Surgical Reconstruction of a Large Defect after Excision of Infiltrative Squamous Cell Carcinoma in the Scalp and Occipital Region: A Case Report

Squamous cell carcinoma(SCC)of the scalp is the second most prevalent skin cancer,following basal cell carcinoma.Notably,it has the capability to infiltrate the skull,dura mater,and even brain tissue.The cornerstone of treatment is the surgical removal of the lesion,with a particular focus on the depth of invasion,which is directly correlated with recurrence rates.Post-surgical strategies may involve immediate or delayed cranial bone reconstruction and repair of scalp defects using either artificial dermis or skin grafts.In the case presented,a substantial defect necessitated more than a single flap for primary repair.Hence,a single pedicle double-island flap was designed for reconstructing the occipital area.Due to increased tension on the flap following cranial bone repair,the bone repair was temporarily deferred.Postoperative care included adjuvant chemotherapy and radiotherapy to mitigate the risk of SCC recurrence.

In this paper,we extend the deterministic learning theory to sampled-data nonlinear systems.Based on the Euler approximate model,the adaptive neural network identifier with a normalized learning algorithm is proposed.It is proven that by properly setting the sampling period,the overall system can be guaranteed to be stable and partial neural network weights can exponentially converge to their optimal values under the satisfaction of the partial persistent excitation(PE)condition.Consequently,locally accurate learning of the nonlinear dynamics can be achieved,and the knowledge can be represented by using constant-weight neural networks.Furthermore,we present a performance analysis for the learning algorithm by developing explicit bounds on the learning rate and accuracy.Several factors that influence learning,including the PE level,the learning gain,and the sampling period,are investigated.Simulation studies are included to demonstrate the efectiveness of the approach.

癌因性疲乏患者易伴有厌食、减重、腹胀等症状或体征,严重降低生存质量,成为影响恶性肿瘤患者生存率的独立危险因素,亟需寻找有效的治疗策略及药物。既往基于络病理论指导提出“改善证候——肿瘤治疗新策略”学术观点,笔者以气络学说核心理论“承制调平”为指导,提出恶性肿瘤癌因性疲乏“脾肾两虚,瘀毒内阻”中医病机及“健脾益肾、化瘀解毒”治法,研制的代表药物养正消积胶囊不仅对实体瘤有良好的治疗作用,在治疗癌因性疲乏方面也显示出良好优势,有助于恢复恶性肿瘤患者“荷瘤生存”自稳平衡状态,为癌因性疲乏临床治疗提供新的药物选择。

In this paper, based on deterministic learning, we propose a method for rapid recognition of dynamical patterns consisting of sampling sequences. First, for the sequences yielded by sampling a periodic or recurrent trajectory(a dynamical pattern) generated from a nonlinear dynamical system, a sampled-data deterministic learning algorithm is employed for modeling/identification of inherent system dynamics. Second,a definition is formulated to characterize similarities between sampling sequences(dynamical patterns) based on differences in the system dynamics. Third, by constructing a set of discrete-time dynamical estimators based on the learned knowledge, similarities between the test and training patterns are measured by using the average L1 norms of synchronization errors, and general conditions for accurate and rapid recognition of dynamical patterns are given in a sampled-data framework. Finally, numerical examples are discussed to illustrate the effectiveness of the proposed method. We demonstrate that not only a test pattern can be rapidly recognized corresponding to a similar training pattern, but also the proposed recognition conditions can be verified step by step based on historical sampling data. This makes a distinction compared with the previous work on rapid dynamical pattern recognition for continuous-time nonlinear systems, in which the recognition conditions are difficult to be verified by using continuous-time signals.

This paper addresses the learning control problem for a group of robot manipulators with homogeneous nonlinear uncertain dynamics, where all the robots have an identical system structure but the reference signals to be tracked differ. The control objective is twofold: to track on reference trajectories and to learn/identify uncertain dynamics. For this purpose, deterministic learning theory is combined with consensus theory to find a common neural network(NN) approximation of the nonlinear uncertain dynamics for a multi-robot system. Specifically, we first present a control scheme called cooperative deterministic learning using adaptive NNs to enable the robotic agents to track their respective reference trajectories on one hand and to exchange their estimated NN weights online through networked communication on the other. As a result, a consensus about one common NN approximation for the nonlinear uncertain dynamics is achieved for all the agents. Thus, the trained distributed NNs have a better generalization capability than those obtained by existing techniques. By virtue of the convergence of partial NN weights to their ideal values under the proposed scheme, the cooperatively learned knowledge can be stored/represented by NNs with constant/converged weights, so that it can be used to improve the tracking control performance without re-adaptation. Numerical simulations of a team of two-degree-of-freedom robot manipulators were conducted to demonstrate the effectiveness of the proposed approach.

目的强直性脊柱炎(AS)是脊柱关节病的一种,属于自身免疫性疾病。在过去几十年中,关于AS的研究总体上呈上升趋势。通过文献计量学的方法,揭示AS研究领域的关注热点。方法基于检索Web of Science Core Collection(WoSCC)的科学引文索引扩展版(SCI-Expanded)中的出版物,记录了2013年至2023年期间发表的信息。通过使用VOSviewer、CiteSpace以及Excel软件对年度产出、领先国家、活跃机构、期刊、作者、共引参考文献和关键词的爆发等进行了可视化分析。结果通过检索和筛查,共获得了1193篇文章纳入分析。在最近十年中,与AS相关的研究数量呈增长态势。美国是该领域最高产的国家。最高产的机构是加拿大的多伦多大学。发文量排名前十的期刊中,《风湿病学年鉴》Annals of the rheumatic diseases获得了最高的引用量。来自德国的学者Poddubnyy发文量表现最为突出。“th 17 cells”、“antitumor necrosis factor”是该研究领域较为突出的关键词。结论本研究阐明了AS研究领域的研究现状及前沿热点,为该领域的下一步研究方向提供了科学性的数据支持。

Peptides exert important biological functions but their application is hindered by their susceptibility to proteolysis and poor stability in vivo.Thus,functional peptide mimics have drawn a great deal of attention to address this challenge.Poly(2-oxazoline)s,a class of biocompatible and proteolysis-resistant polymer,can work as host defense peptide mimics without following the general membrane-targeting mechanism as shown in our previous work.This observation encouraged us to figure out if poly(2-oxazoline)s are special and break the general membrane-targeting mechanism of host defense peptides and their mimics.In this study,we aimed at the connection between structure and antibacterial mechanism of poly(2-oxazoline)s.A new γ-aminobutyric acid(GABA)-pendent poly(2-oxazoline)was synthesized and investigated to compare with glycine-pendent poly(2-oxazoline)in our previous study,with the former polymer has two extra CH2 groups in the sidechain to increase the hydrophobicity and amphiphilicity.Membrane depolarization assay suggested that incorporating two more CH2 groups into the sidechain of poly(2-oxazoline)resulted in a mechanism switch from DNA-targeting to membrane-targeting,which was supported by the slow time-kill kinetics and slightly distorted and sunken membrane morphology.Besides,GABA-pendent poly(2-oxazoline)showed potent activity against methicillin-resistant S.aureus and low hemolysis on human red blood cells.Moreover,repeated use of the antimicrobial poly(2-oxazoline)did not stimulate bacteria to obtain resistance,which was an obvious advantage of membrane-targeting antimicrobial agents.

目的评估全胸腔镜下行单纯左心耳闭合术的有效性和安全性,以探索其对高出血风险非瓣膜性房颤患者相关脑卒中的预防作用。方法共纳入66例抗凝高出血风险的非瓣膜性房颤患者,其中阵发性房颤7例、持续性或永久性房颤59例。行全胸腔镜下单纯左心耳闭合术,评估左心耳闭合的有效性,同时记录手术相关不良事件,进一步随访手术对房颤相关卒中的预防作用。结果纳入患者均成功完成手术,平均手术时长约(2.22±0.97)h,术后约90%患者残根不超过10 mm,所有患者无内漏发生,无心耳夹移位。高龄患者手术时间、术后引流、手术成功率与其他患者相比,差异均无统计意义,仅术后脱机拔管时间较其他患者更久,差异有统计学意义,但均顺利脱机。在安全性评价中,未见非计划二次手术,无围术期死亡事件,有1例出血事件,为术中牵拉心耳尖端出血,释放心耳夹后顺利止血。患者平均随访时间为(17.3±0.7)个月,2例患者出现新发缺血性脑卒中,其中1例表现为新发脑梗死、另1例表现为短暂性脑缺血发作。结论全胸腔镜下行单纯左心耳闭合术为抗凝高出血风险的非瓣膜性房颤患者的卒中预防提供了新的治疗选择,具有成功率高、内漏发生率低和并发症少的特点,且手术对于高龄患者同样安全有效。
