Acute pancreatitis induced by transarterial chemoembolization:a single-center experience of over 1500 cases

BACKGROUND： Acute pancreatitis is a relatively rare but po- tentially lethal complication after transarterial chemotherapy. This study aimed to review the complications such as acute pancreatitis after transarterial chemotherapy with or without embolization for hepatocellular carcinoma. METHODS： A total of 1632 patients with hepatocellular car- cinoma who had undergone transarterial chemoembolization from ]anuary 2000 to February 2014 in a single-center were reviewed retrospectively. We investigated the potential com- plications of transarterial chemoembolization, such as acute pancreatitis and acute pancreatitis-related complications. RESULTS： Of the 1632 patients with hepatocellular carcinoma who had undergone 5434 transarterial chemoembolizations, 1328 were male and 304 female. The median age of these pa- tients was 61 years. Most （79.6%） of the patients suffered from HBV-related hepatoceUular carcinoma. The median tumor size was 5.2 cm. Of the 1632 patients, 145 patients underwent transarterial chemoembolization with doxorubicin elut- ing bead, making up a total of 538 episodes. The remaining patients underwent transarterial chemoembolization with cisplatin. Seven （0.4%） patients suffered from acute pancre- atitis post-chemoembolization. Six patients had chemoembo- lization with doxorubicin and one had chemoembolization with cisplatin. Patients who received doxornbicin eluting bead had a higher risk of acute pancreatitis [6/145 （4.1%） vs 1/1487 （0.1%）, P〈0.0001]. Two patients had anatomical arterial variations. Four patients developed acute pancreatitis- related complications including necrotizing pancreatitis （n=3） and pseudocyst formation （n=1）. All of the 4 patients resolved after the use of antibiotics and other conservative treatment. Three patients had further transarterial chemoembolization without any complication. Hepatopancreatoduodenectomy for advanced hepatobiliary malignancies: a single-center experience

BACKGROUND: Hepatopancreatoduodenectomy is a complicated and challenging procedure but necessary for curative resection for advanced hepatobiliary malignancies. This retrospective study was to examine the safety and survival outcomes of hepatopancreatoduodenectomy in our center. METHODS: Prospectively collected data of 12 patients who underwent hepatopancreatoduodenectomy for advanced hepatobiliary malignancies in our hospital from January 1998 to December 2014 were analyzed. The primary endpoints are treatment-related morbidity and mortality and the secondary endpoints are overall survival and disease-free survival. RESULTS: Curative resection was achieved in 11 (91.7%) patients. Complications developed in 10 (83.3%) patients. Three hospital deaths resulted from multiorgan failure secondary to postoperative pancreatic fistula or hepaticojejunostomy leakage. Six of the nine remaining patients had disease recurrence. The nine patients had a median survival of 39.8 (5.3-151.8) months. The 1-, 3- and 5-year overall survival rates were 66.7%, 55.6% and 27.8%, respectively. The corresponding disease-free survival rates were 55.6%, 44.4% and 29.6%, respectively. CONCLUSIONS: Morbidity and mortality after hepatopancreatoduodenectomy were significant. With RO resection, the 5-year overall survival and disease-free survival rates were 27.8% and 29.6%, respectively.

Intravenous fluid selection rationales in acute clinical management

BACKGROUND: Intravenous fluid(IVF) is commonly used in acute clinical management. This study aimed to review the choice and primary considerations in IVF prescriptions and to evaluate the adequacy of guidelines and trainings on it in the New Territories West Cluster(NTWC) of Hong ***: This is a descriptive study based on data collected from an online survey. Data were processed by SPSS for statistical analysis. This study focused on a general description and doctor-nurse between group comparison. Participants were asked the choice of IVF for nine acute clinical scenarios and provide reason. A 1–10 scale was used to assess the sufficiency of guideline, training and information, and time for revision on IVF ***: 0.9% sodium chloride was the most familiar IVF(36%), followed by 5% Dextrose solution(26%). In the nine scenarios, the most chosen IVF was 0.9% sodium chloride(37%–61%). There was significant difference in the choice of IVF between doctors and nurses in 7 cases. The second most chosen IVF for doctors was Plasma-Lyte A while that for nurses was Gelofusine. Departmental practice was the most chosen reason to account for the prescription. The adequacy of guideline, information and training, and time for revision was rated 5. Doctors had significantly more time at work than nurses to update knowledge in IVF prescription(5.41 versus 4.57).CONCLUSION: 0.9% sodium chloride was mostly chosen. The choice of IVF was mainly based on departmental practice. Adequacy of guideline, information and training, and time for revision on IVF prescription were average, indicating significant training deficit.

Seismic performance of cable-sliding friction bearing system for isolated bridges

During past strong earthquakes, highway bridges have sustained severe damage or even collapse due to excessive displacements and/or very large lateral forces. For commonly used isolation bearings with a pure friction sliding surface, seismic forces may be reduced but displacements are often unconstrained. In this paper, an alternative seismic bearing system, called the cable-sliding friction bearing system, is developed by integrating seismic isolation devices with displacement restrainers consisting of cables attached to the upper and lower plates of the bearing. Restoring forces are provided to limit the displacements of the sliding component. Design parameters including the length and stiffness of the cables, friction coefficient, strength of the shear bolt in a fixed-type bearing, and movements under earthquake excitations are discussed. Laboratory testing of a prototype bearing subjected to vertical loads and quasi-static cyclic lateral loads, and corresponding numerical finite element simulation analysis, were carried out. It is shown that the numerical simulation shows good agreement with the experimental force-displacement hysteretic response, indicating the viability of the new bearing system. In addition, practical application of this bearing system to a multi-span bridge in China and its design advantages are discussed.

Assessment of the damages and direct economic loss in Hong Kong due to Super Typhoon Mangkhut in 2018

Super Typhoon Mangkhut hit Hong Kong on September 16,2018,necessitating the issuance of the highest tropical cyclone warning signal,No.10 Hurricane *** ferocious winds and record-breaking storm surge,Mangkhut brought the most serious and widespread destruction to the territory in the recent three decades.A series of post event information search,field visits and damage surveys has been conducted by the Hong Kong Observatory(HKO)and the findings on the damages and impacts caused by Mangkhut in different parts of the territory are documented in this ***,by analyzing the economic loss data reported by various government departments,public utilities and organizations in Hong Kong and the statistics on insurance claims from the Hong Kong Federation of Insurers(HKFI),the estimated direct economic loss due to Mangkhut in Hong Kong is about HK$4.60 billion,which is about 3.8 times to that of Super Typhoon Hato in *** the contrary,in the Guangdong-Hong Kong-Macao Greater Bay Area,the estimated direct economic loss due to Hato is significantly higher than that of *** could be attributed to the early and effective warnings for Mangkhut,increased public awareness and typhoon preparedness for Mangkhut in 2018 since the fierce attack of Hato in 2017,and infrastructure enhancement of the major impact areas.

X-reality for phantom limb management for amputees:A systematic review and meta-analysis

Phantom limb is a disabling neuropsychiatric condition among amputees resulting in pain and disturbance that impact their functions,quality of life,and *** pharmacological approaches appeared to be ineffec-tive,the emergence and integration of X-reality,including virtual reality,augmented reality,and mixed reality,might elevate the effectiveness of mirror therapy in managing phantom *** objective of this study is to review X-reality for managing phantom pain.A systematic search was conducted on PubMed,Scopus,Web of Science,PsycINFO,Embase,and ***(n=16)studies containing 66 lower-limb and 53 upper-limb amputees were included for the review over the thematic framework of amputee characteristics and intervention designs,while thirteen(n=13)studies were further proceeded for the *** found eleven studies on virtual reality(n=11),four studies on marker-based augmented reality(n=4)and one study on mixed reality(n=1)with a total of 40 game/task themes involving,motor skills,motor control,and ***,all these interventions adopted the movement representation strategies with different ***,the X-reality interventions reduced the pain level of the amputees(mean difference:-2.30,95%CI,-3.38 to-1.22),especially the virtual reality subgroup(mean difference:-2.83,95%CI,-4.43 to-1.22).However,there were substantial heterogeneity and partially explained by the subgroup analysis on publication *** strength of evidence was limited by case reports and case series in this review.
