Endoscopic submucosal dissection vs endoscopic mucosal resection for colorectal polyps:A meta-analysis and metaregression with single arm analysis

BACKGROUND Endoscopic submucosal dissection(ESD)has shown to be effective in management of colorectal neoplasm in the Asian countries,while its implementation in Western countries where endoscopic mucosal resection(EMR)is preferred is still *** To compare the surgical,histological,and oncological outcomes between ESD and EMR in the treatment of colorectal polyps,with subgroup analysis comparing the efficacy of ESD and EMR between Japan and the rest of the *** Embase and Medline databases were searched from inception to October 2020 in accordance with PRISMA guidelines for studies comparing en bloc,complete resection,margin involvement,resection time,need for additional surgery,complications,and recurrence rate of ESD with *** Of 281344 colorectal polyps from 21 studies were *** compared to EMR,the pooled analysis revealed ESD was associated with higher en bloc and complete resection rate,and lower lateral margin involvement and *** led to increased procedural time,need for additional surgery,and perforation *** significant difference in bleeding risk was found between the two ***-regression analysis suggested only right colonic polyps correlated with an increased perforation risk in *** including polyp size and invasion depth did not significantly influence the en bloc and complete resection rate,bleeding risk and *** subgroup analysis,Japan performed better than the rest of the world in both ESD and EMR with perforation risk of 4%and 0.0002%,respectively,as compared to perforation risk of 8%and 1%,respectively,in reports coming from rest of the *** ESD resulted in better resection outcomes and lower recurrence compared to *** appropriate training,ESD is preferred over EMR as the first-line therapy for resection of colorectal polyps,without restricting to lesions greater than 20 mm and those with high suspicion of submucosal invasion.

E dible bird’s nest(EBN)hydrolysates have been proven to exhibit enhanced ***,being a macromolecule,fractions with different molecular weights would have different properties and ***,this research was aimed to determine the chemical properties and antioxidant activities of freezedried(EBN_(FD))and spray-dried EBN(EBN_(SD))hydrolysates fractionated using gel permeation chromatography(GPC).Overall,two well-separated fractions were identified(EBN_(fFD1),EBN_(fFD2),EBN_(fSD1) and EBN_(fSD2)).EBNFD demonstrated significantly higher(P≤0.05)peptide(3.6%),total carbohydrate(27.7%)and sialic acid(18.2%)contents than that of *** trend was observed in low molecular weight fractionates(EBN_(fFD2) and EBN_(fSD2)).Meanwhile,the first fractionates(EBN_(fFD1) and EBN_(fSD1))exhibited significantly higher(P≤0.05)hydroxyl radical(·OH)scavenging *** transform infrared(FTIR)spectroscopy demonstrated that all EBN fractionates have similar spectrum,except in the region of N—H(amideⅡ)and C—H alkyl *** conclusion,EBN fractionates with different molecular weights showed different chemical properties and antioxidant activities.

Finite element analysis of tibio-femoral contact mechanics of a customised knee spacer

With one-third of patients having osteoarthritis predominantly in one compartment of the knee,unicompartmental knee spacers have been introduced as a less invasive alternative to total knee ***,patients with the knee spacer implanted were seen to have persistent pain,resulting in high revision rates.A static finite element model of the knee and its interaction with a knee spacer implant at full extension and 90°flexion was used to investigate the tibio-femoral contact mechanics of the knee joint after a knee spacer *** different knee spacer designs(contoured,flat and C-shaped)were modelled and prescribed with cobalt-chrome,ultra-high molecular polyethylene and polyurethane material *** results suggested that a softer spacer is generally preferred as this helps in the conformity of knee spacer to the condyle,effectively distributing the load subjected to the *** spacers that result in high stresses resulting from lower contact areas should be avoided.

目的:研究复方生肌膏用于肛瘘术后创面愈合的效果。方法:选择2021年1月至2023年1月,医院收治的72例肛瘘患者,依据随机数字法分为观察组和对照组,每组36例。观察组应用复方生肌膏纱条换药,对照组应用凡士林纱条换药。对比2组术后不同时间点创面分泌物评分、肉芽组织评分、创面愈合率及愈合时间等指标。结果:观察组术后第7天创面分泌物评分和创面疼痛评分均低于对照组(P0.05)。结论:肛瘘术后应用复方生肌膏纱条换药能够减少创面分泌物,促进肉芽组织生长,减轻患者疼痛,缩短创面愈合时间,值得临床推广应用。

Different position from traditional left lateral for colonoscopy? A meta-analysis and systematic review of randomized control trials

Background: Colonoscopy requires the intubation of the cecum for screening of colorectal diseases. The conventional position used for colonoscopy is the left lateral position (LLP). However, alternative positions have also been utilized to enhance the success of intubation. Thus, the aim of this study was to perform a meta-analysis of the different positions to determine the effectiveness of the individual positions for successful ***: Medline, Embase and Cochrane trials electronic databases were searched for studies on colonoscopy positions. The primary outcome was defined as the cecal intubation rate. Pooled risk ratios (RR) and 95% confidence intervals (CI) for the rates of cecal intubation were estimated. Secondary outcomes such as the cecal intubation time and adenoma detection rate were further analyzed ***: After reviewing 644 identified records, 7 randomized control trials (RCT) studies were included. No significant difference was observed in either comparisons, between the LLP vs. supine position (SP) (RR = 1.01, 95% CI, 0.98 to 1.04,P = 0.55) or the LLP vs. prone position (PP) (RR = 1.02, 95% CI, 0.98 to 1.06,P = 0.27).Conclusions: Amidst available literature, the use of other positions can be considered when performing colonoscopy. These further highlights that the existential practice is based predominantly on familiarity instead of evidence-based-research.

Disease Progression, Response to ACEI/ATRA Therapy and Influence of ACE Gene in IgA Nephritis

Various studies have shown that angiotensin-converting enzyme （ACE） gene insertion/deletion （ID） polymorphism may play a role in the progression to end stage renal failure （ESRF） in patients with IgA nephritis （IgAN）. In this randomized controlled trial, patients were followed up for 5 years to determine their long-term renal outcome to ACEI/ATRA therapy and to ascertain if their ACE gene profile could play a role in determining their response to therapy. Seventy-five patients with IgAN were enlisted. Thirty-seven were on ACEI/ATRA therapy for 62 ± 5 months and thirty-eight were untreated and served as controls. All patients had their ACE gene ID polymorphism genotyped. Compared to controls, treated patients had lower serum creatinine （p 〈 0.001）, lower proteinuria （p 〈 0.002） and fewer numbers progressing to ESRF （p 〈 0.002）. Among patients with genotype II, there were less ESRF in the treatment group when compared to the untreated control group （p 〈 0.02）. The advantage of therapy was not seen in patients with ID or DD genotypes. ACEI/ATRA therapy was found to be effective in retarding disease progression in IgAN with years to ESRF significantly extended in patients at all levels of renal function, including patients whose outcome were ESRF. Genotyping showed better response to therapy only for those with genotype Ⅱ. The common mechanism is probably through lower levels of ACE, glomerular pressure and proteinuria resulting in reduced renal damage and retardation of progression to ESRF. Background:Intestinal parasitic infections(IPIs)among indigenous people have been widely documented in Malaysia,however,the prevalence of these infections remains *** the past,most studies have focused on specific species of parasites but polyparasitism has received limited *** addition,epidemiology studies on indigenous people tend to consider them as a homogenous group,whereas in reality different sub-ethnic groups have different cultural and living *** in living habits such as personal hygiene practices may predispose different groups to different parasitic *** better understand prevalence and risk factors of intestinal parasitism among different sub-ethnic groups,the present study was conducted among two sub-ethnic groups of indigenous people(Temuan and Mah Meri)residing in Selangor state,***:A cross-sectional study that focused on two distinct sub-ethnic groups was carried out from February to September *** samples were collected from 186 participants and examined using the formalin-ether sedimentation technique.A molecular approach was adopted to conduct a genetic characterisation of the ***,questionnaires were administered to obtain information on the demographics,socioeconomic backgrounds and behavioural risks relating to the participants,as well as information about their *** analyses(*** and multivariate logistic regression analyses)were performed to measure risk ***:For Temuan communities,trichuriasis(64.2%)was the most common infection found,preceding hookworm infection(34%),ascariasis(7.5%),giardiasis(14.2%)and amoebiasis(7.5%).As for the Mah Meri communities,trichuriasis(77.5%)prevailed over ascariasis(21.3%),hookworm(15%),giardiasis(7.5%)and amoebiasis(3.8%).Significant differences in proportions of trichuriasis,ascariasis and hookworm infections were observed between the Temuan and Mah Meri sub-ethnic *** was more
