Massive, Spontaneous Facial Hematoma in a Neurofibromatosis Type 1 Patient

Significant hemorrhage in Neurofibromatosis Type 1 (NF-1) patients occurs infrequently, but has potentially devastateing consequences when occurring in the head and neck region. There have been no prior reports of patients with hemodynamically significant, rapidly-expanding lesions into a neurofibroma in the head and neck region without preceding trauma. This case describes the management of a spontaneous, rapidly expanding facial hematoma in an NF-1 patient with an extensive facial and skull base plexiform neurofibroma. The patient underwent angioembolization of his left external carotid artery prior to operative management. The strategies utilized can be extended to management of facial hematomas arising from more common situations such as fractures, lacerations, and pseudoaneursyms, along with bleeding from subacute conditions like other head and neck cancers.

Experimental investigations of transient pressure variations in a high head model Francis turbine during start-up and shutdown

Penetration of the power generated using wind and solar energy to electrical grid network causing several incidents of the grid tripping, power outage, and frequency drooping. This has increased restart （star-stop） cycles of the hydroelectric turbines significantly since grid connected hydroelectric turbines are widely used to manage critical conditions of the grid. Each cycle induces significant stresses due to unsteady pressure loading on the runner blades. The presented work investigates the pressure loading to a high head （ He = 377 m, De = 1.78 m） Francis turbine during start-stop. The measurements were carried out on a scaled model turbine （HM = 12.5 m, DM = 0.349 m）. Total four operating points were considered. At each operating point, three schemes of guide vanes opening and three schemes of guide vanes closing were investigated. The results show that total head variation is up to 9% during start-stop of the turbine. On the runner blade, the maximum pressure amplitudes are about 14 kPa and 16 kPa from the instantaneous mean value of 121 kPa during rapid start-up and shutdown, respectively, which are about 1.5 times larger than that of the slow start-up and shutdown. Moreover, the maximum pressure fluctuations are given at the blade trailing edge.

＜正＞ Objective: To compare coronary artery disease pattern and effects of coronary artery bypass grafting （CABG） in diabetics having left ventricular （LV） systolic dysfunction （LVD） with those in non- diabetics. Methods: The study included patients with LVD （ejection fraction （EF） less than 35% on echocardgiography） and undergoing CABG. Group - Ⅰ included diabetics and group - Ⅱ included non -diabetics. Records of coronary angiography were compared. Pre- and post CABG echocardiographic data were also compared. Results: Out of 267 patients included. 116 were in group - Ⅰ and 151 in group - Ⅱ. Relatively more patients in group - Ⅰ had significant stenosis in left anterior descending, obtuse marginal and right posterior descending artery than those in group - Ⅰ. Consistently, there was reduced LV contractility before CABG. in group -Ⅰ （EF: 27.5%） as compared to group- Ⅱ （29.5%）. However, improvement following 2 - months of CABG was greater in group - Ⅰ （EF: 35.3%） than group - Ⅱ （34.4 % ）. Reduction in LV diameters was also greater in group - Ⅰ. Conclusions: Indian diabetics having LVD and undergoing CABG are found with more stenosed coronary arteries. Diabetics gain greater improvement by CABG than non - diabetics.
