Comparing posture induced intraocular pressure variations in normal subjects and glaucoma patients

AIM:To compare the posture-induced variations in intraocular pressure(IOP)between the primary open angle glaucoma(POAG)and non-glaucomatous ***:A case-controlled age matched study was conducted in 55 successive newly diagnosed POAG and 56 non-glaucomatous patients seen in glaucoma clinic and general outpatient eye clinic in the Alex ekwueme University Teaching Hospital,*** IOPs of eligible correspondents were measured with Perkin’s hand-held tonometer in the sitting,supine flat and supine with pillow positions *** of IOP in each position was done after 15 min of assuming such ***:The IOP difference between the sitting and supine flat position was significantly higher in the POAG than non-glaucoma subjects(7.68±2.08 vs 4.03±0.13 mm Hg,Pe IOP difference between the sitting and supine with pillow positions was 2.61±1.49 mm Hg for POAG and 1.44±0.70 mm Hg in non-glaucoma(Pe difference between supine flat and supine with pillow positions was 5.07±2.24 mm Hg in POAG and 2.59±0.66 mm Hg in non-glaucomatous patients(Peater variations in posture induced IOP occurred in POAG patients than *** posture induced IOP variation is lowest in the sitting position and highest in the supine flat *** of posture induced IOP changes may be an important tool in the management of glaucoma.

Perception and Practice of Undergoing Glaucoma Screening among Ophthalmologists

Background: Globally, glaucoma remains the leading cause of irreversible blindness. early detection and treatment is mainstay of prevention of blindness from glaucoma. Purpose: This study was done to assess the perception and glaucoma screening practice of ophthalmologist in Nigeria. Methods: This is a cross-sectional descriptive study. A pretested structured questionnaire was administered to all the ophthalmologists who attended the general plenary session of the 2013 annual conference of the Ophthalmological Society of Nigeria (OSN). Result: Of the 115 respondents, 63 (54.8%) were females;mean age was 39.7 ± 9.3 years. Majority 69 (60.0%) were less than 40 years old. Presence of systemic risk factors to glaucoma was reported by 19 (16.5%), out of which 9 (47.4%) were hypertension, 2 (10.5%) were Diabetes Mellitus, 2 (10.5%) were other cardiovascular diseases, and 6 were unspecified others. Among the respondents, 28 (24.3%) had positive family history;29 (25.2%) had Myopia;and 3 (2.6%) were known glaucoma patients already on treatment. Majority 91 (79.1%) had positive attitude towards glaucoma screening. Although 98 (85.2%) have ever had a full eye examination, only 56 (48.7%) were routine. There was no statistically significant relationship between sex, age group, religion, positive family history, professional status, attitude and glaucoma screening. Conclusion: The attitude of Nigerian Ophthalmologists to glaucoma screening was high but actual screening practice was below expectation in view of their presumed high knowledge of the risk of blindness from glaucoma. In-house campaign aimed at promoting self-glaucoma screening among this high-cadre eye care providers is warranted.

A Preliminary Report of Predisposing Factors and Predominant Microbiological Diagnosis of Corneal Ulcers Seen at the Federal Teaching Hospital Abakaliki, Nigeria

Background: Microbial keratitis often results in poor visual outcome despite treatment. A revision of treatment protocol based on local evidence may be required in order to obtain better treatment outcome. Objectives: To determine the predisposing factors and predominant microbiological diagnosis of corneal ulcers seen at the Federal Teaching Hospital Abakaliki (FeTHA), ebonyi State, Nigeria. Materials and Methods: This is a preliminary report of an on-going longitudinal descriptive study of all consenting corneal ulcer patients managed at the FeTHA eye clinic over a 4-month period. Information obtained were socio-demographic data, presenting complaints, duration of symptoms prior to presentation, history of preceding trauma, medications used before presentation, presenting and final visual acuity and microbiological diagnosis. Results: A diagnosis of corneal ulcer was made in 8 out of the 852 outpatients seen over the study period giving a hospital prevalence rate of 0.59%. Five patients (62.50%) were males, five (62.50%) were farmers and 4 patients (50%) were above 60 years of age. The microbial diagnoses were bacterial keratitis 37.5% (Staphylococcus aureus), fungal keratitis 25% (Fusarium spp. and aspergillus) and acanthamoeba (25%). None of the patients ever used contact lenses. There was a history of eye trauma in 50% of the patients. All the eyes presented blind after a period of failed attempts to treat by self or quacks. Mean duration before presentation was two weeks. Treatment improved the visual acuity in 37.5% of patients. Conclusion: Bacteria, fungi and acanthamoeba organisms were the microbiological isolates from the scrapings of corneal ulcer patients seen in the eye clinic of FeTHA;with bacterial organisms being the most common. Farming activities, preceding eye trauma, delayed presentation, self-medication and use of traditional eye medications (TeM) were common findings among the patients.
