A search for the rare decays W^(+)→D_(s)^(+)γ and Z→D0γis performed using proton-proton collision data collected by the LHCb experiment at a centre-of-mass energy of 13TeV,corresponding to an integrated luminosity of 2.0fb−*** significant signal is observed for either decay mode and upper limits on their branching fractions are set using W^(+)→D_(s)^(+)γ and Z→μ+μ−decays as normalization *** upper limits are 6.5×10^(−4) and 2.1×10^(−3) at 95% confidence level for the W^(+)→D_(s)^(+)γ and Z→D^(0)γ decay modes,*** is the first reported search for the Z→D^(0)γ decay,while the upper limit on the W+→D+sγbranching fraction improves upon the previous best limit.

A patient presented with a 4- month history of slowly progressive pruritic papules on her trunk and extremities. Biopsies from 2 of these lesions revealed molluscum contagiosum. One of the biopsies also showed several small foci of granular parakeratosis. Based on the clinical features and course of this patient, the granular parakeratosis seems to be an incidental finding.

目的为了比较FREDDY激光机、钬激光机和气压弹道碎石机在结石上移和碎石方面的临床效果。方法上移的水下试验装置包括一个水平方向放置的硅树脂导管(直径为1.3cm),和一个固定支架,使激光机的石英光纤和气压弹道碎石机的撞针可以直接接触石头。将预先称过重的圆柱状Bego石头(BegoUSA公司提供)放于该装置中,分别由FREDDY激光机、钬激光机和气压弹道碎石机进行碎石。FREDDY激光机和钬激光机都采用相近的脉冲能量和频率。为了具有可比性,气压弹道碎石机采用了半硬性撞针和单脉冲,在100、200、300kpa各档压力设置下分别对石头进行30次撞击。“平均后退距离”定义为直接测得的石头最终静止点到原点的距离(每档设置都记录测试结果)。另外,两台激光机分别对称过重的石头(熟石膏成分)进行碎石,以比较碎石效果。石头固定于一个水下装置(由一个倒置的硅注射器和一个固定装置组成)中。激光光纤(钬激光和FREDDY激光的光纤内径分别是365μm和280μm)穿过注射器的针头出来抵住石头。总共24颗石头被分成4组,每组6颗,其中两组用FREDDY激光分别以300J和400J的总能量碎石,另两组用钬激光分别以300J和480J的总能量碎石。“碎石率”定义为石头重量缺失的百分比。结果FREDDY激光机在能量设置为160mJ时,分别以5、10和15Hz的频率碎石,石头的平均后退距离为7.6、8.1和6.8cm;钬激光机在能量设置为0.8J时,分别以5和10Hz的频率碎石,石头的平均后退距离为3.3和4.9cm;气压弹道碎石机在压力设置分别为100、200、300kpa时,石头的平均后退距离为8.5、9.9和13.8cm。一般来说,FREDDY激光机造成的石头后退距离都要少于气压弹道碎石机,尽管这只有在后者处于最高压力设置时才有统计意义(pppp<0.05)。结论FREDDY激光机造成的石头后退距离要明显大于钬激光机(在低频设置时),但又明显少于气压弹道碎石机(在所有设置时)。因此我们建议,在进行腔内输尿管镜碎石术,或在输尿管肾盂连接处进行腔内激光碎石术时,于接近结石处使用阻断装置,如StoneCone-石椎(BostonScientific公司提供)。FREDDY激光机在体外碎石的效率要明显好于钬激光机,对于结石病人来说,前者是个更优、更经济的选择。

There are indications that the effect of environmental factors on the risk of type 1 diabetes mellitus (T1DM) is increasing over time. This can be documented by the rapid increase of T1DM incidence in genetically stable populations. Our aim was to study an association of T1DM with the variable factors of the perinatal period and of early infancy, using data from children born over a period of changing exposure to some of the studied factors. A case-control dataset was analysed, consisting of 868 diabetic children and 1,466 anonymous controls, mostly schoolmates of the children with T1DM. The data were collected using structured questionnaires completed by parents. After performing univariate analyses, the associations were analysed using multiple logistic regression adjusted for potential confounders, including the year of birth. The risk of T1DM decreased with increasing duration of breast-feeding, while no breast-feeding was associated with an increased T1DM risk, OR=1.93 95%CI: 1.33-2.80 , breast-feeding for more than 12 months was protective, OR=0.42 95%CI: 0.22-0.81 , both being relative to the reference category of breast-feeding for 1-3 months. A short duration of day-care attendance (none or less than 1 year) was weakly associated with the risk of T1DM, OR=1.65 95%CI: 1.05-2.62 . No association was detected between T1DM and signs of prenatal infections, perinatal stress factors, birth size and weight, indicators of crowding or the presence of a domestic pet in the household. Short breastfeeding, and short attendance to day-care is associated with risk of T1DM in Czech children.

A search for the doubly charmed baryon ?cc+with the decay mode ?cc+→Ξc+K-π+is performed using proton-proton collision data at a centre-of-mass energy of 13 TeV collected by the LHCb experiment from 2016 to 2018, corresponding to an integrated luminosity of 5.4 fb-1. No significant signal is observed within the invariant mass range of 3.6 to 4.0 GeV/c2. Upper limits are set on the ratio R of the production cross-section times the total branching fraction of the ?cc+→Ξc+K-π+decay with respect to the Ξcc++→Λc+K-π+π+decay. Upper limits at 95% credibility level for R in the range 0.005 to 0.11 are obtained for diferent hypotheses on the ?cc+mass and lifetime in the rapidity range from 2.0 to 4.5 and transverse momentum range from 4 to 15 GeV/c.

Introduction. pathogenesis of sarcoidosis remains partially unknown. Cutaneous lesions are frequent (20 to 35% of cases). Their clinical features and follow- up data are highly variable. Numerous treatments have been proposed. The clinical features and followup data of four patients with chronic cutaneous sarcoidosis treated with methotrexate are reported. Case report. Mean age of patients (3 female, 1 male) was 40 years old (34- 49 years). One patient presented with a lupus pernio, two patients with papules and nodules, and the last with an annular lesion of the face. All patients had been previously treated with topical corticosteroids and/or hydroxychloroquine without any success. patients were treated with methotrexate at doses ranging from 12.5 mg to 30 mg per week for at least 6 months. Complete remission of cutaneous lesions was observed in 3 of 4 patients after a mean treatment duration of 29 months (16 to 36). Methotrexate side effects were observed in one patient (elevated liver enzymes) leading to methotrexate discontinuation. Discussion. Methotrexate seems to be an effective treatment of cutaneous sarcoidosis. It should be used namely in patients who failed to respond to previous treatments with topical corticosteroids or antimalarial drugs.

Biological drugs opened up new horizons in the management of inflammatory bowel diseases(IBD).This study focuses on access to biological therapy in IBD patients across 9 selected Central and Eastern European(CEE)countries,namely Bulgaria,the Czech Republic,Estonia,Hungary,Latvia,Lithuania,poland,Romania and *** data on the epidemiology and disease burden of IBD in CEE countries was systematically ***,we provide an estimation on prevalence of IBD as well as biological treatment *** all countries with the exception of Romania,lower biological treatment rates were observed in ulcerative colitis(UC)compared to Crohn’s disease despite the higher prevalence of *** heterogeneity(up to 96-fold)was found in access to biologicals across the CEE ***,Bulgaria,Romania and the Baltic States are lagging behind Hungary,Slovakia and the Czech Republic in their access to *** of reimbursement policy may be one of the factors explaining the differences to a certain extent in Bulgaria,Latvia,Lithuania,and poland,but association with other possible determinants(differences in prevalence and incidence,price of biologicals,total expenditure on health,geographical access,and cost-effectiveness results)was not *** assume,nevertheless,that healthdeterioration linked to IBD might be valued differently against other systemic inflammatory conditions in distinct countries and which may contribute to the immense diversity in the utilization of biological drugs for *** conclusion,access to biologicals varies widely among CEE countries and this difference cannot be explained by epidemiological factors,drug prices or total health *** in reimbursement policy could contribute to better access to biologicals in some countries.

背景:人们重新关注成功治疗结核菌潜伏感染对降低结核病总体影响的主要作用。然而,结核菌潜伏感染治疗的依从性在工业化国家(例如美国和加拿大)一直处于较低水平。目的:系统回顾美国和加拿大的相关研究,分析结核菌潜伏感染治疗依从性的测量和治疗完成率,以及结核菌潜伏感染治疗依从性的预测指标和依从性干预。方法:全面搜索了pubMed,Medline和psycINFO电子数据库中发表于1997—2007年的定量研究。为了进行数据提取,对研究方法和严谨性进行了严格的审查和全文评估。本综述展示了78个研究的结果。结果:无论治疗方案如何,结核菌潜伏感染的治疗依从及治疗完成率在高风险人群中均不理想。不同研究所发现的依从性与病人因素、医疗机构,以及治疗特点的相关性不尽相同。在美国和加拿大,制定了很多改善依从性的干预措施,但没有一个干预措施显示出一贯的有效性。结论:要想实现消除结核病的目标,结核菌潜伏感染者必须得到有效的治疗。关键是应用连续的测量和改进依从性的方法。发现依从性和治疗完成的障碍将有助于制定有效的、适当的干预措施,一刀切的办法不可能在所有的情况下取得成功。创新的方法能够激励将来采取更加有效的干预措施,并对目前结核菌潜伏感染规划和病人所面临的困难提出解决方案。
