Saddle pulmonary embolism is not a sign of high-risk deterioration in non-high-risk patients: A propensity score-matched study

BACKGROUND: There is controversy regarding whether saddle main pulmonary artery(MPA) embolism represents a high risk of deterioration in non-high-risk acute pulmonary embolism(PE) patients. This study aims to address this issue by conducting a propensity score matching(PSM) ***: A total of 727 non-high-risk acute PE patients were retrospectively evaluated. We evaluated the Bova score and risk stratification to examine the risk of deterioration. Deterioration defined as any adverse event within 30 days after admission. Computed tomographic pulmonary angiography was used to identify the embolism type. All patients were matched into four subgroups by PSM according to age, sex, Bova score, and risk stratification:(1) MPA and non-MPA embolism;(2) non-saddle MPA and non-MPA embolism;(3) saddle MPA and non-saddle MPA embolism;(4) saddle MPA and non-MPA embolism. Correlations were analyzed using Cox regression analysis, and deterioration risk was compared between subgroups using Kaplan-Meier ***: Cox regression analysis revealed that MPA embolism was correlated with deterioration, regardless of whether saddle MPA embolism was included or excluded. Saddle MPA embolism was not correlated with deterioration, regardless of comparison with non-saddle MPA embolism or non-MPA embolism. Patients with MPA and non-saddle MPA embolism presented a high risk for deterioration(logrank test=5.23 and 4.70, P=0.022 and 0.030, respetively), while patients with saddle MPA embolism were not at a high risk of deterioration(log-rank test=1.20 and 3.17, P=0.729 and 0.077, respetively).CONCLUSIONS: Saddle MPA embolism is not indicative of a high risk of deterioration in nonhigh-risk acute PE patients.

The probiotic Akkermansia muciniphila(A. muciniphila) is an intestinal bacterium that was first identified in human feces in 2004. Its specialization in mucin degradation makes it a key microorganism that maintains intestinal mucosal barrier function. As an unique representative strain of the phylum Verrucomicrobia that can be cultured in vitro, A. muciniphila is much easier to detect by metagenomic analysis of intestinal flora. In the past few years, A. muciniphila has been getting increasing attention for the positive correlation between its intestinal colonization and host homeostatic metabolism. In this review, we summarize the relationship between A. muciniphila and host health and diseases, especially focusing on metabolic diseases and related mechanisms, as well as the natural food and drug-derived substrates affecting its colonization in the host, expecting to provide evidence and clues for the development of drugs targeting A. muciniphila.

This article concerns the existence of multi-bump positive solutions for the following logarithmic Schrödinger equation:{−Δu+λV(x)u=ulogu^(2)inRN,u∈H^(1)(R^(N)),where N≥1,⋋>0 is a parameter and the nonnegative continuous function V:ℝ^(N)→ℝhas potential wellΩ:=int V^(−1)(0)which possesses k disjoint bounded componentsΩ=∪^(k)_(j)=1Ω_(j).Using the variational methods,we prove that if the parameter⋋>0 is large enough,then the equation has at least 2^(k)−1 multi-bump positive solutions.

In this paper, a nonlinear nonautonomous predator-prey dispersion model with continuous distributed delay is studied, where all parameters are time-dependent. In this system consisting of n-patches the prey species can disperse among n-patches, but the predator species is confined to one patch and cannot disperse. It is proved that the system is uniformly persistent under any dispersion rate effect. Furthermore~ some sufficient conditions are established for the existence of a unique almost periodic solution of the system. The example shows that the criteria in the paper are new, general and easily verifiable.

Coastal overwash is a natural phenomenon that commonly occurs during storm events and can cause considerable changes in nearshore morphology within a short *** this study,a complete set of empirical overwash transport algorithms is developed and introduced into a phase-averaged wave-current-sediment transport coupling model that integrates the Finite-Volume Community Ocean Model(FVCOM)and the Simulating Waves Nearshore(SWAN)*** resulting morphological evolution model can simulate coastal *** against the data obtained from multiple sets of laboratory overwash experiments demonstrates that the model performs relatively well in simulating morphological changes caused by runup overwash and inundation overwash under different hydrodynamic and beach profile *** sensitivity of each empirical coefficient in the overwash transport algorithms is comprehensively *** effects of each coefficient on the output of the model are discussed,and a recommended value range is provided for each coefficient.

A simple and efficient procedure for the synthesis of substituted benzothiazoles through condensation of 2-aminothiophenol with aromatic aldehydes in the presence of H2O2/HCl system in ethanol at room temperature is described. The target compounds have been characterized by ^1H NMR, ^13C NMR, IR and MS. Short reaction time, easy and quick isolation of the products, and excellent yields are the main advantages of this procedure.

Parameters Tuning of Model Free Adaptive Control Based on Minimum Entropy

Dynamic linearization based model free adaptive control(MFAC) algorithm has been widely used in practical systems, in which some parameters should be tuned before it is successfully applied to process industries. Considering the random noise existing in real processes, a parameter tuning method based on minimum entropy optimization is proposed,and the feature of entropy is used to accurately describe the system uncertainty. For cases of Gaussian stochastic noise and non-Gaussian stochastic noise, an entropy recursive optimization algorithm is derived based on approximate model or identified model. The extensive simulation results show the effectiveness of the minimum entropy optimization for the partial form dynamic linearization based MFAC. The parameters tuned by the minimum entropy optimization index shows stronger stability and more robustness than these tuned by other traditional index,such as integral of the squared error(ISE) or integral of timeweighted absolute error(ITAE), when the system stochastic noise exists.
