Objective:The purpose of the study was to identify the best sequence of therapy beginning with a tyrosine kinase inhibitor(TKI)as the first-line therapy for patients with metastatic renal cell carcinoma(mRCC)in terms of overall survival(Os),progression-free survival(PFs),and rates of discontinuation and adverse effects during the treatment ***:This is a retrospective,nationwide multicenter study of patients with mRCC after diagnosis at 10 different tertiary medical centers in Korea from January 1992 to December *** focused on patients at either“favorable”or“intermediate”risk according to the International mRCC Database Consortium criteria,and they were followed up(median 335 days).Finally,a total of 1409 patients were selected as the study *** generated a Cox proportional hazard model adjusted for covariates,and the different therapy schemes were statistically tested in terms of Os as well as *** addition,frequencies of discontinuation and adverse events were compared among the therapy ***:Of the primary patterns of treatment sequences(24 sequences),“sunitinib epazopanib”and“sunitinibeeverolimuseimmunotherapy”showed the most beneficial results in both Os and PFs with significantly lower hazards than“sunitinib”,which is the most commonly treated agent in *** that the“TKIeTKI”structure showed relatively higher discontinuation rates with higher adverse effects,the overall beneficial sequence would be“sunitinibeeverolimuseimmunotherapy”.Conclusion:Among several sequential therapy starting with TKIs,“sunitinibeeverolimuse immunotherapy”was found to be the best scheme for mRCC patients with“favorable”or“intermediate”risks.

Background In Viet Nam, tuberculosis (TB) represents a devastating life-event with an exorbitant price tag, partly due to lost income from daily directly observed therapy in public sector care. Thus, persons with TB may seek care in the private sector for its flexibility, convenience, and privacy. Our study aimed to measure income changes, costs and catastrophic cost incurrence among TB-affected households in the public and private *** Between October 2020 and March 2022, we conducted 110 longitudinal patient cost interviews, among 50 patients privately treated for TB and 60 TB patients treated by the National TB Program (NTP) in Ha Noi, Hai Phong and Ho Chi Minh City, Viet Nam. Using a local adaptation of the WHO TB patient cost survey tool, participants were interviewed during the intensive phase, continuation phase and post-treatment. We compared income levels, direct and indirect treatment costs, catastrophic costs using Wilcoxon rank-sum and chi-squared tests and associated risk factors between the two cohorts using multivariate *** The pre-treatment median monthly household income was significantly higher in the private sector versus NTP cohort (UsD 868 vs UsD 578;P=0.010). However, private sector treatment was also significantly costlier (UsD 2075 vs UsD 1313;P=0.005), driven by direct medical costs which were 4.6 times higher than costs reported by NTP participants (UsD 754 vs UsD 164;Ps resulted in no significant difference in catastrophic costs between the two cohorts (Private: 55% vs NTP: 52%;P=0.675). Factors associated with catastrophic cost included being a single-person household [adjusted odds ratio [(aOR=13.71; 95% confidence interval (CI): 1.36–138.14;P=0.026], unemployment during treatment (aOR=10.86; 95%CI: 2.64–44.60;Pstigma (aOR=37.90; 95%CI: 1.72–831.73;P=0.021).Conclusions Persons with TB in Viet Nam face similarly high risk of catastrophic costs whether treated in the

The functionalities and diverse metastable phases of multiferroic BiFeO3（BFO） thin films depend on the misfit *** mixed phase-induced strain relaxation in multiphase BFO thin films is well known,it is unclear whether a singlecrystalline BFO thin film can accommodate misfit strain without the involvement of its ***,understanding the strain relaxation behavior is key to elucidating the lattice strain-property *** this study,a correlative strain analysis based on dark-field inline electron holography（DIH） and quantitative scanning transmission electron microscopy（sTEM） was performed to reveal the structural mechanism for strain accommodation of a single-crystalline BFO thin *** nanoscale DIH strain analysis results indicated a random combination of multiple strain states that acted as a primary strain relief,forming irregularly strained *** sTEM-based bond length measurement of the corresponding strained nanodomains revealed a unique strain accommodation behavior achieved by a statistical combination of multiple modes of distorted structures on the unit-cell *** globally integrated strain for each nanodomain was estimated to be close to-1.5%,irrespective of the nanoscale strain states,which was consistent with the fully strained BFO film on the *** functional theory calculations suggested that strain accommodation by the combination of metastable phases was energetically favored compared to single-phase-mediated *** discovery allows a comprehensive understanding of strain accommodation behavior in ferroelectric oxide films,such as BFO,with various low-symmetry polymorphs.

Commercial purity Ti was subjected to channel die compression in liquid nitrogen for the purpose of studying its deformation characteristics from the viewpoint of the grain refinement induced by severe plastic *** specimens showed microstructural heterogeneity in that the initial blocky grains of about 50 urn in size turned into a mixture of the easily deforming soft grains and the hard grains revealing local concentration of dislocations and deformation *** electron back scattered diffraction analysis the in-grain misorientation-axis distribution was studied,from which it was found that the deformation heterogeneity resulted from the anisotropy of individual grains with respect to the compression die:while the soft grains were those oriented favorably for the prismslip,the hard ones were oriented for non-prism slips and deformation *** difference in the response of individual grains,therefore,led to a remarkable effect of the crystal texture of specimens on the maximum strain that can be imposed on specimens without *** on the present results,a way to achieve expedient grain refinement through the cryogenic plane-strain compression was suggested.

As an interdisciplinary product,water-soluble gold nanoclusters(AuNCs) stabilized by ligands containing carboxyl(-COOH) group have garnered significant attention from synthetic chemists and biologists due to their immense potential for biomedical ***,revealing the crystallographic structures of-COOH-functionalized AuNCs remains a ***,we successfully applied the salting-out method to obtain a series of high-quality single crystals of-COOH-functionalized Au25nanoclusters and revealed their crystallographic ***,K3Au25(2-Hmna)9(mna)6]-(Au25a)protected by 2-mercaptonicotinic acid features an unprecedented tetrameric Au4(sRs)3(sRs,N)2staple motifs surrounding the icosahedral Au13kernel,breaking the traditional perception on the structure of Au25(sR)***25a exhibits a distinct near-infrared emission at 970 nm with long lifetime of 8690 ns,which have been studied by transient absorption spectroscopy and time-dependent density functional *** work compensates for the research gap in the experimental structure of-COOHfunctionalized AuNCs and opens up a new avenue to explore their structure-property correlations.

Geocenter motion (GCM) is one important topic for constructing and maintaining the terrestrial reference frame and its applications. GCM is studied from CHAMP with the multi-step approach in this paper. Geometric orbits of CHAMP in 2001-2006 are precisely determined with the kinematic method only from the satel-lite-borne GPs zero-difference data. Then a GCM time series is estimated from the precise kinematic orbits based on the theory of satellite dynamics to fit the CHAMP’s real geometric orbits. We compare the series with the geocenter series used in ITRF2005. Then the GCM series are analyzed with Fourier transform and wavelet transformation. The mean motions within 6 years in TX, TY and TZ direc-tions are respectively 0.8 mm, 2.2 mm, and 7.9 mm. The trends of GCM in the three directions are 0.495 mm/a, -0.004 mm/a, and 1.309 mm/a, respectively. The long-term movement (2001-2006) indicates that the crustal figure is changing. The seasonal variations are the main component which may be excitated by the mass redistribution of Earth’s fluid layer, e.g. ocean, atmosphere and continental water. The inter-annual variations are also found in the GCM series measured with CHAMP.

Cartan-Fubini type extension of holomorphic maps respecting varieties of minimal rational tangents

A fundamental result in the theory of minimal rational curves on projective manifolds is Cartan- Fubini extension theorem proved by hwang and Mok, which describes the extensibility of biholomorphisms between connected open subsets of two Fano manifolds of Picard number 1 which preserve varieties of minimal rational tangents （VMRT）, under a mild geometric assumption on the second fundamental forms of VMRT＇s. Hong and Mok have developed Cartan-Fubini extension for non-equidimensional holomorphic immersions from a connected open subset of a Pano manifold of Picard number 1 into a uniruled projective manifold, under the assumptions that the map sends VMRT＇s onto linear sections of VMRT＇s and it satisfies a mild geometric condition formulated in terms of second fundamental forms on VMRT＇s. In the current paper, we give a generalization of Hong and Mok＇s result, under the same condition on second fundamental forms, assuming only that the holomorphic immersions send VMRT＇s to VMRT＇s. Our argument is different from Hong and Mok＇s and is based on the study of natural foliations on the total family of VMRT＇s. This gives a substantially simpler proof than Hong and Mok＇s argument.

Background It is debatable whether treating multimorbid nursing home patients with antihypertensive drugs produces beneficial effects. Most cardiovascular guidelines promote treatment; few have advice on how to deprescribe when treatment may no longer be necessary. We investigated the effect of medication review on antihypertensive drug use and the association between cognition, blood pressure, and prescribing. Methods From August 2014 to December 2015, 765 patients from 72 units （clusters） in 32 Norwegian nursing homes were included in a 4-month, multicentre, cluster-randomized, controlled trial, with 9-month follow-up. Patients ≥ 65 years old with antihypertensive treatment （n = 295, 39%） were randomized to systematic medication review where the physician received support from peers （collegial mentoring） or were given care as usual （control condition）. Outcome measures were the number of antihypertensive drugs, systolic blood pressure, and pulse. We used hospitalizations and deaths as criteria to assess harm. Results At baseline, each patient used 9.2 ± 3.5 regular drugs, and 1.6 ± 0.7 antihypertensives. Mean blood pressure was 128/71 mmHg and 9% had a systolic pressure ≥ 160 mmHg. Between baseline and month four, antihypertensives were deprescribed to a significantly higher extent in the intervention group （n = 43, 32%） compared to control （n = 11, 10%）; Incidence Rate Ratio = 0.8, 95% CI = 0.7？0.9. In the intervention group, there was an immediate increase in systolic blood pressure when antihypertensives were reduced, from baseline 128 ± 19.5 mmHg to 143 ± 25.5 mmHg at month four. However, at month nine, the blood pressure had reverted to baseline values （mean 134 mmHg）. Deprescription did not affect pulse and systolic pressure. The number of hospitalizations was higher in control patients at month four （P = 0.031） and nine （P = 0.041）. The quality of satellite radar altimetric data is very important in studies of geodesy,geophysics,and *** coastal oceans,altimeter waveforms are contaminated by the terrain and physical environments so that the accuracy of altimeter data is lower than that over open *** we develop a new multi-subwaveform parametric retracker(MsPR) to improve the quality of altimeter data for the recovery of gravity anomaly in coastal *** least squares collocation method is used to recover the residual gravity anomaly over the coastal water from altimetric *** waveform data records from Geosat/GM around Taiwan Island are practically retracked with *** compared with the Taiwan geoid height,the results retracked by MsPR are more accurate than those retracked by the well-known β-5-parmeter method and from the geophysical data records(GDRs).The gravity anomalies over Taiwan coastal waters are calculated from the retracked altimeter data with the least squares *** we compared gravity anomalies computed using altimeter GDRs with the ship-borne gravity data over Taiwan coastal ocean,we found that the results from retracked data are more accurate than those from GDRs.
