Intensity modulated radiation therapy with simultaneous integrated boost based dose escalation on neoadjuvant chemoradiation therapy for locally advanced distal esophageal adenocarcinoma

AIM:To evaluate impact of radiation therapy dose escalation through intensity modulated radiation therapy with simultaneous integrated boost(IMRT-SIB).METHOdS:We retrospectively reviewed the patients who underwent four-dimensional-based IMRT-SIBbased neoadjuvant chemoradiation *** the concurrent chemoradiation therapy,radiation therapy was through IMRT-SIB delivered in 28 consecutive daily fractions with total radiation doses of 56 Gy to tumor and 5040 Gy dose-painted to clinical tumor volume,with a regimen at the discretion of the treating medical *** was followed by surgical tumor *** analyzed pathological completion response(p CR) rates its relationship with overall survival and ***:Seventeen patients underwent dose escalation with the IMRT-SIB protocol between 2007 and 2014 and their records were available for *** the IMRT-SIB-treated patients,the toxicity appeared mild,the most common side effects were grade 1-3 esophagitis(46%) and pneumonitis(11.7%).There were no cardiac *** Ro resection rate was 94%(n = 16),the p CR rate was 47%(n = 8),and the postoperative morbidity was *** was one mediastinal failure found,one patient had local failure at the anastomosis site,and the majority of failures were distant in the lung or *** 3-year diseasefree survival and overall survival rates were 41%(n = 7) and 53%(n = 9),***:The dose escalation through IMRT-SIB in the chemoradiation regimen seems responsible for down-staging the distal esophageal with well-tolerated complications.

Family resilience is not the sum of the individual psychological resilience of each of the family members,but the family itself as a unit of resilience *** representative models of family stress tolerance theory mainly include Mc Cubbin's series of family stress tolerance models and Walsh's family stress tolerance framework,which are widely used in the practice of family *** the future,empirical studies on family resilience should be strengthened,measurement tools with high reliability and validity should be established,and the combination of family resilience theory and community-based practice should be promoted.

It is well known that the cotton aphid is the major pest in cotton fields of Northwest China, and seven-spot ladybird is an important natural enemy among the various possi- ble natural enemies of cotton aphid. In order to increase the applications of population dynamics in integrated pest management and control the cotton aphids biologically, we need to understand the population dynamics of cotton aphid and their natural enemies. A delay predator prey system on cotton aphid and seven-spot ladybird beetle are pro- posed in this paper. Based on the comparison theorem and an iterative method, we investigate the global attra^tivity of the equilibrium points which have important bio- logical meanings. Furthermore, some numerical simulations were carried out to illustrateand expand our theoretical results, in which a conjecture to generalize the well-known Theorem 16.4 in H. R. Thiemes book was put forward, which was taken as the open problem. The numerical simulations show coexistence of periodic solution, confirming the theoretical prediction.

Propulsion tests on ultra-high-temperature ceramic matrix composites for reusable rocket nozzles

Ultra-high-temperature ceramic matrix composites(UHTCMCs)based on a ZrB_(2)/SiC matrix have been investigated for the fabrication of reusable nozzles for *** de Laval nozzle prototypes,obtained by sintering with either hot pressing(HP)or spark plasma sintering(SPS),were tested 2-3 times in a hybrid rocket motor for proving *** were extracted after oxidation tests to study the microstructural changes and oxidative and thermomechanical stresses induced by the repeated *** to a reference graphite nozzle,no measurable erosion was observed for the UHTCMC-based *** oxidation mechanism consisted in the formation of a ZrO_(2)intermediate layer,with a liquid silicon oxide(SiO_(2))layer on the surface that was displaced by the action of the gas flux towards the divergent part of the nozzle,protecting it from further *** specimens obtained by HP and SPS displayed similar performance,with very slight differences,which were attributed to small changes in *** tests demonstrated the capability of complex-shaped prototypes made of the developed UHTCMCs to survive repeated exposure to environments representative of a realistic space propulsion application,for overall operating time up to 30 s,without any failure nor measurable erosion,making a promising step towards the development of reusable rocket components.

Case Control Study of Post-endoscopic Variceal Ligation Bleeding Ulcers in Severe Liver disease: Outcomes and Management

Background and Aims:The management of post-endoscopic variceal ligation(EVL)bleeding ulcers(PEBUs)is currently based on local expertise and patients liver disease *** present retrospective study investigated associations between the endoscopic morphology of PEBUs and patient ***:Patients underwent EVL(primary or secondary),from January 2015 to January 2018,in two tertiary care hospitals in India(ILBS New delhi and dharamshila Narayana New delhi).Mortality rates were determined at post-EVL day five and week *** were typified based on Jamwal&Sarin classification system as follows:A,ulcer with active spurting;B,ulcer with ooze;C,ulcer base with visible vessel or clot;and d,clean or pigmented ***:Of 3854 EVL procedures,141(3.6%)patients developed PEBU,and 46/141(32.6%)suffered *** the former,the PEBU types A,B,C,and d accounted for 17.7,26.2,36.3,and 19.8%,*** those who died,39.1,30.4,21.7,and 8.8%had PEBU types A,B,C,and *** included transjugular intrahepatic portosystemic shunts(TIPS),esophageal self-expandable metal stent(SEMS),glue and sclerosant injection,Sengstaken-Blakemore tube placement and liver *** univariate analysis,no correlation with hepatic venous pressure gradient,TIPS placement,size of varices,or number of bands was *** Model for EndStage Liver disease(MELd)-sodium score correlated positively with *** adjusting for MELd-sodium score,mortality was best predicted by type-A ulcer(p=0.024;OR 8.95,CI 1.34-59.72).Conclusions:PEBU occurred in 3.6%of a large EVL *** patients based on PEBU type can help predict outcomes,independent of the MELd-sodium *** PEBUs by endoscopic morphology may inform treatment strategies,and warrants further validation.

提出了一种由层状前体合成单一晶相镁铁尖晶石的新方法,首先对Mg Fe(Ⅱ) Fe(Ⅲ)水滑石的制备进行了系统研究,成功合成了Mg2 +/Fe2 +/Fe3+摩尔比分别为 1/2/1,4/5/3,2/1/1的系列水滑石层状前体,结果表明在以上三种投料比下均可制备出晶型较好的水滑石层状前体,并探讨了合成条件对晶体结构的影响规律.在此基础上,利用X射线衍射、振动样品磁强计和穆斯堡尔谱等手段研究了层状前体焙烧产物的结构、组成、磁性及微观信息,研究表明当Mg2 +/Fe2 +/Fe3+投料摩尔比为 2/1/1时。

Even though solid waste management is widely researched,there is scanty literature addressing the influence of physical environmental attributes on solid waste management service provision in informal *** aim of this study was to investigate the nexus between physical infrastructural planning and waste management *** obtain relevant results,this study used the case study of Peleng township in *** study relied on empirical data collected from 91 ran-domly selected *** was used for mapping 550 purposively selected households,and for visualization of the physical *** study concluded that the community is at the forefront of dealing with waste problems in their ***,their interventions have fallen short of becoming profitable and sustainable due to a failure of the government to integrate solid waste management into mainstream urban development *** findings revealed that there is minimal government intervention for improved solid waste management in the *** recommends that adequate attention must be given to road infrastructure and physical developments in order solid waste management in Peleng,Lobatse.

Adaptive optimization on ultrasonic transmission tomography-based temperature image for biomedical treatment

Hyperthermia has proven to be beneficial to treating superficial malignancies, particularly chest wall recurrences of breast cancer. during hyperthermia, monitoring the time–temperature profiles in the target and surrounding areas is of great significance for the effect of therapy. An ultrasound-based temperature imaging method has advantages over other approaches. When the temperature around the tumor is calculated by using the propagation speed of ultrasound, there always exist overshoot artifacts along the boundary between different tissues. In this paper, we present a new method combined with empirical mode decomposition（EdM）, similarity constraint, and continuity constraint to optimize the temperature images. Simulation and phantom experiment results compared with those from our previously proposed method prove that the EMd-based method can build a better temperature field image, which can adaptively yield better temperature images with less computation for assistant medical treatment control.

COMPUTATION OF HOPF BRANCHES BIFURCATING FROM A HOPF/PITCHFORK POINT FOR PROBLEMS WITH Z_2-SYMMETRY

This paper is concerned with the computation of Hopf branches emanating from a Hopf/Pitchfork point in a two-parametor nonlinear problem satisfying a Z2symnletry condition. Our aim is to present a new al,proach to the theoretical and computational analysis of the bifurcating Hopf branches at this singular point by using the system designed to calculate Hopf points and exploring its symmetry. It is shown that a Hopf/Pitchfork point is a pitchfork bifurcation point in the *** standard continuation and branch-switching can be used to compute these Hopf branches. In addition, an effect method based on the extended system of the singular points is developed for the computation of branch of secondary (nonsymmetric) Hopf points. SHRIMP U-Pb dating for a K-bentonite bed in the Tieling Formation,North China

A SHRIMP U-Pb zircon age of 1437±21 Ma was obtained for a recently discovered K-bentonite bed in the Tieling Formation,situated northeast of Beijing at the boundary between Liaoning and Hebei provinces,on the northern margin of the North China Craton(NCC).The SHRIMP U-Pb age places Tieling Formation near the end of the Calymmian Period of the early Mesoproterozoic *** addition,a SHRIMP U-Pb zircon date of 1372±18 Ma was acquired for K-bentonite beds in the overlying,dark-shale-dominated Xiamaling Formation from the same location northeast of *** date assigns a similar Mid-Mesoproterozoic(Ectasian Period) age for the Xiamaling Formation,as have previously determined dates from other sections northwest of *** dates indicate that the Tieling and Xiamaling formations,as well as the related succession in the eastern part of the Yanshan Mountains,represented by the well-known Meso-to Neoproterozoic standard section in Jixian,can be correlated well with sections northwest of Beijing in the western part of the Yanshan *** other words,the boundary between the Calymmian and Ectasian periods in the northern parts of the NCC is marked by the unconformity between the Tieling and Xiamaling formations in the northern *** boundary was previously ascribed to the "Qinyu Orogeny" and thought to be of Grenville *** this regard,the conventional "Qinyu Orogeny" should now be regarded as a short-lived regional uplift during Early Mesoproterozoic time,rather than a result from the Grenvillian assembly of the NCC to the Rodinia Supercontinent(～1.0 Ga).
