Comparison of the Analgesic Effects of Ultrasound Guided Supra-Inguinal Fascia Iliaca Block with Femoral Nerve Block for Surgical Fixation of Hip Fractures

Introduction: Regional anaesthesia techniques have been used for perioperative analgesia for hip fractures. The supra-inguinal approach to fascia iliaca block (FIB) can potentially provide superior analgesia compared to femoral nerve block (FNB) by blocking the obturator and lateral femoral cutaneous nerves. We aimed to evaluate the analgesic effect of single shot FIB and FNB for surgical fixation of hip fractures. Methods: After obtaining ethics approval and written, informed consent, 30 patients undergoing surgical fixation of hip fractures were recruited and randomized into 2 groups. Patients received either a single shot FIB or FNB with 0.5% Ropivacaine 30 mls, and a subarachnoid block. Pain scores were assessed pre-operatively, post-block, in recovery and at 24 hours post-operatively. Time to first analgesic, oxynorm consumption, opioid related side effects and block related complications were assessed at 24 hours. Results: There were no statistically significant difference in post-block pain scores, median (IQR) of 0 (0 - 0) versus 0 (0 - 0) at rest and 3 (2 - 6) versus 5 (2 - 6) on positioning for spinal;and 24 hour pain scores were 0 (0 - 0) versus 0 (0 - 0) at rest and 4 (2 - 5) versus 5 (2 - 6) on movement for FIB and FNB groups respectively. 5 patients from each group required post-operative opioids, post-operative opioids requirement were similar. Conclusions: Though ultrasound guided supra-inguinal FIB was more consistent in blocking the lateral femoral cutaneous nerve then a femoral nerve block, this did not translate to any difference in terms of pain scores, opioid consumption and side effects.

The expression of the Maxwell magnetic monopole was employed to correlate the space to space projection that gives rise to the Gell-Mann standard model, and space to time projection which gives the leptons;and how does it correlate to the Perelman mappings from the homogeneous 5D manifold to the Lorentz 4D manifold, together with correlating the physical consequences caused by the breaking of the Diagonal Long Range Order [DLRO] of the monopoles quantum states affected by the motion of massive particles in the Lorentz 4D boundary of the 5D manifold, which leads to gravitons and the gravity field via the General Relativity covariant Riemannian 4D curvatures metric equation.

为揭示小角度空间交错轴变厚齿轮副的时变啮合特性与非线性动态特性,在精确几何建模的基础上,建立空间小角度交错轴变厚齿轮传动时变啮合模型,获取其时变啮合刚度与时变传动误差;考虑外部载荷与侧隙变化的影响,采用集中参数法建立齿轮传动非线性动力学模型,对其系统非线性特征进行仿真。结果表明外部载荷的增加直接导致齿轮副啮合刚度、传动误差、动态传动误差和动态啮合力的增加,同时啮合刚度的增加使得系统共振频率增加;侧隙的增加使得系统在轻载下出现与单边、双边冲击耦合的突跳现象,在重载下双边冲击区域变大,动态啮合力增加。

Metabolic and physiological regulation of Chlorella sp.(Trebouxiophyceae, Chlorophyta) under nitrogen deprivation

A freshwater green microalgae Chlorella sp., UMACC344 was shown to produce high lipid content and has the potential to be used as feedstock for biofuel production. In this study, photosynthetic effciency, biochemical pro?les and non-targeted metabolic pro?ling were studied to compare between the nitrogen-replete and deplete conditions. Slowed growth, change in photosynthetic pigments and lowered photosynthetic effciency were observed in response to nitrogen deprivation. Biochemical pro?les of the cultures showed an increased level of carbohydrate, lipids and total fatty acids, while the total soluble protein content was lowered. A trend of fatty acid saturation was observed in the nitrogen-deplete culture with an increase in the level of saturated fatty acids especially C16:0 and C18:0, accompanied by a decrease in proportions of monounsaturated and polyunsaturated fatty acids. Fifty-nine metabolites, including amino acids, lipids, phytochemical compounds, vitamins and cofactors were signi?cantly dysregulated and annotated in this study. Pathway mapping analysis revealed a rewiring of metabolic pathways in the cells, particularly purine, carotenoid, nicotinate and nicotinamide, and amino acid metabolisms. Within the treatment period of nitrogen deprivation, the key processes involved were reshu ? ing of nitrogen from proteins and photosynthetic machinery, together with carbon repartitioning in carbohydrates and lipids.

Arsenic availability and transportation in soil-rice system affected by iron-modified biochar

Iron-modified biochar(FeOS)is known to be effective at immobilization of arsenic(As)in soils.A pot experiment was conducted to investigate the effects of FeOS on As availability and ttransportation in the soil-rice system at different growth stages of rice with different pollution *** results showed that Fe concentration decreased and As concentration increased in paddy soils with the FeOS addition,especially in 120 mg/kg As treatment,the As concentration decreased by 16.46%and 30.56%at the maturity stage with 0.5%and 1%FeOS additions,*** with the control,the application of FeOS reduced the arsenic content in rice tissues and increased the biomass,with the root biomass increased by 12.68%and the shoot biomass was increased by 8.94%with the addition of 1%*** may be related to the promotion of iron plaque formation and the transformation of microbial community structure in FeOS treatments,in accordance with the result of gene abundance and Fe/As contents of iron plaque in the *** study is expected to provide further support and theoretical basis for the application of FeOS in the remediation of As contaminated paddy soil.

Recurrent giant fibrovascular polyp of the esophagus

Giant fibrovascular polyps of the esophagus and hypopharynx are rare benign esophageal tumors. They arise most commonly in the upper esophagus and may, rarely, originate in the hypopharynx. They can vary significantly in size. Even though they are benign, they may be lethal due to either bleeding or, rarely, asphyxiation if a large polyp is regurgitated. Patients commonly present with dysphagia or hematemesis. The polyps may not be well visualized on endoscopy and imaging plays a vital role in aiding diagnosis as well as providing important information for preoperative planning, such as the location of the pedicle, the vascularity of the polyp and the tissue elements of the mass. They can also be recurrent in rare cases, especially if the resection margins of the base are involved. We review the recent literature and report a case of a 61-year-old man with a recurrent giant esophageal fibrovascular polyp with illustrative contrast barium swallow, CT and intra-operative images, who required several surgeries via a combination of endoscopic, trans-oral, trans-cervical, trans-thoracic and trans-abdominal approaches.

Single Channel Source Separation Using Filterbank and 2D Sparse Matrix Factorization

We present a novel approach to solve the problem of single channel source separation (SCSS) based on filterbank technique and sparse non-negative matrix two dimensional deconvolution (SNMF2D). The proposed approach does not require training information of the sources and therefore, it is highly suited for practicality of SCSS. The major problem of most existing SCSS algorithms lies in their inability to resolve the mixing ambiguity in the single channel observation. Our proposed approach tackles this difficult problem by using filterbank which decomposes the mixed signal into sub-band domain. This will result the mixture in sub-band domain to be more separable. By incorporating SNMF2D algorithm, the spectral-temporal structure of the sources can be obtained more accurately. Real time test has been conducted and it is shown that the proposed method gives high quality source separation performance.

Hydrogen as Carbon Gasifying Agent During Glycerol Steam Reforming over Bimetallic Co-Ni Catalyst

Alumina-supported bimetallic cobalt-nickel catalyst has been prepared and employed in a fixed-bed reactor for the direct production of synthesis gas from glycerol steam reforming. Physicochemical properties of the 5Co-10Ni/85Al2O3 catalyst were determined from N2-physisorption, H2-chemisorption, CO2 and NH3-temperature-programmed desorption measurements as well as X-ray diffraction analysis. Both weak and strong acid sites are present on the catalyst surface. The acidic:basic ratio is about 7. Carbon deposition was evident at 923 K;addition of H2 however has managed to reduce the carbon deposition. Significantly, this has resulted in the increment of CH4 formation rate, consistent with the increased carbon gasification and methanation. Carbon deposition was almost non-existent, particularly at 1023 K. In addition, the inclusion of hydrogen also has contributed to the decrease of CO2 and increase of CO formation rates. This was attributed to the reverse water-gas-shift reaction. Overall, both the CO2:CO and CO2:CH4 ratios decrease with the hydrogen partial pressure.

Planar scintigraphy in assessment of mandibular asymmetry: Unilateral condylar hyperplasia vs asymmetric mandibular hyperplasia

Objectives: To 1) explore the planar scintigraphic findings in asymmetry patients caused by unilateral condylar hyperplasia (UCH) or asymmetric mandibular hyperplasia (AMH);2) develop a local agecondylar activity reference norm;and 3) check the accuracy of the new norm and compare with other analytical methods. Material and Method: chinese patients with mandibular asymmetry and scintigraphic findings available were recruited. Clinical, radiographic records and scintigraphic condylar activity ratio (CAR) were studied. Regression analysis was performed to quantify the relationship between age and CAR to develop a new norm. The sensitivity of condylar activity assessment using a) traditional norm;b) new norm;and c) percentile difference was compared. In patients with serial data available, longitudinal analysis of the scintigraphic changes were checked. Result: 109 patients were eligible for the study. Significant difference in CAR was noted between UCH and AMH patients. Linear relationship was observed between age and CAR. A new norm of the age-CAR was established, which showed improved sensitivity in condylar activity prediction in UCH and AMH when compared with traditional norm. Relative to percentile difference, the sensitivity of new norm was lower in AMH but not in UCH patients. Serial analysis revealed gradual decline in CAR with minimal change in percentile difference. Conclusion and Clinical Relevance: There is difference in scintigraphic condylar activity between UCH and AMH patients. A norm of age-CAR relationship was established, which showed improved sensitivity in condylar activity prediction in UCH patients. Further study is required to confirm the role of scintigraphy in AMH.

A Study of Different Unwarping Methods for Omnidirectional Imaging

Omni-directional imaging system is becoming more and more common in reducing the maintenance fees and the number of cameras used as well as increasing the angle of view in a single camera. Due to omni-directional images are not directly understandable, an approach namely the un-warping process, has been implemented in converting the omni-directional image to a panoramic image, making it understandable. There are different kinds of methods used for the implementation of this approach. This paper evaluates the performance of the 3 universal un-warping methods currently applied actively around the world in transforming omni-directional image to panoramic image, namely the pano-mapping table method, discrete geometry method （DGT） and the log-polar mapping method. The algorithm of these methods will first be proposed, and the code will then be generated and be tested on several different omni-directional images. The images converted will then be compared among each other and be evaluated based on their performance on the resolutions, quality, algorithm used, complexity based on Big-O computations, processing time, and finally their data compression rate available for each of the methods. The most preferable un-warping method will then be concluded, taking into considerations all these factors.
