In Mali, the low utilization of reproductive health services in general and family planning (FP) in particular contributes strongly to illnesses or deaths of women during and/or after childbirth and of children under the age of one year. This situation is, among other things, the logical consequence of harmful practices, including closely spaced pregnancies, early pregnancies, clandestine abortions, access difficulties and the lack of adequate obstetric and neonatal care. Objective: The objective of our study was to study the difficulties of access to contraception in the health district of commune IV. Methodology: This was a descriptive and multicenter prospective cross-sectional study, carried out in the reference health center of commune IV in the district of Bamako, in the ten (10) CSCOMs and the RENEE CISSE maternity hospital (MRC). Study appalled from June 1 to december 30, 2019. Two subgroups of the study population were involved in the study: Service providers;Users (clients) made up of women of childbearing age (13 - 45 years old). The premises of the FP units, the service providers, the clients who answered our questionnaires constituted our sample;the interviewed providers were also evaluated for their capacity in Counseling with the different methods used. The variables retained for the analysis concerned: the provision of contraceptive products;as well as user files and their characteristics. Results: The result of our survey reveals that the method most used at the time of our survey was Norplant 49.5% and the least used is the pill 1.8;some products, such as the diaphragm, cervical cap, female condom and spermicide, are not available in the various FP units. Side effects were the cause of discontinuation for 17.9% of our clients;50% against the husband;10.7% for no reason;21.4% for desire for children. 87.2% of clients are satisfied with the service delivery compared to 12.8%. 79.8% of clients say that the cost of contraceptive products is affordable

Electronic System Using Artificial Intelligence for Queue Management

The Covid-19 pandemic has brought changes in behaviour in public places. Indeed, the health and political authorities, in order to fight against the virus in a preventive manner, require the respect of barrier gestures: social distance, mask, vaccine, gel. Still in terms of public health, long waits in a place for a service have a negative impact on the health of fragile categories such as the disabled, pregnant women and the elderly. The technical devices used for queue management must now take into account the health context, identity, particularity and behaviour of people. This paper presents an electronic system developed with artificial intelligence for queue management in public facilities. This design personalises the user’s ticket by automatically integrating the name, facial image, age and possible disability status. At the counters, a system of name calling, sound and screen display, allows users to follow the queue without having a ticket printed on thermal paper with a high carbon footprint. This solution also makes illiterate users autonomous in the queue, allowing them to maintain their dignity and to respect the safety distance between people. The device allows the establishment’s manager, depending on the context, to activate positive discrimination of the disabled or the elderly, to control the Covid-19 mark or the health pass by QR Code. This queue manager performs biometric authentication by facial recognition before the user is registered in the queue register, which prevents fraud by people who do not want to respect the order of arrival of users. This work has led to the improvement of the technical management of queues by introducing more equity, inclusion, solidarity, health and ecology.

Identification of pests and assessment of their damage on Carapa procera and Lophira lanceolata in Burkina Faso,West Africa

Nontimber forest products are a source of income for women in rural African communities. However, these products are frequently damaged by insect pests. The present study investigates the diversity and damage rates of insect pests that attack Carapa procera seeds and Lophira lanceolata fruits. The experiment was set up in western Burkina Faso and, for C. carapa, consisted of pests collected from seeds that had fallen to the ground and from stockpiled seeds. For L. lanceolata, pests were collected from fruits on the trees, and on the ground. The collected samples were sent to the laboratory to estimate the proportion of damaged seeds/fruits and rear the insects. The results showed that Ephestia spp., Tribolium castaneum, Oryzeaphilus spp., and Tenebroides mauritanicus were the pests of Carapa procera seeds and Lophira lanceolata fruits. Ephestia spp. was recorded as the main pest of both C. proeera and L. lanceolata, whereas T. castaneum was only detected from seeds of L. lanceolata. For C. procera, the stocks were the most infested （29 %） by Ephestia spp. The infestation rate of fruits of L. lanceolata by Ephestia spp, on trees （31.42 ± 3.75 %） was less than the rate of fruits by T. castaneum on the ground （44.00 ± 3.5 %）. The different body sizes of Ephestia spp. may indicate the occurrence of two putative species, one from C. procera and another one from L. lanceolata. This work provides important information that could contribute to the setting up of a local-scale sustainable management framework for oil tree pests in Burkina Faso and surrounding countries.

Management of Urological Emergencies at the Nianankoro Fomba Hospital in Segou: A Case Report of 72 Patients

PURPOSE: Our aim was to present the diagnostic and therapeutic aspects of urological emergencies in a regional hospital. MATERIAL ANd METHOdS: We conducted a prospective study over a period of 6 months (April 2021 to September 2021) collecting all the emergencies received by the on-call urology team at the Nianankoro Fomba Hospital in Segou. This team was led by a dES in urology under the supervision of a urological surgeon. We were interested in age, sex of patients, diagnosis, number of patients hospitalised and type of surgery performed in emergency. RESULTS: We registered 72 patients. The mean age of our patients was 58.8 years with extremes of 6 and 90 years. Acute bladder retention was represented in 61% and total haematuria in 24%. Renal colic was reported in 8%. Acute prostatitis was reported in 4% of cases, and acute pyelonephritis in 1%. Urogenital trauma accounted for 6%. CONCLUSION: Although underestimated, in our context, the management of urological emergencies remains a regular activity of the urology department in view of the number of patients managed. Bladder drainage remains the most frequent procedure.

Introduction: The diabetic foot remains a public health problem due to its high frequency, difficult and costly management. The aim of this study was to determine the epidemiological, therapeutic and evolutionary aspects of the diabetic foot in a hospital setting in Bamako. Methodology: This is a retrospective, descriptive, cross-sectional and monocentric study conducted between September 1, 2011 and december 31, 2015 on diabetic patients aged 14 years and older arriving in our department with a foot infection. Results: We identified 94 cases of diabetic foot infection out of a total of 828 hospitalized patients, a prevalence of 11.35%. The age range 41 - 60 years represented 57 cases (60.6%), the extreme ages were 14 and 81 years. Men (38.3%), women (61.7%) with a sex ratio of 0.62%. Housewives 50%, illiterate 51 cases (54.3%), low economic standard of living 40 cases (42.6%), presence of osteitis 40 cases (42.6%), foot at stage d Grade3 28 cases (29.8%). Management was medical in 48 cases (51.1%), treatment with insulin 58 cases (61.7%), Amoxicillin + Metronidazole used 36 cases (38.29%). Amputation was performed in 35 cases (37.2%). deaths concerned 5 patients (9.6%) with hypoglycemia as the main cause in 4 cases. Conclusion: diabetic foot is a frequent complication of diabetes. The establishment of a multidisciplinary team should contribute to the improvement of the prognosis of the diabetic foot in a management center.

Evaluation of the Adequacy between the Workload and the Number of State Midwives and Maieuticians Practicing in the Gynecology-Obstetrics departments of the University Hospitals of Burkina Faso

Objective: To evaluate the adequacy between the workload and the number of midwives and state midwives practicing in the gynecology-obstetrics services of the university hospitals of Burkina Faso. Methodology: This is a multicenter study conducted in 4 university hospitals representing this country (for brevity referred here to as A, B, C, and d) from May 1, 2018, to April 30, 2019. The Workload Indicators of Staffing Need (WISN) method was used to analyze the workload. Results: dystocic delivery was the activity that took the most time. Inpatient activities were the most performed in terms of volume. At the University Hospitals d and C, there was an over-staffing of midwives and maïeuticians with a ratio of 1.93 and 2.12 respectively. At University Hospital B, the workload was in line with the existing number of State midwives and maieuticians. A low workload pressure (04%) was found at the University Hospital A. Conclusion: In most of the University Hospital Centers, there was no match between the workload and the number of midwives and maïeuticians. These results highlight the need for redeplayment of midwives and maïeuticians in order to ensure good health coverage for midwives and maïeuticians.

Staphylococcus xylosus is a species of bacteria belonging to the genus Staphylococcus. This genus currently includes several species and subspecies. We distinguish between 2 groups: the group of coagulase-positive staphylococci (CoPS) and coagulase-negative staphylococci (CoNS), a larger group that includes more than forty species and subspecies. Among the CoNS, Staphylococcus xylosus occupies a special place because it is frequently isolated from meats, dairy products, and agricultural environments. It is commonly used in fermentation due to its primary role in the flavor and color development of fermented products. It is a commensal bacterium of the skin of small mammals and farm animals, where it constitutes the majority of flora. In this species, there are a large number of strains, including phenotypes and genotypes. Although S. xylosus is actually a recognized non-pathogenic species, a few strains can be potentially dangerous. They can thus be responsible for mastitis and dermatitis in cattle. With the multiplicity of breeding farms, the consumption of livestock products and the increasingly frequent contact of humans with animals a few rare cases of human opportunistic infections have also been reported in the literature. These include pyelonephritis, endocarditis, urinary, lymphatic, and lymph node infections, contamination of orthopedic materials and catheters. Thereby, we report the case of a lingering and chronic infection of the lacrimal tract by Staphylococcus xylosus in an 8-year-old child in the Ophthalmology department of Sikasso hospital in Mali.

Stand-Alone Patient Reception and Referral System with Health data Management

The COVId-19 pandemic has exposed vulnerabilities within our healthcare structures. Healthcare facilities are often faced with staff shortages and work overloads, which can have an impact on the collection of health data and constants essential for early diagnosis. In order to minimize the risk of error and optimize data collection, we have developed a robot incorporating artificial intelligence. This robot has been designed to automate and collect health data and constants in a contactless way, while at the same time verifying the conditions for correct measurements, such as the absence of hats and shoes. Furthermore, this health information needs to be transmitted to services for processing. Thus, this article addresses the aspect of reception and collection of health data and constants through various modules: for taking height, temperature and weight, as well as the module for entering patient identification data. The article also deals with orientation, presenting a module for selecting the patient’s destination department. This data is then routed via a wireless network and an application integrated into the doctors’ tablets. This application will enable efficient queue management by classifying patients according to their order of arrival. The system’s infrastructure is easily deployable, taking advantage of the healthcare facility’s local wireless network, and includes encryption mechanisms to reinforce the security of data circulating over the network. In short, this innovative system will offer an autonomous, contactless method for collecting vital constants such as size, mass, and temperature. What’s more, it will facilitate the flow of data, including identification information, across a network, simplifying the implementation of this solution within healthcare facilities.

Ocular Manifestations in Severe Acute Malnutrition in Children under 60 Months in a Secondary Health Center

Introduction: Severe acute malnutrition (SAM) is one of the main public health problems in the world. It is responsible in addition to the general symptoms, vitamin A deficiency which can cause serious eye damage including keratomalacia. This work aims to study possible ophthalmological lesions in severely acutely malnourished children from 0 to 60 months. Patients and Method: This study was carried out in a secondary ophthalmology center. It concerned all severely acute malnourished patients admitted for consultation in the pediatric department. Results: A total of 174 eyes of 87 children aged 0 to 60 months were examined. Two-thirds of the patients were aged between 6 and 18 months. Palpebral lesions are rare, however, we observed 12.64% blepharitis and palpebral dermatitis. Ocular involvement due to vitamin A deficiency was 6.9%, including one case of BITOT SPOT, 4 cases of conjunctival xerosis and one case of keratomalacia. Conclusion: Xerophthalmia is an ocular lesion due to a dietary intake deficient in vitamin A that occurs during severe acute malnutrition. Its occurrence is increasingly rare even in developing countries. However, some cases are observed in practice and may be responsible for profound visual impairments.

胜利露天矿是中国重要的煤炭基地。然而,由于煤炭资源的开采,该矿区的生态环境严重恶化。植物基质是矿区排土场边坡重建与生态修复的重要因素。因此,本文旨在提高半干旱矿区排土场边坡在蒸发、入渗、径流、水土流失和土壤湿度影响下的生态恢复效果,以减少土壤侵蚀,促进边坡上的种子萌发和植物生长。在盆栽试验的基础上,加入污泥、粉煤灰、矿渣、秸秆等基质材料,与氮、磷、钾混合,提高土壤肥力。测定了植物基质的理化性质、种子萌发、植物生长、植物对径流和水土流失的影响。试验在20°单坡(K1)、30°单坡(K2)和多层坡(K3)三种不同类型的斜坡上进行,降雨间隔超过24小时。K1、K2和K3坡每次降雨量分别为500 ml、550 ml和575 ml。本文采用高压液压喷雾器进行降雨模拟研究,研究降水和浇灌植物的特性。基质材料均匀分布在长40 cm、宽30 cm、高20 cm的聚乙烯罐中。这些盆栽在播种前浇水一周以改善土壤质量。播种后覆盖约2 cm厚的材料,然后浇水至饱和,每天观察土壤水分,以适当水化。本试验选取了5种植物:紫花苜蓿、高羊茅、小麦、羊草和紫穗槐。理化性质研究结果表明,与其他处理土壤相比,未处理土壤(对照土壤)植物基质的电导率和p H值较高,属于碱性土壤。与处理过的土壤相比,氮、磷、钾等有机质含量相对较低,表明基质中养分含量较低,抑制了植物的生长。污泥比含量、粉煤灰比含量、矿渣比含量是影响入渗、蒸发、p H、电导率、有机质、植物生长速率等指标的关键因素。随着坡角的增大,各坡型土壤减湿幅度逐渐减小,初始生产时间减少,且坡角越高有利于雨水的充分入渗,因此侵蚀量相对较低。在间歇降水作用下,由于表层土壤含水量较高,土壤水分增加的趋势更接近。随着降水的增加,初始生产时间明显缩短,达到稳定径流所需时间减少,稳定流量增加,水土流失量增加。在盆栽试验的基础上,确定了植株的发芽率和生长指标。结果表明,在种子萌发过程中,小麦首先萌发,其次是高羊茅属植物。两株萌发质量均较好,平均发芽率分别为93.33%和88.88%。紫穗槐植株发芽最晚,平均发芽率为16.66%。在植物光合作用方面,坡地黄叶数量和生态干扰最小,不同水平的绿叶数量最大。测定了植物生物量。根据植物生长试验,有平台、上坡、中坡、下坡。对植物生长的相关指标进行了测定,结果表明,虽然保护地有利于植物生长和根系发育,但对植物地上部分的抑制作用比地下部分更为显著。小麦和高羊茅的根长非常发达,平均长度分别为40.5 cm和18.56 cm,对露天矿排土场边坡的稳定具有重要作用。这些植物还在到达排土场斜坡底部之前拦截了快速落下的雨滴,减少了雨水溅落的侵蚀以及径流和水土流失。这些植物有利于减少径流、水和土壤流失,并可以通过拦截降雨和稳定根系发育的斜坡表面来控制侵蚀。本研究中不同类型边坡生态恢复的适宜坡度为边坡K3,适宜角度为20°。全文共有图39张,表格28张,参考文献83篇。
