Attempt Suicide by Inserting a Ball-Pen into the Brain: A Case Report

Purpose: Closed head injuries are more common than penetrating head injuries in our clinical practices. Penetrating head injuries can be happened in industrial accidents, car accidents, fall accidents or criminal activities, also they can be occurred in self-inflicted injuries. We reported a case of self-inflicted penetrating head injury to highlight the importance of suicide precaution and review the principle of management of penetrating head injury. Methods and Results: A 58 year-old woman, who has hypertension and major depression, attempted suicide by inserting a ball-pen into her brain through the right temple in the psychiatric ward. CT of brain showed a foreign body penetrating through the right sphenoid ridge to intracranial right frontal-temporal junction, right parasella and suprasella cistern to interpeduncular cistern and acute subarachnoid hemorrhage in the basal cistern and right sylvian fissure. She underwent an emergent neurosurgical procedure to remove the ball-pen, to evacuate the intracranial hematoma and to repair the lacerated dura. She recovered gradually and was clear after surgery, but a right oculomotor palsy and very mild left hemiparesis were left. Discussion: We learn a lesson from this patient, even a ball-pen can become a weapon in the hands of depression patients. All clinicians should be aware of the psychological condition of each patient and suicide precaution should be kept in mind in clinical practice.

Background: Choroid plexus carcinoma is a highly aggressive malignant, infrequent tumor with poor prognosis. About 80% of choroid plexus carcinoma occurs in children, but it is uncommon in adults. Because of the rarity of the choroid plexus carcinoma, there is no established treatment protocol for this malignancy. Case Description: A 21-year-old man with past medical history of asthma presented to us with the chief complaint of morning headache for one month. His brain magnetic resonance imaging (MRI) showed a mass in the trigone and occipital horn of the left lateral ventricle. He had undergone a left occipitoparietal craniotomy, posterior interhemispheric precuneus approach with grossly total removal of the tumor. The histology examination of the tumor proved to be choroid plexus carcinoma. This patient achieved a favorable outcome after having a grossly complete surgical resection followed by postoperative adjuvant radiotherapy and chemotherapy. Conclusions: Choroid plexus carcinoma is aggressive and is associated with dismal prognosis. The 5-year survival rates for choroid plexus carcinoma vary between 10% and 50%. Currently, there is no established treatment protocol for choroid plexus carcinoma. Complete resection of this malignant tumor is the primary goal of treatment since it allows best chance of survival and improves the overall prognosis.

Rathke Cleft Cyst with a Coexisting Gonadotropin Producing Pituitary Adenoma

Rathke cleft cyst is thought to arise from incomplete obliteration of the lumen of Rathke pouch. The cells of the anterior pituitary lobe, from which pituitary adenomas develop, are also derived from the cells of Rathke pouch. Although Rathke cleft cyst and pituitary adenoma have a shared ancestry, they rarely occur coincidentally. Rathke cleft cysts have been found incidentally in 11% - 33% of post-mortem examinations, and were associated with 1.7% - 2.1% of the pituitary adenoma cases. These coexisting lesions are difficult to diagnose pre-operatively due to the variable signal intensity and position of the Rathke cleft cysts. Treatment of these lesions involves surgical resection to decrease mass effect and medical management to normalize hormonal imbalances. To our best knowledge, only 42 cases of Rathke cleft cyst with a coexisting pituitary adenoma have been reported in the English or Japanese literatures, but none of the coexisting pituitary adenoma is gonadotropin producing. We would like to report a case of Rathke cleft cyst with a coexisting gonadotropin producing adenoma that was successfully treated by endoscopic endonasal transsphenoidal approach with the removal of the sellar lesion and temporary hormone replacement.

chiral luminescence materials have potential applications in the field of three-dimensional displays due to their circularly polarized luminescence(CPL)***,the further development of circularly polarized organic light-emitting diodes(CP-OLEDs)needs to meet the requirements of high efficiency,high color purity,low cost,and high dissymmetry factor(gPLor gEL),chiral multiple resonance thermally activated delayed fluorescence(MR-TADF)materials are considered as candidates in these ***,based on a pair of chiral spirofluorene precursors,two pairs of high-performance chiral MR-TADF emitters((R/S)-p-Spiro-DtBuCzB and(R/S)-m-Spiro-DtBuCzB)are developed,which exhibit strong emissions peaking at 491 and 502 nm in toluene with full-width at half-maximum values of 25 and 33 nm,*** addition,small singlet–triplet energy gaps of 0.15 and 0.10 eV with high absolute photoluminescence efficiencies of 95.0%and 96.7%are observed for p-Spiro-DtBuCzB and m-Spiro-DtBuCzB molecules,*** based on p-Spiro-DtBuCzB and m-Spiro-DtBuCzB display high maximum external quantum efficiencies of 29.6%and 33.8%,*** importantly,CP-OLEDs present symmetric circularly polarized electroluminescence spectra with|gEL|factors of 3.36×10^(-4)and 7.66×10^(-4)for devices based on(R/S)-p-Spiro-DtBuCzB and(R/S)-m-Spiro-DtBuCzB enantiomers,respectively.

All-inorganic CsPbIBr_(2) perovskite has attracted widespread attention in photovoltaic and other optoelectronic devices because of its superior thermal ***,the deposition of high-quality solutionprocessed CsPbIBr_(2) perovskite films with large thicknesses remains ***,we develop a triple-component precursor(TCP) by employing lead bromide,lead iodide,and cesium bromide,to replace the most commonly used double-component precursor(DCP) consisting of lead bromide and cesium ***,the TCP system significantly increases the solution concentration to 1.3 M,leading to a larger film thickness(~390 nm) and enhanced light *** resultant CsPbIBr_(2) films were evaluated in planar n-i-p structured solar cells,which exhibit a considerably higher optimal photocurrent density of 11.50 mA cm^(-2) in comparison to that of DCP-based devices(10.69 mA cm^(-2)).By adopting an organic surface passivator,the maximum device efficiency using TCP is further boosted to a record efficiency of 12.8% for CsPbIBr_(2) perovskite solar cells.
