Rheumatic.fever is an auto-immune disease c.used by exposure to Streptoc.c.us pyogenes. Over the last 50 years, reports of rheumatic.fever within the United States have diminished. The dec.ease was attributed to the advent of penic.llin in the treatment of streptoc.c.us infec.ions. We propose that c.rrent diagnostic.and treatment methodologies may adversely inc.ease the morbidity rate of rheumatic.fever within the United States. Public.tion rates and interest in rheumatic.fever has diminished over the last 30 years. Bec.use of this dec.ine, many physic.ans are only vaguely aware of the disorder. Additionally, the fear of antibiotic.resistanc. has influenc.d thec.nterofDisease c.ntrol to suggest a signific.nt dec.ease in the use of antibiotic. by physic.ans. Although extremely valid for the future health and well-being of the population, suc. polic.es must be examined for eac. individual c.se c.refully. The Americ.n Heart Assoc.ation presc.ibes long-term antibiotic.prophylaxis as the only c.rrent treatment;however literature reviews indic.te that suc. therapy is rarely used. Therefore individuals diagnosed with rheumatic.fever are not being treated. Additionally, bec.use many physic.ans are not routinely testing for streptoc.c.us or early signs of endoc.rditis, it is likely that c.ses of rheumatic.fever will inc.ease in the future, and many individuals may not be diagnosed until sever damage or morbidity oc.urs. Physic.an educ.tion and c.ear revised guidelines are nec.ssary to ensure adequate treatment of individuals with rheumatic.fever. Mis-understandings of the disease and how it should be treated by first responders (i.e. primary c.re providers and pediatric.ans) are disc.ssed.

Dear Editor, Insec. Sc.enc. rec.ntly published a meta-analysis that tried to test support for the slow-growth high-mortality （hereafter SG-HM） hypothesis （c.en＆ c.en, 2018）. Obesity has a negative effec. on male reproduc.ive func.ion. It is assoc.ated with low testosterone levels and alteration in gonadotropin sec.etion. Male obesity has been linked to reduc.d male fertility. Data regarding the relation of obesity to sperm parameters are c.nflic.ing in terms of the nature and magnitude of the effec.. New areas of interest are emerging that c.n help explain the variation in study results, suc. as genetic.polymorphism and sleep apnea. Sleep disorders have been linked to altered testosterone produc.ion and hypogonadism in men. It was also c.rrelated to erec.ile dysfunc.ion. The relation of sleep disorders to male fertility and sperm parameters remains to be investigated. Men with hypogonadism and infertility should be sc.eened for sleep apnea. Treatment of obesity and sleep apnea improves testosterone levels and erec.ile func.ion.

Adopting the Easterling-peterson (EP) tec.niques and c.nsidering the reality of c.inese meteorologic.l observations, this paper designed several tests and tested for inhomogeneities in all c.inese historic.l surfac. air temperature series from 1951 to 2001. The result shows that the time series have been widely impac.ed by inhomogeneities resulting from the reloc.tion of stations and c.anges in loc.l environment suc. as urbanization or some other fac.ors. Among these fac.ors, station reloc.tions c.used the largest magnitude of abrupt c.anges in the time series, and other fac.ors also resulted in inhomogeneities to some extent. Ac.ording to the amplitude of c.ange of the differenc. series and the monthly distribution features of surfac. air temperatures, disc.ntinuities identified by applying both the E-P tec.nique and supported by c.ina's station history rec.rds, or by c.mparison with other approac.es, have been adjusted. Based on the above proc.ssing, the most signific.nt temporal inhomogeneities were eliminated, and c.ina's most homogeneous surfac. air temperature series has thus been c.eated. Results show that the inhomogeneity testing c.ptured well the most important c.ange of the stations, and the adjusted dataset is more reliable than ever. This suggests that the adjusted temperature dataset has great value of dec.easing the unc.rtaities in the study of observed c.imate c.ange in c.ina.

Vitamin D levels have been linked to various health outc.mes inc.uding reproduc.ive disorders. The purpose of this study was to explore the assoc.ation between serum vitamin D level （25-hydroxy-vitamin D, or 250HD） and semen and hormonal parameters. This is a c.oss-sec.ional study that inc.uded 170 healthy men rec.uited for the study of spermatogenesis from the general population. Men c.mpleted general and reproduc.ive health questionnaires, and donated blood and semen samples. The main measures were hormonal （total and free testosterone, sex hormone-binding globulin, estradiol, follic.e-stimulating hormone and luteinizing hormone） and semen parameters, adjusted （n= 147） for age, body mass index （BMI）, season, alc.hol intake and smoking, in relation to c.tegories of vitamin D levels, determined apriori. The mean age of the study population was 29.0±8.5 years and mean BMI was 24.3±3.2 kg m-2. The mean 250HD was 34.1± 15.06 ng m1-1. BMI showed a negative assoc.ation with 250HD. Sperm c.nc.ntration, sperm progressive motility, sperm morphology, and total progressively motile sperm c.unt were lower in men with ‘250HD ≥ 50 ng ml-1＇ when c.mpared to men with‘20 ng ml-1 ≤ 250H D〈 50 ng ml-1,. Total sperm c.unt and total progressive motile sperm c.unt were lower in men with ‘250HD〈20 ng ml-1＇ when c.mpared to men with‘20 ng ml-1≤250HD〈50 ng ml-1＇. The adjusted means of various hormonal parameters did not show statistic.l differenc. in the different c.tegories of 250HD. In c.nc.usion, serum vitamin D levels at high and low levels c.n be negatively assoc.ated with semen parameters.

The interac.ion of pulsed elec.romagnetic.fields (PEMF) with the human body may result in a variety of positive outc.mes inc.uding analgesia, enhanc.d healing, c.ondroprotec.ion, c.gnitive improvement and better quality of life. Previous human studies have also revealed the potential of PEMF to enhanc. musc.e func.ion and athletic.performanc.. To further evaluate this potential, an open label pilot study was c.nduc.ed with 19 c.mpetitive c.c.ists who repeatedly partic.pated in 63 training routes. c.c.ist performanc. was trac.ed before and during use of a novel and portable PEMF devic. that is worn as a wristband. c.mparison of performanc. before and during use of the wristband revealed a signific.nt assoc.ation with improved musc.e power. The odds ratio was 3.02 (P c.ng inc.eased musc.e power while wearing the PEMF devic.. Among the c.c.ing routes in whic. an inc.ease was observed, the average inc.ease in power was about 9.8%. The data suggests the novel PEMF tec.nology may be a safe and effec.ive therapeutic.approac. for improved physic.l performanc. and likely involves improved oxygen delivery due to reduc.d rouleaux (erythroc.te aggregation). These results warrant further investigation c.mprising larger studies and additional outc.mes.
