AIM To determine the sensitivity of macroscopic appearance for predicting histological diagnosis at sites other than duodenum in pediatric celiac disease(Cd).METHOdS Endoscopic and histologic findings in pediatric patients undergoing upper endoscopy for first-time diagnosis of Cd at Stollery Children's Hospital from 2010-2012 were retrospectively *** Clinical charts from 140 patients were reviewed. Esophageal and gastric biopsies were taken in 54.3% and 77.9% of patients, respectively. Endoscopic appearance was normal in the esophagus and stomach in 75% and 86.2%. Endoscopic esophageal diagnoses were eosinophilic esophagitis(EE)(11.8%), esophagitis(7.9%), glycogenic acanthosis(1.3%) and non-specific abnormalities(3.9%). Endoscopic gastric diagnoses were gastritis(8.3%), pancreatic rest(0.9%), and nonspecific abnormalities(4.6%). Histology was normal in 76.3% of esophageal and 87.2% of gastric speci-mens. Abnormal esophageal histology was EE(10.5%), esophagitis(10.5%), glycogenic acanthosis(1.3%) and non-specific(1.3%). Gastritis was reported in 12.8% of specimens. Sensitivity and specificity of normal endoscopy for predicting normal esophageal histology was 86.2% and 61.1%, and for normal gastric histology was 87.4% and 21.4%.CONCLUSION In the absence of macroscopic abnormalities, routine esophageal and gastric biopsy during endoscopy for pediatric Cd does not identify major pathologies. These findings have cost and time saving implications for clinical practice.

Perimesencephalic subarachnoid hemorrhage (P-SAH) is a benign subset of subarachnoid hemorrhage. Only two cases of P-SAH during pregnancy have been reported in the literature. we describe a case of P-SAH detected at 29 weeks’ gestation, arose with severe bilateral frontal headache, neck rigidity, and vomiting. A diagnosis of P-SAH was confirmed on magnetic resonance angiography with no evidence of vascular malformation or bleeding sources and conservatively treated. Hemorrhage, vasospasm, and cerebellar edema observed on initial magnetic resonance angiography resolved after two months. The clinical course of P-SAH during pregnancy appears to be favorable. Our case shows for the first time in literature that magnetic resonance angiography is a valid diagnostic test for P-SAH during pregnancy that circumvents radiological risk associated with conventional imaging.

The sorption study of La(Ⅲ) was carried out on poly[dibenzo-18-crown-6] and L-valine medium. The quantitative adsorption of La(Ⅲ) was found at 1×10-2 to 1×10-5 mol/L L-valine. The various eluting agents were found efficient eluents for La(Ⅲ). The capacity of crown polymer for La(Ⅲ) was found to be 0.43 ±0.01 mmol/g. The tolerance limit of various cations and anions for La(Ⅲ) was determined. La(Ⅲ) was quantitatively separated from other metal ions in binary as well as multicomponent mixtures. The study was extended to sequential separation of La(Ⅲ), U(VI) and Th(IV). The good separation yields were obtained and had good reproducibility (±2%). The method incorporated the determination of La(Ⅲ) in real sample. The method was simple, rapid and selective.

Crohn's disease(Cd) is a chronic inflammatory bowel disease that can affect the entire gastrointestinal ***,up to 70% of all patients will need surgery,despite optimized medical ***,about half of the patients will need redo-surgery because of disease *** introduction of anti-tumor necrosis factor(TNF) drugs(Infliximab in 1998) revolutionized the treatment of *** randomized trials assessed the efficacy of anti-TNF treatment not only to induce,but also to maintain,steroid-free ***,these agents can rapidly lead to mucosal *** aspect is important,as it is a major predictor for long-term disease *** analyses of responding patients seemed to suggest a reduction in the need for surgery at median-term follow up(1-3 years).However if one looks at population surveys,one does not observe any decline in the need for surgery since the introduction of Infliximab in *** short follow-up term and the exclusion of patients with imminent surgical need in the randomized trials could bias the *** 60% of patients respond to induction of anti-TNF therapy,moreover,some patients will actually develop resistance to *** patients are diagnosed when stenosing disease has already occurred,obviating the need for biological *** a further attempt to change the actual course of the disease,top down strategies have been progressively *** this will indeed obviate surgery for a substantial group of patients remains *** the time being,surgery will still play a pivotal role in the treatment of Cd.
