Is convenience really king? Comparative evaluation of catastrophic costs due to tuberculosis in the public and private healthcare sectors of Viet Nam: a longitudinal patient cost study

Background In Viet Nam, tuberculosis (TB) represents a devastating life-event with an exorbitant price tag, partly due to lost income from daily directly observed therapy in public sector care. Thus, persons with TB may seek care in the private sector for its flexibility, convenience, and privacy. Our study aimed to measure income changes, costs and catastrophic cost incurrence among TB-affected households in the public and private *** Between October 2020 and March 2022, we conducted 110 longitudinal patient cost interviews, among 50 patients privately treated for TB and 60 TB patients treated by the National TB Program (NTP) in Ha Noi, Hai Phong and Ho chi Minh City, Viet Nam. Using a local adaptation of the WHO TB patient cost survey tool, participants were interviewed during the intensive phase, continuation phase and post-treatment. We compared income levels, direct and indirect treatment costs, catastrophic costs using Wilcoxon rank-sum and chi-squared tests and associated risk factors between the two cohorts using multivariate *** The pre-treatment median monthly household income was significantly higher in the private sector versus NTP cohort (USD 868 vs USD 578;P=0.010). However, private sector treatment was also significantly costlier (USD 2075 vs USD 1313;P=0.005), driven by direct medical costs which were 4.6 times higher than costs reported by NTP participants (USD 754 vs USD 164;P<0.001). This resulted in no significant difference in catastrophic costs between the two cohorts (Private: 55% vs NTP: 52%;P=0.675). Factors associated with catastrophic cost included being a single-person household [adjusted odds ratio [(aOR=13.71; 95% confidence interval (CI): 1.36–138.14;P=0.026], unemployment during treatment (aOR=10.86; 95%CI: 2.64–44.60;P<0.001) and experiencing TB-related stigma (aOR=37.90; 95%CI: 1.72–831.73;P=0.021).Conclusions Persons with TB in Viet Nam face similarly high risk of catastrophic costs whether treated in the

目的分析Miller金字塔结合HEART沟通模式在护生《基础护理学》教学中的应用效果。方法整群随机抽样选取我院2022级分组实习的一、二组63名本科护生为观察组, 选取三、四组63名本科护生为对照组。对照组护生在常规教学基础上接受HEART沟通教学, 观察组护生在HEART沟通模式基础上接受Miller金字塔教学, 比较两组护生教学前后理论考核成绩、操作考核成绩、人文关怀能力、批判性思维能力。应用SPSS 24.0进行卡方检验、t检验。结果教学后, 两组护生理论考核成绩和操作考核成绩均较教学前高, 且观察组较对照组高(t=5.79、8.56, P<0.001);两组护生人文关怀能力评估问卷各维度评分及总分均较治疗前高, 且观察组较对照组高(t=4.45、5.78、3.31、4.50, P<0.001);两组护生批判性思维能力测量表中各维度评分均较教学前高, 且观察组较对照组高(t=7.43、4.10、5.26、4.01、5.33、3.49、3.62、4.55, P<0.001)。结论 Miller金字塔教学结合HEART沟通模式有利于提高《基础护理学》教学效果, 提高护生人文关怀能力、自主学习能力以及批判性思维能力。

Comparison of Layer-stacking and Dempster-Shafer Theory-based Methods Using Sentinel-1 and Sentinel-2 Data Fusion in Urban Land Cover Mapping

Data fusion has shown potential to improve the accuracy of land cover mapping,and selection of the optimal fusion technique remains a *** study investigated the performance of fusing Sentinel-1(S-1)and Sentinel-2(S-2)data,using layer-stacking method at the pixel level and Dempster-Shafer(D-S)theory-based approach at the decision level,for mapping six land cover classes in Thu Dau Mot City,*** the pixel level,S-1 and S-2 bands and their extracted textures and indices were stacked into the different single-sensor and multi-sensor datasets(*** datasets).The datasets were categorized into two *** group included the datasets containing only spectral and backscattering bands,and the other group included the datasets consisting of these bands and their extracted *** random forest(RF)classifier was then applied to the datasets within each *** the decision level,the RF classification outputs of the single-sensor datasets within each group were fused together based on D-S ***,the accuracy of the mapping results at both levels within each group was *** results showed that fusion at the decision level provided the best mapping accuracy compared to the results from other products within each *** highest overall accuracy(OA)and Kappa coefficient of the map using D-S theory were 92.67%and 0.91,*** decision-level fusion helped increase the OA of the map by 0.75%to 2.07%compared to that of corresponding S-2 products in the ***,the data fusion at the pixel level delivered the mapping results,which yielded an OA of 4.88%to 6.58%lower than that of corresponding S-2 products in the groups.

Microstructure and Properties of Hot Working Die Steel H13MOD

Compared with H13 steel, the influences of different heat treatment process on the microstructure and properties of the new type of hot working die steel H13MOD were studied. The results show that the complete aus tenitizing temperature of H13MOD is around 1030 ℃ and the quenching hardness achieves the maximum value at this temperature. While for H13, the complete austenitizing temperature is above 1100 ℃ and the quenching hardness rise constantly with the quenching temperature increasing. In quenching process, the undissolved MC carbides can prevent the coarsening of grain in both steels. With the rise of quenching temperature, when MC carbides dissolve completely, the grain grows quickly. The hardness and strength of H13MOD at higher tempering temperature （above 570 ℃） are nearly the same as those of H13, but its toughness is higher than that of H13. Mo2C carbide is the main strengthening phase in H13MOD, which is attributed to the higher content of Mo. The quantity of VC eutectic car- bides is reduced because of lower content of V in H13MOD, which plays an important role in enhancing the impact toughness of H13MOD. Under a certain strength condition, H13MOD steel can be used in the environment that higher toughness is required and the service life of die casting mold can be improved.

Phase Transformation of a Cold Work Tool Steel during Tempering

The hardness and microstructure evolution of a 8% Cr cold work tool steel during tempering for 40 h were investigated. Transmission electron microscope examinations showed that M_3C carbides precipitated from supersaturated martensite after tempering at 350 ℃. When the tempering temperature was higher than 520 ℃,the M_（23）C_6 carbides precipitated to substitute for M_3C carbides. After ageing at the temperature of 520 ℃ for 40 h,it was observed that very fine and dense secondary Mo_2C precipitates were precipitated. Thus,it can be concluded that the early stage of Mo_2C-carbide precipitation is like to be Gunier-Preston（ G-P） zone formed by [Mo-C] segregation group which is responsible for the secondary hardening peak at 520 ℃. Overageing at 700 ℃ resulted in recovery of martensitic microstructure and precipitation of M_（23）C_6 *** ageing at 700 ℃ for more than 20 h,recrystallization occurred,which resulted in a change of the matrix morphology from martensitic plates to equiaxed ferrite. It was noticed that the size of recrystallized grain / subgrain was very fine,which was attributed to the pinning effect of M_（23）C_6 precipitates.
