BACKGROUND In Korea,infliximab was approved for use in children with ulcerative colitis(UC)in October *** To compare the clinical course of UC before and after the introduction of biological agents,and to compare with the IBSEN *** Patients under 18 years of age,who were diagnosed with UC and followed from January 2003 to October 2020,were included in the *** A(n=48)was followed for at least 2 years between January 2003 and October 2012,and Group B(n=62)was followed for at least 2 years between November 2012 and October *** compared endoscopic remission,drug composition,relapse rate,steroidfree period,and the quality of life of each *** plotted the clinical course of the included patients using the pediatric UC activity index score,and compared our patients with those in the IBSEN *** After 2 years of treatment,colonoscopy evaluation revealed different outcomes in the two treatment *** was confirmed in 14 patients(29.2%)of Group A,and in 31 patients(50.0%)of Group B(P<0.012).The median cumulative corticosteroid-free period was 3.0 years in Group A and 4.4 years in Group ***-free period of Group B was significantly longer than that of Group A(P<0.001).There was a statistically significant difference between the two groups in evaluation of the relapse rate during the observation period(P<0.001).The plotted clinical course graphs of Group A showed similar proportions to the graphs in the IBSEN ***,in Group B,the proportion of patients corresponding to curve 1(remission or mild severity after initial high activity)was high at 76%(47/62).CONCLUSION The incidence of relapse has decreased and the steroid-free period has increased after the introduction of the biological *** clinical course also showed a different pattern from that of IBSEN *** active use of biological agents may change the long-term disease course in moderate to severe pediatric UC.

AIM: To investigate the effectiveness of early infliximab use for induction and maintenance therapy in pediatric Crohn’s disease. METHODS: We performed a retrospective chart review of 36 patients with Crohn’s disease. Ten patients (group A) were treated with mesalamine after induction therapy with oral prednisolone, and 13 patients (group B) were treated with azathioprine after induction therapy with oral prednisolone. Thirteen patients (group C) received infliximab and azathioprine for induction and maintenance therapy for the first year, and were treated with azathioprine after 1 year. All patients were followed for at least 24 mo. Efficacy was determined by the relapse rate using the pediatric Crohn’s disease activity index score in each group at 12 and 24 mo. RESULTS: At the 1 year follow-up, the relapse ratee, Mi jin Kim, Hae Jeong Lee, Yon Ho choe (23.1%, 3 of 13 patients) in group C was lower than that (61.5%, 8 of 13 patients) in group B (P = 0.047). At the 2 years follow-up, the relapse rate (38.5%, 5 of 13 patients) in group C was lower than that (76.9%, 10 of 13 patients) in group B (P = 0.047). Adverse events in group C were fewer than in groups A and B. CONCLUSION: Early induction with infliximab at diagnosis, known as "top-down" therapy, was effective for reducing the relapse rate compared to conventional therapies for at least 2 years.

LaNi（0.6）Fe（0.4）O（3-δ）（LNF） powders were synthesized by the glycine-nitrate process and LNF-gadolinium-doped ceria （GDC） nanocomposite cathodes for solid oxide fuel cells （SOFCs） were fabricated by infiltration from LNF porous backbones. Electrochemical properties and Cr-poisoning behavior of LNF-GDC cathodes were studied. Single phase perovskite LNF could be obtained at the glycine to nitrate molar ratio of 1：1. The polarization resistance of the LNF-GDC nanocomposite cathode was significantly decreased in comparison with the LNF. This phenomenon was associated with enhanced catalytic activity and enlarged triple-phase boundary （TPB） length by GDC nano particles. In addition, the nanocomposite cathode showed good Cr tolerance under open circuit condition. The LNF-GDC nanocomposite cathodes were expected for use as a potential cathode in intermediate- temperature solid oxide fuel cells （IT-SOFC）.

AIM:To evaluate seroprevalence of hepatitis A virus (HAV) antibody and investigate demographic,clinical,and laboratory features of recent cases in ***:For the evaluation of hepatitis A seroprevalence,we analyzed the data from 3127 subjects including,healthcare workers and patients who visited Konkuk University Hospital,a secondary referral center,from January to October *** sera with positive IgM were excluded from seroprevalence data for total HAV *** retrospectively reviewed the electronic medical records of 419 patients with HAV,who were diagnosed by the presence of serum IgM antibodies against *** patients presented at Konkuk University Hospital between August 2005 and September ***:Among 3127 sera tested,1428 (45.7%)were positive for anti-HAV *** seroprevalence was very low in teenagers or those in their twenties,increased in those in their thirties,and was > 90% in older *** children younger than 10 years,seroprevalence was increased *** patients with HAV hepatitis were in their twenties and *** γ-glutamyl transpeptidase increased with age and was significantly higher in patients older than 30 *** of severity,such as decreased albumin and increased bilirubin,were also more prominent in the older age group;however,the leukocyte count was higher and the frequency of leukopenia was lower in younger patients than in older ***:There has been an apparent epidemiological shift in HAV seroprevalence and a change in the peak age of HAV *** study could provide baseline data of recent hepatitis A in Asia.

Transition metal phosphides(TMPs)and transition metal dichalcogenides(TMDs)have been widely investigated as photoelectrochemical(PEC)catalysts for hydrogen evolution reaction(HER).Using high-temperature processes to get crystallized compounds with large-area uniformity,it is still challenging to directly synthesize these catalysts on silicon photocathodes due to chemical incompatibility at the ***,a graphene interlayer is applied between p-Si and MoP nanorods to enable fully engineered interfaces without forming a metallic secondary compound that absorbs a parasitic light and provides an inefficient electron path for hydrogen ***,the graphene facilitates the photogenerated electrons to rapidly transfer by creating Mo-O-C covalent bondings and energetically favorable band *** a bridging role of graphene,numerous active sites and anti-reflectance of MoP nanorods lead to significantly improved PEC-HER performance with a high photocurrent density of 21.8 mA cm−2 at 0 V versus RHE and high ***,low dependence on pH and temperature is observed with MoP nanorods incorporated photocathodes,which is desirable for practical use as a part of PEC *** results indicate that the direct synthesis of TMPs and TMDs enabled by graphene interlayer is a new promising way to fabricate Si-based photocathodes with high-quality interfaces and superior HER performance.

The prognostic significance of percentage of tumour involvement according to disease risk group in men treated with radical prostatectomy

We investigated the prognostic significance of percentage of tumour involvement （PTI） according to the clinicopathological features of prostate cancer among patients who underwent radical prostatectomy （RP）. A retrospective study of 534 patients who underwent RP between September 2003 and March 2008 without any neoadjuvant or adjuvant therapy was performed. The associations of PTI with various clinicopathological features and biochemical recurrence-free survival were examined via uni- and multivariate analyses. The predictive accuracy of the multivariate model was assessed with a receiver operating characteristics-derived area under the curve. PTI was demonstrated to be significantly associated with preoperative prostate-specific antigen （PSA） level （P=0.001）, pathological Gleason score （P〈0.001）, extraprostatic tumour extension （P〈0.001）, seminal vesicle invasion （P〈0.001） and positive surgical margin （P〈0.001） in univariate analyses. When patients were stratified into disease risk groups, PTI was an independent predictor of biochemical recurrence-free survival in multivariate analysis only among the low-risk group （P=0.033） but not the intermediate- （P=0.287） or the high-risk groups （P=0.828）. The addition of the PTI did not significantly increase the accuracy of the multivariate model devised for the prediction of biochemical recurrence-free survival among both total patients （P=-0.459） and the low-risk group （P=0.268）, respectively. In conclusion, although PTI appeared to be a more significant prognostic factor among patients with low-risk disease than among those with higher risk diseases, overall, the PTI may not provide additional prognostic information beyond what can already be obtained via established prognostic factors.

Importance of the surrounding colonic mucosa in distinguishing between hyperplastic and adenomatous polyps during acetic acid chromoendoscopy

AIM: To examine the characteristics of colonic polyps, where it is difficult to distinguish adenomatous polyps from hyperplastic polyps, with the aid of acetic acid chromoendoscopy. METHODS: Acetic acid spray was applied to colonic polyps smaller than 10 mm before complete excision. Endoscopic images were taken before and 15-30 s after the acetic acid spray. Both pre-and post-sprayed images were shown to 16 examiners, who were asked to interpret the lesions as either hyperplastic or adenomatous polyps. Regression analysis was performed to determine which factors were most likely related to diagnostic accuracy. RESULTS: In 50 cases tested by the 16 examiners, the overall accuracy was 62.4% (499/800). Regression analysis demonstrated that surrounding colonic mucosa was the only factor that was significantly related to accuracy in discriminating adenomatous from hyperplastic polyps (P < 0.001). Accuracy was higher for polyps with linear surrounding colonic mucosa than for those with nodular surrounding colonic mucosa (P < 0.001), but was not related to the shape, location, or size of the polyp. CONCLUSION: The accuracy of predicting histology is significantly related to the pattern of colonic mucosa surrounding the polyp. Making a histological diagnosis of colon polyps merely by acetic acid spray is helpful for colon polyps with linear, regularly patterned surrounding colonic mucosa, and less so for those with nodular, irregularly patterned surrounding colonic mucosa.
