Retrospective Study of 23 Cases of Psoriasis Association with HIV Infection Observed in the department of dermatology-STd in the University Hospital of donka Conakry Guinea

Introduction :The psoriasis is one of the inflammatory dermatoses with unknown etiology, with chronic evolution having episodic appearance and disappearance. Its prevalence in HIV patients varied from 2% to 5% in most of the times. Objective of this study was to describe the demographic, clinical and therapeutic characteristics of psoriasis patients infected with HIV. Methods: It is a descriptive retrospective study done from January 2003 to december 2013 based on the information from the hospital card of hospitalized patients and outpatients taken care in the department of dermatology-STd for psoriasis at the University hospital center in donka Conakry. We included all the cases of psoriasis associated with HIV infection diagnosed from clinical and paraclinical elements. Results: We recorded 23 (24.73%) cases of psoriasis associated with HIV infection among 93 patients observed for psoriasis in which there are 4 cases of psoriasis vulgaris, 10 cases of erythrodermic psoriasis and 9 cases of arthropathic form among these numbers. We had 7 women and 16 men. The medium age of our patients was 44.5 ± 12 years [27 - 62 years]. The delayed duration time of consultation varied from 30 to more than 180 days. The psoriasis was the circumstance of the discovery of the HIV infection among 55% of cases. The pruritus was the functional sign which is the most frequent in 20 cases among 23 cases and 71.4% of cases were accompanied with pain. Family history was found in 7.10% of cases. Anxiety was the dominant factor cause in 42% and the infection 38%. The cutaneous alteration was noted in all patients;92.9% of patients had nails alteration and intertrigineous association in 78.6% of cases. The clinical forms found were psoriasis vulgaris 4/23 cases, arthropathic psoriasis 9/23 cases, erythrodermic psoriasis 10/23. More than half (13/23) cases of our patients were diagnosed stage III of the classification of WHO. The complicated forms like erythrodermic and arthropathic psoriasis

Lyell Syndrome with Fatal Outcome in Two Pregnant Women Receiving Antiretroviral Therapy in Guinea Conakry

An old woman, 28 years old, weighing 74 kg was referred for skin detachment associated with bubbles and mucosal erosions. This clinical picture had occurred 15 days after the institution of a triple combination: AZT (300 mg) + 3TC (150 mg) and nevirapine (200 mg), during a pregnancy of 20-week gestation. The woman was in her third gestation. The second case is aged 26, weighing 65 kg. This clinical picture had occurred 20 days after the administration of a triple therapy combination: AZT (300 mg) + 3TC (150 mg) and nevirapine (200 mg), during a pregnancy of 32-week gestation. It was the fourth gestation. The birth of a stillborn had occurred the day before his admission into our service. The diagnosis of Toxic Epidermal Necrolysis with nevirapine was selected on the basis of the clinical and anamnestic arguments. The biological tests showed the following results: transaminases (ALT and AST) were within normal limits, blood glucose, and urea > 10 mmol/l. The evolution was marked by the patient death to day 9 (d9) and at d2 of hospitalization. discussion: NET and the SJS have been reported in pregnant women as potentially dangerous complications that are often associated with suspected d.ugs. Among these d.ugs, antiretroviral d.ugs, prescribed to pregnant women to prevent mother to child transmission of HIV are often reported. Our case is an illustration of the problem of HIV management in pregnant women. It also focuses on the association between pregnancy and Toxic Epidermal Necrolysis. Nevirapine is an effective d.ug in the regimens proposed in the management of HIV patients in limited resources countries. But the high frequency of toxidermia with this d.ug should lead to its replacement by other antiretrovirals least providers of toxidermia as anti proteases in the context of PMTCT programs.

disseminated peritoneal leiomyomatosis (dPL) is a benign tumor of smooth muscle tissue. It is rare and is characterized by the development of multiple peritoneal nodules simulating peritoneal carcinosis. Less than 200 cases have been reported in the literature so far. We are reporting a case of dPL detected during a Caesarean section 6 years ago in a 41-year-old female patient, G5P2. The patient underwent an elective iterative caesarean section at 38 weeks of amenorrhea for a fetus in breech presentation. during laparotomy, there was a marked regression of the peritoneal nodules varying in size from 0.1 to 0.5 cm. Histological examination showed a proliferation of smooth muscle fibers without mitosis, atypia or necrosis. dPL is a benign, confusing condition that simulates peritoneal carcinomatosis, which must be recognized as such in order to avoid a dilapidated and unnecessary surgical procedure.

Obstetrical Complications of Female Genital Mutilation: Management Maternal-Fetal Medical Care and Prognosis, Obstetrical Gynecology Regional Hospital, Unit of N'zérékoré

Introduction: Female Genital Mutilation (FGM) is a public health problem. There are 100 to 140 million girls and women who suffer every year in the world [1]. The aim of this study is to improve the medical care and reduce complications of FGM at the Regionalhospital of N’zérékoré, to determine their frequency, and to evaluate the maternal-fetalprognosis. Methods: The study was conducted at the Regional Hospital of N’zérékoré. This was a cross-sectional, descriptive and analytic study of 6 months, from 1 September 2016 to 28 February 2017, including all pregnant women admitted for childbirth who had a complication of female genital mutilation. Results: A total of 1295 women gave birth in the service, of which 1204 women were women with female genital mutilation. Given a frequency of 92.97%, of these 1204 mutilated women, 223 presented obstetrical complications during their delivery, a proportion of 17.22%. They were mostly young patients, mostly housewives who were not in school. Type II FGM was the most common (53.06%). Obstetric complications were dominated by complicated perinatal tears (54.08%), and hemorrhages (40.81%). The catch was dominated by perineorrhaphy. Conclusion: The frequency of FGM was 92.97% and that of their obstetric complications 17.22%. Most were house-wives, not in school. There was FGM type II. The abandonment of FGM would reduce maternal and perinatal morbidity and mortality.
