Background: In the last 6 months, cases of acute hemorrhagic fever (AHF) have been recorded in Sao Tome and Principe (STP). Objective: To identify the clinical, epidemiological and laboratory parameters associated with AHF cases found in patients hospitalized in STP. Methods: Descriptive and quantitative analysis of AHF cases hospitalized in STP in the period December 2021 to May 19, 2022 through the variables: demographic data;clinical data;laboratory data and clinical outcome. Results: Analyzed 18 of 22 AHF cases, 50% of them male, mean age 33.7 years, 85.7% residing in the 2 largest districts of the country, 66.7% rural workers, administrative near maritime areas, sailors and domestics. 66.7% were healthy individuals. ≥50% had a fever, asthenia/weakness, myalgia, headache, lethargy, nausea, vomiting, and diarrhea. 22.2% to 33.3% had retro-orbital pain, abdominal pain, decreased consciousness, dyspnea/hypoxia, and cough. In a smaller register (5.6% - 11.1%), exanthema, convulsion, arthralgia, low back pain, chills, and chest pain. Bleeding from the digestive tract was found in 72.2%, followed by vaginal (33.3%), urethral (27.8%), nasal (22.2%), and oral (16.7%). 50% had laboratory confirmation for dengue, and another 50% with suspected clinical diagnosis without laboratory confirmation of the etiologic agent. Despite hemorrhage, 66.7% of confirmed cases were hemodynamically non-severe, and 88.9% of suspected cases were severe. Coinfection with malaria is evidenced in 11.1% of cases. 72.2% recovered and 27.8% died (deaths in 55.5% of suspected cases). Case deaths were male (100%), resident foreigners (40%), tool store employees, sailors and students, healthy (80%), and residing in the largest district in the country (60%). All deaths were hospitalized in serious condition, 80% were hospitalized for hemorrhage and shock, with bleeding arising between day 4 - 5 of illness in 60% and hospitalization on day 5 of illness. 60% died within ≤24 hours of hospitalization. 80

AIM: To identify demographic and clinical factors asso-ciated with disabling Crohn's disease (CD). METHODS: A systematic review and meta-analysisof observational studies, focusing on the factors that can predict the prognosis of different outcomes of CD was undertaken. PubMed, ISI Web of Knowledge and Scopus were searched to identify studies investigat-ing the above mentioned factors in adult patients with CD. Studies were eligible for inclusion if they describe prognostic factors in CD, with inclusion and exclusion criteria defined as follows. Studies with adult patients and CD, written in English and studying association between clinical factors and at least one prognosis out-come were included. Meta-analysis of effects was un-dertaken for the disabling disease outcome, using odds ratio (OR) to assess the effect of the different factors in the outcome. The statistical method used was Mantel-Haenszel for fixed effects. The 16-item quality assess-ment tool (QATSDD) was used to assess the quality of the studies (range: 0-42). RESULTS: Of the 913 papers initially selected, sixty studies were reviewed and three were included in the systematic review and meta-analysis. The global QA-TSDD scores of papers were 18, 21 and 22. Of a total of 1961 patients enrolled, 1332 (78%) were classified with disabling disease five years after diagnosis. In two studies, age at diagnosis was a factor associated with disabling disease five years after diagnosis. Individu-als under 40 years old had a higher risk of developing disabling disease. In two studies, patients who were treated with corticosteroids on the first flare developed disabling disease five years after diagnosis. Further, perianal disease was found to be relevant in all of the studies at two and five years after diagnosis. Finally, one study showed localization as a factor associated with disabling disease five years after diagnosis, with L3 being a higher risk factor. Structural optimization of patient-specific temporomandibular joint replacement implants for additive manufacturing:novel metrics for safety evaluation and biomechanical performance

Total temporomandibular joint(TMJ)replacement is recommended only when there is irreversible damage to the joint and no conservative treatment can provide functional *** stock and custom-made TMJ implants have been made available;however,retrospective and comparative studies were unable to find significant differences between the two types of *** introduction of additive manufacturing(AM)techniques in medical practice allows for a greater freedom of design and a higher degree of device *** combination of AM with structural optimisation may streamline development and provide the key for fabricating biomechanic ally enhanced TMJ *** this study,structural optimis ation techniques were applied to develop and numerically validate a patient-specific TMJ *** biomechanical behaviour of each intermediate TMJ design was assessed under four different nominal and maximum biting tasks using finite element *** addition,a new set of metrics were proposed to compare each design regarding biomechanical performance and implant *** results suggest that 55-82%of the natural/intact strain patterns can be recovered with the finally selected TMJ *** represents an increase of 15%in biomechanical performance for incisor biting,15%for right molar biting,17%for left molar biting and a decrease of 2%for left group biting compared with the initial *** results also suggest that load transfer at the proximal ramus reduces the implant’s impact on the mandible’s strain ***,structural optimisation allows for a volume reduction of up to 44%with a minimum loss of implant safety and biomechanical performance.

Developing a protocol based on the Brazilian Dietary Guidelines for individual dietary advice in the primary healthcare:theoretical and methodological bases

class III Reversion with Immediate Complete Denture and Immediate Implant-Retained Overdenture: Case Report

With the increased life expectancy, complete dentures have been used in the treatment of edentulism progressively. It restores stomatognathic system functions, providing improvements in chewing, aesthetics, and phonetics. The aim was to report an oral rehabilitation of a patient with severe aesthetic and functional impairment, with Angle’s class III malocclusion. The patient sought the extension project of the School of Dentistry of the Federal University of Minas Gerais with aesthetic complaints. After clinical and radiographic evaluation, the proposed planning was immediate complete denture maxilla and immediate implant-retained overdenture in mandible. During clinical evaluation it was observed that the patient was Angle’s III. The rehabilitation was performed as proposed and at the end of the treatment, it was possible to revert the class III occlusal pattern to Angle’s I. The excellent maxillomandibular relationship was achieved, which ensured optimal facial aesthetics and functional condition. It was concluded that the immediate complete dentures and immediate implant-retained overdenture might be beneficial for restoring the aesthetics and function of patients with Angle’s class III malocclusion.

Prediction of ground-borne vibration induced by impact pile driving:numerical approach and experimental validation

Deep foundations are currently used in engineering practice to solve problems caused by weak geotechnical characteristics of the *** pile driving is an interesting and viable solution from economic and technical points of ***,it is necessary to ensure that the environmental drawbacks,namely ground-borne vibration,are adequately *** this purpose,the authors propose an axisymmetric finite element method-perfectly matched layer(FEM-PML)approach,where the nonlinear behavior of the soil is addressed through an equivalent linear *** the complexity of the problem,an experimental test site was developed and fully *** experimental work comprised in-situ and laboratory soil characterization,as well as the measurement of vibrations induced during pile *** comparison between experimental and numerical results demonstrated a very good agreement,from which it can be concluded that the proposed numerical approach is suitable for the prediction of vibrations induced by impact pile *** experimental database is available as supplemental data and may be used by other researchers in the validation of their prediction models.
