Tuberculoma of the lung is one of manifestations in tuberculosis and usually presents as a solitary pulmonary nodule(SPN).It is difficult to differentiate tuberculoma from SPN by other benign or malignant *** present,the crucial role of video-assisted thoracoscopic surgery(VATS) in diagnosis and treatment of pulmonary diseases has been well ***,we reported 53 patients undergoing VATS resection for tuberculomas in our *** postoperative mortality was found and only two patients experienced prolonged air-leakage(>7 d) and two had minor wound infections that were recovered after anti-tuber-culosis or antibiotic ***-tuberculosis chemotherapy from 6 to 12 months was routinely used *** conclude that VATS is a satisfactory tool for the diagnosis and treatment of tuberculoma and can also establish a reliable diagnosis for all patients with SPNs.

BACKGROUND Hepatitis C virus(HCV)genotype 6(HCV-6)infection is prevalent predominantly in Southeast Asia,and the data on the virologic response of HCV-6 to direct-acting antivirals(DAAs)are sparse in people living with human immunodeficiency virus(HIV)(PLWH).AIM To assess the virologic response of HCV-6 to DAAs in *** From September 2016 to July 2019,PLWH coinfected with HCV-6 initiating DAAs were *** investigations were performed at baseline,the end of treatment,and 12 wk *** Of the 349 PLWH included(mean age 48.9 years,82.5%men),80.5%comprised people who inject drugs,18.1%men who have sex with men,and 1.4%*** hepatitis B virus infection was present in 12.3%of the included PLWH,liver cirrhosis 10.9%,hepatocellular carcinoma 0.9%,and previous HCV treatment experience 10.9%.The mean baseline plasma HCV RNA was 6.2 log10 IU/m *** with glecaprevir/pibrentasvir was initiated in 51.9%,sofosbuvir/ledipasvir 41.5%,sofosbuvir/velpatasvir 6.3%,and sofosbuvir/daclatasvir 0.3%.At DAA initiation,antiretroviral therapy containing tenofovir alafenamide was given in 26.4%,tenofovir disoproxil fumarate 34.4%,non-tenofovir alafenamide/tenofovir disoproxil fumarate 39.3%,non-nucleoside reverse-transcriptase inhibitors 30.4%,protease inhibitors 4.0%,and integrase strand transfer inhibitors 66.8%;94.8%of the included patients had CD4 counts≥200 cells/mm3 and 96.0%had plasma HIV RNA<50 copies/m ***,96.8%achieved undetectable plasma HCV RNA(<30 IU/m L)at end of treatment;and 92.3%achieved sustained virologic response 12 wk off-therapy in the intention-to-treat analysis(93.5%in patients receiving sofosbuvir-based DAAs and 91.2%in those receiving glecaprevir/pibrentasvir).CONCLUSION Similar to the observation made in HIV-negative patients,sustained virologic response 12 wk offtherapy with DAAs is high in PLWH coinfected with HCV-6.

Aim: Sorafenib is a multi-tyrosine kinase inhibitor and the standard therapy for advanced hepatocellular carcinoma (HCC). This retrospective study aimed to observe the anti-fibrotic effect of sorafenib in patients with advanced HCC. Methods: Seventeen patients with advanced HCC were recruited. Shear wave velocity (SWV) using acoustic radiation force impulse elastography and non-invasive serum markers for liver fibrosis, such as the aspartate aminotransferase (AST) to alanine aminotransferase ratio (AAR), the AST to platelet ratio index, the fibrosis-4 index and the Lok index, were recorded at the beginning of sorafenib treatment and 3-6 months after sorafenib treatment in 2014-2015. Results: Nine (52.9%) patients achieved disease control status and 8 had progressive disease after a mean duration of 11.1 months with sorafenib treatment. The mean SWV decreased from 2.37 m/s at the beginning to 1.90 m/s after sorafenib treatment (P < 0.01). This trend was observed in patients with and without liver cirrhosis (from 2.49 to 2.06 m/s, P = 0.06, and from 2.32 to 1.69 m/s, P < 0.05, respectively). Among the non-invasive serum markers, no statistically significant differences were observed except for the AAR in the cirrhotic group. Conclusion: Sorafenib has potential antif-ibrotic effects in patients with advanced HCC.

AIM:To elucidate the role of insulin resistance(IR) and serum adiponectin level in hepatocellular carcinoma(HCC) associated with chronic hepatitis ***:Clinical and biochemical characteristics were collected from 165 consecutive patients with newly diagnosed *** model assessment of IR(HOMA-IR) and serum adiponectin level were investigated in 188 patients with different stages of hepatitis C virus(HCV) ***:Among HCC patients,type 2 diabetics(DM) was more prevalent in HCV subjects(35.6%,n = 59) compared to hepatitis B virus(HBV;12.7%,n = 63) or non-HBV,non-HCV cases(7.1%,n = 28).In patients with chronic hepatitis C,HCC subjects had higher blood sugar(P < 0.001),insulin level(P = 0.003) and HOMAIR(P = 0.018) than those with chronic hepatitis and advanced ***,male sex and body mass index were significantly associated with serum adiponectin level,whereas HOMA-IR was *** on stepwise logistic regression analysis,age(OR:1.124,P < 0.001),serum insulin level(OR:1.585,P < 0.001),HOMA-IR(OR:0.495,P = 0.001),DM(OR:11.601,P = 0.002) and male sex(OR:3.877,P = 0.016) were independently associated with *** result was similar even if the diabetic subjects were excluded for ***:Insulin resistance measured by HOMAIR,regardless of the presence of diabetes,is significantly associated with HCC development in patients with chronic HCV infection.

Two measurement systems, one being comprised of three HPGe detectors and the other being a 4π measurement system of the assembly of 10 sets of scintillation detectors, were compared for their measurement performance in operations of radioactive waste drum radioactivity measurement and clearance and release operation of decommissioning. The system features of the former were high costs, capable of identifying radionuclides, a measurement time of about 1000 sec. The system features of the latter were high sensibility, capable of identifying hotspots, a measurement time of about 100 sec. Various comparisons were made for the differences of the two systems on reference drums, whole-drum homogeneous and non-homogeneous metal drums and non-metal drums, whole-drums of different filling rates. The aim was to compare the differences of the two measurement systems in measuring radioactivity of critical radionuclides of 137Cs, 54Mn and 60Co.

AIM:To evaluate the effect of hands-on training of gastroenterology fellows in gastric polypectomy using an ex vivo ***:Eight gastroenterology fellows at Mackay Memorial Hospital,Taipei were evaluated in gastricpolypectomy techniques using a pig stomach with artificial polyps created by a rubber band ligation *** performance of four second year(year-2)fellows who had undergone one year of clinical training was compared with that of four f irst year(year-1)fellows both before and after a 4-h workshop using the ex vivo *** workshop allowed for hands-on train-ing in the removal of multiple artif icial polyps and the placement of hemoclips at the excision *** included observation of technical skills,procedure time,and the fellows' conf idence ***:One week after the workshop,the year-1 fellows were re-evaluated and had significantly im-proved mean performance scores(from 17.9 ± 1.8 to 22.5 ± 0.7),conf idence scale(from 4.5 ± 1.0 to 7.8 ± 0.5)and procedure time(from 615.0 ± 57.4 s to 357.5 ± 85.0 s)compared with their baseline *** 4 h of training using the ex vivo simulator,the skills of the year-1 fellows were statistically similar to those of the year-2 ***:Use of this ex vivo simulator significantly improved the endoscopic gastric polypectomy skills of gastroenterology fellows who had not had previous clinical training in gastric polypectomy.

The recent trends in Industry 4.0 and Internet of Things have encour-aged many factory managers to improve inspection processes to achieve automa-tion and high detection ***,the corresponding cost results of sample tests are still used for quality control.A low-cost automated optical inspection system that can be integrated with production lines to fully inspect products with-out adjustments is introduced *** corresponding mechanism design enables each product to maintain aﬁxed position and orientation during inspec-tion to accelerate the inspection *** proposed system combines image recognition and deep learning to measure the dimensions of the thread and iden-tify its defects within 20 s,which is lower than the production-line productivity per 30 *** addition,the system is designed to be used for monitoring production lines and equipment *** dimensional tolerance of the proposed system reaches 0.012 mm,and a 100%accuracy is achieved in terms of the defect *** addition,an attention-based visualization approach is utilized to verify the rationale for the use of the convolutional neural network model and identify the location of thread defects.

This study focused on the strategic leadership in the organization and the internal and external social capital can be seen as the ambidexterity innovation assets for building organization *** research selected 500 top managers and 385 ones were returned,using purposive sampling in Southern Tainan Real Estate firms as subjects to explore the interactions and relationships among strategic leadership,social capital,innovation ambidexterity,and contextual ambidexterity,and examines the fitness of model in this *** data were analyzed by a variety of statistical methods,such as regression analysis,hierarchical regression analysis,ANOVA,and SEM to test the hypotheses of this study and model *** conclusions are as follows:(1)Strategic leadership has significant positive influence on social capital and innovation ambidexterity.(2)Social capital has significant positive influence on innovation ambidexterity.(3)Good model fitness of relationship exists among each construct,and the social capital has significant mediating effects between strategic leadership and innovation ambidexterity.(4)The relationship between social capital and innovation ambidexterity has a moderating influence of contextual ambidexterity.
