Role of adjuvant chemotherapy on recurrence and survival in patients with resected ampulla of Vater carcinoma

BACKGROUND Owing to rarity of disease and lack of prospective studies, data supporting the role of adjuvant chemotherapy in ampulla of Vater(AoV) carcinoma is *** To evaluate whether adjuvant chemotherapy cases for AoV carcinoma had better disease-free survival(DFS) rates than cases of observation following curative *** We retrospectively analyzed the association between adjuvant chemotherapy and DFS and overall survival(OS) in patients with stage IB-Ⅲ AoV carcinoma who underwent curative surgical resection. Fluorouracil-based adjuvant chemotherapy was administered after surgery at the discretion of the physician. Adjusted multivariate regression models were used to evaluate the association between adjuvant chemotherapy and survival *** Of the total 104 patients who underwent curative surgery, 52 received adjuvant chemotherapy. Multivariate analysis revealed that higher histologic grade [hazard ratio(HR) = 2.24, P = 0.046], advanced tumor stage(HR = 1.85, P = 0.030), and vascular invasion(HR = 2.14, P = 0.010) were associated with shorter DFS. Adjuvant chemotherapy improved DFS compared to the observation group(HR =0.50, P = 0.015) and tended to be associated with a longer OS, although the difference was not statistically significant(HR = 0.58, P = 0.098).CONCLUSION Among patients with resected AoV carcinoma, the adjuvant chemotherapy group was not associated with a significant survival benefit compared to the observation group. however, on multivariate analysis adjusting for prognostic factors, adjuvant chemotherapy following surgery was an independent prognostic factor for DFS in patients with resected AoV carcinoma. Further studies are needed to investigate the effectiveness of adjuvant chemotherapy according to histologic phenotype.

Size-dependent transition of the deformation behavior of Au nanowires

Inspired by the controversy over tensile deformation modes of single-crystalline 〈110〉/{111} Au nanowires, we investigated the dependency of the deformation mode on diameters of nanowires using the molecular dynamics technique. A new criterion for assessing the preferred deformation mode-slip or twin propagation--of nanowires as a function of nanowire diameter is presented. The results demonstrate the size-dependent transition, from superplastic deformation mediated by twin propagation to the rupture by localized slips in deformed region as the nanowire diameter decreases. Moreover, the criterion was successfully applied to explain the superplastic deformation of Cu nanowires.

Electrical Stimulation of Auricular Acupressure for Dry Eye:A Randomized Controlled-Clinical Trial

Objective： To evaluate the clinical efficacy of electrical stimulation（ES） of auricular acupressure on reducing the ocular symptoms and signs before and after treatment for dry eye. Methods： The inclusion criteria were the tear film break-up time（TFBUT） below 5 s and a Schirmer test-Ⅰ below 5 mm in dry eyes with ocular symptoms for at least 6 months. Subjects were randomized into a treatment group（50 cases） with continuous low frequency ES under auricular acupressure at acupoints and a no ES under auricular acupressure（no-ES, control group, 50 cases） on the same acupoints. Auricular acupressure were stimulated with ES at 4 master points of both ears, which were performed twice a week for 4 weeks at each point for 30 s. The ocular symptoms, the TFBUT, and Schirmer test-Ⅰ were evaluated before and after this procedure. Results： There were significantly better scores in TFBUT（P=0.032）, the Schirmer test-Ⅰ（P=0.044） and ocular symptoms（P=0.029） at 3 months post-treatment in the treatment group than in the control group. The total effective rate in the treatment group was accomplished in 41（82%） of the 50 cases of dry eye. Conclusions： Auricular acupressure with ES at auricular acupoint improves ocular symptoms and signs of dry eye for a period of at least 3 months.

Characterization of Ga-doped ZnO Nanorods Synthesized via Microemulsion Method

Gallium-doped ZnO （GZO） nanorods were synthesized by microemulsion method with different types of surfactants. The phase and morphology of the above nanorods were investigated by scanning electron microscopy （SEM）, X-ray diffraction （XRD）, high resolution transmission electron microscopy （HR-TEM） and X-ray photoelectron spectroscopy （XPS）. SEM observations show that the ZnO nanorods have diameters around 70-200 nm and lengths up to several micrometers. The room temperature photoluminescence （PL） spectrum of GZO nanorods exhibited a sharp and strong ultraviolet bandgap at 383 nm and a relatively weaker emission associated with the defect level. Resistivity of GZO nanorods synthesized with sodium lauryl sulfate （SLS） and sodium benzene sulfonate （SBS） surfactants showed 2.84 Ω m and 14.2 Ω m, respectively.

Acute phlegmonous gastritis complicated by delayed perforation

Here, we report on a case of acute phlegmonous gastritis (PG) complicated by delayed perforation. A 51-year-old woman presented with severe abdominal pain and septic shock symptoms. A computed tomography scan showed diffuse thickening of the gastric wall and distention with peritoneal fluid. Although we did not find definite evidence of free air on the computed tomography (CT) scan, the patient’s clinical condition suggested diffuse peritonitis requiring surgical intervention. Exploratory laparotomy revealed a thickened gastric wall with suppurative intraperitoneal fluid in which Streptococcus pyogenes grew. There was no evidence of gastric or duodenal perforation. No further operation was performed at that time. The patient was conservatively treated with antibiotics and proton pump inhibitor, and her condition improved. however, she experienced abdominal and flank pain again on postoperative day 10. CT and esophagogastroduodenoscopy showed a large gastric ulcer with perforation. Unfortunately, although the CT showed further improvement in the thickening of the stomach and the mucosal defect, the patient’s condition did not recover until a week later, and an esophagogastroduodenoscopy taken on postoperative day 30 showed suspected gastric submucosal dissection. We performed total gastrectomy as a second operation, and the patient recovered without major complications. A pathological examination revealed a multifocal ulceration and necrosis from the mucosa to the serosa with perforation.

Characteristics of Atmospheric Circulation Associated with Cold Surge Occurrences in East Asia:A Case Study During 2005/06 Winter

The characteristics of the upper-level circulation and thermodynamical properties for the period when two distinct cold surges broke out over East Asia during the 2005/06 winter are investigated. From early December 2005 to early January 2006, exceptionally cold weather lasted for approximately one month due to two successive cold surges that took place on 2 December 2005 and 2 January 2006, respectively. This study reveals that both involve the upper-tropospheric circulation, which induces the amplification and expansion of the surface Siberian high toward East Asia, but arose from different causes： the former is caused by the upper-level blocking originated from the North Pacific and the latter is caused by the upper-level wave train across the Eurasian Continent. In particular, it is suggested that the lower-tropospheric anomalous wind caused by upper-level circulation anomalies and a steep meridional temperature gradient amplified by phase-locked annual cycle combined to induce very strong cold advection in East Asia, which resulted in exceptionally cold weather that lasted for several weeks. The present results emphasize that the characteristics of the upper-tropospheric circulation can be considered as important precursors to cold surge occurrences in East Asia.

Hepatic cyst misdiagnosed as a gastric submucosal tumor:A case report

We describe here a case of 51-year-old woman with a symptomatic hepatic cyst that was misdiagnosed as a gastric submucosal tumor (SMT) with endoscopic ultrasound (EUS) and CT scan. The patient presented with an epigastric pain for two months. On endoscopy, a submucosal tumor was found on the cardia of the stomach. Based on EUS and abdominal CT scan, the lesion was diagnosed as a gastric duplication cyst or a gastrointestinal stromal tumor (GIST). The operative plan was laparoscopic wedge resection for the GIST of the gastric cardia. A cystic mass arising from the left lateral segment of the liver was found at the laparoscopic examination. There was no abnormal finding at the gastric cardia. She was treated by laparoscopic hepatic wedge resection including the hepatic cyst using an endoscopic linear stapler.

Ataxic gait following total gastrectomy for gastric cancer

A 58-year-old woman, who had undergone total gastrectomy for early gastric cancer 9 years previously, visited the outpatient clinic complaining of progressive difficulty in walking for 15 d. Laboratory examinations showed macrocytic anemia and a decreased serum vitamin B12 concentration and increased serum concentrations of folate, vitamin E and copper. Magnetic resonance imaging showed multifocal high signal intensities along the posterior column of the cervical and thoracic spinal cord. Treatment consisted of intramuscular injections of vitamin B12 for 7 d, which increased her serum level of vitamin B12 to normal. This was followed by weekly intramuscular injections of vitamin B12 for another 2 wk and oral administration of vitamin B12 three times per day. After comprehensive rehabilitation for 4 wk, she showed sufficient improvements in strength and ataxic gait, enabling her to return to her normal daily activities.

This paper investigates the effects of types and aging of asphalt binder on healing performance of asphalt mixture modified with steel wool fibers(SWF)using induction heating *** binders PG58-22,PG64-22 and PG76-22,widely used in Korea,were applied in this *** rolling thin film oven(RTFO)and pressure ageing vessel(PAV)tests were conducted to simulate the aging process of asphalt *** healing performance of mixtures was evaluated by the crack-heal process,which included semi-circular bending and induction heating *** cooking oil waste was used as a rejuvenator in order to refresh the aged binder and its healing performance was also *** results indicated that the performance grade(PG)of asphalt binder slightly affected the healing performance of mixtures due to the difference of binder *** addition,the healing level of asphalt mixture was reduced significantly by aged asphalt ***,it was found that using cooking oil waste as a rejuvenator for the aged asphalt mixture can improve its healing performance.

The immune-modulating effects of viable Weissella cibaria JW15 on RAW 264.7 macrophage cells

The objective of this study is to investigate the immune-enhancing ability of viable and heat-killed Weissella cibaria JW15(JW15)isolated from Kimchi in RAW 264.7 *** immune effects were evaluated by measuring the production of NO,cytokines,inflammatory enzyme,and activation of NF-κ*** JW15 executed higher activity on stimulating the release of TNF-αas well as activating NF-κB compared to that of heatkilled ***,viable and heat-killed JW15 significantly increased the production of NO,IL-6 and TNF-αmore than that of Lactobacillus rhamnosus GG(LGG).Furthermore,viable JW15 induced higher production of i NOS compared with that of viable ***,our finding indicates that viable JW15 had similar,if not more,immune-enhancing activities as heat-killed *** addition,viable JW15 had higher immune-enhancing activity than commercial strain ***,viable JW15 has the potential to be used as a functional food to improve the host immune response.
