Acute calculous cholecystitis(ACC) is the most frequent complication of cholelithiasis and represents one-third of all surgical emergency hospital admissions, many aspects of the disease are still a matter of debate. Knowledge of the current evidence may allow the surgical team to develop practical bedside decision-making strategies, aiming at a less demanding procedure and lower frequency of complications. In this regard, recommendations on the diagnosis supported by specific criteria and severity scores are being implemented, to prioritize patients eligible for urgency surgery. Laparoscopic cholecystectomy is the best treatment for ACC and the procedure should ideally be performed within 72h. Early surgery is associated with better results in comparison to delayed surgery. In addition, when to suspect associated common bile duct stones and how to treat them when found are still debated. The antimicrobial agents are indicated for high-risk patients and especially in the presence of gallbladder necrosis. The use of broad-spectrum antibiotics and in some cases with antifungal agents is related to better prognosis. Moreover, an emerging strategy of not converting to open, a difficult laparoscopic cholecystectomy and performing a subtotal cholecystectomy is recommended by adept surgical teams. Some authors support the use of percutaneous cholecystostomy as an alternative emergency treatment for acute Cholecystitis for patients with severe comorbidities.

The Phytochemical Profile and Photoprotective Potential of Bark and Leaves of Erythrina velutina Willd.—A Medicinal Species from the Caatinga Region of Brazil

The use of medicinal plants as therapeutic and economic resources has been on the rise in recent years. In Brazil, however, increasing doubt is being cast on the quality of these products, owing to the prevalence of adulteration and fraud. Solar radiation can cause serious damage to human skin, as a result, mostly, of ultraviolet light, which is a cause of skin cancer. Photoprotective substances are capable of absorbing, reflecting, or refracting ultraviolet radiation and thus protecting skin from exposure to sunlight. The present study aimed to characterize samples and examine the phytochemical profile and photoprotective potential of bark and leaves of Erythrina velutina Willd. The samples underwent five extraction methods using 80% ethanol. The phenolic content was measured using spectrophotometry. Antioxidant activity was examined using the dPPH and the photoprotective properties of the plant extracts were assessed using the method developed by Mansur. There was a quantitative difference in some groups of metabolites, with higher levels of tannin in the bark and of flavonoids in the leaves. The latter showed greater dPPH free radical scavenging capacity than the bark, although higher levels of SPF were obtained from the bark, with no statistically significant differences between methods. The results indicate that Erythrina velutina Willd. has potential as a photoprotector.

Hepatic encephalopathy(HE)is a cognitive disturbance characterized by neuropsychiatric *** occurs in acute and chronic hepatic disease and also in patients with portosystemic *** presence of these portosystemic shunts allows the passage of nitrogenous substances from the intestines through systemic veins without liver ***,the embolization of these shunts has been performed tocontrol HE manifestations,but the presence of portal vein thrombosis is considered a *** this presentation we show a cirrhotic patient with severe HE and portal vein thrombosis who was submitted to embolization of a large portosystemic *** report:a 57 years-old cirrhotic patient who had been hospitalized many times for persistent HE and hepatic coma,even without precipitant *** had a wide portosystemic shunt and also portal vein *** abdominal angiography confirmed the splenorenal shunt and showed other *** larger shunt was embolized through placement of microcoils,and the patient had no recurrence of overt *** was a little increase of esophageal and gastric varices,but no endoscopic treatment was *** portosystemic shunts are frequent causes of recurrent HE in cirrhotic patients,portal vein thrombosis should be considered a relative contraindication to perform a shunt ***,in particular cases with many shunts and severe HE,we found that one of these shunts can be safely embolized and this procedure can be sufficient to obtain a good HE *** conclusion,we reported a case of persistent HE due to a wide portosystemic shunt associated with portal vein *** the patient had other shunts,she was successfully treated by embolization of the larger shunt.

Prognostic Role of Preoperative Tricuspid Annular Plane Systolic Excursion (TAPSE) in Mitral Valve Replacement (MVR) for Rheumatic Mitral Stenosis Patients

Tricuspid annular plane systolic excursion has been proposed as a simple and reproducible parameter for quantitative assessment of the right ventricular ejection fraction. The prognostic importance of preoperative TAPSE in patients with mitral valve replacement for rheumatic mitral stenosis patients is still under focused. Therefore, the objective of the study was to predict the outcome after MVR in rheumatic mitral stenosis patients in relation to preoperative TAPSE. This comparative cross-sectional study was conducted at the department of Cardiac Surgery, National Heart Foundation Hospital and Research Institute. A total of 72 patients of rheumatic mitral stenosis patients who underwent mitral valve replacement were included in the study. They were divided into two groups: Group A and B. Group A included 36 patients with TAPSE 0.05) except for the preoperative TAPSE. Mean TAPSE of Group A was 13.17 (±1.40) and Group B was 18.61 (±1.57), the difference was statistically significant (p 0.05). Among the postoperative complications, including postoperative atrial fibrillation was higher in Group A (30.56%) than Group B (11.11%), mean ventilation time was higher in Group A (27.78%) than Group B (5.56%), length of intensive care was higher in Group A (33.33%) than Group B (11.12%), and hospital stay was higher in Group A (25.0%) than Group B (5.56%), (p d be used as a prognostic tool for MVR in rheumatic mitral stenosis patients in our settings.

Intake,digestibility and nitrogen utilization in cattle fed tropical forage and supplemented with protein in the rumen,abomasum,or both

Background： There is little information in the tropics with regard the comparative understanding of how an increased nitrogen supply in the rumen or in the intestines affects efficiency of nitrogen utilization in cattle. This study evaluated the effects of supplementation with nitrogenous compounds in the rumen, abomasum, or both on intake, digestibility and the characteristics of nitrogen utilization in cattle fed tropical forage. Four rumen- and abomasum-fistulated Nellore bulls （227 ± 11 kg） were used. Four treatments were evaluated： control, ruminal supplementation （230 g/d of supplemental protein in the rumen）, abomasal supplementation （230 g/d of supplemental protein in the abomasum）, and ruminal and abomasal supplementation （115 g/d protein in both the rumen and the abomasum）. The basal forage diet consisted of Tifton 85 hay with a crude protein （CP） level of 78.4 g/kg dry matter. Casein was used as a supplement. The experiment was conducted using a 4 x 4 Latin square. Results： There were no differences between the treatments （P 〉 0.10） with regard to forage intake. The intake and tota digestibility of CP increased （P 〈 0.01） with supplementation. The nitrogen balance in the body increased （P 〈 0.01） and muscle protein mobilization decreased （P 〈 0.01） with supplementation, regardless of the supplementation site. The efficiency of nitrogen utilization did not differ among the treatments （P 〉 0.10）. Conclusions： The supplementation of cattle fed tropical forage with protein in the rumen, abomasum, or both similarly increased the nitrogen accretion in animal, which reflects improvements on nitrogen status in animal body.

A geomechanics classification for the rating of railroad subgrade performance

Leishmaniasis is an infectious disease that affects both animals and humans, caused by flagellated parasites belonging to the genus Leishmania. The disease is estimated to reach about 700,000 to 1 million people, causing the deaths of 20 to 30,000 individuals annually. Thus, the present study aims to perform molecular docking tests and evaluation of antileishmania activity in vitro of a ruthenium complex with epiisopiloturine and nitric oxide. AutodockTools-1.5.6 software was used to perform molecular docking tests. Molecular targets were considered rigid, and Epiruno2 considered flexible. The genetic algorithm Lamarckian (AGL) with global search and pseudo-Solis and Wets with local search were the methods adopted in the docking. The most promising results of molecular interaction were achieved in the targets Pteridine reductase and UdP-glucose Pyrophosphorylase with rates of -10.68 Kcal·mol-1 and -10.51 Kcal·mol-1, respectively. This demonstrates that Epiruno2 has molecular affinity with the targets of L. major. In vitro assays prove the antileishmania activity of the complex in the face of promastigote forms with inhibition of growth, concluding through this study that the Epiruno2 complex has antileishmania activity.

Quality of Service to Users of Primary Health Care:A Study of Evaluation

The quality of service to people in Primary Healthcare (PHC) is linked to the provision of quality service by professionals who integrate this service in the community. Accordingly, this study aims to evaluate the service given by health professionals in primary health care, from the user’s perception. This is an evaluative and quantitative study, conducted with people who use the PHC service. 180 adults are interviewed in the city of Santa Cruz, Rio Grande do Norte, Brazil. The results show that most respondents are women. The average age is of 36.65, median of 34.00 and standard deviation of 11,554. With regards to the quality of service, it ranges from fair to good. When performing correlation between the quality of service and time spent to reach the service, most people say that they spend between 0 and 15 minutes from their house to the healthcare service unit. About reception, respondents reported being satisfied and that the service has been responsible and quality for health monitoring. Thus, it is noted that the user classifies the quality of the service in primary healthcare as regular, showing weaknesses that need to be corrected to prevent damage in service provision in Primary Healthcare.
