Use of topical vancomycin powder in total joint arthroplasty:Why the current literature is inconsistent?

Periprosthetic joint infection(PJI)is a rare but terrible complication in hip and knee arthroplasty,and the use of topical vancomycin powder(VP)has been investigated as a tool to potentially reduce its ***,there remains no consensus on its ***,the aim of this review is to provide an overview on the application of topical vancomycin in orthopaedic surgery focusing on the recent evidence and results in total joint *** systematic reviews and meta-analyses on topical VP in hip and knee arthroplasty have been recently published reporting sometimes conflicting *** from all being limited by the quality of the included studies(mostly level III and IV),confounding variables are often included potentially leading to biased *** taken into consideration the exclusive use of VP in isolation,the available data,although very limited,suggest that it does not reduce the infection rate in routine primary hip and knee ***,we still cannot advise for a routinary application.A properly powered randomized-controlled trial would be necessary to clarify the role of VP in hip and knee *** on the analysis of the current evidence,the use of topical VP appears to be safe when used locally in terms of systemic adverse reactions,hence,if proven to be effective,it could bring great benefits due to its low cost and accessibility.

Severe acute pancreatitis： pathogenesis, diagnosis and surgical management

BACKGROUND： Severe acute pancreatitis is a subtype of acute pancreatitis, associated with multiple organ failure and systemic inflammatory response syndrome. In this qualitative review we looked at the principles of pathogenesis, classification and surgical management of severe acute pancreatitis. We also looked at the current shift in paradigm in the management of severe acute pancreatitis since the guideline developed by the British Society of *** SOURCES： Studies published between 1st January 1991 and 31st December 2015 were identified with Pub Med, MEDLINE, EMBASE and Google Scholar online search engines using the following Medical Subject Headings： “acute pancreatitis, necrosis, mortality, pathogenesis, incidence” and the terms “open necrosectomy and minimally invasive necrosectomy”.The National Institute of Clinical Excellence（NICE） Guidelines were also included in our study. Inclusion criteria for our clinical review included established guidelines, randomized controlled trials and non-randomized controlled trials with a follow-up duration of more than 6 ***： The incidence of severe acute pancreatitis within the UK is significantly rising and pathogenetic theories are still controversial. In developed countries, the most common cause is biliary calculi. The British Society of Gastroenterology,acknowledges the Revised Atlanta criteria for prediction of severity. A newer Determinant-based system has been *** principle of surgical management of acute necrotizing pancreatitis requires intensive care management, identifying infection and if indicated, debridement of any infected necrotic area. Characterisation of Neonicotinoid and Pymetrozine Resistance in Strains of Bemisia tabaci(Hemiptera:Aleyrodidae)from China

Four strains of the Q biotype and one of the B biotype of the whitefly Bemisia tabaci collected from China were characterised for resistance to four neonicotinoid insecticides and pymetrozine.Q biotype strains showed moderate to strong resistance to imidacloprid,thiamethoxam and acetamiprid,but little or no cross-resistance to *** to neonicotinoids was consistently associated with resistance to pymetrozine,despite the latter having a distinct（though unresolved）mode of *** single B biotype strain proved largely susceptible to all the insecticides *** in the Q biotype strains was associated with over-expression of a cytochrome P450 monooxygenase gene,CYP6CM1,whose substrate specificity presumably accounts for the observed cross-resistance profiles.

Seismic considerations for spent nuclear fuel storage in dry casks

To aid the United States Nuclear Regulatory Commission,Sandia National Laboratories (SNL) was contracted to investigate the seismic behavior of typical dry cask storage systems. Parametric evaluations characterized the sensitivity of calculated cask response characteristics to input parameters. The parametric evaluation investigated two generic cask designs (cylindrical and rectangular),three different foundation types (soft soil,hard soil,and rock),and three different casks to pad coefficients of friction (0.2,0.55,0.8) for earthquakes with peak ground accelerations of 0.25g,0.6g,1.0g and 1.25g. A total of 1 165 analyses were completed,with regression analyses being performed on the resulting cask response data to determine relationships relating the mean (16 % and 84 % confidence intervals on the mean) to peak ground acceleration,peak ground velocity,and pseudo-spectral acceleration at 1 Hz and 5 % damping. In general,the cylindrical casks experienced significantly larger responses in comparison to the rectangular cask. The cylindrical cask experienced larger top of cask displacements,larger cask rotations (rocking),and more occurrences of cask toppling (the rectangular cask never toppled over). The cylindrical cask was also susceptible to rolling once rocking had been initiated,a behavior not observed in the rectangular cask. Cask response was not overly sensitive to foundation type,but was significantly dependent on the response spectrum employed.

Low ADAMTS-13 in plavix induced thrombotic thrombocytopenic purpura

Thrombotic thrombocytopenia purpura(TTP) was fi rst described in 1924 as a "pathologic alteration of the microvasculature, with detachment or swelling of the endothelium, amorphous material in the sub-endothelial space, and luminal platelet aggregation leading to compromise of the microcirculation". Ticlopidine induced TTP has been highly associated with autoimmune induced reduction in ADAMTS-13 activity. These findings, to a lesser extent, have also been found in clopidogrel induced TTP. We report a case of clopidogrel associated TTP in a patient that presented with acute stroke, renal failure, and non-ST elevation myocardial infarction.
