Relationship between Macroinvertebrates and Physico-Chemical Parameters to Access Water Quality of the Affon River in Bénin

The Affon River is one of the important rivers of the Ouémé River whose benthic diversity remained unknown. The present study aims to make the relationship between macroinvertebrates and physico-chemical parameters to access water quality of the Affon River. The measurement of physico-chemical parameters (temperature, pH, conductivity, transparency, depth and TDS, dissolved oxygen, ammonium, nitrite and phosphate) and the sampling of macroinvertebrates using a Surber net were carried out during floods in eight stations. A principal component analysis (PCA) and canonical correspondence analysis (CCA) were used. The indices of Shannon, Piélou, Simpson, Hilsenhoff, EPT and EPT/Chironomidae were used to assess the level of water pollution. The study identified 9755 macroinvertebrates divided into 4 classes, 14 orders and 49 families. Pollution-sensitive families (14 families) that are organic pollution indicators, as well as pollution-tolerant families (Chironomidae, Limnaeidae, Bithynidae, Physidae) were captured. Chironomidae were the most predominant and abundant family (FO = 100%). The predominance of Chironomidae coupled with the rarity of the Ephemeroptera, Trichoptera and Plecoptera would reflect the poor quality of the Affon River waters. The principal components analysis yielded groups of associations: The first group of stations Taneka 2 and 3 characterized by high values of ammonium and phosphate;and low values of conductivity and TDS;the second group of Tanéka 1 and Kolokondé 1 stations with low values of pH, transparency, ammonium and phosphates;and the third group of stations Kolokondé 2, Kpébouko1, Kpébouko 2 and Affon marked by high values of conductivity, TDS, transparency, depth and temperature. The canonical correspondence analysis revealed a strong positive correlation between Nemouridae and ammonium as well as between Perlidae, Taeniopterygidae, Ephemeridae, Heptageniidae, Isonychiidae, Elmidae and phosphate. Medication Error (ME) in the Pediatrics Service of the University Teaching Hospital (CHU) of Bouaké(Côte d’Ivoire)

Introduction: Medication error (ME) is a common cause of childhood morbidity and mortality reported in developed countries. It has not yet been described in the pediatric services in Cote d’Ivoire. Objective: To evaluate ME in pediatrics for the improvement of professional practice. Materials and Methods: This was a cross-sectional, descriptive and analytical study conducted in the service of pediatrics at the University Teaching Hospital (CHU) of Bouaké from January 11 to February 25, 2016. It involved 204 children hospitalized and treated with drugs. The variables studied were the prescription, dispensing and administration of the drugs. Results: A total of 483 prescriptions notes were recorded for 733 therapeutic lines, i.e. 2.3 prescriptions notes and 3.6 lines per child. The overall prevalence of ME was 31% (prescription 83%, dispensing 0%, administration 11%). The prescription error concerned the illegible writing (20%), the omission of the stamp & signature (47%), the omission of the date (33%), weight (66%), age (25%) and name of the child (9%).The drug name and the dosage were incorrect in 99% and 1% respectively. Regarding the administration error, care was provided by a person without a nursing qualification in 65.5% of cases. On the patient’s care record, the care giver’s name was omitted in 100%, the child’s name in 57% and the date in 47%. The route of administration and the administered dose were each incorrect in 5%. The student nurses and assistant nurse caregivers committed more prescription errors than doctors and nurses and midwives (p= 0,000 X2 = 44.91). Administration error was not significantly related to the service, the socio-professional category, the gender and the age of the patient. Conclusion: EM is common in pediatrics at the University Teaching Hospital (CHU) of Bouaké. To prevent it, it is necessary to strengthen the staff’s capacity to prescribe and administer the drug in children.

This theoretical chemical reactivity study was conducted using the Density Functional Theory (DFT) method, at computational level B3LYP/6-31G (d). It involved a series of six (06) 5-arylidene rhodanines and allowed to predict the chemical reactivity of these compounds. DFT global chemical reactivity descriptors (HOMO and LUMO energies, chemical hardness, softness, electronegativity) were examined to predict the relative stability and reactivity of rhodanin derivatives. Thus, the compound 6 which has an energy gap between the orbitals of ΔEgap = 3.004 eV is the most polarizable, the most reactive, the least stable, the best electron donor and the softest molecule. Calculation of the local indices of reactivity as well as dual descriptors revealed that the sulfur heteroatom of the Rhodanine ring is the privileged site of electrophilic attack in a state of sp3 hybridization and privileged site of nucleophilic attack in a state of sp2 hybridization.

Background: Neonatal mortality remains a major public health problem in low income countries. The survival within the first 28 days of life remains a challenge in such countries. Many strategies have been implemented to reduce deaths in children under five especially in sub-Saharan Africa. Laquintinie Douala Hospital benefits from some of these measures including a perinatal network and an emergency voucher. We aimed to describe the main causes of neonatal deaths at Laquintinie Douala Hospital. Methods: We carried out a cross-sectional study including files of all deceased neonates in the neonatal unit during a 24 months period from January 1st, 2017 to December 31st, 2018. Data collection included socio-demographic characteristics of the mothers and the newborns, clinical and therapeutic data and the evolution of the newborn. Assessment of Dietary Habits and Nutritional Status of Women of Childbearing Age in Cameroon: A Cross Sectional Study

Background: Malnutrition due to poor feeding habits, is still a serious public health issue in Cameroon. The objective of this study was to assess the dietary patterns and nutritional status of women of childbearing age in various geographical settings in Cameroon so as to propose intervention strategies. Methods: In a cross sectional study conducted from January 2014 to August 2015, women aged 14 - 49 years were randomly selected from the capital city Yaounde and four regions of Cameroon. Anthropometric measures were collected for nutritional status assessment. Data on diet habits and socio-demographic parameters were collected. Results: 608 women of reproductive age with average age 34.68 ± 0.39 years made up the study. Dietary patterns assessment revealed that, most study participants had a frequent intake of carbohydrate rich foods and fats and oils rich foods;but an infrequent intake of protein rich foods and vitamin and minerals rich foods in a week. Nutritional status assessment revealed that, 2.1% were underweight, 28.6% had a normal weight, 30.8% overweight, and 38.5% obese. The grass field regions presented the highest prevalence of underweight, overweight and obesity. Higher malnutrition levels were linked to low intakes of pulses and beans;milk and dairy products;vegetables and fruits food groups. Conclusion: Women of the study population had an unhealthy dietary pattern and a poor nutritional status. Therefore, strategies such as nutritional education are warranted and should be age and region specific, so as to target specific groups of women and ensure adequate nutritional status and health.

Childhood Heart Disease in Côte d’Ivoire: Retrospective Multicentric Study about 228 Cases

Introduction: In Cote d’Ivoire, the paediatrician has to face a diagnosis and management problem in front of pediatric heart disease, with consequent a high lethality. Objective: To describe the epidemiological, diagnostic, therapeutic and progressive aspects of childhood cardiopathy for the improvement of prognosis and professional practice. Materials and methods: This was a multicenter retrospective and descriptive study conducted from January 2011 to March 2016 in two main hospitals universities, one located in Abidjan and the other in Bouaké. It concerned children aged 0 to 15 admitted for cardiac disease diagnosed on clinical and/or echocardiographic arguments. The variables studied were the epidemiological, diagnostic, therapeutic and evolutionary aspects. Results: A total of 49.760 admissions including 228 infantile heart disease cases (congenital 113, acquired 51, indeterminate 64) are overall prevalence of 4.6‰. They involved 106 boys and 122 girls. In 74.6% of cases, the age was between 0 and 2 years. Respiratory distress 73.7% was the main reason for consultation. The main congenital heart diseases (CHD) are ventricular septal defect (VSD) (31%), atrial septal defect (ASD) (20.4%), atrioventricular canal (AVSD) (12.4%) and tetralogy of Fallot (TOF) (11.5%). As for acquired forms (AHD), rheumatic mitral insufficiency (41.2%) and tuberculous pericarditis (15.7%) were the two main causes. Treatment progress is marked by stabilization (71.1%) and death (14%). Death was significantly associated with low socioeconomic status (p = 0.01) and with complication (p 0.001). Conclusion: Infantile heart diseases are relatively less frequent and serious in Cote d’Ivoire. To improve the prognosis, close collaboration between the pediatrician and the cardiologist is required for early diagnosis and management.

Congenital nephrotic syndrome (CNS) is defined as the presence of proteinuria > 50 mg/kg/24h associated with a protein concentration g/L or albuminemia 30 g/L in an infant less than 3 months old. The CNS is rare, of various clinical forms dominated by the Finnish type caused by a mutation of the NPHS1 gene located on chromosome 19. The edematous syndrome is the most common mode of discovery. We report a case discovered in an infant of 50 days admitted for ascites of great abundance. The aim of this study was to describe the main epidemiological, diagnostic, therapeutic and evolutionary aspects of this syndrome. Improving the prognosis of this condition requires advocacy with the political authorities of Cote d’Ivoire to provide Teaching Hospital for the resources needed to perform kidney transplantation.

Anesthetic Management of Hepatic Resection in a Low-Resource Setting: First Bulletin from Africa

Introduction: General anesthesia and antisepsis have made intraperitoneal surgery safer. With the improvement of anesthetic techniques and the development of new surgical approaches, the liver, an organ with high haemorrhagic risks, has become the subject of many successful therapeutic indications. Although widely performed in high-income settings due to a better technical platform, the reverse is true for low-resource settings in Africa where there is little or no report on the anesthetic management of this dreadful surgery. Hence, this study is one of the first from Africa to report on the perioperative anesthetic management and outcome of hepatic resections. Methods: This was a retrospective case series study conducted over four years (2019-2022) through a chart review of all medical and anesthesia records of patients admitted to Douala General Hospital, Cameroon for hepatic resection. We collected socio-demographic, clinical, laboratory and intraoperative data, the estimated financial cost and patient outcomes. Results: Twelve open hepatic resections were performed mainly electively (11/12 cases) for localized hepatic tumours (7/12 cases) on ASA II patients (11/12 cases) with a mean age of 36.5 years and sex ratio of 1.2. Pre-anesthetic consultations were usually done within 24 days before the surgery. General anesthesia maintained with sevoflurane was the mainstay anesthetic technic. Continuous hemodynamic monitoring with an arterial catheter was done. The blood-sparing strategy was based on tranexamic acid, calcium gluconate and controlled hypotension with an intraoperative transfusion rate of 33.33%. The cost of the procedures was well above the minimum wage in Cameroon but relatively cheap compared to the cost in high-income countries. No perioperative death was recorded. Conclusion: Our hepatic resections were managed with satisfactory blood savings, low morbidity and zero perioperative mortality. Overall, this study is the first to provide data on perioper

With the current limited drug therapy for the core symptoms of autism spectrum disorder(ASD),we herein report a randomized,double-blind,placebo-controlled trial to investigate the efficacy,safety,and potential neural mechanism of bumetanide in children with ASD aged 3-6 years old.A total of 120 children were enrolled into the study and randomly assigned to either 0.5 mg bumetanide or *** the final sample,119 children received at least one dose of bumetanide(59 children) or placebo(60 children) were included in the final *** primary outcome was a reduction in the Childhood Autism Rating Scale(CARS) score,and the secondary outcomes were the Clinical Global Impressions Scale(CGI)-Global Improvement(CGI-I) score at 3 months and the change from baseline to 3-month in the Autism Diagnostic Observation Schedule(ADOS).Magnetic resonance spectroscopy(MRS) was used to measure y-aminobutyric acid(GABA) and glutamate neurotransmitter concentrations in the insular cortex(IC) before and after the *** compared with the placebo,bumetanide treatment was significantly better in reducing the *** patient withdrew from the trial due to adverse *** superiority of bumetanide to placebo in reducing insular GABA,measured using MRS,was *** clinical improvement was associated with a decrease in insular GABA in the bumetanide *** conclusion,this trial in a large group of young children with predominantly moderate and severe ASD demonstrated that bumetanide is safe and effective in improving the core symptoms of ***,the clinical significance remains uncertain,and future multi-center clinical trials are required to replicate these findings and confirm the clinical significance using a variety of outcome measures.

Unusual Posterior Traumatic Hip Dislocations at Yopougon/Abidjan Teaching Hospital

Traumatic dislocation of the hip is quite rare and accounts for 2% - 5% of all dislocations. We report on a case of a bilateral posterior hip dislocation associated with symmetric acetabular fracture and two cases of infracotyloid dislocation. The report included 2 men and one woman. The mean age of patients was 33 years. All dislocations followed a road traffic accident. Non-operative closed reduction was achieved within 19 hours of admission. The average follow-up time was 15 months. The hips were stable, mobile, and painless;and there was no radiographic sign of avascular necrosis of the femoral head.
