Intestinal endometriosis:Diagnostic ambiguities and surgical outcomes

BACKGROUND Endometriosis is a common disease for women of reproductive age. However,when it involves intestines, it is difficult to diagnose preoperatively because its symptoms overlap with other diseases and the results of evaluations can be unspecific. Thus it is important to know the clinical characteristics of intestinal endometriosis and how to exactly *** To analyze patients in whom intestinal endometriosis was diagnosed after surgical treatments, and to evaluate the clinical characteristics of preoperatively misdiagnosed *** We retrospectively reviewed the pathologic reports of 30 patients diagnosed as having intestinal endometriosis based on surgical specimens between January2000 and December 2017. We reviewed their clinical characteristics and surgical *** Twenty-three(76.6%) patients showed symptoms associated with endometriosis,with dysmenorrhea being the most common(n = 9, 30.0%). Thirteen patients(43.3%) had a history of pelvic surgeries. Ten patients(33.3%) had a history of treatment for endometriosis. Only 4 patients(13.3%) had a diagnosis of endometriosis based on endoscopic biopsy findings. According to preoperative evaluations, 13 patients(43.3%) had an initial diagnosis of pelvic endometriosis and 17 patients(56.6%) were misdiagnosed as having other diseases. The most common misdiagnosis was submucosal tumor in the large intestine(n = 8, 26.7%),followed by malignancies of the colon/rectum(n = 3, 10.0%) and ovary(n = 3,10.0%). According to the Clavien-Dindo classification, 5 complications were grade I or II and 2 complications were grade IIIa. YBa2Cu3O7-x(YBCO) films were fabricated on an LAO substrate using the trifluoroacetic acid-metal organic deposition(TFA-MOD) method and the effects of the humidity and heat treatment temperatures on the microstructure,degree of texture and critical properties of the films were *** order to understand the combined effects of the humidity and the calcining and firing temperatures on critical properties,heat-treatment was performed at various temperatures with the other processing variables *** films were calcined at 400-430 ℃ and fired at 750-800 ℃ in a 0-12.1% humidified Ar-O2 *** texture was determined by pole-figure *** amount of the BaF2 phase was effectively reduced and a sharp and strong biaxial texture was formed under a humidified atmosphere,which led to increased critical *** addition,the microstructure varied significantly with firing temperature but changed little with calcining *** highest IC of 40 A/cm-width,which corresponds to JC value of 1.8 MA/cm2,was obtained for the films fired at 775 ℃(in 12.1% humidity) after calcining at 400-430 ℃.It is likely that the highest IC value is due to the formation of a more pure YBCO phase,c-axis grains,and a denser microstructure.

在加热过程中,应用一种新型的快速加热装置——铂膜反应器,及时准确地获得样品质量和颗粒温度,来研究石灰石的煅烧分解,并对石灰石加热速率高达750℃/s下的分解反应特性,及其对分解后产生的氧化钙吸收CO2和脱硫的化学反应特性进行了研究.实验表明,在初始加热阶段,高升温速率明显阻碍了煅烧分解的进行,这对分解后的CaO吸收CO2的效果影响不大.当加热速率升高时,石灰石分解后产生的CaO吸收CO2和脱硫过程中,钙利用率显著下降.

BACKGROUND Optimal surveillance strategies for stage III colorectal cancer(CRC)are lacking,and intensive surveillance has not conferred a significant survival *** To examine the association between surveillance intensity and recurrence and survival rates in patients with stage III *** Data from patients with pathologic stage III CRC who underwent radical surgery between January 2005 and December 2012 at Asan Medical Center,Seoul,Korea were retrospectively *** consisted of abdominopelvic computed tomography(CT)every 6 mo and chest CT annually during the 5 year follow-up period,resulting in an average of three imaging studies per *** who underwent more than the average number of imaging studies annually were categorized as high intensity(HI),and those with less than the average were categorized as low intensity(LI).RESULTS Among 1888 patients,864(45.8%)were in HI ***,sex,and location were not different between *** group had more advanced T and N stage(P=0.002,0.010,each).Perineural invasion(PNI)was more identified in the HI group(21.4%vs 30.3%,P<0.001).The mean overall survival(OS)and recurrence-free interval(RFI)was longer in the LI group(P<0.001,each).Multivariate analysis indicated that surveillance intensity[odds ratio(OR)=1.999;95%confidence interval(CI):1.680–2.377;P<0.001],pathologic T stage(OR=1.596;95%CI:1.197–2.127;P=0.001),PNI(OR=1.431;95%CI:1.192–1.719;P<0.001),and circumferential resection margin(OR=1.565;95%CI:1.083–2.262;P=0.017)in rectal cancer were significantly associated with *** mean post-recurrence survival(PRS)was longer in patients who received curative resection(P<0.001).Curative resection rate of recurrence was not different between HI(29.3%)and LI(23.8%)groups(P=0.160).PRS did not differ according to surveillance intensity(P=0.802).CONCLUSION Frequent surveillance with CT scan do not improve OS in stage III CRC *** need to evaluate role of other surveillance method rather tha

This study was conducted to recover edible bird’s nest(EBN)hydrolysates from different grades of EBN,including the industrial by-products,using enzymatic *** nutrient,physicochemical properties and antioxidant activities of the recovered hydrolysates at different hydrolysis times were *** showed that the recovery yield of enzymatic hydrolysis was above 89%for all grades of EBN and the degree of hydrolysis increased over *** content(0.321-0.433 mg/L)was below the permissible tolerance level for all ***,the antioxidant activities(DPPH and ABTS scavenging activities and ferric reducing antioxidant powder(FRAP)activity)were significantly higher(P≤0.05)in hydrolysates recovered from EBN by-products(EBNhC and EBNhD)as compared to the high grade EBN hydrolysates(EBNhA and EBNhB).The in-vitro probiotic activity of EBN and its hydrolysates were examined using the probiotic bacterium Lactobacillus ***,EBN by-products hydrolysate(EBNhD)recorded the highest number of ***(1.1×1011 CFU/mL),indicating that low grade EBN has the potential as prebiotic material that promotes probiotic *** study demonstrated the concept of using EBN by-products hydrolysates for various applications,such as functional ingredients with enhanced bioactivities,to improve its economic value.

The flowability of a root canal sealer is clinically important because it improves the penetration of the sealer into the complex root canal system. The purpose of this study was to compare the flowabilities of four root canal sealers, measured using the simple press method （ISO 6876）, and their viscosities, measured using a strain-controlled rheometer. A newly developed, calcium phosphate-based root canal sealer （Capseal） and three commercial root canal sealers （AH Plus, Sealapex and Pulp Canal Sealer EWT） were used in this study. The flowabilities of the four root canal sealers were measured using the simple press method （n= 5） and their viscosities were measured using a strain-controlled rheometer （n=5）. The correlation between these two values was statistically analysed using Spearman＇s correlation test. The flow diameters and the viscosities of the root canal sealers were strongly negatively correlated （p= -0.8618）. The viscosity of Pulp Canal Sealer EWT was the lowest and increased in the following order： AH Plus〈Sealapex〈Capseal （P〈0.05）. All of the tested root canal sealers showed characteristic time- and temperature-dependent changes in their rheological properties. The viscosities measured using the strain-controlled rheometer were more precise than the flowabilities measured using the simple press method, suggesting that the rheometer can accurately measure the rheological properties of root canal sealers.

BACKGROUND Endoscopic submucosal dissection(ESD)has shown to be effective in management of colorectal neoplasm in the Asian countries,while its implementation in Western countries where endoscopic mucosal resection(EMR)is preferred is still *** To compare the surgical,histological,and oncological outcomes between ESD and EMR in the treatment of colorectal polyps,with subgroup analysis comparing the efficacy of ESD and EMR between Japan and the rest of the *** Embase and Medline databases were searched from inception to October 2020 in accordance with PRISMA guidelines for studies comparing en bloc,complete resection,margin involvement,resection time,need for additional surgery,complications,and recurrence rate of ESD with *** Of 281344 colorectal polyps from 21 studies were *** compared to EMR,the pooled analysis revealed ESD was associated with higher en bloc and complete resection rate,and lower lateral margin involvement and *** led to increased procedural time,need for additional surgery,and perforation *** significant difference in bleeding risk was found between the two ***-regression analysis suggested only right colonic polyps correlated with an increased perforation risk in *** including polyp size and invasion depth did not significantly influence the en bloc and complete resection rate,bleeding risk and *** subgroup analysis,Japan performed better than the rest of the world in both ESD and EMR with perforation risk of 4%and 0.0002%,respectively,as compared to perforation risk of 8%and 1%,respectively,in reports coming from rest of the *** ESD resulted in better resection outcomes and lower recurrence compared to *** appropriate training,ESD is preferred over EMR as the first-line therapy for resection of colorectal polyps,without restricting to lesions greater than 20 mm and those with high suspicion of submucosal invasion.

AIM:There is some evidence of functional superiority of colonic J-pouch over straight coloanal anastomosis (CM) in ultralow anterior resection (ULAR) or intersphincteric resection. On the assumption that colonic J-pouch anal anastomosis is superior to straight CM in ULAR with upper sphincter excision (USE: excision of the upper part of the internal sphincter) for low-lying rectal cancer, we compare functional outcome of colonic J-pouch vsthe straight CM. METHODS: Fifty patients of one hundred and thirty-three rectal cancer patients in whom lower margin of the tumors were located between 3 and 5 cm from the anal verge received ULAR including USE from September 1998 to January 2002. Patients were randomized for reconstruction using either a straight (n = 26) or a colonic J-pouch anastomosis (n = 24) with a temporary diverting-loop ileostomy. All patients were followed-up prospectively by a standardized questionnaire [Fecal Inco-ntinence Severity Index (FISI) scores and Fecal Incontinence Quality of Life (FIQL) scales]. RESULTS: We found that, compared to straight anastomosis patients, the frequency of defecation was significantly lower in J-pouch anastomosis patients for 10 mo after ileostomy takedown. The FISI scores and FIQL scales were significantly better in J-pouch patients than in straight patients at both 3 and 12 mo after ileostomy takedown. Furthermore, we found that FISI scores highly correlated with FIQL scales. CONCLUSION: This study indicates that colonic J-pouch anal anastomosis decreases the severity of fecal incontinence and improves the quality of life for 10 mo after ileostomy takedown in patients undergoing ULAR with USE for low-lying rectal cancer.
