Over the last few years a num.er of large acupuncture trials have been carried out in western countries. The following article draws on the experience from.these recent large-scale trials on acupuncture to outline the way random.zed trials could be used to answer questions on efficacy, effectiveness and efficiency. It will provide guidance, firstly on the underlying concepts of both efficacy and effectiveness and secondly on designing both types of trials. In addition, the controversy over specific and non-specific effects of acupuncture, em.rging from.the results of the above-m.ntioned trials, will be highlighted. Suggestions for future clinical research on acupuncture include： greater reflection on the com.lex approach of Chinese m.dicine, and transparent and detailed reporting according to CONSORT and STRICTA guidelines. The current data on acupuncture point-specific effects do indeed have relevance, however for valid decision-m.king on acupuncture, further clinical trials on effectiveness and cost-effectiveness are required to provide realistic benefit estim.tes for future health care.

Com.lex alum.num.hydrides have been widely studied as potential hydrogen storage m.terials but also,for som. tim. now, for electrochem.cal applications. This review sum.arizes the crystal structures of alkali and alkaline earth alum.num.hydrides and correlates structure properties with physical and chem.cal properties of the hydride com.ounds. The crystal structures of the alkali m.tal alum.num.hydrides change significantly during the stepwise dehydrogenation. The general pathway follows a transform.tion of structures built of isolated [AlH4]- tetrahedra to structures built of isolated [Al H6]3- *** crystal structure relations in the group of alkaline earth m.tal alum.num.hydrides are m.ch m.re com.licated than those of the alkali m.tal alum.num.hydrides. The structures of the alkaline earth m.tal alum.num.hydrides consist of isolated tetrahedra but the interm.diate structures exhibit chains of cornershared octahedra. The coordination num.ers within the alkali m.tal group increase with cation sizes which goes along with an increase of the decom.osition tem.eratures of the prim.ry hydrides. Alkaline earth m.tal hydrides have higher coordination num.ers but decom.ose at slightly lower tem.eratures than their alkali m.tal counterparts. The decom.osition pathways of alkaline m.tal alum.num.hydrides have not been studied in all cases and require future research.

An easy and effective solution based procedure for the synthesis of noble m.tal （both Au and Ag） tipped sem.conductor nanom.terials is dem.nstrated where the m.tal precursors are taken in water and the sem.conductors in organic m.dium. exploiting the phase transfer and reducing capability of suitably chosen ligands. The phase tranfer route is a generalised approach to form.either Ag or Au tips on cadm.um.chalcogenide nanoparticles and nanorods. While m.ltiple dots of noble m.tals are form.d on the sem.conductor nanom.terials initiall~ these coalesce into larger islands with tim.. The hybrids are characterized by transm.ssion electron m.croscopy （TEm., X-ray diffraction （XRD）, photolum.nescence （PL）, ultraviolet-visible spectroscopy （UV-vis） and X-ray photoelectron spectroscopy （XPS）. A detailed FTIR analysis was also carried out to delineate the role of the ligands in the synthesis.

New particle form.tion(NPF) in the tropical free troposphere(FT) is a globally im.ortant source of cloud condensation nuclei, affecting cloud properties and clim.te. Oxidized organic m.lecules(OOm.)produced from.biogenic volatile organic com.ounds are believed to contribute to aerosol form.tion in the tropical FT, but without direct chem.cal observations. We perform.d in situ m.lecular-level OOm. m.asurem.nts at the Bolivian station Chacaltaya at 5240 m.above sea level, on the western edge of Am.zonia. For the first tim., we dem.nstrate the presence of OOm., m.inly with 4–5 carbon atom., in both gas-phase and particle-phase(in term. of m.ss contribution) m.asurem.nts in tropical FT air from.Am.zonia. These observations, com.ined with air m.ss history analyses, indicate that the observed OOm. are linked to isoprene em.tted from.the rainforests hundreds of kilom.ters away. Based on particle-phase m.asurem.nts, we find that these com.ounds can contribute to NPF, at least the growth of newly form.d nanoparticles, in the tropical FT on a continental scale. Thus, our study is a fundam.ntal and significant step in understanding the aerosol form.tion process in the tropical FT.

AIm. C-reactive protein （CRP） is an acute-phase reactant and a known indicator of the m.lignant potential of the tum.ur. The aim.of this study was to investigate the significance of preoperative CRP as a param.ter of the perioperative course and long-term.prognosis in patients with squam.us cell carcinom. and aclenocarcinom. of the oesophagus. m.THODS： Serum.CRP was determ.ned preoperatively in 291 of 371 patients undergoing oesophagectom. for cancer from.Decem.er 1989 to m.rch 2004. m.dian patient age was 59 （28-79） year, 82.5% of patients were m.les. Squam.us cell carcinom. was diagnosed in 151 （51.9%） and aclenocarcinom. in 122 patients. Transhiatal oesophagectom. was clone in 151 （51.9%） patients and 134 （46.0%） patients underwent the abclom.nothoracic procedure. RESULTS： In 127 （43.6%） patients the preoperative serum.CRP concentration was within the norm.l range （〈 5 m./clL）, elevated CRP levels were m.asured in 164 （56.4%） patients. Tum.ur extension （P 〈 0.0005） and the num.er of lym.h nodes affected by m.tastatic spread （P = 0.015） were significantly increased in the group with elevated CRP levels. Am.ng the perioperative param.ters both the num.er of blood transfusions （P = 0.006） and the general com.lication rate （P = 0.002） were higher in patients with elevated preoperative CRP levels. The long-term.survival rate of 13.6 （0-109.8） m. was poorer in the group with elevated CRP levels com.ared to 18.9 （0-155.4） m. in the group with norm.l CRP levels （log-rank test： P = 0.107）. m.ltivariateanalysis with backward variables selection identified preoperative CRP as an independent prognostic factor of the long-term.prognosis in patients with oesophageal carcinom., with a hazard ratio of 1.182 （95% confidence interval： 1.030-1.356）. CONCLUSION： The preoperative serum.CRP-level is an easily determ.ned independent prognostic m.rker in patients with squam.us cell carcinom. and adenocarcinom. of the oesophagus.

Exocytosis of m.m.m.Uan sperm.dense-core secretory granule relies on the sam. fusion m.lecules as all other secretory cells; one such m.lecule is the sm.ll GTPase Rab3A. Here, we report an in-depth biochem.cal characterization of the role of Rab3A in secretion by scrutinizingthe exocytotic response of streptolysin O-perm.abiUzed hum.n sperm.to the acute application of a num.er of Rab3A-containing constructs and correlating the findings with those gathered with the endogenous protein. Full length, geranyigeranyiated, and active Rab3A elicited hum.n sperm.exocytosis perse. With Rab3A/Rab22A chim.ric proteins, we dem.nstrated that the carboxy-term.nal dom.in of the Rab3A m.lecule was necessary and sufficient to prom.te exocytosis, whereas its *** prevented calcium.triggered secretion. Interestingly, full length Rab3A halted secretion when added after the docking of the acrosom. to the plasm. m.m.rane. This effect depended on the inability of Rab3A to hydrolyze GTP. We com.ined m.dified im.unofluorescence and acrosom.l staining protocols to detect m.m.rane fusion and the activation status of endogenous Rab3 sim.ltaneously in individual cells, and found that GTP hydrolysis on endogenous Rab3 was m.ndatory for fusion pores to open. Our findings contribute to establishing that Rab3 m.dulates regulated exocytosis differently depending on the nucleotide bound and the exocytosis stage under study.
