Disruption of c.lonic.barrier func.ion and induc.ion of mediator release by strains of c.mpylobac.er jejuni that invade epithelial c.lls

AIM: To study the mec.anisms by whic. c.mpylobac.er jejuni (c. jejuni) c.uses inflammation and diarrhea. In partic.lar, direc. interac.ions with intestinal epithelial c.lls and effec.s on barrier func.ion are poorly under- stood. METHODS: To model the initial pathogenic.effec.s of c. jejuni on intestinal epithelium, polarized human c.lonic.Hc.-7 monolayers were grown on permeabilized filters and infec.ed apic.lly with c.inic.l isolates of c. jejuni. Integrity of the monolayer was monitored by c.anges in monolayer resistanc., release of lac.ate dehydrogenase, mannitol fluxes and elec.ron mic.osc.py. Invasion of Hc.-7 c.lls was assessed by a modified gentamic.n protec.ion assay, transloc.tion by c.unting c.lony forming units in the basal c.amber, stimulation of mediator release by immunoassays and sec.etory responses in monolayers stimulated by bradykinin in an Ussing c.amber. RESULTS: All strains transloc.ted ac.oss monolayers but only a minority invaded Hc.-7 c.lls. Strains that invaded Hc.-7 c.lls destroyed monolayer resistanc. over 6 h, ac.ompanied by inc.eased release of lac.ate dehydrogenase, a four-fold inc.ease in permeability to [3H] mannitol, and ultrastruc.ural disruption of tight junc.ions, with rounding and lifting of c.lls off the filter membrane. Synthesis of interleukin (IL)-8 and prostaglandin E2 was inc.eased with strains that invaded the monolayer but not with those that did not. c.Nc.USION: These data demonstrate two distinc. effec.s of c. jejuni on c.lonic.epithelial c.lls and provide an informative model for further investigation of initial host c.ll responses to c. jejuni.

Wave breaking plays an important role in wave-struc.ure interac.ion. A novel c.ntrol volume finite element method with adaptive unstruc.ured meshes is employed here to study 3-D breaking waves. The numeric.l framework c.nsists of a ＂volume of fluid＂ type method for the interfac. c.pturing and adaptive unstruc.ured meshes to improve c.mputational effic.enc.. The numeric.l model is validated against experimental measurements of breaking wave over a sloping beac. and is then used to study the breaking wave impac. on a vertic.l c.rc.lar c.linder on a slope. Detailed c.mplex interfac.al struc.ures during wave impac., suc. as plunging jet formation and splash-up are c.ptured in the simulation, demonstrating the c.pability of the present method.

LiFePO4 materials have bec.me inc.easingly popular as a c.thode material due to the many benefits they possess inc.uding thermal stability, durability, low c.st, and long life span. Nevertheless, to broaden the general appeal of this material for prac.ic.l elec.roc.emic.l applic.tions, it would be useful to develop a relatively mild, reasonably simple synthesis method of this c.thode material. Herein, we desc.ibe a generalizable, 2-step methodology of sustainably synthesizing LiFePO4 by inc.rporating a template-based, ambient, surfac.antless, seedless, U-tube protoc.l in order to generate size and morphologic.lly tailored, c.ystalline, phase-pure nanowires. The purity, c.mposition, c.ystallinity, and intrinsic.quality of these wires were systematic.lly assessed using transmission elec.ron mic.osc.py （TEM）, high-resolution TEM （HRTEM）, sc.nning elec.ron mic.osc.py （SEM）, X-ray diffrac.ion （XRD）, selec.ed area elec.ron diffrac.ion （SAED）, energy dispersive analysis of X-rays （EDAX）, and high-resolution sync.rotron XRD. From these tec.niques, we were able to determine that there is an absenc. of any obvious defec.s present in our wires, supporting the viability of our synthetic.approac.. Elec.roc.emic.l analysis was also employed to assess their elec.roc.emic.l ac.ivity. Although our nanowires do not c.ntain any notic.able impurities, we attribute their less than optimal elec.roc.emic.l rigor to differenc.s in the c.emic.l bonding between our LiFePO4 nanowires and their bulk-like c.unterparts. Spec.fic.lly, we demonstrate for the first time experimentally that the Fe-O3 c.emic.l bond plays an important role in determining the overall c.nduc.ivity of the material, an assertion whic. is further supported by rec.nt ＂first-princ.ples＂ c.lc.lations. Ac.te bleeding after argon plasma c.agulation for weight regain after gastric.bypass: A c.se report

BAc.GROUND Roux-en-Y gastric.bypass(RYGB) is the most c.mmonly performed surgic.l proc.dure used to treat obesity worldwide. Despite satisfac.ory results in terms of weight loss, over time many patients experienc. weight regain. There are many fac.ors that c.ntribute to weight regain after RYGB, inc.uding the diameter of the gastric.jejunal anastomosis(GJA). One of the most c.mmonly performed endosc.pic.proc.dures for weight regain after RYGB is argon plasma c.agulation(APc.. We report a c.se of hematemesis after outlet revision with APc. We highlight several treatment modalities that c.n be used to treat this *** SUMMARY A 45-year-old female with a history of weight regain after RYGB was referred for possible endosc.pic.treatment for weight regain. On endosc.pic.evaluation, the diameter of the GJA was 22 mm. Due to the dilated GJA, treatment with APc.was performed. Several months later she reported a return of poor satiety and an inc.eased appetite. A repeat endosc.py was then performed. The GJA was approximately 15 mm and was inc.mpetent. APc.was performed. One day post proc.dure she had four episodes of hematemesis. An endosc.py was performed and a large ulc.r with a visible arterial vessel was visualized at the *** was attempted using a c.agrasper and after initial c.ntac. with the vessel, the vessel started oozing. Due to fibrosis and the depth of ulc.ration in the area, c.ips and repeat APc.c.uld not be used. Therefore, an attempt to injec. epinephrine injec.ion was made. However, persistent oozing was noted. As a result, hemostatic.powder was applied to the region of the bleeding ***, no more bleeding was observed. On follow-up, the patient remained hemodynamic.lly stable and a sec.nd look endosc.py was not performed. Pressure Ulc.r Prevention: Effec.iveness of Heel Off-Loading Methodologies

Skin integrity breakdown and pressure ulc.r formation on the heel are predic.able and preventable healthc.re oc.urrenc.s. By minimizing surfac. c.ntac. pressures that c.n c.use heel c.pillary oc.lusion, heel off-loading devic.s help mitigate the risk of dec.bitus ulc.ration. The purpose of this study is to investigate the effic.c. of six off-loading methods c.mmonly used in c.inic.l settings to dec.ease heel c.ntac. pressure. Pressure mapping is used to evaluate c.ntac. pressure at the heel for underweight, normal weight, and overweight simulations. Based upon averages ac.oss four trials, it is shown that all devic.s reduc. heel c.ntac. pressure when c.mpared to no off-load- ing. The PRUventor™ boot exhibits signific.ntly lower average pressure values than other off-load- ing strategies tested, suggesting that the PRUventor™ is the most effec.ive off-loading devic.. From the analysis of the experimental data and c.mparison to similar data for respec.ive off-loading alternatives, it is c.nc.uded that the PRUventor™ boot is an effec.ive heel off-loading devic. for use in c.inic.l settings.
