Organ transplantation and drug eluting stents:Perioperative challenges

Patients listed for organ transplant frequently have severe coronary artery disease(CAD), which may be treated with drug eluting stents(DES). Everolimus and zotarolimus eluting stents are commonly used. Newer generation biolimus and novolimus eluting biodegradable stents are becoming increasingly popular. Patients undergoing transplant surgery soon after the placement of DES are at increased risk of stent thrombosis(ST) in the perioperative period. Dual antiplatelet therapy(DAPT) with aspirin and a P2Y12 inhibitor such as clopidogrel, prasugrel and ticagrelor is instated post stenting to decrease the incident of ST. Cangrelor has recently been approved by Food and Drug Administration and can be used as a bridging antiplatelet drug. The risk of ischemia vs bleeding must be considered when discontinuing or continuing DAPT for surgery. Though living donor transplant surgery is an elective procedure and can be optimally timed, cadaveric organ availability is unpredictable, therefore, discontinuation of antiplatelet medication cannot be optimally timed. The type of stent and timing of transplant surgery can be of utmost importance. Many platelet function point of care tests such as Light Transmittance Aggregrometry, Thromboelastography Platelet Mapping, VerifyN ow, Multiple Electrode Aggregrometry are used to assess bleeding risk and guide perioperative platelet transfusion. Response to allogenic platelet transfusion to control severe intraoperative bleeding may differ with the antiplatelet drug. In stent thrombosis is an emergency where management with either a drug eluting balloon or a DES has shown superior outcomes. Post-transplant complications often involved stenosis of an important vessel that may need revascularization. DES are now used for endovascular interventions for transplant orthotropic heart CAD, hepatic artery stenosis post liver transplantation, transplant renal artery stenosis following kidney transplantation, etc. Several antiproliferative drugs used

In machine learning and data mining,feature selection(FS)is a traditional and complicated optimization *** the run time increases exponentially,FS is treated as an NP-hard *** researcher’s effort to build a new FS solution was inspired by the ongoing need for an efficient FS framework and the success rates of swarming outcomes in different optimization *** paper presents two binary variants of a Hunger Games Search Optimization(HGSO)algorithm based on V-and S-shaped transfer functions within a wrapper FS model for choosing the best features from a large *** proposed technique transforms the continuous HGSO into a binary variant using V-and S-shaped transfer functions(BHGSO-V and BHGSO-S).To validate the accuracy,16 famous UCI datasets are considered and compared with different state-of-the-art metaheuristic binary *** findings demonstrate that BHGSO-V achieves better performance in terms of the selected number of features,classification accuracy,run time,and fitness values than other state-of-the-art *** results demonstrate that the BHGSO-V algorithm can reduce dimensionality and choose the most helpful features for classification *** proposed BHGSO-V achieves 95%average classification accuracy for most of the datasets,and run time is less than 5 *** low and medium dimensional datasets and less than 10 sec for high dimensional datasets.

In this paper, we establish the existence and uniqueness of fixed points of operator , when n is an arbitrary positive integer and X is a partially ordered complete metric space. We have shown examples to verify our work. Our results generalize the recent fixed point theorems cited in [1]-[4] etc. and include several recent developments.

Face transplantation:Anesthetic challenges

Face transplantation is a complex vascular composite allotransplantation(VCA) surgery. It involves multiple types of tissue, such as bone, muscles, blood vessels, nerves to be transferred from the donor to the recipient as one unit. VCAs were added to the definition of organs covered by the Organ Procurement and Transplantation NetworkFinal Rule and National Organ Transplant Act. Prior to harvest of the face from the donor, a tracheostomy is usually performed. The osteotomies and dissection of the midface bony skeleton may involve severe hemorrhagic blood loss often requiring transfusion of blood products. A silicon face mask created from the facial impression is used to reconstruct the face and preserve the donor's dignity. The recipient airway management most commonly used is primary intubation of an existing tracheostoma with a flexometallic endotracheal tube. The recipient surgery usually averages to 19-20 h. Since the face is a very vascular organ, there is usually massive bleeding, both in the dissection phase as well as in the reperfusion phase. Prior to reperfusion, often, after one sided anastomosis of the graft, the contralateral side is allowed to bleed to get rid of the preservation solution and other additives. Intraoperative product replacement should be guided by laboratory values and point of care testing for coagulation and hemostasis. In face transplantation, bolus doses of pressors or pressor infusions have been used intraoperatively in several patients to manage hypotension. This article reviews the anesthetic considerations for management for face transplantation, and some of the perioperative challenges faced.

Split liver transplantation:What's unique?

The intraoperative management of split liver transplantation(SLT) has some unique features as compared to routine whole liver transplantations. Only the liver has this special ability to regenerate that confers benefits in survival and quality of life for two instead of one by splitting livers. Primary graft dysfunction may result from small for size syndrome. Graft weight to recipient body weight ratio is significant for both trisegmental and hemiliver grafts. Intraoperative surgical techniques aim to reduce portal hyperperfusion and decrease venous portal pressure. Ischemic preconditioning can be instituted to protect against ischemic reperfusion injury which impacts graft regeneration. Advancement of the technique of SLT is essential as use of split cadaveric grafts expands the donor pool and potentially has an excellent future.

Monitoring Trace Metals in Some Cultured Freshwater Fish Species in Saudi Arabia

Three species of freshwater fishes, Oreochromis niloticus, Clarias lazera and Cyprinus carpio, were collected in marketing size from some fish farms in Al-Kharj region, Riyadh area, Saudi Arabia and examined throughout the year 2012 for the following trace metals contents： chromium （Cr）, lead （Pb）, cobalt （Co）, nickel （Ni）, copper （Cu）, zinc （Zn）, iron （Fe） and cadmium （Cd）. The results showed no detection of Cd in all examined species, however, the mean values of other tested metals were within the standard permissible levels except for Pb which showed higher mean levels ranged from 2.05 ± 0.65 mg/kg to 5.30 ± 0.60 mg/kg in O. niloticus, 0.15 ± 0.13 mg/kg to 6.40 ± 0.10 mg/kg in C. lazera and 1.7 ± 0.20 mg/kg in C. carpio compared to the maximum permissible limits recorded by Saudi Arabia Standards Organization （SASO） for fish and shellfish. Consequently, from the public health point of view, efficient routine examination of cultured fishes to control trace metals during culture processes and before marketing is recommended.

The Successive Approximation Method for Solving Nonlinear Fredholm Integral Equation of the Second Kind Using Maple

In this paper, we will use the successive approximation method for solving Fredholm integral equation of the second kind using Maple18. By means of this method, an algorithm is successfully established for solving the non-linear Fredholm integral equation of the second kind. Finally, several examples are presented to illustrate the application of the algorithm and results appear that this method is very effective and convenient to solve these equations.

Philosophy of organ donation: Review of ethical facets

Transplantation ethics is a philosophy that incorporates systematizing, defending and advocating concepts of right and wrong conduct related to organ donation. As the demand for organs increases, it is essential to ensure that new and innovative laws, policies and strategies of increasing organ supply are bioethical and are founded on the principles of altruism and utilitarianism. In the field of organ transplantation, role of altruism and medical ethics values are significant to the welfare of the society. This article reviewsseveral fundamental ethical principles, prevailing organ donation consent laws, incentives and policies related to the field of transplantation. The Ethical and Policy Considerations in Organ Donation after Circulatory Determination of Death outline criteria for death and organ retrieval. Presumed consent laws prevalent mostly in European countries maintain that the default choice of an individual would be to donate organs unless opted otherwise. Explicit consent laws require organ donation to be proactively affirmed with state registries. The Declaration of Istanbul outlines principles against organ trafficking and transplant tourism. World Health Organization's Guiding Principles on Human Cell, Tissue and Organ Transplantation aim at ensuring transparency in organ procurement and allocation. The ethics of financial incentives and non-financial incentives such as incorporation of non-medical criteria in organ priority allocation have also been reviewed in detail.
