Characterizing the patient experience during neoadjuvant therapy for pancreatic ductal adenocarcinoma:A qualitative study

BACKGROUND Neoadjuvant therapy(NT)has increasingly been utilized for patients with localized pancreatic ductal adenocarcinoma(PDAC).It is the recommended approach for borderline resectable(BR)and locally advanced(LA)cancers and an increasingly utilized option for potentially resectable(PR)*** its increased use,little research has focused on patient-centered metrics among patients undergoing NT,including patient experiences,preferences,and recommendations.A better understanding of all aspects of the patient experience during NT may identify opportunities to design interventions aimed at improving quality of life;it may also facilitate the completion of NT and receipt of surgery,ultimately optimizing long-term *** To understand the experience of patients initiating and receiving NT to identify opportunities to improve neoadjuvant cancer care *** Semi-structured interviews of patients with localized PDAC during NT were conducted to explore their experience initiating and receiving *** took place between August 2020 and October *** to the descriptive nature of the research,questions were open *** were conducted over the phone,audio recorded and then *** interviews were coded by two independent researchers using NVivo 12,iteratively identifying themes until thematic saturation was *** integrative approach to qualitative analysis was used,utilizing both inductive and deductive *** A total of 12 patients with localized PDAC were *** with BR(n=7),PR(n=2),and LA(n=3)cancers participated in the *** patients indicated that choosing NT was the doctor’s recommendation,while most reported not being familiar with the concept of NT(n=11)and that NT was presented as the only option(n=8).Five themes describing the patient experience emerged:physical symptoms,emotional symptoms,coping mechanisms,access to care,and life *** most commonly cited recommendation for

In the issue of March 25,2021 of the New England Journal of Medicine,Newsome et *** the results of a phase II clinical trial of semaglutide,a glucagon-like peptide-1 receptor agonist(GLP-1RA),given once-daily,subcutaneously for 72 weeks for the treatment of non-alcoholic steatohepatitis(NASH)(1).The authors found that among 230 NASH patients with fibrosis stage of F2 or F3 the primary endpoint of the study(NASH resolution with no worsening of fibrosis)was achieved in 59%of patients with semaglutide at a dose of 0.4 mg compared to 17%in the placebo group(OR 6.87;95%CI:2.60-17.63;P<0.001).

Dear Editor, The introduction of intracytoplasmic sperm injection （ICSI） technology has revolutionised the treatment of patients with severe oligozoospermia and azoospermia. A patient is usually considered oligozoospermic if his sperm concentration is less than 20 million mL^-1. A patient is labelled azoospermic according to the World Health Organisation guideline , if, on two different occasions, no spermatozoa can be detected by high-power microscopy after the seminal fluid has been centrifuged for 15 min at a centrifugation force of 3 000 × g or greater.

Colorectal cancer vaccines: Tumor-associated antigens vs neoantigens

Therapeutic options for the treatment of colorectal cancer(CRC) are diverse but still not always satisfying. Recent success of immune checkpoint inhibition treatment for the subgroup of CRC patients suffering from hypermutated tumors suggests a permanent role of immune therapy in the clinical management of CRC. Substantial improvement in treatment outcome could be achieved by development of efficient patient-individual CRC vaccination strategies. This mini-review summarizes the current knowledge on the two general classes of targets: tumor-associated antigens(TAAs) and tumorspecific antigens. TAAs like carcinoembryonic antigen and melanoma associated antigen are present in and shared by a subgroup of patients and a variety of clinical studies examined the efficacy of different TAA-derived peptide vaccines. Combinations of several TAAs as the next step and the development of personalized TAA-based peptide vaccines are discussed. Improvements of peptidebased vaccines achievable by adjuvants and immunestimulatory chemotherapeutics are highlighted. Finally, we sum up clinical studies using tumor-specific antigens-in CRC almost exclusively neoantigens-which revealed promising results; particularly no severe adverse events were reported so far. Critical progress for clinical outcomes can be expected by individualizing neoantigen-based peptide vaccines and combining them with immunestimulatory chemotherapeutics and immune checkpoint inhibitors. In light of these data and latest developments, truly personalized neoantigen-based peptide vaccines can be expected to fulfill modern precision medicine's requirements and will manifest as treatment pillar for routine clinical management of CRC.

The onset of global urbanization has led to a population shift from rural to urban areas and the expansion of urban development [1-3]. Urban land areas are growing at faster rates than their populations in most parts of the world [4]. The urban sprawl process refers to dispersed land development characterized by low-density, unplanned, and uneven patter ns of growth duri ng urban expansion. This process can result in inefficient use of land resources contradictory to the principle of sustainable development [5]. Additionally, urban sprawl has various environmental, economic, and social consequences, including the following: loss of agricultural land, higher costs for transportation infrastructure, increased landscape fragmentation, degeneration of soil ecological functions, and reduced ecosystem resilience. It is crucial to understand the trends and patterns of urban sprawl to implement a framework for sustainable urban development in developing regions.

A Study of Radiation-Induced Instability for the Gene Locus Associated with Intellectual Disorders or Developmental Delays

Multiplex Ligation-Dependent Probe Amplification (MLPA) was used to study the integrity of the chromosomes for two WIL2-derived lymphoblastoid cell lines (TK6 and WTK1) in the presence and absence of ionizing radiation. WTK1 cells contain a p53 mutation, whereas the TK6 cell line has the native p53 tumor-suppressor gene. Each cell line was isolated pre- and post-irradiation (2 and 3 Gy) and analyzed by MLPA. Using probes that target specific regions on chromosomes associated with a distinct subset of microdeletions and microduplications either established or thought to be responsible for intellectual disability or developmental delay, we have demonstrated that WTK1 and TK6 are not impacted in the same way by irradiation. Instead, each cell line presents its own unique MLPA profile. The most notable differences are the appearance of nine unique probe signals only seen in WTK1 cells. These results are important in the study of how different cell lines can be affected in significantly different ways depending on the presence or absence of wild type p53.

Complex Effective Relative Permittivity of Soil Samples from the Taunus Region (Germany)

The most important parameter affecting ground-penetrating radar （GPR） measurements is the complex effective relative permittivity εr^＊,eff because it controls the propagation velocity and the reflection of GPR pulses. Knowing εr^＊,eff of soils passed through by electromagnetic waves increases accuracy in soil thickness and interface identification. Complex effective relative permittivity εr^＊,eff= εr^＊,eff - jεr^＊,effof 25 soil samples with textures ranging from loamy sand to silty clay was measured using the two-electrode parallelplate method. The measurements were conducted at defined water contents for frequencies from 1 MHz to 3 GHz. The results confirm the frequency dependence of εr^＊,eff and show that the dielectric behavior of soil-water mixtures is a function of water content. Applying the experimental data of this study with predictions based on the empirical model by Toppet aL （1980）, we find that Topp et aL＇s curve tends to underestimate the real part of εr^＊,eff measured. Along with frequency and water content, soil texture and organic matter affect soil permittivity. Moreover, the real part of εr^＊,eff increases at higher dry bulk densities. Output from our calibration model enables us to predict εr^＊,eff for the soil samples which were tested under the actual in situ soil water content. This results in high accuracy of soil thickness prediction.

Aims Soil sample preservation is a challenging aspect in molecular studies on soil microbial *** demands for specialized sample storage equipment,chemicals and standardized protocols for nucleic acid extraction often require sample processing in a home laboratory that can be continents apart from sampling *** sampling procedures,especially when dealing with RNA,comprise immediate snap freezing of soils in liquid nitrogen and storage at−80℃until further *** these instances,organizing a reliable cooling chain to transport hundreds of soil samples between continents is very costly,if possible at *** this study we tested the effect of soil sample preservation by freeze-drying with subsequent short-term storage at 4℃or ambient temperatures compared to−80℃freezing by comparative barcoding analyses of soil microbial *** Two grassland soil samples were collected in Central Germany in the Biodiversity Exploratory Hainich-Dü*** were freeze-dried or stored at−80℃as ***-dried samples were stored at 4℃or ambient *** storage times for both storage temperatures were 1 and 7 *** DNA and RNA were extracted and bacterial and arbuscular mycorrhizal(AM)fungal communities were analyzed by amplicon 454 pyrosequencing of the 16S(V4-V5 variable region)and 18S(NS31-AM1 fragment)of ribosomal RNA(rRNA)marker genes,*** Findings Bacterial communities were sufficiently well preserved at the rDNA and rRNA level although storage effects showed as slightly decreased alpha diversity indices for the prolonged storage of freeze-dried samples for 7 *** fungal communities could be studied without significant changes at the rDNA and rRNA *** results suggest that proper sampling design followed by immediate freeze-drying of soil samples enables short-term transportation of soil samples across continents.
