The objective of this study was to identify risk factors and study the causes of neonatal mortality of LBW newborns referred to the Institute of Nutrition and Child Health. Material and Methods: This is a prospective descriptive and analytical study from March 1 to August 31, 2015, involving 250 newborns referred to the Neonatology Department of the Institute of Child Nutrition and Health (INSE). Results: During our study period, we recorded 1169 newborns, 250 of whom were referred, representing a referral frequency of 21.38%. The overall mortality rate for transferring newborns was 46.8%. Risk factors associated with neonatal mortality were gravidity (p = 0.0019), parity (p = 0.0323), occupational activities requiring physical effort (p = 0.0257), birth weight (p = 0.0008) and prematurity (p = 0.0039). Conclusion: Neonatal mortality associated with referrals is a major health problem in developing countries. There is currently no doubt that it is possible to significantly reduce perinatal mortality and morbidity rates by organizing the identification of “mother-child” couples at risk, to refer them intently to centres better equipped with technical and human resources to care for them.

The Factors of the Failure in the Treatment of Congenital Equinus Clubfoot Varus by the Ponseti Method at the Reference Health Care Center (CSREF) of Commune III of Bamako

Clubfoot malformation is the most common serious congenital anomaly affecting the foot in children. Its treatment by the Ponseti method is simple, profitable and widely used in the world. Objective: The objective of this study was to identify the factors of the failure of its treatment by the Ponseti method. Material and Method: We carried out a retrospective and descriptive study of cases of congenital equinus clubfoot varus at the Reference Health Care Center of Commune III of Bamako over 26 months from September 2020 to November 2022. Data were treated with the utmost anonymity. Result: This study was performed on 44 children seen for clubfoot: male (68%) and female (32%), with a sex ratio of 2.1. We obtained 13 cases of recidivism including 7 boys and 6 girls. We found 21 cases of unilateral and 23 bilateral;among which 9 recurrences were found against 4 in the unilateral forms. 85% of recurrences did not have good adherence to the splint and 62% did not come regularly for follow-up consultation. We obtained 33 children with idiopathic clubfeet (75%) with a recurrence of 24%, and 7 children with secondary clubfeet with 71 % recurrence. There was no recurrence in the postural type. Among the recurrences, 38.5% started treatment between 1 and 6 months, 23.1% from 0 to 1 month and 15.4% at 2 years and more. 85% of recurrences had a Pirani score between 4.5 to 6 at the start of treatment and 15% at a score of 2.5 to 4. Conclusion: The factors of the failure of the Ponseti method are mainly non-compliance with treatment, secondary clubfeet, and a high Pirani score at the start of treatment.

Early Prosthetic Valve Endocarditis with Mycobacterium Tuberculosis after Mitral Valve Replacement: A Case Report

Background: Tuberculous endocarditis is a rare but serious complication of heart valve replacement surgery. We report the case of a 24-year-old patient, who presented with tuberculous endocarditis after mechanical mitral valve replacement, with a favorable clinical course following anti-tuberculosis treatment. Case Presentation: We report a 24-year-old male patient, admitted to the cardiac surgery department of the Fann Hospital (Dakar, Senegal), for the management of severe mixed (rheumatic and endocarditic) mitral insufficiency with associated tricuspid insufficiency. He had a history of recurrent angina and polyarthralgia in childhood, was hospitalized several times for refractory global cardiac decompensation, and for a suspected infective endocarditis a month before his admission. On admission, the clinical examination revealed signs suggestive of mitral and tricuspid insufficiency. Transthoracic echocardiography revealed severe post-endocarditic mitral insufficiency with A3 amputation, highly mobile 15 mm vegetations on the free edge of the large valve, moderate tricuspid insufficiency, and severe pulmonary artery hypertension. Mechanical mitral valve replacement and tricuspid valve annuloplasty using autologous pericardial strip were performed via median sternotomy. After ten days, the patient presented with global cardiac decompensation associated with a clinico-biological infectious syndrome, and tans-oesophageal echography revealed an abscess at the sinotubular junction, communicating with the aorta. A thoraco-abdomino-pelvic CT scan was done, which revealed a bilateral alveolar-interstitial syndrome with mediastinal lymphadenopathy. Anti-tuberculosis treatment with RHZE was initiated for 06 months. The clinical course was favorable. Conclusion: Tuberculous endocarditis in prostheses is a serious complication of heart valve replacement surgery, which may evolve favorably under medical treatment.

Optimization of Diesel and Crude Oil Degradation in a Ghanaian Soil Using Organic Wastes as Amendment

Soil contamination by hydrocarbons poses numerous environmental, health and agricultural problems. The degradation of these pollutants can occur naturally but very slowly. It is therefore generally necessary to stimulate this degradation by different means. Thus, this study aimed to improve the bio-degradation of diesel and crude oil in a Ghanaian soil by biostimulation. For this, the sampled soil was characterized by standard methods and contaminated with diesel and crude oil at a proportion of 1% (w/w). Then, contaminated soil samples were supplemented with biochar-compost, poultry manure or cow dung at the proportion of 10% (w/w). Periodically, fractions of these samples were taken to evaluate the density of hydrocarbon utilizing bacteria (HUB) and the residual quantities of diesel or crude oil. The characteristics of the soil used show the need for supplementation for better degradation of hydrocarbons. The results of the study show that supplementing the soil with organic substrates increases HUB loads in soils contaminated by diesel and crude oil. They also show that the residual quantities of diesel and crude oil are generally significantly lower in supplemented soils (p = 0.048 and p < 0.0001 respectively). In addition, the study shows that degradation was generally greater in soils contaminated by diesel compared to those contaminated by crude oil, especially at the end of the study.

Results of the First 10 Cases of Coronary Bypass Surgery in Senegal

This is a review of the first 10 coronary artery bypass surgeries performed by the local team. The mean age was 62 years old [45 - 74]. The patients were predominantly male, with a M/F ratio of 4:1. Cardiovascular risk factors were mainly myocardial infarction (MI) (60%), hypertension (50%), obesity (40%) and diabetes (30%), with at least two risk factors per patient. Angina was the main symptom (80%). The average time from presentation to surgery was 8 months. The mean Euroscore 2 was 2.92 ± 1.65 [1.33 - 6.60]. Coronary angiography revealed an average of 2 lesions per patient, with 3-vessel involvement in 70% of cases: the Interventricular artery (IVA) (100%), the right coronary artery (90%) and the circumflex artery (70%). On echocardiography, the mean Left ventricular ejection fraction (LVEF) was 59% [33% - 76%]. All patients underwent median sternotomy with bypass grafting. The average duration of the cardiopulmonary bypass was 150 min [46 - 275 min];that of aortic clamping, 120 min [43 - 232 min]. The grafts used were internal thoracic artery (ITA) in 100% of cases (80% on the left and 20% on the right), and the great saphenous vein (GSV) in 60% of cases (50% on the left and 10 on the right). Double bypass was performed in 60% of cases, single bypass in 30% and triple bypass in 10%. The bypasses were performed on the IVA (100%), the middle lateral of the circumflex (30%) and the bisector (20%). The average time to extubation was 11 hours and the length of stay in the intensive care unit was 7 days [03 - 17 days]. One patient had a reoperation on Day 0 post-op. The average hospital stay was 13 days [06 - 27 days]. Complications occurred in nine of the patients (90%), with a predominance of infectious and neurological complications. Overall operative mortality was 3%, all in intensive care.

Measurement of Radon Concentration and Estimation of Cancer Risk in Twenty-Four Model Houses in the Town of Koudougou

The objective of our study is to evaluate the concentration of radon (86Rn) inside houses in the town of Koudougou in order to estimate its impact on the health of the population. Indeed, when uranium-rich minerals are found near the surface of the ground, radon concentrations can reach tens of becquerels per cubic meter in enclosed spaces. Given the nature of the geological base of Burkina Faso, this situation is quite probable and certain places that are sometimes poorly ventilated (house, school, office, etc.) can have radon levels high enough to constitute a health problem for occupants. Thus, twenty-four (24) sample houses were identified. In each house, the Corentium digital detector was between 0.8 m and 2 m for at least one week in a place where the occupants estimate that they spend more time of time and measure the concentration of radon in the long term and short term. The recorded data allowed us to determine the Absorbed Dose and the Annual Effective Dose of radon gas for each house in order to estimate the Risk of Cancer and the probable Number of Cases of Lung Cancer per million inhabitants. Thus, the results indicate that the long-term radon concentration varies between 6 Bq/m3 and 285 Bq/m3 respectively in houses 11 and 4 compared to 1 Bq/m3 to 208 Bq/m3 in the short term in the same houses. Also, in the long term, in control houses 1, 3 and 4, the radon level is above the recommended threshold interval. For the short term, these are houses 1, 3, 4 and 17 respectively with 110 Bq/m3, 142 Bq/m3, 208 Bq/m3 and 105 Bq/m3. As for the long-term and short-term effective doses, only houses 1, 3, 4, 17 and 24 have values between 3 - 10 Sv/year. The estimation of the relative risk of lung cancer gives values relatively close to unity and between 1.006 and 1.142 with an average of 1.035 and that of the Number of Lung Cancer Cases per million inhabitants gives values between 8 and 166 with an average of 42. Introduction: Work environments create the basis for arduous nursing work. This is what led us to research the factors of arduousness in the exercise of the health profession in North Benin, through this study. Methods: This is a cross-sectional and descriptive study which focused on health professionals in the health zones of Parakou-N’dali and Tchaourou. The sampling was an exhaustive census. All health workers who had given their free and informed consent were included. The data was collected using a questionnaire. The variables studied were the factors of arduous work and socio-professional characteristics. The data collected was processed and analyzed with the Epi info 7.***.0.1 software. Results: A total of 692 were surveyed, the participation rate was 85.11% and the sex ratio (M/F) equal to 0.7. The average age was 38 years old. Nurses made up 32.66% of the sample. They were 86.42% subject to night work and 82.66% to alternating work. Overall, 70.23% worked more than 40 hours per week. Between 78.12% and 96.46% of non-specialist doctors, nurses, midwives and biomedical analysis technicians were required to work night shifts. Nurses, midwives, radiology and laboratory technicians were between 89.53% and 97.35%, organized in alternating work. In the sample, 55.20% complained about the insufficient number of work materials, 26.29% handled harmful chemical substances. Among those surveyed, 58.14% had been attacked. Conclusion: All professional categories of caregivers are subject to arduous work. Measures are needed to reverse the situation.

Management of Acute Myeloblastic Leukaemia (AML) Treated with Intensive Chemotherapy: Experience in a Single Centre

Introduction: Acute myeloblastic leukaemia (AML) is a haematological malignancy with a poor prognosis, despite significant therapeutic progress. This study presents the results of AML management in Mali according to GFAOP recommendations. Methodology: This was a retrospective, cross-sectional study. It included patients aged 0 - 15 years treated in the paediatric oncology unit for AML and followed up between January 2016 and December 2020. Results: During the study period, 85 cases of acute leukaemia were diagnosed in the paediatric oncology unit (including 51 cases of ALL), of which 34 cases of AML were included in this study. The majority were boys (59%). The mean age was 8 years, with extremes of 18 months and 15 years. The mean time to diagnosis was 68 days. In 79% of cases, patients were referred by 1st or 2nd level hospitals. Anaemia was observed in 91% of cases, an infectious syndrome in 68%, haemorrhage in 56% and a tumour syndrome in 85%. The haemogram showed hyperleukocytosis in 15% of cases, thrombocytosis in 22% and severe anaemia in 73%. Death occurred in 85% of cases, most often in the context of sepsis or haemorrhage. Conclusion: AML is probably underestimated in Mali and diagnosis delayed, which may be explained by patient-related factors (lack of knowledge, financial constraints) and a cumbersome referral system. These results suggest the need to implement an appropriate diagnostic and therapeutic strategy, with strong involvement of the political authorities.

Sweeteners and flavor enhancers are food additives widely used in industry, respectively, to add sweetness and flavor to foods. However, the presence of these substances is often criticized by consumers for their effects on health. What’s more, some scientific studies link these substances to certain pathologies. To guarantee food safety, competent authorities should have food standards based on risk analysis using consistent, reliable data. However, in developing countries, such data is often weak or non-existent. The aim of this study is therefore to carry out a pilot survey to establish the profile of sweeteners and flavour enhancers present in industrial food products marketed in Senegal. The methodology consisted of sampling various food products sold on the Senegalese market, based on analysis of labels containing information on ingredients, including additives. The investigation involved nine stores, one supermarket and 5 mini-markets in Dakar. The results showed the presence of 6 taste enhancers in food products, the most frequent being sodium L-monoglutamate (E621), inosinate (E631) and disodium guanylate (E627). Solid broths are the foodstuffs with the highest number of taste exhalters. As for sweeteners, 12 substances were identified, the most frequent being acesulfame potassium (E950), aspartame (E951), sucralose (E955) and saccharin (E954). Given the potential health risks associated with the consumption of these food additives, their control and monitoring on the market should be a priority for the competent authorities.

Consumers’ Perception and Knowledge of Food Additives in Senegal: A Pilot Study

Food safety has become a major concern for consumers, as well as a priority for regulatory authorities. Faced with the growing industrial and domestic use of food additives, many questions are being asked and concerns are being felt by consumers around the world. Consumer perception defines the acceptability or rejection of food products, and has an impact on consumption patterns and behavior. To assess the level of knowledge and perception of food additives, a pilot study was carried out on a sample of 200 people in Dakar and Saint-Louis. A questionnaire was used to assess the acceptance or rejection, use and impact of food additives by consumers in Senegal. The results revealed several aspects. On the whole, the people surveyed expressed great mistrust and even rejection of these substances added to food products. This consumer perception is shared throughout the world, as indicated in numerous surveys. It also emerges from this study that, although most consumers are aware of the existence of these additives and their uses in the home, they feel that the use of these substances in industrial production is too excessive. What’s more, consumers associate food additives with numerous pathologies such as cancer, diabetes, hypertension, stroke and even sexual impotence. For some of these indexed pathologies, scientific studies have reached the same conclusions, although controversy still persists. On the other hand, for some of the other adverse effects mentioned, no cause-and-effect relationship has been scientifically demonstrated. In these latter cases, it seems that negative communication, misinformation and misconceptions have a major influence on consumer perception of food additives.
