Multi-aspect analysis of ureteral access sheath usage in retrograde intrarenal surgery: A RIRSearch group study

Objective:To evaluate the effect of ureteral access sheath(UAS)use and calibration change on stone-free rate and complications of retrograde intrarenal surgery(RIRS).Methods:Data from 568 patients undergoing RIRS for kidney or upper ureteral stones were retrospectively ***,patients were compared after 1:1 propensity score matching,according to UAS usage during RIRS(UAS used[+]87 and UAS non-used[−]87 patients).Then all UAS+patients(n=481)were subdivided according to UAS calibration:9.5-11.5 Fr,10-12 Fr,11-13 Fr,and 13-15 *** outcomes of the study were the success and complications of ***:Stone-free rate of UAS+patients(86.2%)was significantly higher than UAS−patients(70.1%)after propensity score matching(p=0.01).Stone-free rate increased with higher caliber UAS(9.5-11.5 Fr:66.7%;10-12 Fr:87.0%;11-13 Fr:90.6%;13-15 Fr:100%;p<0.001).Postoperative complications of UAS+patients(11.5%)were significantly lower than UAS−patients(27.6%)(p=0.01).Complications(8.7%)with 9.5-11.5 Fr UAS was lower than thicker UAS(17.2%)but was not statistically significant(p=0.09).UAS usage was an independent factor predicting stone-free status or peri-and post-operative complications(odds ratio[OR]3.654,95%confidence interval[CI]1.314-10.162;OR 4.443,95%CI 1.350-14.552;OR 4.107,95%CI 1.366-12.344,respectively).Conclusion:Use of UAS in RIRS may increase stone-free rates,which also increase with higher caliber *** usage may reduce complications;however,complications seemingly increase with higher UAS calibration.

In today’s manufacturing industries,hard competition between rival firms makes it compulsory for researchers to design lighter and cheaper machine components due to the megatrends of cost-effectiveness and *** this point,aluminum syntactic foams(ASFs)are new-generation engineering composites and come into the upfront as a *** to their features like low density,sufficient elongation,and perfect energy absorption ability,these advanced foams have been considerably seductive for many industrial sectors *** this study,an industrial-oriented automatic die casting technology was used for the first time to manufacture the combination of AA7075/porous expanded clay(PEC).Micro evaluations(optical and FESEM)reveal that there is a homogenous particle distribution in the foam samples,and inspections are compatible with the other ASF ***,T6 aging heat treatment was operated on one half of the produced foams to explore the probable impact of aging on the compressive *** results show that PEC particles can be an alternative to expensive hollow spheres used in the previous ***,a favorable relationship is ascertained between the aging treatment and mechanical properties such as compression strength and plateau strength.

How has the COVID-19 pandemic changed treatment preferences of patients with proximal ureteral stones?

background:The impact of the coronavirus disease 2019(COVID-19)pandemic on patient decision making remains *** study aimed to investigate the effect of the pandemic on treatment preferences of patients with proximal ureteral stones.Materials and methods:Retrospective data regarding treatment preferences of patients diagnosed with symptomatic proximal ure-teral stones between July 2018 and November 2021 at a single center were *** from 493 patients were analyzed according to 2 groups,including patients diagnosed during the COVID-19 pandemic and those diagnosed during an equivalent period of time be-fore the pandemic.Results:Preference for conservative treatment increased during the COVID-19 pandemic(p=0.009).In patients who had previously undergone shock wave lithotripsy(SWL),the preference for SWL decreased and the preference for conservative treatment increased during the COVID-19 pandemic(p=0.042).Multiple logistic regression analysis revealed a significant correlation between a preference for conservative treatment during the pandemic and no prior spontaneous stone passage(p=0.003;odds ratio[OR],2.48;95％con-fidence interval[CI],1.45-4.23),no hydronephrosis(p=0.035;OR,3.57;95％Cl,1.34-9.49),and a visual analog scale score of 4 or less(p=0.018;OR,1.97;95％Cl,1.15-3.38).Conclusions:A significant increase in the preference for conservative treatment was observed among patients diagnosed during the pandemic,and patients with a history of SWL demonstrated a preference shift from SWL to conservative treatment.

Automated evaluation of parapapillary choroidal microvasculature in crowded optic discs:a controlled,optical coherence tomography angiography study

AIM:To compare superficial and deep vascular properties of optic discs between crowded discs and controls using optical coherence tomography angiography(OCT-A).METHODS:Thirty patients with crowded discs,and 47 control subjects were enrolled in the *** eye of each individual was included and OCT-A scans of optic discs were obtained in a 4.5×4.5 mm^(2) rectangular *** peripapillary capillary(RPC)density,peripapillary retinal nerve fiber layer(pRNFL)thickness,cup volume,rim area,disc area,cup-to-disc(c/d)area ratio,and vertical c/d ratio were obtained automatically using device *** parapapillary choroidal microvasculature(PPCMv)density was calculated using MATLAB *** the vertical c/d ratio of the optic disc was absent or small cup,it was considered as a crowded ***:The mean signal strength index of OCT-A images was similar between the crowded discs and control eyes(P=0.740).There was no difference in pRNFL between the two groups(P=0.102).There were no differences in RPC density in whole image(P=0.826)and peripapillary region(P=0.923),but inside disc RPC density was higher in crowded optic discs(P=0.003).The PPCMv density in the inner-hemisuperior region was also lower in crowded discs(P=0.026).The pRNFL thickness was positively correlated with peripapillary RPC density(r=0.498,P<0.001).The inside disc RPC density was negatively correlated with c/d area ratio(r=-0.341,P=0.002).CONCLUSION:The higher inside disc RPC density and lower inner-hemisuperior PPCMv density are found in eyes with crowded optic discs.

Risk Factors in Surgical Site Infections

Purpose of the Research: Our study aimed to determine and understand the risk factors of surgical site infections in patients operated on in general surgery wards. Method of Research: Between May 2008 and November 2008, 422 consecutive patients who underwent emergency and elective surgeries, excluding breast, goitre and laparoscopic surgeries, were included in the study to determine surgical site infections and risk factors. The patients’ risk factors were selected in the preoperative period, and postoperative 3rd, 7th, and 30th days were checked by the physician responsible for conducting the study regarding surgical site infection. Results and Conclusion: Surgical site infections were defined as incisional and organ/space based on NNIS data. Gender, diabetes mellitus, malignancy, degree of contamination, ASA score ≥ 3, preoperative skin cleansing, duration of intensive care unit stay, duration of hospital stay, growth in wound culture, duration of surgery and incision length were determined as risk factors increasing CAI rates. Especially the degree of contamination, duration of intensive care unit stay, hospitalisation, duration of operation, and growth in wound culture were decisive factors in the development of CAI. As a result of our study, in the light of these data, we concluded that to prevent the development of CAI can be modified, interventions to foreseeable factors can be assessed.

Invisible face of COVID-19 pandemic on the freshwater environment:An impact assessment on the sediment quality of a cross boundary river basin in Turkey

In the current research,the impact of the COVID-19 lockdown period on sediment quality of the MericErgene River basin was evaluated by determining the potentially toxic elements(PTEs) in sediment samples collected from 25 sampling points in the *** some important ecological indicators including potential ecological risk index(PERI),contamination factor(CF),pollution load index(PLI),biological risk index(BRI),and geo-accumulation index(I) and some important statistical indicators including the Pearson correlation index(PCI),factor analysis(FA),and cluster analysis(CA) were applied to collected *** of all the investigated PTEs in sediment of the basin show significant differences between the pre-lockdown and lockdown *** PTE contents have considerably decreased by12.8%-45.4% during the lockdown ***,the results of applied sediment quality risk assessment indices showed significant improvement and the values have decreased by 22.4%-35.8%.In the lockdown period,reduction of industrial activities or limited production at many industrial facilities located in the basin reduced the amount of effluent,leading to significant improvement in sediment quality.

A New Privacy-Preserving Data Publishing Algorithm Utilizing Connectivity-based Outlier Factor and Mondrian Techniques

Developing a privacy-preserving data publishing algorithm that stops individuals from disclosing their identities while not ignoring data utility remains an important goal to *** finding the trade-off between data privacy and data utility is an NP-hard problem and also a current research *** existing approaches are investigated,one of the most significant difficulties discovered is the presence of outlier data in the *** data has a negative impact on data ***,k-anonymity algorithms,which are commonly used in the literature,do not provide adequate protection against outlier *** this study,a new data anonymization algorithm is devised and tested for boosting data utility by incorporating an outlier data detection mechanism into the Mondrian *** connectivity-based outlier factor(COF)algorithm is used to detect *** is selected because of its capacity to anonymize multidimensional data while meeting the needs of real-world ***,on the other hand,is used to discover outliers in high-dimensional datasets with complicated *** proposed algorithm generates more equivalence classes than the Mondrian algorithm and provides greater data utility than previous algorithms based on *** addition,it outperforms other algorithms in the discernibility metric(DM),normalized average equivalence class size(Cavg),global certainty penalty(GCP),query error rate,classification accuracy(CA),and F-measure ***,the increase in the values of theGCPand error ratemetrics demonstrates that the proposed algorithm facilitates obtaining higher data utility by grouping closer data points when compared to other algorithms.

Solving PDEs with a Hybrid Radial basis Function:Power-Generalized Multiquadric Kernel

Radial basis Function(RBF)kernels are key functional forms for advanced solutions of higher-order partial differential equations(PDEs).In the present study,a hybrid kernel was developed for meshless solutions of PDEs widely seen in several engineering *** kernel,Power-Generalized Multiquadric-Power-GMQ,was built up by vanishing the dependence of e,which is significant since its selection induces severe problems regarding numerical instabilities and convergence *** drawback of e-dependency is that the optimal e value does not exist in *** present the Power-GMQ kernel which combines the advantages of Radial Power and Generalized Multiquadric RBFs in a generic ***-GMQ RBF was tested in higher-order PDEs with particular boundary conditions and different ***-Finite Difference(RBF-FD)discretization was also implemented to investigate the characteristics of the proposed *** results revealed that our proposed kernel makes similar or better estimations as against to the Gaussian and Multiquadric kernels with a mild increase in computational ***-QR method may achieve better accuracy in some cases with considerably higher computational *** using Power-GMQ RBF,the dependency of solution on e was also substantially relaxed and consistent error behavior were obtained regardless of the selected e accompanied.
