Utilization of HIv and Sexually Transmitted Infections Prevention Services, and Associated Factors among the Long-Distance Truck Drivers along the Northern Corridor Highway, Kenya

Background: Often, long-distance truck drivers’ (LDTDs’) work predisposes them to sexually transmitted infections (STIs) whose outcomes are influenced by access and behavior of seeking sexual health care. Methodology: In this study, we assessed the utilization of HIV/STI preventive services and associated factors among 296 LDTDs operating along the northern corridor highway using an interviewer-administered questionnaire for data collection at Mlolongo stopover in Machakos, Kenya. Responses for the investigated variables, including condom use, history of HIV testing, frequency of HIV testing, antiretroviral therapy (ART) use and follow-up for the HIV positive and STI treatment, were assigned a score of either 1 or 0 depending on the question’s dimension. Following summing up for each participant, we computed a weighted score ranging between 0 and 1 by dividing the summed responses by the number of eligible variables. We arbitrarily multiplied these scores by 8 to generate endpoint scores ranging from one to eight for each participant to help create a dichotomized outcome variable for utilization levels: limited utilization (1 to 4) and good utilization (5 to 8). Association between certain independent variables and the outcome variable (level of utilization of H.I.V./STIs preventive services) was analyzed using binomial logistic regression analysis in R statistical software. Results: The mean age of the LDTDs was 38.4 years, ranging from 24 - 57 years. The majority (n = 287, 97%) of the LDTDs had been tested on HIv at least once since the beginning of their career. An improved surface-plasmonic nanobeam cavity for higher Q and smaller v

We demonstrate a high-Q hybrid surface-plasmon-polariton-photonic crystal(SP3C) nanobeam *** proposed cavities are analyzed numerically using the three-dimensional finite difference time domain(3D-FDTD) *** results show that a Q-factor of 2076 and a modal volume v of 0.16(/2n) 3 can be achieved in a 50 nm silica-gap hybrid SP3C nanobeam cavity when it operates at telecommunications wavelengths and at room temperature.v can be further reduced to 0.02(/2n) 3 when the silica thickness decreases to 10 nm,which leads to a Q/v ratio that is 11 times that of the corresponding plasmonic-photonic nanobeam cavity(without silica).The ultrahigh Q/v ratio originates from the low-loss nature and deep sub-wavelength confinement of the hybrid plasmonic waveguide,as well as the mode gap effect used to reduce the radiation *** proposed structure is fully compatible with semiconductor fabrication techniques and could lead to a wide range of applications.

Concurrent use of aromatase inhibitors and hypofractionated radiation therapy

AIM: To retrospectively assess the acute and long-term toxicity using aromatase inhibitors (AI) therapy concurrently with hypofractionated radiotherapy (HFRT) in breast cancer patients. METHODS: From November 1999 to October 2007, 66 patients were treated with breast HFRT and concurrent AI. In 63 patients (95.5%), HFRT delivered a total dose of 32.5 Gy to the whole breast within 5 wk (five fractions, one fraction per week). Other fractionations were chosen in three patients for the patients' personal convenience. A subsequent boost to the tumor bed was delivered in 35 patients (53.0%). Acute toxicities were scored according to the Common Toxicity Criteria for Adverse Events v3. Late toxicity was defined as any toxicity occurring more than 6 mo after completion of HFRT and was scored according to the Late Effects Normal Tissue Task Force-Subjective, Objective, Management and Analytic scale. RESULTS: At the end of the HFRT course, 19 patients (28.8%) had no irradiation-related toxicity. Acute grade 1-2 epithelitis was observed in 46 patients (69.7%). One grade 3 toxicity (1.5%) was observed. With a median follow-up of 34 mo (range: 12-94 mo), 31 patients (47%) had no toxicity, and 35 patients (53%) presented with grade 1-2 fibrosis. No grade 3 or greater delayed toxicity was observed. CONCLUSION: We found that AI was well tolerated when given concurrently with HFRT. All toxicities were mild to moderate, and no treatment disruption was necessary. Further prospective assessment is warranted.

volumetric modulated arc radiotherapy for limited osteosclerotic myeloma

AIM:To assess the feasibility of volumetric intensity-modulated arc radiotherapy (vMAT) in patients with limited polyneuropathy, organomegaly, endocrinopathy, monoclonal gammopathy, and skin changes syndrome. METHODS:A 70-year-old male with histologically confirmed osteosclerotic myeloma was treated in our department in July 2010 with vMAT. Fourty-six Gray in 23 fractions were given on three bone lesions. Doses delivered to target volume and critical organs were compared with a tridimensional conformal radiotherapy (3D-RT) plan. Treatment was well tolerated without any side ***:vMAT improved dose homogeneity within the target volume, as compared to 3D-RT (standard deviations:2.9 Gy and 1.6 Gy for 3D and vMAT, respectively). vMAT resulted in a better sparing of critical organs. Dose delivered to 20% of organ volume (D20) was reduced from 22 Gy (3D-RT) to 15 Gy (vMAT) for small bowel, from 24 Gy (3D-RT) to 17 Gy (vMAT) for bladder and from 47 Gy (3D-RT) to 3 Gy (vMAT) for spinal cord. volumes of critical organs that received at least 20 Gy (v20) were decreased by the use of vMAT, as compared to 3D-RT (v20 bladder:10% vs 99%; v20 small bowel:6% vs 21%). One year after treatment completion, no tumor progression has been reported. CONCLUSION:vMAT improved dose distribution as compared to 3D-RT for limited osteosclerotic myeloma, with better saving of critical organs.
