婴儿纤维性错构瘤:一项意大利多中心试验

Background: Fibrous hamartoma (FH) of infancy is a benign mesenchymal tumor, occurring as a superficial mass. Complete excision is curative. Objective and Methods: The clinical features and treatment results of 18 children with FH are described. Results: Local excision was the most common procedure. Surgery was radical in 10 patients, with microscopic residual disease in 6; all of them are alive with no evidence of disease 2 to 49 months after diagnosis. One patient, treated with a local reexcision for macroscopic residual disease (and chemotherapy for a synchronous desmoid fibromatosis) is well 83 months after diagnosis; the last patient, with a lesion of the labia majora, only underwent biopsy and is doing well, awaiting plastic surgery. Limitations: The results did not reach statistical significance due to difficulties in collecting cases. Conclusions: FH should be treated by complete excision; in our experience a nonradical excision was also able to achieve the cure. An aggressive approach should be avoided, as the overall prognosis is excellent.

prey animals are often faced with uncertainty due to having imperfect information regarding current local conditions,including predation risks and the availability of reliable foraging opportunities(dall 2010).As this uncertainty increases,the probability of making costly behavioral errors also increases,leading to more risk averse behavioral tactics(Feyten et al.2019).For example,Trinidadian guppies poecilia reticulata are far more risk averse when faced with multiple unknown sources of risk assessment information than when faced with a mix of known and unknown cues(Feyten et al.2019).prey can reduce uncertainty by gathering and integrating private and social information regarding current risk levels within microhabitats(dall 2010;Munoz and Blumstein 2012).

Cyclic electron transport/flow(CET/CEF)in chloroplasts is a regulatory process essential for the optimization of plant photosynthetic efficiency.A crucial CEF pathway is catalyzed by a membrane-embedded NADH dehydrogenase-like(NDH)complex that contains at least 29 protein subunits and associates with photosystem I(pSI)to form the NDH-pSI ***,we report the 3.9Åresolution structure of the Arabidopsis thaliana NDH-pSI(AtNDH-pSI)*** constructed structural models for 26 AtNDH subunits,among which 11 are unique to chloroplasts and stabilize the core part of the NDH *** the supercomplex,one NDH can bind up to two pSI-light-harvesting complex I(pSI-LHCI)complexes at both sides of its membrane *** minor LHCIs,Lhca5 and Lhca6,each present in one pSI-LHCI,interact with NDH and contribute to supercomplex formation and ***,our study reveals the structural details of the AtNDH-pSI supercomplex assembly and provides a molecular basis for further investigation of the regulatory mechanism of CEF in plants.

Objective: to assess the evolution of serum creatinine in patients undergoing radical nephrectomy (malignant) or total (benign), to identify risk factors connected with an unfavorable renal function outcome. Material and Methods: observational, transversal and retrospective study, through analysis of records and lab exams of 146 patients undergoing nephrectomy from January 2015 to December 2018. Results: Statistically significant difference was found between etiology and patients’ age (p-value Conclusion: This study shows that a large number of young patients undergo nephrectomy, many times avoidable;within a benign disease context with an increased risk of developing renal failure. Greater attention from the public service is required to tackle such chronic condition and its complications.

Dear Editor,The tumor suppressor gene CDKN1B,encoding for the p27^(Kip1)(p27)protein,defines the smallest region of deletion on chromosome 12p13 described in clonal hematopoietic disorders(CHDs)[1].Among them,myelodysplastic syndromes(MDSs)display typical onset in the elderly and an indolent behavior that may evolve in acute myeloid leukemia(AML)[2].Recent evidences support a model of parallel clonal evolution at the stem or progenitor cell level and implicate the need of more preclinical,translational and clinical research to identify better ways to timely target clones that may evolve toward malignancy[3,4].

BACKGROUND AND OBJECTIVE Extracorporeal shockwave therapy (ESWT) is effective in reducing shoulder pain and improving function in calcific supraspinatus tendinopathy. Eccentric exercise has been introduced as an effective treatment choice for Achilles tendinopathy, but poor evidence exists about its role in the treatment of rotator cuff tendinopathy. METHODS The study-group was assigned to receive focal ESWT (f-ESWT) plus a supervised eccentric training (SET) of the shoulder abductor muscles. The matched control-group received f-ESWT only. The post-treatment assessment at follow-up (T1) was performed nine weeks after the enrollment (T0). We assessed shoulder pain and function by the means of a numeric rating scale (p-NRS) and a DASH scale. As secondary outcome, we measured the isometric strength of the abductor muscles of the affected shoulder using a handheld dynamometer. RESULTS At T1, we recorded a significant decrease in pain (pprovement in upper limb function (pps. However, we observed no statistical differences in favor of the study-group, in terms of p-NRS and DASH total score. A mild increase (+13% from baseline) of the maximum isometric abduction strength was noticed in the study group at T1. CONCLUSION Our findings did not support the hypothesis that adding a supervised eccentric training of the shoulder abductor muscles could improve the outcome (pain and function) of shock wave therapy.
