AIm:To evaluate the referrals with suspected arrhythmogenic right ventricular cardiomyopathy(ARVC)and compare cardiac mR(cmR)findings against clinical ***:A retrospective analysis of 114(age range16 to 83,males 55%and females 45%)patients referred for cmR with a suspected diagnosis of ARVC between may 2006 and February 2010 was performed after obtaining institutional approval for service *** for referral including clinical symptoms and family history of sudden death,electrocardiogram and echo abnormalities,cmR findings,final clinical diagnosis and information about clinical management were *** results of cmR were classified as major,minor,non-specific or negative depending on both functional and tissue characterisation and the cmR results were compared against the final clinical ***:The most common reasons for referral included arrhythmias(30%)and a family history of sudden death(20%).Of the total cohort of 114 patients:4 patients(4%)had major cmR findings for ARVC,13patients(11%)had minor cmR findings,2 patients had non-specific cmR findings relating to the right ventricle and 95 patients had a negative *** the 4 patients who had major cmR findings,3(75%)had a positive clinical *** contrast,of the 13 patients who had minor cmR findings,only 2(15%)had a positive clinical *** of the 95 negative patients,clinical details were available for 81 patients and none of them had *** the 14 patients with no clinical data and final diagnosis,the sensitivity of the test was 100%,specificity 87%,positive predictive value29%and the negative predictive value 100%.CONCLUSION:CmR is a useful tool for ARVC evaluation because of the high negative predictive value as the outcome has a significant impact on the clinical decision-making.

Dear Editor, Potyviruses such as Papaya ring-spot virus （PRSV） cause important yield losses in cucurbits. Two distinct resistant alleles were identified in the Cucumis melo germplasm. Accession PI 414723 （Supplemental Table 1） possesses mono- genic resistance, controlled by the Prv2 allele, and reacts to PRSV by systemic necrotic lesions; plants with the PryI allele, described in cultivar WmR-29, remain symptomless （Pitrat and Lecoq, 1983）. Fusarium oxysporum f. sp. melonis （FUS） exclusively attacks melon, causing severe wilt. monogenic dominant resistance was described against races O, 1, and 2. The Fore-2 gene, controlling resistance to races 0 and 1, was cloned by Joobeur et al. （2004）, and encodes a nucleotide bindina domain （NB）-Ieucine rich repeat （LRR） protein.

Background and purpose:Providing participants with evidence-based care for secondary prevention is an ethical and scientific priority for trials in stroke *** optimal strategy,however,is *** report the performance of a new approach for delivering preventive care to trial ***:Participants were enrolled in the Insulin Resistance Intervention after Stroke trial,which examined the insulin sensitiser,pioglitazone versus placebo for prevention of stroke and myocardial infarction after ischaemic stroke or transient ischaemic *** care was the responsibility of the participants’personal healthcare providers,but investigators monitored care and provided feedback *** studied achievement of 8 prevention goals at baseline and 3 annual visits,with a focus on 3 priority goals:blood pressuremm Hg,lowdensity lipoprotein(LDL)cholesterolmmol/L and antithrombotic ***:The proportion of participants achieving the priority goals was highest for antithrombotic use(96–99%in each year)and similar for blood pressure(66–72%in each year)and LDL(68–70%in each year).All 3 priority goals were achieved by 47–52%of participants in any given ***,only 22%of participants achieved all 3 goals in each ***:A strategy of monitoring care and providing feedback was associated with high average yearly achievement of 3 priority secondary prevention goals,but the majority of trial participants did not persist in being at goal over *** registration number:NCT00091949.

Background: The determination of prognosis in heart failure (HF) has focused primarily on the identification of potential biological and physiological markers and not on communication. High morbidity and mortality rates suggest that patients require prognostic information to assist in life planning. This study examined the preferences of both patients with HF and cardiologists for prognosis communication in the outpatient clinical setting, with the aim of guiding practitioners in undertaking prognosis conversations. methods: Using qualitative descriptive techniques informed by a grounded theory approach, 32 patients with HF and 9 cardiologists from outpatient settings in Ontario, Canada were interviewed to identify convergent preferences for prognosis communication. Strategies to enhance methodological rigor were employed. Results: Individualized, context-specific prognosis communication between patients and cardiologists was preferred. Two main themes and ten related attributes were identified to describe convergent preferences;1) Set the Stage for Prognosis Communication, and 2) map the HF route. Attributes reflected the complex, dynamic, interactive and iterative nature of prognosis communication preferences. Conclusions: Prognosis communication occurs within a complex, adaptive healthcare system. While specific preferences exist, changing contextual elements within and outside of the system create conditions that require cardiologists to adjust their approach to individual patients. Patients with HF and cardiologists each have preferences that affect their willingness and ability to engage in dyadic HF-specific prognosis communication. Findings have relevance for the implementation of any efforts, including HF guidelines, aimed at improving prognosis communication. Our findings, informed by a complexity science approach, offer an innovative and robust alternative to traditional linear approaches to prognosis communication.

Dear Editor, In its latest edition, the World Health Organization （WHO） provides guidelines regarding the standardization of techniques for the measurement of sperm concentrationI and recommends use of a 100 μm-deep hemocytometer. However, these instructions are not always followed correctly.2 Furthermore, although hemocytometers provide more accurate and consistent results, they present several drawbacks： the chamber must be properly assembled prior to its use so as to ensure that the counts are correct and it needs to be cleaned thoroughly to remove all traces of sperm, which risks breaking the glass coverslip.

由于世界液态烃类燃料供应明显减少,使煤衍生油改质为合成原油最终将成为商业化的实体。虽然半焦-油-能开发(COED)项目暂时被搁置起来了,但 COED 法中用加氢处理技术使煤衍生油改质有关的反应工程知识应当对其它煤液化方法的衍生油改质有用。COED 法是在六十年代后期到七十年代初期由 FmC 公司与煤炭研究局(现在的能源部)共同开发的。这个方法用多级流化床热解煤生产合成原油,中热值煤气和半焦。用固定床催化加氢处理把热解产生的粗焦油改质为合成原油。粗煤焦油与石油衍生油有不同的性质,用加氢处理改进其质量还没有一个现行的技术。在 COED 中试厂,建设并操作了一套日处理30桶油的固定床加氢处理装置。这套中试厂的加氢处理装置设计是根据一般石油残油加氢处理技术,并结合阿尔柯(ARCO)在六十年代完成的煤焦油加氢处理台架试验结果,这些煤焦油是在过程开发单元中生产的。中试厂的加氢处理装置操作了大约四年,提供了关于加氢处理过程中反应工程方面有价值的资料,又提供了大量的加氢处理样品给其它能源部的订约者和对COED 潜在用途感兴趣的用户去完成应用试验。值得注意的是,50,000加仑的COED 合成原油供给美国船舰工程中心,在1973年11月15—16日的航行试验中,作*** 号的锅炉燃料。本文讨论 COED 装置中的保护室和加氢反应器的反应工程问题。极为重要的是研究原料(热解煤焦油)性质的作用和它们对反应器中催化剂的影响。已经得到一个工作动力学模型,它能使设计者把一组特定的参数最佳化,又能使工厂操作者确定催化剂寿命。关于煤衍生油和石油残油中的金属含量对催化剂的钝化影响也作了定量比较。

Atrial Septal Defect and Left Recurrent Laryngeal Nerve Paralysis: A Case of Ortner’s Syndrome and Literature Review

Introduction: Cardiovocal syndrome, or hoarseness resulting from vocal fold paralysis secondary to cardiovascular pathology, is commonly referred to as Ortner’s syndrome. We present a brief overview of vocal fold paralysis, present an illustrative case of Ortner’s syndrome, and provide a review of the pertinent literature. Here we aim to broaden one’s differential for vocal fold paralysis, discuss its importance as pertains to cardiovascular pathology and outcomes, and highlight the difficulties in therapeutic planning for these unique patients. methods: A case report and literature review. Results: A 26-year-old female with an atrial septal defect and pulmonary hypertension presented with 5 months of hoarseness. Laryngoscopy revealed left vocal fold paralysis. Imaging from the skull base to chest showed an enlarged pulmonary artery (PA) in the absence of other abnormalities. Literature review suggests that this left laryngeal nerve paralysis results from nerve compression within the aortopulmonary window, a triangle defined by the aortic arch, PA, and ligamentumarteriosum. Imaging in our patient over 8 months demonstrated an increase in PA size from 3.9 to 4.2 cm correlating with the onset of hoarseness. Conclusions: Importantly, hoarseness second ary to laryngeal nerve compression in cardiovascular disease may correlate with a poorer prognosis, i.e., in thoracic aortic aneurysms and mitral valvestenosis. Awareness of vocal changes in the setting of cardiovascular disease improves diagnostic acumen in vocal foldparalysis.
