Analysing vaccine efficacy evaluated in phase 3 clinical trials carried out during outbreaks

In this paper we examine several definitions of vaccine efficacy(VE)that we found in the literature,for diseases that express themselves in outbreaks,that is,when the force of infection grows in time,reaches a maximum and then *** fact that the disease occurs in outbreaks results in several problems that we *** propose a mathematical model that allows the calculation of VE for several *** trials usually needs a large number of volunteers that must be ***,all volunteers should be enrolled in approximately the same time,but this is generally impossible for logistic reasons and they are enrolled in a fashion that can be replaced by a continuous density function(for example,a Gaussian function).The outbreak can also be replaced by a continuous density function,and the use of these density functions simplifies the ***,for example Gaussian functions,one of the problems one can immediately notice is that the peak of the two curves do not occur at the same *** model allows us to conclude:First,the calculated vaccine efficacy decreases when the force of infection increases;Second,the calculated vaccine efficacy decreases when the gap between the peak in the force of infection and the peak in the enrollment rate increases;Third,different trial protocols can be simulated with this model;different vaccine efficacy definitions can be calculated and in our simulations,all result are approximately the *** final,and perhaps most important conclusion of our model,is that vaccine efficacy calculated during outbreaks must be carefully examined and the best way we can suggest to overcome this problem is to stratify the enrolled volunteer's in a cohort-by-cohort basis and do the survival analysis for each cohort,or apply the Cox proportional hazards model for each cohort.

Charge density wave(CDW) in kagome materials with the geometric frustration is able to carry unconventional ***, a CDW has been observed below the antiferromagnetic order in kagome FeGe, in which magnetism and CDW are intertwined to form an emergent quantum ground state. However, the CDW is only short-ranged and the structural modulation originating from it has yet to be determined experimentally. Here we realize a long-range CDW order by post-annealing process,and resolve the structure model through single crystal X-ray diffraction. Occupational disorder of Ge resulting from short-range CDW correlations above T_(CDW) is identified from structure refinements. The partial dimerization of Ge along the c axis is unveiled to be the dominant distortion for the CDW. Occupational disorder of Ge is also proved to exist in the CDW phase due to the random selection of partially dimerized Ge sites. Our work provides useful insights for understanding the unconventional nature of the CDW in FeGe.

Objective Detection and quantification of rNA synthesis in cells is a widely used technique for monitoring cell viability,health,and metabolic *** exposure to environmental stimuli,both the internal reference gene and target gene would be *** a result,it is imperative to consider the accurate capture of nascent rNA and the detection of transcriptional levels of rNA following environmental *** study aims to create a Click Chemistry method that utilizes its property to capture nascent rNA from total rNA that was stimulated by the *** The new rNA was labeled with 5-ethyluridine(5-EU)instead of uracil,and the azido-biotin medium ligand was connected to the magnetic sphere using a combination of“Click Chemistry”and magnetic bead *** the new rNA was captured and the transcription rate of 16S rrNA was detected by fluorescence molecular beacon(M.B.)and quantitative reverse transcription PCr(qrT-PCr).results The bacterial nascent rNA captured by“Click Chemistry”screening can be used as a reverse transcription template to form *** with the fluorescent molecular beacon M.B.1,the synthesis rate of rrNA at 37℃is 1.2 times higher than that at 15℃.The 16S rrNA gene and cspI gene can be detected by fluorescent quantitative PCr,it was found that the measured relative gene expression changes were significantly enhanced at 25℃and 16℃when analyzed with nascent rNA rather than total rNA,enabling accurate detection of rNA transcription *** Compared to other article reported experimental methods that utilize screening magnetic columns,the technical scheme employed in this study is more suitable for bacteria,and the operation steps are simple and easy to implement,making it an effective rNA capture method for researchers.

Body Contour in the Post-Bariatric Male Patient

Background: Obesity is currently considered a public health problem. Bariatric procedures have become an important part of obesity management and, consequently, the number of male patients seeking post-bariatric reconstructive procedures have increased. Therefore, the clinical approach and understanding of the body contour of this population have become more relevant. The goal of post-bariatric reconstruction is to enhance the male silhouette through removal of skin and adipose tissue excess, and abdominal rectus diastasis repair. Material and Methods: We conducted a retrospective study at the Plastic and reconstructive Surgery Department of the National Medical Center “20 de Noviembre”. All male patients referred to our department to start a post-bariatric reconstruction protocol from January 2018 to December 2022 were included in this study. results: In total, 15 patients who underwent corporal contouring procedures were included;median age was 49.2 years with minimum of 33 years, and a maximum of 57 years. Median Body mass index was 28.4 kg/m2 with minimum of 22 kg/m2 and maximum of 38 kg/m2. All patients were treated 18 months after their bariatric surgery. All patients underwent an abdominoplasty as a body contouring procedure. 4 (26.7%) patients presented complications related to the surgery. Conclusion: We described a comprehensive and systematic approach to massive weight loss for male patients, suggesting an abdominal marking based on the patient’s clinical features and the expected results avoiding feminization of the abdominal body contour.

The investigation of the interplay between geometry and nonlinearity may open the road to the control of extreme waves. We study three-dimensional localization and dispersive shocks in a bent cigar shaped potential by the nonlinear Schro¨ dinger equation. At high bending and high nonlinearity, topological trapping is frustrated by the generation of curved wave-breaking. Four-dimensional parallel simulations confirm the theoretical model. This work may contribute to novel devices based on geometrically constrained highly nonlinear dynamics and tests and analogs of fundamental physical theories in curved space.

Linear and Nonlinear Anderson Localization in a Curved Potential

Disorder induced localization in the presence of nonlinearity and curvature is investigated. The time-resolved three-dimensional expansion of a wave packet in a bent cigar shaped potential with a focusing Kerr-like interaction term and Gaussian disorder is numerically analyzed. A self-consistent analytical theory, in which randomness, nonlinearity and geometry are determined by a single scaling parameter, is reported, and it is shown that curvature enhances localization.

The study of animal personality has attracted considerable attention, as it has revealed a number of similarities in personality between humans and several nonhuman species. At the same time the adaptive value and evolutionary maintenance of different personalities are the subject of debate. Since Pavlov＇s work on dogs, students of comparative cognition have been aware that animals display vast individual differences on cognitive tasks, and that these differences may not be entirely accounted for differences in cognitive abilities. Here, we argue that personality is an important source of variation that may affect cognitive performance and we hypothesise mutual influences between personality and cognition across an individual＇s lifespan. In particular, we suggest that： 1） personality profiles may be markers of different cognitive styles; 2） success or failure in cognitive tasks could affect different personalities differently; 3） ontogenetic changes of personality profiles could be reflected in changes in cognitive performance. The study of such interplay has implications in animal welfare as well as in neuroscience and in translational medicine [Current Zoology 57 （4）： 491-498, 2011].

A VSC-based Model for Power Flow Assessment of Multi-terminal VSC-HVDC Transmission Systems

This paper puts forward a new practical voltage source converter(VSC)based AC-DC converter model suitable for conducting power flow assessment of multi-terminal VSCbased high-voltage direct current(VSC-MTDC)*** model uses an advanced method to handle the operational limits and control modes of VSCs into the power flow *** new model is incorporated into a unified framework encompassing AC and DC power grids and is solved by using the Newton-raphson method to enable quadratically convergent iterative *** use of complementarity constraints,together with the Fischer-Burmeister function,is proposed to enable the seamless incorporation of operational control modes of VSC and automatic enforcement of any converter’s operational limits that become violated during the iterative solution ***,a dedicated process for checking limits is no longer ***,all existing relationships between the VSC control laws and their operational limits are considered directly during the solution of the power flow *** applicability of the new model is demonstrated with numerical examples using various multi-terminal AC-DC transmission networks,one of which is a utility-sized power system.

Crucial steps in the natural history of inflammatory bowel disease

Inflammatory bowel diseases(IBD),including ulcerative colitis(UC) and Crohn's disease(CD),are chronic,progressive and disabling *** the last few decades,new therapeutic approaches have been introduced which have led not only to a reduction in the mortality rate but also offered the possibility of a favorable modification in the natural history of *** identification of clinical,genetic and serological prognostic factors has permitted a better stratification of the disease,thus allowing the opportunity to indicate the most appropriate *** treatment with immunosuppressive drugs and biologics has offered the opportunity to change,at least in the short term,the course of the disease by reducing,in a subset of patients with IBD,hospitalization and the need for *** this review,the crucial steps in the natural history of both UC and CD will be discussed,as well as the factors that may change their clinical *** methodological requirements for high quality studies on the course and prognosis of IBD,the true impact of environmental and dietary factors on the clinical course of IBD,the clinical,serological and genetic predictors of the IBD course(in particular,which of these are rel-evant and appropriate for use in clinical practice),the impact of the various forms of medical treatment on the IBD complication rate,the role of surgery for IBD in the biologic era,the true magnitude of risk of colorectal cancer associated with IBD,as well as the mortality rate related to IBD will be stressed;all topics that are extensively discussed in separate reviews included in this issue of World Journal of Gastroenterology.
