Background:Currently,surgical resection is the mainstay for colorectal liver metastases(CRLM)management and the only potentially curative treatment *** tools can support patient selection for surgical resection to maximize therapeutic *** study aimed to develop a survival prediction model using machine learning based on a multicenter patient sample in Hong ***:Patients who underwent hepatectomy for CRLM between 1 January 2009 and 31 December 2018 in four hospitals in Hong Kong were included in the *** analysis was performed using Cox proportional hazards(CPH).A stepwise selection on Cox multivariable models with Least Absolute Shrinkage and Selection Operator(LASSO)regression was applied to a multiply-imputed dataset to build a prediction *** model was validated in the validation set,and its performance was compared with that of Fong Clinical Risk Score(CRS)using concordance ***:A total of 572 patients were included with a median follow-up of 3.6 *** full models for overall survival(OS)and recurrence-free survival(RFS)consist of the same 8 established and novel variables,namely colorectal cancer nodal stage,CRLM neoadjuvant treatment,Charlson Comorbidity Score,pre-hepatectomy bilirubin and carcinoembryonic antigen(CEA)levels,CRLM largest tumor diameter,extrahepatic metastasis detected on positron emission-tomography(PET)-scan as well as KRAS *** CRLM Machine-learning Algorithm Prognostication model(CMAP)demonstrated better ability to predict OS(C-index=0.651),compared with the Fong CRS for 1-year(C-index=0.571)and 5-year OS(C-index=0.574).It also achieved a C-index of 0.651 for ***:We present a promising machine learning algorithm to individualize prognostications for patients following resection of CRLM with good discriminative ability.

Hepatocellular carcinoma(HCC)with inferior vena cava(IVC)invasion and metastatic lymph node metastases has a poor prognosis,and surgical resection is seldom *** report how an initially unresectable HCC in a 36-year-old Chinese male with distant lymph node metastases and tumor thrombosis in the IVC was successfully downstaged and ultimately resected together with the *** the disease had been downstaged,curative resection of the tumor and IVC was conducted with immediate reconstruction of the *** patient has survived for more than 2 years after the *** conclusion,tumor and IVC resection can cure metastatic HCC after downstaging treatment combining sorafenib and transarterial chemoembolization.

Dear Editor,In recent years,criteria based on the combinationof morphology and biology have been proposed forimproving the selection of hepatocellular cancer(HCC)patients waiting for liver transplantation(LT)[1,2].Since all the proposed models showed suboptimalresults in predicting the risk of postLT recurrence,aprediction model constructed using artificial intelligence(Al)could be an attractive way to surpass this limit[3,4].Therefore,the Time_Radiological-response_Alpha-fetoproteIN_Artificial-Intelligence(TRAIN-AI)modelwas developed,combining morphology and biology tumorvariables.

Aim:Recurrence of hepatocellular carcinoma (HCC) after liver transplantation (LT) for chronic hepatitis B (CHB) can be associated with reappearance of hepatitis B surface antigen (HBsAg). The current study determined the significance of HBsAg qualitatively and quantitatively using a highly sensitive assay in recurrent HCC after transplantation. Methods:Consecutive patients with HBV-related HCC with LT were included. Oral nucleos(t)ide analogues without hepatitis B immune globulin were used as hepatitis B virus (HBV) prophylaxis. Quantitative HBsAg levels were performed at time of transplant, at 1 month, 3 and 6 months post transplant using a highly sensitive (hs)-HBsAg assay. Results:One hundred and fourteen patients were included, with a median follow-up of 80 months, with 24 cases of HCC recurrence, and a cumulative rate of 20.7% at 5 years. There was significant correlation between time of tumor recurrence and time of HBsAg reappearance (r = 0.551,P = 0.027). Early HCC recurrence was associated with higher median level of hs-HBsAg at the time of transplant (72.85vs. 69.70 IU/mL,P = 0.018). Using a hs-HBsAg cut-off level of 0.0005 IU/mL, patients with levels above this threshold at 3 and 6 months were associated with higher rate of early HCC recurrence (28.6%vs. 3.0% and 26.9%vs. 2.9% respectively, bothP =0.0006). There was no significant difference in HCC recurrence between positive and negative HBsAg using the conventional qualitative HBsAg assay. Conclusion:Serum hs-HBsAg levels of≥ 0.0005 IU/mL at 3 to 6 months after LT is associated with higher rates of early HCC recurrence, and may be useful as an early tumor marker.

回顾性分析2015年7月至2017年7月因区域性肝胆管结石病行沿肝中静脉解剖性半肝切除的34例患者资料,统计其临床表现、结石类型、手术方式、术后并发症及治疗效果。患者平均年龄54.8岁,其中左半肝切除29例(85.3%),右半肝切除5例(14.7%),手术时间(321.0±78.0)min,术中出血量(551.0±662.0)ml,出现并发症8例(23.5%)。术后结石残留1例(2.9%),所有患者未发生死亡及肝功能衰竭。研究显示在重视围手术期处理和精准手术技术的前提下,沿肝中静脉解剖性半肝切除术可有效降低区域性肝胆管结石病术后并发症发生率和结石复发率。

In the era of the nanometer CMOS technology, due to stringent system requirements in power, performance and other fundamental physical limitations (such as mechanical reliability, thermal constraints, overall system form factor, etc.), future VLSI systems are relying more on ultra-high data rates (up to 100 Gbps/pin or 20 Tbps aggregate), scalable, re-configurable, highly compact and reliable interconnect fabric. To overcome such challenges, we first explore the use of multiband RF/wireless-interconnects which can communicate simultaneously through multiple frequency bands with low power signal transmission, reconfigurable bandwidth and excellent mechanical flexibility and reliability. We then review recent advances in RF/wireless-interconnect in four different potential application domains, which include network-on-chips (NoCs), 3-dimensional integrated circuit (3DIC), advanced memory interface and ultra-high speed contactless connectors. Based on those developments, we further propose the future research direction on future inter- and intra-VLSI interconnect system through the comparison of performance and the proper communication range for all three types of interconnects, including communication data throughput, range and power consumption (pJ/bit) among the RF/wireless-interconnects, the optical interconnects and traditional parallel repeated bus.

In recent years, e-learning system has been adopted as a training tool in many firms, as it can help employees to complete training in a short time and at a low cost. Within this context, since 2003 the Industrial Development Bureau （IDB） of the Ministry of Economic Affairs （MOEA） in Taiwan sponsored more than 50 enterprises each year to help them implement customized e-learning systems. Past research on e-learning mostly focused on one criterion and adopted regression approach, such as examining the relationships between satisfaction or continued usage intentions and various antecedent factors. Although the regression method could uncover the significant factors of e-learning adoption, they can not show the can （weighting） of each factor, and thus firms cannot allocate the optimal levels of resources. To overcome this problem, this study collected factors related to finns implementing customized e-learning systems from a review of the literature and expert opinions, and then constructed a hierarchical factor table. Based on the appropriateness and independence of factors, and as assessed by experts in the project promotion office of the IDB, e-learning solution providers and project leaders of the IDB sponsored enterprises, this paper then designed an Analytical Hierarchy Process （AHP） format questionnaire and sent it to these same experts to gather their responses. The factor weightings were derived based on the Fuzzy Analytic Hierarchy Process （FAHP） method. Finally, analysis of key factors was performed and recommendations provided. Based on the results for this work, service industry firms with limited resources can achieve proper resource allocation and more quickly and successfully adopt customized e-Learning systems.
