BACKGROUND Porocarcinoma is a rare type of skin cancer that originates from sweat gland *** is an aggressive malignant skin cancer that is difficult to diagnose clinically owing to its rarity and similarity to squamous cell carcinoma(SCC).CASE SUMMARY This case involved a 92-year-old woman,a farmer by profession,presented with an exophytic and verrucous mass on her left palm that had formed 2 years prior and caused chronic pain and frequent ***,the patient was diagnosed with SCC using a punch biopsy;however,a repeat biopsy with addi-tional immunohistochemical tests was performed for ***,the patient was diagnosed with porocarcinoma and reconstruction was planned using a full-thickness skin *** treatment,the range of motion of the palm was preserved,and the aesthetic outcome was *** 6 mo of follow-up,the patient was satisfied with the *** Porocarcinoma is commonly misdiagnosed as SCC;therefore,clinicians should consider porocarcinomas when evaluating mass-like lesions on the hands.

Strengthening mechanisms analysis and tailoring of bimodal grain structures for enhanced strength in CoCrFeMnNi high-entropy alloys

The CoCrFeMnNi high-entropy alloys(HEAs)with a(face-centered cubic) FCC structure has garnered considerable attention for its exceptional ductility and strain hardening ***,its yield strength is insufficient for structural *** this study,strengthening mechanisms in these HEAs were investigated to gain insight into the mechanical properties according to alloy powder ***,we present a novel approach to achieve both high strength and high ductility through the creation of a bimodal structure consisting of both coarse and fine grains via gas atomization and spark plasma sintering processes.A bimodally structured HEA prepared with a mass ratio of 2:8 between coarse particles(75-106 μm) and fine particles(≤25 μm)yielded optimal results,with a strength of 491.95 MPa and elongation of 19.64%.This strength value represents an~41% increase compared with the sample that displayed a fine single microstructure(347.08 MPa for yield strength).The strength enhancement was attributed to the prevention of plastic deformation initiation from the fine particles during *** innovative approach to the creation of HEAs with bimodal structures shows promise for various applications,such as structural components that require a combination of high strength and high ductility.

Background: Acupotomy, a more invasive procedure than acupuncture, involves the use of a thicker needle with an integrated knife at the tip, necessitating safety research. We aimed to define relevant adverse events(AEs) and create a standardized form of the ACUPOtomy-related AEs CHECKlist(ACUPOCHECK).Methods: Before conducting the Delphi process, a systematic review and pilot prospective study were conducted to gather information on previously reported AEs. Using these data, pilot versions of the ACUPOCHECK and Delphi questionnaires were developed. The Delphi questionnaire involved selecting types of AE for inclusion, establishing separate criteria for acupotomy-related AEs, and achieving a consensus on AE assessment. Thirteen Korean doctors with experience in acupotomy or AE research were recruited to participate in each Delphi round. Consensus was considered to have been reached if the critical value for the content validity ratio met or exceeded ***: The final ACUPOCHECK was developed using four rounds of the Delphi method and one face-toface consensus meeting. It included 12 local AEs(pain, hemorrhage, bruise, hematoma, edema, pruritus,rash, infection, nerve damage, dysesthesia, movement impairment, and pneumothorax) and 14 systemic AEs(disease aggravation, needle fatigue, sleepiness, procedural nausea, procedural vomiting, procedural headache, procedural dizziness, sweating, procedural shock, syncope, dyspnea, procedural pain, sleep disorder, and postprocedural infection). Separate criteria were established for pain, hemorrhage and bruising: pain was defined as pain that occurrs during daily activities and persists for longer than 72 h,hemorrhage as bleeding that continues for ≥ 3 min despite pressure application, and bruising as having a bruise with a diameter of ≥ 3 cm. Long-term outcome of stem cell transplantation with and without anti-tumor necrotic factor therapy in perianal fistula with Crohn’s disease

BACKGROUND Stem cell transplantation is a promising therapeutic option for curing perianal fistula in Crohn’s disease(CD).Anti-tumor necrotic factor(TNF)therapy combined with drainage procedure is effective as ***,previous studies are limited to proving whether the combination treatment of biologics and stem cell transplantation improves the effect of fistula *** This study aimed to evaluate the long-term outcomes of stem cell transplantation and compare Crohn’s perianal fistula(CPF)closure rates after stem cell transplantation with and without anti-TNF therapy,and to identify the factors affecting CPF closure and *** The patients with CD who underwent stem cell transplantation for treating perianal fistula in our institution between Jun 2014 and December 2022 were *** data were compared according to anti-TNF therapy and CPF *** A total of 65 patients were *** median age of females was 26 years(range:21-31)and that of males was 29(44.6%).The mean follow-up duration was 65.88±32.65 months,and complete closure was observed in 50(76.9%)*** closure rates were similar after stem cell transplantation with and without anti-TNF therapy(66.7%vs 81.6%at 3 year,P=0.098).The patients with fistula closure had short fistulous tract and infrequent proctitis and anorectal stricture(P=0.027,0.002,and 0.008,respectively).Clinical factors such as complexity,number of fistulas,presence of concurrent abscess,and medication were not significant for *** cumulative 1-,2-,and 3-year closure rates were 66.2%,73.8%,and 75.4%,*** Anti-TNF therapy does not increase CPF closure rates in patients with stem cell ***,both refractory and non-refractory CPF have similar closure rates after additional anti-TNF *** tract length,proctitis,and anal stricture are risk factors for non-closure in patients with CPF after stem cell transplantation.
