Molecular aspects of carcinogenesis in pancreatic cancer

BACKGROUND:Pancreatic cancer(PCa)is one of the most aggressive human solid tumors,with rapid growth and metastatic spread as well as resistance to chemotherapeutic drugs,leading rapidly to virtually incurable *** the last 20 years,however,significant advances have been made in our understanding of the molecular biology of PCa,with a focus on the cytogenetic abnormalities in PCa cell growth and differentiation. DATA SOURCES:A MEDLINE search and manual cross- referencing were utilized to identify published data for PCa molecular biology studies between 1986 and 2008, with emphasis on genetic alterations and developmental oncology. RESULTS:Activation of oncogenes,deregulation of tumor suppressor and genome maintenance genes,upregulation of growth factors/growth factor receptor signaling cascade systems,and alterations in cytokine expression,have been reported to play important roles in the process of pancreatic *** in the K-ras proto- oncogene and the p16INK4a,p53,FHIT,and DPC4 tumor suppressor genes occur in a high percentage of tumors. Furthermore,a variety of growth factors are expressed at increased *** addition,PCa often exhibits alterations in growth inhibitory pathways and evades apoptosis through p53 mutations and aberrant expression of apoptosis-regulating genes,such as members of the Bcl *** pathways in the development of an aggressive phenotype,local infiltration and metastasis are still under ongoing genetic *** present paper reviews recent studies on the pathogenesis of PCa,and includes a brief reference to alterations reported for other types of pancreatic tumor. CONCLUSIONS:Advances in molecular genetics and biology have improved our perception of the pathogenesis of ***,further studies are needed to better understand the fundamental changes that occur in PCa,thus leading to better diagnostic and therapeutic management.

The treatment of patients with inflammatory bowel disease(IBD),especially those with severe or refractory disease,represents an important challenge for the clinical *** seems to be no exaggeration to say that in these patients,not only the scientific background of the gastroenterologist is tested,but also the abundance of“gifts”that he should possess(insight,intuition,determ-ination,ability to take initiative,etc.)for the successful outcome of the *** daily clinical practice,depending on the severity of the attack,IBD is treated with one or a combination of two or more pharmaceutical *** combin-ations include not only the first-line drugs(e.g.,mesalazine,corticosteroids,antibiotics,etc)but also second-and third-line drugs(immunosuppressants and biologic agents).It is a fact that despite the significant therapeutic advances there is still a significant percentage of patients who do not satisfactorily respond to the treatment ***,a part of these patients are going to *** recent years,several small-size clinical studies,reviews,and case reports have been published combining not only biological agents with other drugs(e.g.,immunosuppressants or corticosteroids)but also the combination of two biologi-cal agents simultaneously,especially in severe *** our opinion,it is at least a strange(and largely unexplained)fact that we often use combinations of drugs in a given patient although studies comparing the simultaneous administration of two or more drugs with monotherapy are very *** mentioned above,there is a timid tendency in the literature to combine two biological agents in severe cases unresponsive to the applied treatment or patients with severe extraintestinal *** appropriate dosage,the duration of the administration,the suitable timing for checking the clinical and laboratory outcome,as well as the treatment side-effects,should be the subject of intense clinical research *** this

Mechanical performance of 22SiMn2TiB steel welded with lowtransformation- temperature filler wire and stainless steel filler wire

TX-80 low-transformation-temperature(LTT)welding wire was used to replace the traditional ER 307Si welding wire to realize the connection of 22SiMn2TiB armor steel in manual overlay *** previously existing issues,such as welding cracks,large welding deformation,and severe welding residual stress,were solved to ensure good strength and ductility *** particular,with the same welding conditions,TX-80 LTT wire eliminates welding *** reduces the welding deformation no matter the base pretreatment of pre-setting angle or no pre-setting *** comparison,it was found that the microstructure at the TX-80 weld is mainly composed of martensite and a small amount of retained *** contrast,the microstructure of the ER 307Si weld consists of a large amount of austenite and a small amount of skeleton-like *** variation trend of residual stress and microhardness from the weld to the base were investigated and compared with the mechanical properties of base *** TX-80 and the ER 307Si tensile samples elongation is 6.76%and 6.01%,while the ultimate tensile strengths are 877 and 667 MPa,*** average impact toughness at room temperature of the ER 307Si weld is 143.9 J/cm^(2),much higher than that of the TX-80 weld,which is only 36.7 J/cm^(2).The relationship between impact and tensile properties with microstructure species and distribution was *** addition,the fracture surface of the tensile and the impact samples was observed and *** dimples,fewer pores,larger radiation zone,and shear lips of TX-80 samples indicate better tensile ductility and worse impact toughness than those of ER 307Si weld.

The most prominent risk assessment techniques are founded on the values of measuring and controlling the frequency and the consequences of risks in order to assure an“acceptable level”of“safeness”mainly in the lines of environmental,health and hygiene and port product *** paper examines security risk assessment approaches within the emerging role of *** paper contributes to the current literature by considering the ports of Greece as a case in point and by measuring the degree of its security risk orientation based on certain valid risk factors drawn from the current ***,it presents a security risk assessment methodology into the domain of port container *** potential for ports were quantitatively and qualitatively assessed by discussing issues of security approaches within the maritime industry,in order to facilitate improvement strategies.A two-dimension empirical study was conducted,in a time range of ten years(2010-2020)in order to provide evidence regarding security risk assessment in the port container terminal of Thessaloniki,in *** findings of this study have significant strategic policy implications and shed more light on the role of security risks in the overall risk orientation of container terminals in ***,further research directions in security risk in ports are proposed.

Nowcasting and forecasting solar irradiance are vital for the optimal prediction of grid-connected solar photovoltaic(PV)power *** plants face operational challenges and scheduling dispatch difficulties due to the fluctuating nature of their power *** the generation capacity within the electric grid increases,accurately predicting this output becomes increasingly essential,especially given the random and non-linear characteristics of solar irradiance under variable weather *** study presents a novel prediction method for solar irradiance,which is directly in correlation with PV power output,targeting both short-term and medium-term forecast *** proposed hybrid framework employs a fast trainable statistical learning technique based on the truncated-regularized kernel ridge regression *** proposed method excels in forecasting solar irradiance,especially during highly intermittent weather periods.A key strength of our model is the incorporation of multiple historical weather parameters as inputs to generate accurate predictions of future solar irradiance values in its scalable *** evaluated the performance of our model using data sets from both cloudy and sunny days in Seattle and Medford,USA and compared it against three forecasting models:persistence,modified 24-hour persistence and least *** on three widely accepted statistical performance metrics(root mean squared error,mean absolute error and coefficient of determination),our hybrid model demonstrated superior predictive accuracy in varying weather conditions and forecast horizons.

Percutaneous antegrade management of large proximal ureteral stones using non-papillary puncture

Artificial Intelligence and Computer Vision during Surgery: Discussing Laparoscopic Images with ChatGPT4—Preliminary Results

Introduction: Ultrafast latest developments in artificial intelligence (ΑΙ) have recently multiplied concerns regarding the future of robotic autonomy in surgery. However, the literature on the topic is still scarce. Aim: To test a novel AI commercially available tool for image analysis on a series of laparoscopic scenes. Methods: The research tools included OPENAI CHATGPT 4.0 with its corresponding image recognition plugin which was fed with a list of 100 laparoscopic selected snapshots from common surgical procedures. In order to score reliability of received responses from image-recognition bot, two corresponding scales were developed ranging from 0 - 5. The set of images was divided into two groups: unlabeled (Group A) and labeled (Group B), and according to the type of surgical procedure or image resolution. Results: AI was able to recognize correctly the context of surgical-related images in 97% of its reports. For the labeled surgical pictures, the image-processing bot scored 3.95/5 (79%), whilst for the unlabeled, it scored 2.905/5 (58.1%). Phases of the procedure were commented in detail, after all successful interpretations. With rates 4 - 5/5, the chatbot was able to talk in detail about the indications, contraindications, stages, instrumentation, complications and outcome rates of the operation discussed. Conclusion: Interaction between surgeon and chatbot appears to be an interesting frontend for further research by clinicians in parallel with evolution of its complex underlying infrastructure. In this early phase of using artificial intelligence for image recognition in surgery, no safe conclusions can be drawn by small cohorts with commercially available software. Further development of medically-oriented AI software and clinical world awareness are expected to bring fruitful information on the topic in the years to come.

An alternative surgical approach to a difficult case of Mirizzi syndrome: A case report and review of the literature

Mirizzi syndrome (MS) is an uncommon complication of gallstone disease and occurs in approximately 1% of all patients suffering from cholelithiasis. The syndrome is characterized by extrinsic compression of the common hepatic duct frequently resulting in clinical presentation of intermittent or constant jaundice. Most cases are not identifi ed preoperatively. Surgery is the indicated treat- ment for patients with MS. We report here a 71-year- old male patient referred to the surgical outpatient department for diffuse upper abdominal pain and mild jaundice (bilirubin rate: 4.2 mg/dL). Ultrasound examina- tion revealed a stone in the cystic duct compressing the common hepatic duct. The patient had a history of gas- trectomy for gastric ulcer 30 years ago. MRCP revealed a stone impacted in the cystic duct causing obstruction of the common hepatic duct by extrinsic compression. With these fi ndings the preoperative diagnosis was indicative of MS. At laparotomy a moderately shrunken gallbladder was found embedded in adhesions containing a large stone which was palpable in the common bile duct. The anterior wall of the body of the gallbladder was opened by an incision which extended longitudinally along the gallbladder towards the common bile duct. The stone measuring 3.0 cm in diameter, was then removed set- ting astride a large communication with the common bile duct. A Roux-en-Y cholecysto-choledocho-jejunostomy was performed. The subhepatic region was drained. The patient had an uneventful recovery. He was discharged eleven days after operation and remained well after a 30-mo follow-up.
