A three-year longitudinal study of the relation between left atrial diameter remodeling and atrial fibrillation ablation outcome

Background The long-term prognostic influence of left atrial diameter （LAD） remodeling on the status of post-radiofrequency catheter ablation （RI CA） atrial fibrillation （AF） is unclear. This study employed a two-stage model from 3-year echocardiographic data to ascertain whether the two-stage model predicts RFCA outcome more favorably than models using the baseline LAD. Methods Data were retrospectively collected from 263 consecutive patients with drug-refractory AF undergoing RFCA. Regular echocardiographic measurements of LAD were performed at baseline, 1, 3, 6, and 12 months and then every 6 months after RFCA. Sex, age, type of AF, number of RFCA, and AF status were recorded. We obtain the actual （predicted） 3-year LAD using a longitudinal linear mixed model （1st stage）. Logistic regression models based on the baseline LAD （Model 1）, actual （predicted） 3-year LAD （Model 2）（2nd stage）, and observed 3-year LAD （Model 3） were constructed to predict RFCA outcome. The area under the receiver operating characteristic curve （AUC） were used to assess the performance of models. Results The lowess smoothed curve indicated that the LAD declined over the first three months and remained stable up to 36 months after RFCA. The degree of LAD reduction was significantly influenced by the baseline LAD. Non-paroxysmal AF, large LAD and female gender were significant predictors of AF recurrence. Model 2 had the largest AUC among the three models. Conclusions This longitudinal study-based two-stage model outperforms the original logistic model using the baseline LAD. Non-paroxysmal AF, larger LAD and female gender are significant predictors of RFCA failure.

Estimation of plasma parameters of X-pinch with time-resolved x-ray spectroscopy

We estimate the parameters of a Cu plasma generated by an X-pinch by comparing experimentally measured x-rays with synthetic data.A filtered absolute extreme ultraviolet diode array is used to measure time-resolved x-ray spectra with a spectral resolution of∼1 keV in the energy range of 1–10 *** synthetic spectra of Cu plasmas with different electron temperatures,electron densities,and fast electron fractions are calculated using the FLyCHK *** quantitative comparison with the measured spectrum,two x-ray power ratios with three different spectral ranges are *** observe three x-ray bursts in X-pinch experiments with two Cu wires conducted on the SNU X-pinch at a current rise rate of∼0.2 kA/*** of the spectra reveals that the first burst comprises x-rays emitted by hot spots and electron beams,with characteristics similar to those observed in other *** second and third bursts are both generated by longlived electron beams formed after the neck structure has been completely *** the second burst,the formation of the electron beam is accompanied by an increase in the electron density of the background ***,the long-lived electron beams generate the additional strong x-ray bursts while maintaining a plasma channel in the central region of the ***,they emit many hard x-rays(HXRs),enabling the SNU X-pinch to be used as an HXR *** study confirms that the generation of long-lived electron beams is crucial to the dynamics of X-pinches and the generation of strong HXRs.

Overexpression of the promyelocytic leukemia gene suppresses growth of human bladder cancer cells by inducing Gl cell cycle arrest and apoptosis

Objectives To examine the anti-oncogenic effects of promyelocytic leukemia (PML) on bladder cancer and to explore its molecular mechanisms of growth *** Wild-type PML was transfected into bladder cancer cells (5637 cell) and expressed in a replication-deficient adenovirus-mediated gene delivery system and introduced into human bladder cancer cells (5637 cell) in vitro and in vivo. The effect and mechanisms of the PML gene in cell growth, clonogenicity, and tumorigenicity of bladder cancer cells were studied using in vitro and in vivo growth assays, soft agar colony-forming assay, cell cycle analysis, apoptosis assay and in vivo tumorigenicity *** Overexpression of PML in 5637 cells significantly reduced their growth rate and clonogenicity on soft agar. PML suppressed bladder cancer cell growth by inducing G1 cell cycle arrest and apoptosis. Adenovirus-mediated PML ( Ad-PML) significantly suppressed the tumorigenicity and growth of bladder cancer cells. Novel rapid tissue lysis method to evaluate cancer proteins: Correlation between elevated Bcl-X_L expression and colorectal cancer cell proliferation

AIM： We optimized a rapid and efficient tissue lysis method using the MagNA Lyser （Roche, Germany）. Using this novel method combined with immunoblot analysis, we investigated the correlation between abnormal Bcl-XL expression and clinicopathological characteristics in colorectal cancer. METHODS： Tissue samples from Sprague-Dawley rats were tested to determine optimal lysis conditions for use with MagNA Lyser. We next used the new method to extract tissue proteins from the tumor tissue of a colorectal cancer patient. The availability of extractable tissue proteins for proteomic study was demonstrated by two-dimensional （2D） gel electrophoresis and subsequent matrix-assisted laser desorption/ionization time-of-flight （MALDI-TOF） mass spectrometry. In addition, we prepared tissue lysates from paired tumor tissues and adjacent nontumor tissues of 50 colorectal carcinoma patients. Ensuing immunoblot analyses were performed to detect the level of Bcl-X, expression. RESULTS： The optimal sample sizes processed were found to be around 200 mg, with oscillation frequency of 6 500 r/rain for 80 s. Test of the first human tissue lysate confirmed that the MagNA Lyser method was adequate for protein extraction and subsequent identification by current proteomic protocols. The method was also applicable to immunoblot analysis. Thirty of 50 （60%） colorectal patients exhibited higher level of Bcl-XL expression in their tumor tissues. Raised level of Bcl-XL expression correlated with patients＇ gender and tumor cell proliferation index （P= 0.037 and P〈0.001, respectively）, but was independent of clinicopathological characteristics and overall survival. CONCLUSION： We report a novel tissue lysis method applicable to proteomic and immunoblot analyses, which can facilitate the discovery and detection of cancer protein alterations.

Multiple bowel intussusceptions from metastatic localized malignant pleural mesothelioma:A case report

Localized malignant pleural mesothelioma (LMPM) is a rare occurrence, and gastrointestinal intra-luminal metastases have not previously been reported. Herein, we report a patient with LMPM who presented with a local recurrence 10 mo after initial en bloc surgical resection. Abdominal computed tomography was performed for intractable, vague abdominal pain with episodic vomiting, which showed a "target sign" over the left lower quadrant. Laparotomy revealed several intra-luminal metastatic tumors in the small intestine and colon and a segmental resection of metastatic lesions was performed. Unfortunately, the patient died of sepsis despite successful surgical intervention. Though local recurrence is more frequent in LMPM, the possibility of distant metastasis should not be ignored in patients with non-specifi c abdominal pain.

Candidal liver abscesses and cholecystitis in a 37-year-old patient without underlying malignancy

We report a case of candidal liver abscesses and concomitant candidal cholecystitis in a diabetic patient, in whom differences were noted relative to those found in patients with hematologic malignancies. In our case, the proposed entry route of infection is ascending retrograde from the biliary tract. Bile and aspirated pus culture repeatedly tested positive, and blood negative, for Candida albicans and Candida glabrata. Cholecystitis was cured by percutaneous gallbladder drainage and amphotericin B therapy. The liver abscesses were successfully treated by a cumulative dosage of 750 mg amphotericin B. We conclude that in cases involving less immunocompromised patients and those without candidemia, a lower dosage of amphotericin B may be adequate in treating candidal liver abscesses.

We demonstrate the transmission of directly modulated 10-Gb/s WDM signals over 320 km of negative dispersion fiber (dispersion: -2.5 ps/km/nm @1550 nm) without dispersion compensation. The results indicate that a regional metro WDM network could be implemented cost-effectively by using the proposed negative dispersion fiber and direct modulated lasers.
