Fish oil-containing lipid emulsions prevention on parenteral nutrition-associated cholestasis in very low birth weight infants:a meta-analysis

Background The effect of fish oil-containing lipid emulsions on preventing parenteral nutrition-associated cholestasis(PNAC)in very low birth weight(VLBW)infants is not ***,we conducted a meta-analysis to identify any preven-tion *** PubMed,EMBASE,and CENTRAL were searched up to 26 January 2021 for studies related to the preventive effect of fish oil-containing lipid emulsions and fish oil-free lipid emulsions on cholestasis in VLBW *** 5.3 was used to synthesize the results.A fixed-effect model was used to summarize the data when the heterogeneity was non-significant(I250%).Results Of 728 articles,11 randomized controlled trials met the inclusion *** meta-analysis indicated that fish oil-containing lipid emulsion reduced the occurrence of PNAC significantly with risk ratio(RR)=0.53,95%confidence interval(CI)0.36-0.80,P=*** heterogeneity was non-significant with I2=23%.Subgroup analysis based on par-enteral nutrition duration and median birth weight was *** synthesis results for patients with parenteral nutri-tion duration exceeding 14 days revealed I2=35%(P=0.15)and pooled RR=0.47,95%CI 0.30-0.73,P=0.0008;and for patients with duration less than 14 days revealed I2=0%(P=0.72)and pooled RR=1.14,95%CI 0.39-3.35,P=*** synthesis for patients with birth weight more than 1000 g revealed I2=0%(P=0.41)and pooled RR=0.55,95%CI 0.26-1.18,P=0.12;and for patients with birth weight below 1000 g revealed I2=44%(P=0.11)and pooled RR=0.53,95%CI 0.33-0.85,P=*** The fish oil-containing lipid emulsion can reduce the occurrence of PNAC in VLBW infants based on the avail-able original randomized controlled trial studies,especially for patients with parenteral nutrition duration exceeding 14 days and extremely low birth weight *** studies should be performed before a definitive conclusion can be established.

Safety and tolerability of the ketogenic diet used for the treatment of refractory childhood epilepsy: a systematic review of published prospective studies

Background:To review the available evidence from prospective studies on the safety and tolerability of the ketogenic diet (KD) for the treatment of refractory childhood ***:A comprehensive bibliographic search was performed with the aim of retrieving prospective studies that monitored adverse effects (AEs) in children after receiving the classic or medium-chain triglyceride KD therapy for refractory ***:A total of 45 studies were retrieved,including 7 randomized controlled *** than 40 categories of AEs were *** most common AEs included gastrointestinal disturbances (40.6%),hyperlipidemia (12.8%),hyperuricemia (4.4%),lethargy (4.1%),infectious diseases (3.8%) and hypoproteinemia (3.8%).Severe AEs,such as respiratory failure and pancreatitis,occurred in no more than 0.5% of ***,patients receiving KD therapy should be monitored for osteopenia,urological stones,right ventricular diastolic dysfunction,and growth *** total retention rates of the diet for 1 year and 2 years were 45.7% and 29.2%,*** half of the patients discontinued the diet because of lack of *** were not the main reason for the KD *** of the 24 deaths reported after initiation of the diet was attributed to the ***:KD is a relatively safe dietary ***,because the KD can cause various AEs,it should be implemented under careful medical *** follow-up is needed to address the long-term impact of the diet on the overall health of children.

A New Peptide with Membrane-permeable Function Derived from Human Circadian Proteins

Basic peptides such as human immunodeficiency virus type 1 (HIV-1) Tat-(48-60) and Droso- phila Antennapedia-(43-58) have been reported to have a membrane permeability and a carrier function for intracellular protein delivery. Based on the fluorescence microscopic observations of the vascular endothelial cells (ECV-304) and the primary cultured neuroglial cells, we found that human Clock protein DNA-binding peptide [residue 35–47, hClock-(35-47)] had a translocation activity very similar to Tat-(48-60). The cellular uptake of hClock-(35-47) increases with the increase of incubation time and concentration. The internalization effect at 4 °C was same as that at 37 °C. Internalization of hClock-(35-47) was saturable and could be inhibited by the excess of the other MPPs. Moreover, The uptake of these peptides were significantly inhibited in the presence of heparan sulfate. These results strongly suggested that the hClock-(35-47) shared a common or very similar internalization pathway with other MPPs. Furthermore, we injected rat through the common carotid artery with hClock-(35-47)-FITC peptide, and cryostat sections of the brain were prepared and observed using a fluorescence microscope. Result showed that the peptide had the ability to translocate through the blood-brain barrier. It is promising to provide a new safe carrier for the intracellular and encephalic treatment.

Background The guidelines addressed the evidence-based indications for the management of children with acute infectious diarrhea in Chinese pediatric population. Data sources The experts group of evidence development put forward clinical problems, collects evidence, forms prelimi-nary recommendations, and then uses open-ended discussions to form recommendations. The literature review was done for developing this guideline in databases including PubMed, Cochrane, EMBASE, China Biomedical Database, and Chinese Journal Full-text Database up to June 2013. Search the topic 'acute diarrhea' or 'enteritis' and 'adolescent' or 'child' or'Pediatric patient' or 'Baby' or 'Infant'. Results For the treatment of mild, moderate dehydration, hypotonic oral rehydration solutions (ORS) are strongly recom-mended. Intravenous (IV) rehydration is recommended for severe dehydration, with a mixture of alkali-containing dextrose sodium solution. Nasogastric feeding tube rehydration is used for children with severe dehydration without IV infusion conditions with ORS solution. Regular feeding should resume as soon as possible after oral rehydration or IV rehydration. The lactose-free diet can shorten the diarrhea duration. Zinc supplements are recommended in children with acute infectious diarrhea. Saccharomyces boulardii and Lactobacillus Rhamnus are recommended to be used in acute watery diarrhea. Sac-charomyces boulardii is recommended in children with antibiotic-associated diarrhea as well. Montmorillonite and Racec-adotril (acetorphan) can improve the symptoms of diarrhea or shorten the course of acute watery diarrhea. Antibiotics are recommended with dysenteric-like diarrhea, suspected cholera with severe dehydration, immunodeficiency, and premature delivery children with chronic underlying disease;otherwise, antibiotics are not recommended. Conclusion The principles of the most controversial treatments with of acute infectious disease are reaching to a consensus in China.

对4根十字形型钢混凝土柱进行了火灾后的力学性能试验研究,并按照ISO834标准升温过程曲线进行火灾试验。1根未受火试件作为对比。异形柱采用桁架式配钢形式,横向腹杆间距为260mm。试件通过静力荷载作用下的单向偏心受压试验,研究加载偏心距对力学性能的影响。通过试验,得出了试件的破坏形式以及极限承载力、截面平均应变分布、柱中侧向位移曲线、荷载-滑移曲线和截面特性。试验结果表明:(1)受火1小时后的十字形型钢混凝土柱仍然具有较高的竖向承载力;(2)试验测量的混凝土与槽钢之间滑移较小,说明型钢与混凝土之间能够协同工作,平截面假定在十字形型钢混凝土柱的截面中仍然适用;(3)在90°荷载角的作用下,十字形型钢混凝土柱会发生单向弯曲;(4)随着荷载的增加,挠度曲线存在水平段,说明火灾后型钢混凝土柱仍具有较大的后期变形能力,并且随着偏心距的增大,试件的变形能力更强。

巨细胞病毒(cytomegalovirus,CMV)感染在我国流行广泛,胎儿及免疫力低下的婴儿易受侵害。肝脏是巨细胞病毒感染的主要靶器官。此外,巨细胞病毒还可侵犯呼吸、神经、血液系统,严重者导致胎儿畸形,甚至危及患儿生命。巨细胞病毒感染在健康人群中多表现为亚临床不显性感染和潜伏感染。而孕期和儿童巨细胞病毒感染较为严重,是引起出生缺陷和各种不可逆损伤的重要因素。因此巨细胞病毒感染早期诊治的重要性越来越受到关注。本文对巨细胞病毒的诊断及治疗进展综述如下。

Constitutive behavior and microstructural evolution in hot deformed 2297 Al-Li alloy

The microstructural evolution mechanism and constitutive behavior of 2297 Al-Li alloy were studied via thermal compression test with the constant strain rates of 0.001–1 s-1 and the deformation temperatures ranging from 623 to 773 *** verify the predictable ability of diverse constitutive models under different stress states,the hot compression experiments with stress triaxiality varying from-0.33 to 0.46 were *** microstructures of the deformed specimens under diverse deformation conditions are probed to reveal the mechanism of hot deformation *** experimental results indicate that the work-hardening and dynamic softening are competitive during the hot compression process,and the dynamic softening is more obvious under low deformation temperature and high strain *** microstructural analysis manifests that the dynamic recovery gets predominant at high deformation temperature to produce fine ***,the dynamic recrystallization becomes more dominant as the strain rate decreases,which is sensitive to the stress *** addition,both the modified Johnson-Cook model and straincompensated Arrhenius-type function are suitable for describing the flow behavior of 2297 alloy,while the latter reveals a more accurate ***,the predictability of the two kinds of models is worsened with the transformation of stress triaxiality,and the validity of the Arrhenius-type model is restricted by high stress triaxiality.

BACKGROUND Primary squamous cell carcinoma(SCC)of the renal parenchyma is extremely rare,with only nine cases *** SUMMARY This study reports a 51-year-old man with primary SCC of the renal *** patient was admitted with recurrent dull pain and discomfort in the right lumbar region,which had worsened over 2 weeks,accompanied by painful gross *** antigen(SCCA)levels were elevated,and imaging revealed a renal mass with associated *** patient underwent laparoscopic unilateral nephrectomy and lymph node *** pathology confirmed highly differentiated SCC with necrosis in the right renal parenchyma,with negative renal pelvis and *** pathological stage was *** months after surgery,the tumor recurred with involvement of the liver,right psoas major muscle,and inferior vena *** patient refused chemotherapy and succumbed to the disease 6 months postoperatively due to disease *** We report a case of primary SCC of the renal parenchyma,a rare renal *** clinical symptoms,laboratory tests,and imaging findings are nonspecific,making accurate and timely diagnosis *** to the literature,for patients with renal calculi accompanied by a renal mass,elevated serum SCCA levels,and magnetic resonance imaging showing cystic or cystic-solid masses within the kidney with pseudocapsules and heterogeneous mild enhancement,the possibility of this disease should be considered.

A 2.5D finite element approach for predicting ground vibrations generated by vertical track irregularities

Dynamic responses of track structure and wave propagation in nearby ground vibration become significant when train operates on high speeds. A train-track-ground dynamic interaction analysis model based on the 2.5D finite element method is developed for the prediction of ground vibrations due to vertical track irregularities. The one-quarter car mode,1 is used to represent the train as lumped masses connected by springs. The embankment and the underlying ground are modeled by the 2.5D finite element approach to improve the computation efficiency. The Fourier transform is applied in the direction of train＇s movement to express the wave motion with a wave-number. The one-quarter car model is coupled into the global stiffness matrix describing the track-ground dynamic system with the displacement compatibility condition at the wheel-rail interface, including the irregularities on the track surface. Dynamic responses of the track and ground due to train＇s moving loads are obtained in the wave-number domain by solving the governing equation, using a conventional finite element procedure. The amplitude and wavelength are identified as two major parameters describing track irregularities. The irregularity amplitude has a direct impact on the vertical response for low-speed trains, both for short wavelength and long wavelength irregularities. Track irregularity with shorter wavelength can generate stronger track vibration both for low-speed and high-speed cases. For low-speed case, vibrations induced by track irregularities dominate far field responses. For high-speed case, the wavelength of track irregularities has very little effect on ground vibration at distances far from track center, and train＇s wheel axle weights becomes dominant.
