Ankle arthrodesis:A systematic approach and rev.ew of the literature

Ankle arthrodesis is a common treatment used for patients with end-stage ankle arthritis(ESAA). The surgical goal of ankle arthrodesis is to obtain bony union between the tibia and talus with adequate alignment [slight v.lgus(0°-5°)], neutral dorsiflexion, and slight external rotation positions) in order to prov.de a pain-free plantigrade foot for weightbearing activ.ties. There are many v.riations in operativ. technique including deferring approaches(open or arthroscopic) and differing fixation methods(internal or external fixation). Each technique has its adv.ntage and disadv.ntages. Success of ankle arthrodesis can be dependent on sev.ral factors, including patient selection, surgeons' skills, patient comorbidities, operativ. care, etc. Howev.r, from our experience, the majority of ESAA patients obtain successful clinical outcomes. This rev.ew aims to outline the indications and goals of arthrodesis for treatment of ESAA and discuss both open and arthroscopic ankle arthrodesis. A systematic step by step operativ. technique guide is presented for both the arthroscopic and open approaches including a postoperativ. protocol. We rev.ew the current ev.dence supporting each approach. The rev.ew finishes with a report of the most recent ev.dence of outcomes after both approaches and concerns regarding the dev.lopment of hindfoot arthritis.

AIM:To present statistical tools to model and optimize the cost of a randomized clinical trial as a function of the stringency of patient inclusion ***: We consider a two treatment, dichotomous outcome trial that includes a proportion of patients who are strong responders to the tested interv.ntion. Patients are screened for inclusion using an arbitrary number of test results that are combined into an aggregate suitability score. The screening score is regarded as a diagnostic test for the responsiv. phenotype, hav.ng a specific cutoff v.lue for inclusion and a particular sensitiv.ty and specificity. The cutoff is a measure of stringency of inclusion criteria. Total cost is modeled as a function of the cutoff v.lue, number of patients screened, the number of patients included, the case occurrence rate, response probabilities for control and experimental treatments, and the trial duration required to produce a statistically significant result with a specified power. Regression methods are dev.loped to estimate relev.nt model parameters from pilot data in an adaptiv. trial design. RESULTS: The patient numbers and total cost are strongly related to the choice of the cutoff for inclusion. Clear cost minimums exist between 5.6 and 6.1 on arepresentativ. 10-point scale of exclusiv.ness. Potential cost sav.ngs for typical trial scenarios range in millions of dollars. As the response rate for controls approaches 50%, the proper choice of inclusion criteria can mean the difference between a successful trial and a failed trial. CONCLUSION: Early formal estimation of optimal inclusion criteria allows planning of clinical trials to av.id high costs, excessiv. delays, and moral hazards of Type II errors.

An inv.stigation of hydrotalcite-deriv.d ternary Mg-Al-Ce oxides as catalysts for v.pour phase condensation of ethanol to 1-butanol in a flow reactor under atmospheric pressure was carried *** Mg-Al-Ce oxide systems with Mg/(Al+Ce)ratio from 1 to 4 were synthesized and characterized by XRD,SEM,NMR,and *** study of acid-base characteristics of the systems with different Mg/(Al+Ce)ratio by NH3/CO_(2)quasi-equilibrium thermal desorption techniques shows that the ratio of the catalyst oxide components(Mg,Al,Ce)can prov.de acid/base capacity ratio close to 3 for the effectiv.ty of the target *** highest selectiv.ty 68%is reached ov.r Mg-Al-Ce oxide catalyst with the ratio of Mg/(Al+Ce)=2.

Pulmonary embolism occurs more frequently after hepatectomy than prev.ously thought but is infrequently associated with peripheral deep v.in thrombosis. In this paper, we report 2 cases of postoperativ. hepatic v.in thrombosis after liv.r resection. Both patients had undergone major hepatectomy of a non-cirrhotic liv.r largely exposing the middle hepatic v.in. Clots were incidentally found in the middle hepatic v.in 4 and 17 d after surgery despite routine systemic thrombo-prophylaxis with low molecular weight heparin. Coagulation of the transition plan in a context of mutation of the prothrombin gene and inflammation induced biloma were the likely predisposing conditions. Clots disappeared following curativ. anticoagulation. We conclude that thrombosis of hepatic v.ins may occur after liv.r resection and is a potential source of pulmonary embolism.

Creep of squeeze-cast Mg-3Y-2Nd-1Zn-1Mn alloy was inv.stigated at the constant load in the stress range of 30-80 MPa. Tensile creep tests were performed at 300℃up to the final *** tests at 50 MPa were interrupted after reaching the steady state creep;and another set of creep tests was interrupted after the onset of ternary *** of cav.tated dendritic boundaries was ev.luated using optical *** of grain boundary sliding by observ.tion of the offset of marker lines was carried out on the surface of the crept specimens after the test interruption by scanning electron microscopy and by confocal laser scanning *** results show that the dominant creep mechanism in this alloy is dislocation creep with minor contribution of the grain boundary *** failure took place by the nucleation,growth and coalescence of creep cav.ties on the boundaries predominantly oriented perpendicular to the applied *** amount of cav.tated boundaries with time of creep exposure supports the mechanism of continuous cav.ty nucleation and growth.

Studies that compare tumor genotype with phenotype hav. prov.ded the basis of a new histological/molecular classification of hepatocellular adenomas. Based on two molecular criteria （presence of a TCFI/HNF1α or β-catenin mutation）, and an additional histological criterion （presence or absence of an inflammatory infiltrate）, subgroups of hepatocellular adenoma can be defined and distinguished from focal nodular hyperplasia. Analysis of 96 hepatocellular adenomas performed by a French collaborativ. network showed that they can be div.ded into four broad subgroups： the first one is defined by the presence of mutations in TCF1 gene inactiv.ting the hepatocyte nuclear factor 1 （HNF1α）, the second by the presence of β-catenin activ.ting mutations; the category without mutations of HNF1α or β-catenin is further div.ded into 2 subgroups depending on the presence or absence of inflammation. Therefore, the approach to the diagnosis of problematic benign hepatocytic nodules may be entering a new era directed by new molecular information. It is hoped that immunohistological tools will improv. significantly diagnosis of liv.r biopsy in our ability to distinguish hepatocellular adenoma from focal nodular hyperplasia （FNH）, and to delineate clinically meaningful entities within each group to define the best clinical management. The optimal care of patients with a liv.r nodule will benefit from the recent knowledge coming from molecular biology and the combined expertise of hepatologists, pathologists, radiologists, and surgeons.
