First Nationwide Survey on the Resistance to First Line Anti-Tuberculosis Drugs in Ghana

Background/objective: A nationwide survey on the resistance to first line anti-tuberculosis (anti-TB) drugs was conducted in Ghana from 2007-2008 by Noguchi Memorial Institute for Medical Research in collaboration with the National Tuberculosis Control Programme. We aimed to characterize mycobacterial species causing pulmonary tuberculosis (PTB) and determine the resistance pattern to first line anti-TB drugs among newly diagnosed and previously treated PTB patients in Ghana. Methods: Two sputum samples from consented new smear positive PTB patients who had never been treated for TB or had been on anti-TB treatment for less than a month and patients who had been treated for TB previously for more than a month in selected diagnostic centres nationwide were collected for culture, identification and drug susceptibility test. Culture positive isolates were tested against streptomycin (S), isoniazid (h), rifampicin (R) and ethambutol (E) using the simplified proportion method and line probe assay (LPA). The LPA was performed in mid-2017. Results: Among 410 samples, 345 positive cultures were obtained and identified as Mycobacterium tuberculosis complex (MTBC). Of the 345 isolates, 133 were further differentiated by GenoType MTBC®as M. tuberculosis, 126 (94.7%) and M. africanum 7 (5.3%). The overall drug resistance patterns were as follows: 43/345 (12.5%), 6/345 (1.7%), 9/345 (2.6%) and 71/345 (20.6%) were resistant to h, R, E and S respectively and 5/345 (1.4%) were multi-drug resistant (MDR). Conclusion: The results indicate high levels of resistance to S and h among new and previously treated TB patients. We recommend adequate surveillance systems including periodic national anti-TB drug resistance surveys.

The Bambili maars are twin contemporaneous maars embedded in trachytic rocks. The two maars are separated by a low lying inter-Maar dome whose inner walls dip towards the Southern Maar. The Bambili Northern Maar (BNM) contains water and its floor is found at an altitude lower than the Bambili Southern Maar (BSM) (50 m) which is swampy and drains into the BNM. Both maars have very steep vertical walls. Three different units are identified in the area. Unit 1 is 1 m thick sequence, characterize by the presence of matrix supported fine-grained fallouts (ash and lapilli size) with stratified diffused bedding planes. This unit is further subdivided into three different layers: L1, L2 and L3 from bottom to top with increasing sizes of particles respectively. This layer shows reverser grading and its grain sizes rarely exceed 2 cm and this unit shows no depositional structures. Unit 2 is an unstratified unit predominantly made up of tephra deposits. Within these deposits are found blocks of sizes ranging between 2 and 10 cm of different petrographic types. They include scoria, basalts, trachyte, and granite cognate. The last unit, Unit 3 is distinctly stratified characterized by well sorted, heterolithologic, graded bedding layers which are matrix supported. Base on the composition and grain sizes, this unit was subdivided into 7 horizons: h1 - h7. Within this unit, two sequences of deposits are distinguished: U1 and U2. Both sequences are characterized by alternation of lapilli and tuff beds. The clast comprises of juvenile materials, mudstone which are highly vesicular and altered rock fragments. Other products identified within the area are ferolithic volcanic bombs (5 - 20 cm), xenocrystals (2 - 4 cm) within finer deposits. They are elongated and show visible twinned planes. Explosion breccias are equally identified occurring as bolders. Along the inner walls of the maars are also found large basaltic and trachytic blocks which are remnants of pre-maar unit within maar

Background: hypertension is the leading cause of cardiovascular disease worldwide. hypertensive disorders in pregnancy also constitute a major global health threat. There are different types of hypertension that can occur during pregnancy;with different mechanisms and consequences for mother and fetus. Objectives: To determine the frequency of hypertension (hypertension) during pregnancy. Document the risk factors for pregnancy-related hypertension. Review the material and fetal complications which determine the prognosis. Methods and Patients: This was a retrospective cross-sectional study from January 1 to June 30, 2022 in the Obstetrics and Gynecology Department of the Brazzaville hospital and University Center. We noted 72 cases of hypertension among 1188 births admitted during the reference period. hypertension was defined as blood pressure 40/90mmhg. Results: hypertension was observed in 6% of those giving birth. The average age of the patients was 28 ± 8 years (range 15 to 39 years). The age group of 15 to 34 years was the most affected, 56 cases (778%). The risk factors were young age (15 - 34 years), late transfer of women in labor, 49 cases (68%). Primiparity, 33 cases (45.8%), absence of prenatal consultation, 7 cases (9.7%), hypertension, 8 cases (11%), twinning, 4 cases (5%), fetal macrosomia, one case (1.3%). Maternal complications recorded were: 21 cases of eclampsia (29%), 6 cases of left ventricular failure (8.3%), 5 cases of anemia (6.9%), 2 cases of retroplacental hematoma (2.8%), 1 case of hELLP syndrome, as much renal failure. Fetal and neonatal complications were 23 cases of prematurity (32%), 10 cases of acute fetal distress (14%), 4 cases of hypotrophy (5.6%). Eleven cases of death (15.3%) were: 3 in utero. B in the neonatal period.

Stability Analysis of Electromagnetic Ordinary and Extraordinary Modes

By using kinetic theory, we derived the general dispersion relations for ordinary mode (O-mode) and Extra-ordinary mode (X-mode) in anisotropic magnetized plasma. The effects of energy anisotropy, magnetic field to density ratio () and the plasma beta on the propagation characteristics, have been analyzed. The stability analysis and the growth rates have been presented. The marginal threshold condition for oscillatory and purely growing mode has been obtained for higher harmonics and we have also calculated their growth rates in terms of plasma beta and energy anisotropy . The X-mode satisfies the instability condition according to difference of geometry with the O-Mode. These modes are important for spherical tokamaks, and their coupling leads to the generation of the Bernstein mode, which causes the heating effects.

Root hairs are single cells that develop by tip growth, a process shared with pollen tubes, axons, and fungal hyphae. however, structural plant cell walls impose constraints to accomplish tip growth. In addition to polysaccharides, plant cell walls are composed of hydroxyproline-rich glycoproteins （hRGPs）, which include several groups of O-glycoproteins, including extensins （EXTs）. Proline hydroxylation, an early post-translational modification （PTM） of hRGPs catalyzed by prolyl 4-hydroxylases （P4hs）, defines their subsequent O-glycosylation sites. In this work, our genetic analyses prove that P4h5, and to a lesser extent P4h2 and P4h13, are pivotal for root hair tip growth. Second, we demonstrate that P4h5 has in vitro preferred specificity for EXT substrates rather than for other hRGPs. Third, by P4h promoter and protein swapping approaches, we show that P4h2 and P4h13 have interchangeable functions but cannot replace P4h5. These three P4hs are shown to be targeted to the secretory pathway, where P4h5 forms dimers with P4h2 and P4h13. Finally, we explore the impact of deficient proline hydroxylation on the cell wall architec- ture. Taken together, our results support a model in which correct peptidyl-proline hydroxylation on EXTs, and possibly in other hRGPs, is required for proper cell wall self-assembly and hence root hair elongation in Arabidopsis thaliana.

Ecologic study of serum selenium and upper gastrointestinal cancers in Iran

AIM:Both observational and experimental studies have shown that higher selenium status reduces the risk of upper gastrointestinal cancers in selenium deficient populations. Recent cancer registry data have shown very different rates of esophageal cancer(EC)and gastric cancer(GC)in four Provinces of Iran,namely Ardabil,Mazandaran,Golestan, and *** aim of this study was to have a preliminary assessment of the hypothesis that high rates of EC in Golestan and high rates of GC in Ardabil may be partly attributable to selenium deficiency. METhODS:We measured serum selenium in 300 healthy adults from An:labil(n=100),Mazandaran(n=50),Golestan (n=100),and Kerman(n=50),using inductively coupled plasma,with dynamic reaction cell,mass spectrometry(ICP- DRC-MS)at the US Centers for Disease Control(Atlanta, Georgia). RESULTS:The median serum selenium concentrations were very different in the four *** medians(IQR)for selenium in Ardabil,Mazandarn,Golestan,and Kerman were 82(75-94),123(111-132),155(141-173),and 119 (110-128)μg/L,respectively(Phe results of linear regression showed that the Province variable,by itself, explained 76% of the variance in log selenium(r^2=0.76). The proportion of the populations with a serum selenium more than 90 μg/L(the concentration at which serum selenoproteins are saturated)was 100% in Golestan, Kerman,and Mazandaran but only 29% in Ardabil. CONCLUSION:Our findings suggest that selenium defidency is not a major contributor to the high incidence of EC seen in northeastern Iran,but it may play a role in the high incidence of GC in Ardabil Province. Paraclinical, Therapeutic and Evolutionary Profile of Osteoarticular Tuberculosis: A Report about 550 Cases in Abidjan

Objective: To describe paraclinical, therapeutic and evolutionary aspects of osteoarticular tuberculosis in Abidjan. Materials and Method: We realized a descriptive retrospective study carried out in the Rheumatology Department of the University hospital Center of Cocody in Abidjan about 550 patients over a period of 10 years from January 2004 to December 2013. The paraclinical, therapeutic and evolutionary data had interested us. Results: Osteoarticular tuberculosis represented 12.13% of 4531 rheumatic diseases identified during the study period. The tuberculin skin test was positive in 78.49% of cases. An inflammatory syndrome was present with erythrocyte sedimentation rate and C-reactive protein respectively into 69.45% and 76.54% of cases. The research of Mycobacterium tuberculosis was realized into 176 of cases which 50% was positive. Polymerase chain reaction and biopsy of tissue were performed respectively in 8 and 6 cases. Plain radiography (89.82%) and computed tomography (83.63%) were the most realized imaging. Computed tomography has contributed for the diagnostic in 99.78% of cases. The majority of our patients (82.91%) received anti-tuberculous treatment which consisted of two months of combination of rifampicin (R), isoniazid (h), pyrazinamide (Z) and Ethambutol (E) (2RhZE), followed by 10 months of rifampicin (R) and isoniazid (h) (10 Rh). The outcome in patients on this treatment was favorable in 528 cases (96%). Conclusion: This study revealed that the research of Mycobacterium tuberculosis was often realized and positive. On the contrary, polymerase chain reaction and biopsy of tissue were rarely performed. The main imaging is computed tomography. Our protocol of treatment is effective.

为了更好地理解各种配位聚合物[MX2y](M=Zn,Cd:X=C1,Br:py=m二卤代吡啶)的电子结构差异,我们在100K收集了五个配位聚合物单晶(图1a)的高分辨率X光衍射数据,用hansen-Coppens非球型对称的原子中心多极子电子密度模型进行了优化,以Bade的QTAIM为工具作了拓扑分析。除了得到传统化学键的键临界点外,还观测到了弱相互作用的(3,-1)型临界点。例如,PY中的对位h原子与另一相邻MX链中的卤离子(图1c);相邻晶格的吡啶环上的卤代基R...R等。并用量子化学从头算方法计算了弱相互作用的强度。

A high angular resolution near-infrared image that shows the intensity of polarization for the GG Tau A binary system was obtained with the Subaru Telescope. The image shows a circumbinary disk scattering the light from the central binary. The azimuthal profile of the intensity of polarization for the circumbinary disk is roughly reproduced by a simple disk model with the henyey-Greenstein phase function and the Rayleigh function, indicating there are small dust grains at the surface of the disk. Combined with a previous observation of the circumbinary disk, our image indicates that the gap structure in the circumbinary disk orbits counterclockwise, but material in the disk orbits clockwise. We propose that there is a shadow caused by material located between the central binary and the circumbinary disk. The separations and position angles of the stellar components of the binary in the past 20 yr are consistent with the binary orbit with a = 33.4 AU and e = 0.34.
