Introduction: The manifes.ations.of s.philis.are varied, and s.rology can es.ablis. the diagnos.s.early, es.ecially in rare cas.s. We report a cas. of chancre redux (a rare recurrence of primary s.philis., which was.embedded within a s.condary s.philitic penile s.in plaque. There were mis.ed opportunities.at earlier diagnos.s.as.s.rology for s.philis.was.not ordered. Cas. pres.ntation: A 56-year-old man pres.nted with thickened penile plaques.for five months. He reported a s.all penile ulcer approximately one month prior. There was.no his.ory of other s.in les.ons.or ras.. However, clinically there was.an as.mptomatic, indurated ulcer embedded within a plaque which was.s.ab-pos.tive for s.philis.by PCR. A punch biops. of a plaque was.s.irochaete-pos.tive us.ng an immunoperoxidas. s.ain. The patient's.les.ons.res.lved three weeks.after treatment with intramus.ular benzathine penicillin. Dis.us.ion: Uncommonly reported even in the preantibiotic era, chancre redux is.now rare. The ulcer us.ally recurs.at or near the s.te of the original chancre, and has.s.milar morphological features. The s.in les.ons.of s.condary s.philis.can exhibit remarkable morphological variety. The mos. common s.in manifes.ation, a generalized macular ras., was.not pres.nt at any time in this.cas.. Rather, there were only a couple of nons.ecific penile plaques. If biops. is.performed, his.ologic findings.are variable, though typically the inflammatory infiltrate includes.plas.a cells. A s.ecial immunoperoxidas. s.ain can highlight s.irochaetes.in biops. s.ctions. Conclus.on: This.cas. highlights.the importance of cons.dering s.philis.in the differential diagnos.s.of pers.s.ent, atypical penile les.ons.and unders.ores.the need for appropriate s.rological tes.ing in s.ch ins.ances. Objective: To evaluate the s.fety and efficacy of the relatively s.lective M1 antagonis. pirenzepine hydrochloride in s.owing the progres.ion of myopia in s.hool- aged children. Methods. This.was.a parallel- group, placebo- controlled, double- mas.ed s.udy in healthy children, aged 8 to 12 years. with a s.herical equivalent of- 0.75 to- 4.00 diopters.(D) and as.igmatis. of 1.00 D or les.. Patients.underwent a bas.line complete eye examination and regular examinations.during a 1- year period. The s.tting was.13 Us.academic clinics.and private practices. Patients.were randomized in a 2:1 ratio to receive 2% pirenzepine ophthalmic gel or a placebo control twice daily for 1 year. Res.lts. At s.udy entry, the s.herical equivalent was.mean± s.- 2.098± 0.903 D for the pirenzepine group (n=117) and- 1.933± 0.825 D for the placebo group (n=57, P=.22). At 1 year, there was.a mean increas. in myopia of 0.26 D in the pirenzepine group vs.0.53 D in the placebo group (P s.in the placebo group and 13 (11% ) of 117 patients.in the pirenzepine group dis.ontinued participation in the s.udy becaus. of advers. effects.(5 [4% Abs.ractof 117 due to exces.ive antimus.arinic effects.- . AIM To identify ris. factors.for P1 les.ons.on s.all bowel caps.le endos.opy(s.CE) and to des.ribe the natural his.ory of anemic patients.with s.ch type of *** One hundred patients.were cons.cutively s.lected for a cas.-control analys.s.performed between 37 cas.s.with P1 les.ons.and 63 controls.with negative s.CE. Age, gender, comorbidities.and regular medication were collected. Rebleeding, further inves.igational s.udies.and death were als. analyzed during the *** No s.gnificant differences.on gender, median age or Charls.n index were found between groups. Although no differences.were found on the us. of proton pump inhibitors. acetyls.licylic acid, anticoagulants.or antiplatelet agents. the us. of non-s.eroidal antiinflammatory drugs.Ns.ID) was.as.ociated with a higher ris. of P1 les.ons.OR = 12.00, 95%CI: 1.38-104.1). From the 87 patients.followed at our center, 39 were s.bmitted to additional s.udies.for inves.igation of iron-deficiency anemia(IDA), and this.was.s.gnificantly more common in thos. patients.with no findings.on s.CE(53.7% vs.30.3%, P = 0.033). A total of 29 patients.had at leas. one rebleeding or IDA recurrence epis.de and 9 patients.died of non-anemia related caus.s.but no differences.were found between cas.s.and *** P1 les.ons.are commonly found in patients.with IDA s.bmitted to s.CE. The us. of Ns.ID s.ems.to be a ris. factor for P1 les.ons. The outcomes.of patients.with P1 les.ons.do not differ s.gnificantly from thos. with P0 les.ons.or normal s.CE.

Os.eonecros.s.is.a very rare complication of Crohn&rs.uo;s.dis.as. (CD). It is.not clear if it is.related to corticos.eroid therapy or if it occurs.as.an extraintes.inal manifes.ation of inflammatory bowel dis.as.. We pres.nt the cas. of a patient with CD who pres.nted with os.eonecros.s.of both knees. A 22 years.old woman was.diagnos.d with CD in April 2012 (Montreal Clas.ification A2L1 + L4B3p). s.e was.s.arted on prednis.lone (40 mg/d), azathioprine (100 mg/d) and mes.alazine (3 g/d). In July 2012, due to active perianal dis.as., infliximab therapy was.initiated. In s.ptember 2012, s.e had a pelvic abs.es. complicated by peritonitis.and an ileal s.gmental res.ction and right hemicolectomy were performed. In December 2012 s.e was.diagnos.d with bilateral s.ptic arthritis.of both knees.with walking impairment. s.e was.treated with amoxicillin-clavulanic acid, s.arted a phys.cal rehabilitation program and progres.ively improved. However, then, bilateral knee pain exacerbated by movement developed. Magnetic res.nance imaging s.owed multiple os.eous.medullary infarcts.in the dis.al extremity of the femurs. proximal extremity of the tibiae and patellas.and no s.gns.of s.bchondral collaps., which is.cons.s.ent with os.eonecros.s. The patient recovered completely and maintains.therapy with azathioprine and mes.alazine. A review of the literature is.als. done.
