A six-year descriptive epidemiological study of human coronavirus infections in hospitalized patients in Hong Kong

We conducted a six-year epidemiological study on human coronaviruses（HCoVs） circulating in Hong Kong, using 8275 nasopharyngeal samples from patients with acute respiratory tract infections. HCoVs were detected in 77（0.93%） of the samples by a pan-HCoV RT-PCR assay. The most frequently detected HCoV species was HCoV-OC43（0.58%）, followed by HCoV-229E（0.15%）,HCoV-HKU1（0.13%） and HCoV-NL63（0.07%）. HCoVs were detected throughout the study period（September 2008–August 2014）, with the highest detection rate from September 2010 to August2011（22/1500, 1.47%）. Different seasonal patterns of each HCoV species in Hong Kong were ***-OC43 was predominant in the fall and winter, whereas HCoV-HKU1 showed peak activity in winter, with a few cases occurred in spring and summer. HCoV-229 E mainly occurred in winter and spring, while HCoV-NL63 was predominant in summer and autumn. HCoVs most commonly infect the elderly and young children, with median age of 79.5 years（range, 22 days to 95 years）.Intriguingly, the detection rate of HCoV-OC43 in the age group of ＆gt; 80 years（26/2380, 1.09%） was significantly higher than that in the age group of 0–10 years（12/2529, 0.47%）（P 〈 0.05）. These data provides new insight into the epidemiology of coronaviruses.

Ischemic preconditioning（IPC） is a condition of sublethal transient global ischemia and exhibits neuroprotective effects against subsequent lethal ischemic ***,in this study,examined the neuroprotective effects of IPC and its effects on immunoreactive changes of antioxidant enzymes including superoxide dismutase（SOD） 1 and SOD2,catalase（CAT） and glutathione peroxidase（GPX） in the gerbil hippocampal CA1 region after transient forebrain *** neurons of the stratum pyramidale（SP） in the hippocampal CA1 region of animals died 5 days after lethal transient ischemia without IPC（8.6%（ratio of remanent neurons） of the sham-operated group）;however,IPC prevented the pyramidal neurons from subsequent lethal ischemic injury（92.3%（ratio of remanent neurons） of the sham-operated group）.SOD1,SOD2,CAT and GPX immunoreactivities in the sham-operated animals were easily detected in pyramidal neurons in the stratum pyramidale（SP） of the hippocampal CA1 region,while all of these immunoreactivities were rarely detected in the stratum pyramidale at 5 days after lethal transient ischemia without ***,their immunoreactivities in the sham-operated animals with IPC were similar to（SOD1,SOD2 and CAT） or higher（GPX） than those in the sham-operated animals without ***,their immunoreactivities in the stratum pyramidale of the ischemia-operated animals with IPC were steadily maintained after lethal ischemia/*** of western blot analysis for SOD1,SOD2,CAT and GPX were similar to immunohistochemical *** conclusion,IPC maintained or increased the expression of antioxidant enzymes in the stratum pyramidale of the hippocampal CA1 region after subsequent lethal transient forebrain ischemia and IPC exhibited neuroprotective effects in the hippocampal CA1 region against transient forebrain ischemia.

Central Neurocytoma and Epidermoid Tumor Occurring as Collision Tumors: A Rare Association

Different brain tumors of distinct histology can co-exist in the setting of phakomatoses or as a complication of radiotherapy. In the absence of these predisposing factors, this phenomenon is uncommon. When the lesions are in close proximity they are described as collision tumors and are extremely rare. A 58-year-old woman presented with persistent headache and cognitive decline for three months. Magnetic resonance imaging revealed a tumor arising from the atrium of the left lateral ventricle with heterogeneous contrast enhancement. This intraventricular lesion was adjacent to another extensive infiltrating tumor of the basal cisterns. Operative findings revealed a vascular ventricular tumor and gross total resection was achieved. An adjacent avascular basal cistern tumor with a pearly white sheen was encountered and partial excision was performed. The histopathological diagnosis was central neurocytoma and epidermoid tumor. There is only one documented description of a central neurocytoma co-existing with a tumor of different pathology. To our knowledge, this is the first reported collision tumor case involving central neurocytoma. Since the incidence of both lesions co-existing juxtaposed is extremely low, a chronic oncogenetic inflammatory process stimulated by the epidermoid tumor to the subventricular region is suggested. Other mechanisms for tumor collision are discussed and we suggest a classification system for this rare association to reflect their pathogenesis.

Evaluation of the Surface of Nitinol after MR Polishing Process

Nitinol is used in various industries, especially for biomaterials. Generally, the surface integrity of sub-micron level and non-surface flaw is required in nitinol for biomaterial. However, surface flaws, such as scratches, pits, and residual stress, can occur on the polished surface, and it is difficult to remove these surface flaws. The corrosion is accelerated continuously and causes bad influence in the body. Therefore, in this study, conventional polishing and MR polishing were used to minimize the corrosion and residual stress when the nitinol is used as biomaterial. X-ray diffraction and Tafel extrapolation were used to investigate the residual stress and the corrosion, and the strain hardening was investigated by nanoindentation. Surface integrities were also evaluated by SEM image and surface roughness results. As a result, the increased residual stress in conventional polishing was decreased by MR polishing. The result of corrosion test was improved from ?301 mV to ?280 mV. The residual stress was improved from ?38.18 MPa to ?31.17 MPa. Finally, it is certified that MR polishing can be used to minimize surface flaws and to improve the corrosion properties.

Live attenuated vaccines might elicit mucosal and sterilizing immunity against SARS-CoV-2 that the existing mRNA,adenoviral vector and inactivated vaccines fail to ***,we describe a candidate live attenuated vaccine strain of SARS-CoV-2 in which the NSP16 gene,which encodes 2′-O-methyltransferase,is catalytically disrupted by a point *** virus,designated d16,was severely attenuated in hamsters and transgenic mice,causing only asymptomatic and nonpathogenic infection.A single dose of d16 administered intranasally resulted in sterilizing immunity in both the upper and lower respiratory tracts of hamsters,thus preventing viral spread in a contact-based transmission *** also robustly stimulated humoral and cell-mediated immune responses,thus conferring full protection against lethal challenge with SARS-CoV-2 in a transgenic mouse *** neutralizing antibodies elicited by d16 effectively cross-reacted with several SARS-CoV-2 *** immunoglobulin A was detected in the blood and nasal wash of vaccinated *** work provides proof-of-principle evidence for harnessing NSP16-deficient SARS-CoV-2 for the development of live attenuated vaccines and paves the way for further preclinical studies of d16 as a prototypic vaccine strain,to which new features might be introduced to improve safety,transmissibility,immunogenicity and efficacy.

背景强迫症是一种较为常见的慢性、致残的疾病，患者出现无法自控的强迫思维及强迫行为。10％以上的强迫症病人对药物和行为治疗无效。对一些常规治疗无效的难治性强迫症患者，神经外科已经开展一些手术治疗，例如：扣带前回毁损、脑白质切断术、尾状核下束切断、内囊前支毁损。扣带前回毁损相对来说比较安全同时没有明显的副作用，但是也有术后存在认知障碍的报道。为判断立体定向双侧扣带前回毁损是否是一种适合难治性强迫症的治疗，我们评价了立体定向双侧扣带前回毁损术的长期疗效及其副作用。方法患者筛选：在1999年3月至2003年4月间，17例难治性强迫症患者（男10例，女7例）接受了双侧扣带前回毁损术，并且接受了超过2年的随访。患者的选择标准如下：患者符合美国精神障碍诊断与统计手册第4版（DSM-Ⅳ）强迫症的诊断标准，病程超过3年，疾病导致社会心理功能降低。每位患者皆由两位精神科医师依照DSM-Ⅳ的AxisⅠ和AxisⅡ的疾病诊断标准行临床诊断。如伴有精神病性症状，就先用一线药物彻底治疗。手术方法：立体定向的靶点通过与MRI配套的Leksell适配器，在GESigma1．5 T的机器上得以确认。局部麻醉下，在两侧的冠状缝中线外20mm颅骨钻孔。将尖端直径．1．8mm，长度10mm的电极插至靶点，用85℃，90S进行射频毁损。在双侧扣带前回，每侧各4个射频毁损灶，2个针道。第一个射频毁损灶在两侧侧脑室前角后缘15mm，侧脑室顶部上2mm，旁开中线7mm处。射频电极接着回撤8mm，制作第二个射频毁损灶。第三个射频毁损处在两侧侧脑室前角后缘22mm，侧脑室顶部上2mm，旁开中线7mm处。再往上8mm，制作第四个射频毁损灶。结果是出现类似椭圆柱形的射频毁损灶，大约高18mm、前后径13mm，横径6mm。术前和术后随访评估：两位精神科医师用Yale-Brown强迫症量表（Y-BOCS）、临床总体印象量表（CGI）、汉密顿抑郁量表（HAMD）、汉密顿焦虑量表（HAMA），分别在手术前1个月、术后12个月、术后24个月对患者进行评估。评估的精神科医师接受了培训，Kappa值超过0．75，以确保评估的准确性。在我们研究中，患者的Y-BOCS得分改善35％或更高、CGI评分为1（极其显著改善）或者2（显著改善）；由专业的临床精神科医生对患者进行一套固定的神经心理测试，测试分别在术前1个月、术后12个月、24个月进行。这些测试包括：K—WAIS测验，视觉空间功能（RCFT），Hopkins语言学习测验（HVLT），威斯康星卡片分类测验（WCST），口语流畅测验（COWAT）。K-WAIS评分由韦氏成人智力量表（WAIS—R）改编而来，以产生总智商（IQ）、言语IQ和行为IQ来测量智力水平。RCFT评分用于测试空间构建能力。记忆力也包括回忆的精确性、瞬时回忆、延迟回忆和识别指数等的得分。HVLT用于测量口头记忆能力和学习能力。使用WCST和COWAT评分来检查认知的灵活性和执行功能。在WCST中，正确、错误反应的总数，完成的项目数，持续的反应，持续的错误数都用于评分。COWAT评分采用字母翻译和目录翻译的单词数目，字母翻译用来检查语音的流利，目录翻译用来评价语义流利性。使用Wileoxon符号秩检验对术前的Y—BOCS，HAMD，HAMA评分和术后6、12个月的评分进行比较，使用Wileoxon符号秩检验对手术前和手术后12个月的K—WAIS，RCFT，HVLT，WCST，COWAT评分进行比较。统计软件采用SPSS10。结果扣带回毁损术后强迫、抑郁、焦虑症状均改善：Y-BOCS，CGI、HAMD，HAMA评分显示了扣带回毁损术的有效性，在12个月的随访中，与术前Y-BOCS评分相比，改善率为36％，在24个月的随访中，改善率升至48％。术前、术后认知功能和记忆的改变：手术前和手术后24个月的K-WAIS，RCFT，HVLT，COWAT评分没有显著性差异。在WCST评分中，与术前基线相比，术后的错误总数、持续错误数和反应数明显减少。术后的错误总数、持续错误数和反应数的改变说明在扣带回毁损术后执行功能的提高。在术后2—3个月，3例患者有记忆障碍，但持续时间少于2个月。结论强迫症的扣带回毁损术术后24个月的随访结果显示症状明显改善，但没有认知功能的改变，包括智商、言语、视觉空间记忆和执行功能等均没有改变。

为了解重要的生化模型甲酰胺在水溶液中的微观结构,采用全原子力场在全浓度范围内对甲酰胺溶液进行了分子动力学模拟,得到了溶液的径向分布函数,分析计算了溶质和溶剂分子间的相互作用,对甲酰胺和水分子的氢键缔合情况进行了分析.研究发现羰基侧的H原子与水分子能形成C—H…O弱相互作用.在作者早期的研究中发现,此相互作用对于阻碍甲酰胺的异构化具有重要意义,特别是当甲酰胺在溶液中含量增大时,此相互作用更加不能忽视.全浓度溶液的模拟表明,甲酰胺在稀浓度区可以促进水局部结构的增强,随FM浓度增加,由水的自身缔合转变为水与FM的交叉缔合,在FM高浓度区,两者的交叉缔合将逐渐被甲酰胺自身的线状缔合代替.

Post-inflammatory hyperpigmentation is common problem, but its treatment still remains challenging. Tranexamic acid has been used to treat or prevent excessive bleeding loss in various medical conditions. There have been some reports of the effect of oral and topical tranexamic acid for treatment of pigmented disorder. Herein we report on a case of female patient who showed improvement of PIH after oral and topical tranexamic acid administration.

Background:IgG4-related hypertrophic pachymeningitis is a relative newly recognized and rare manifestation of IgG4-related disease,an immune-mediated fibroinflammatory tumefactive *** than 80 patients have been reported in the literature,and it can mimic common neurosurgical *** describe the clinical presentation of two patients that were initially considered to have a subdural collection,tuberculous meningitis,and a cervical spinal meningioma,but were eventually diagnosed with this *** presentation:Two ethnic Chinese men,86 and 62 years old,experienced a 4-week history of *** patients had a history of autoimmune disease,namely glomerulonephritis and Grave’s disease,*** resonance brain imaging revealed diffuse dural thickening with the latter patient exhibiting homogeneous and intense gadolinium-contrast *** the 86-year-old patient also had progressive bilateral visual loss,giant cell arteritis was suspected and a 2-week course of glucocorticoid therapy was prescribed,but his symptoms failed to *** 62-year-old patient also had accompanying low-grade fever and was treated empirically as having tuberculous meningitis although there were no confirmatory microbiological *** patient further developed right hemiparesis,and additional imaging revealed a C4/5 intradural-extramedullary contrast-enhancing lesion resembling a meningioma causing cord *** patients underwent neurosurgical intervention with the former undergoing a dural biopsy and the latter having the cervical lesion *** final diagnosis was IgG4-related hypertrophic pachymeningitis with the hallmark histological features of lymphoplasmacytic infiltration of IgG4+plasma cells,storiform fibrosis,and obliterative *** addition,their serum IgG4 levels were elevated(i.e.,>135 mg/dL).Both patients received at least 6 months of glucocorticoid therapy while the latter also had *** symptom
