Outcomes in randomized controlled trials of exercise interventions in solid organ transplant

AIm To identify the outcome measures that have been used in randomized controlled trials(RCTs) of exercise training in solid organ transplant(SOT) recipients and to link these outcomes to the International Classification of Functioning, Disability and Health(ICF) *** Electronic literature searches of mEDLINE, EmBASE, CINAHL, Cochrane, Scopus, and Web of Science were performed. We sought RCTs that investigated the effect of exercise training in SOT recipients. Reference lists of all eligible publications were searched for other appropriate studies not identified by the electronic search. A complete list of outcome measures used in the RCTs was generated and each of these was linked to an ICF *** Four hundred and thirteen articles were retrieved, of which 35 met our inclusion criteria. The studies included were designed to compare the effects of exercise training programs to usual care or to another exercise training program and reported on recipients of heart(n = 21), kidney(n = 9), lung(n = 3) or liver(n = 2) transplant. Of the 126 outcome measures identified, 62 were used as primary outcome measures. The most commonly occurring primary outcomes were aerobic capacity using the peak VO2(n = 13), quality of life using the shortform-36(n = 8), and muscle strength(n = 7). Theseoutcome measures were linked to 113 ICF categories and the majority of outcomes fall into the body function domain(n = 93). CONCLUSION There is little standardization in outcome measures used in RCTs of exercise interventions in SOT recipients. The ICF framework can be used to select a core set of outcomes that cross all domains of ICF and that would be appropriate to all SOT recipients.

Anatomy of the 2009 Fiordland earthquake(m_w 7.8),South Island,New Zealand

We have analyzed the July 15, 2009, Fiordland earthquake （mw 7.8）, which occurred in the southwestern part of the South Island of New Zealand. This thrust-related earthquake in the southern Fiordland subduction zone is the largest New Zealand earthquake in the past 80 years. We have constrained a rupture model for this earthquake using coseismic offsets derived from the continuous geodetic network （Global Positioning System） of the New Zealand Institute of Geological and Nuclear Sciences （GNS Science）. Our analysis concludes that this earthquake significantly increased the Coulomb stress on the overriding plate, particularly on the offshore portion of the Alpine fault.

Factors associated with mortality in neonatal surgical emergencies in a developing tertiary hospital in Nigeria

Background: The outcome of neonatal surgery depends on safe anaesthesia, competent surgery and good nursing care. The University of Uyo Teaching Hospital, Uyo, Nigeria, established in February 2008, has specialist anaesthetic and surgical manpower. The aim of the study was to determine the outcome and contributing factors to mortality in neonatal surgical emergencies at this new tertiary health institution. method: It was a retrospective descriptive study of neonates that underwent emergency surgery at the University of Uyo Teaching Hospital between June 2008 and may 2011. Data was obtained from the anaesthetic register, ward admission and discharged register, nurses report books and patient case files. Results: Forty-five neonates were operated upon during the three year period. There were 28 males and 17 females with a male to female ratio of 1.7:1. Forty-four (97.8%) of the neonates were referred to the University of Uyo Teaching Hospital. The mean age and body weight at presentation were 47.5 ± 44.4 hours and 2.65 ± 0.61 kg respectively. The mean interval between admission and surgical intervention was 4.9 ± 6.2 days. malformations of the gut (40%) and anterior abdominal wall (26.7%) were the major pathologies. The overall mortality following surgery was 62.2%. Case fatality rates ranged from 0% for Hirschprung’s disease to 100% for tracheoesophageal fistula. The immediate causes of death among these neonates were peritonitis from gangrenous gut, hypovolaemia and repeat surgery. Contributing factors to mortality were delivery in unorthodox health facilities, delay in presentation as well as surgical intervention and inefficient postoperative monitoring. Conclusion: Emergency neonatal surgeries at the UUTH are associated with unacceptable high mortality. Reduction in such mortality would require campaign for early presentation, a lot more timely surgical interventions and upgrading of monitoring facili- ties to help in improving perioperative monitoring and care.

Is cardiac mRI an effective test for arrhythmogenic right ventricular cardiomyopathy diagnosis?

AIm:To evaluate the referrals with suspected arrhythmogenic right ventricular cardiomyopathy(ARVC)and compare cardiac mR(cmR)findings against clinical ***:A retrospective analysis of 114(age range16 to 83,males 55%and females 45%)patients referred for cmR with a suspected diagnosis of ARVC between may 2006 and February 2010 was performed after obtaining institutional approval for service *** for referral including clinical symptoms and family history of sudden death,electrocardiogram and echo abnormalities,cmR findings,final clinical diagnosis and information about clinical management were *** results of cmR were classified as major,minor,non-specific or negative depending on both functional and tissue characterisation and the cmR results were compared against the final clinical ***:The most common reasons for referral included arrhythmias(30%)and a family history of sudden death(20%).Of the total cohort of 114 patients:4 patients(4%)had major cmR findings for ARVC,13patients(11%)had minor cmR findings,2 patients had non-specific cmR findings relating to the right ventricle and 95 patients had a negative *** the 4 patients who had major cmR findings,3(75%)had a positive clinical *** contrast,of the 13 patients who had minor cmR findings,only 2(15%)had a positive clinical *** of the 95 negative patients,clinical details were available for 81 patients and none of them had *** the 14 patients with no clinical data and final diagnosis,the sensitivity of the test was 100%,specificity 87%,positive predictive value29%and the negative predictive value 100%.CONCLUSION:CmR is a useful tool for ARVC evaluation because of the high negative predictive value as the outcome has a significant impact on the clinical decision-making.

Dear Editor, Potyviruses such as Papaya ring-spot virus （PRSV） cause important yield losses in cucurbits. Two distinct resistant alleles were identified in the Cucumis melo germplasm. Accession PI 414723 （Supplemental Table 1） possesses mono- genic resistance, controlled by the Prv2 allele, and reacts to PRSV by systemic necrotic lesions; plants with the PryI allele, described in cultivar WmR-29, remain symptomless （Pitrat and Lecoq, 1983）. Fusarium oxysporum f. sp. melonis （FUS） exclusively attacks melon, causing severe wilt. monogenic dominant resistance was described against races O, 1, and 2. The Fore-2 gene, controlling resistance to races 0 and 1, was cloned by Joobeur et al. （2004）, and encodes a nucleotide bindina domain （NB）-Ieucine rich repeat （LRR） protein.

The complexity of prognosis communication in heart failure: Patient and cardiologists’ preferences in the outpatient clinical setting

Background: The determination of prognosis in heart failure (HF) has focused primarily on the identification of potential biological and physiological markers and not on communication. High morbidity and mortality rates suggest that patients require prognostic information to assist in life planning. This study examined the preferences of both patients with HF and cardiologists for prognosis communication in the outpatient clinical setting, with the aim of guiding practitioners in undertaking prognosis conversations. methods: Using qualitative descriptive techniques informed by a grounded theory approach, 32 patients with HF and 9 cardiologists from outpatient settings in Ontario, Canada were interviewed to identify convergent preferences for prognosis communication. Strategies to enhance methodological rigor were employed. Results: Individualized, context-specific prognosis communication between patients and cardiologists was preferred. Two main themes and ten related attributes were identified to describe convergent preferences;1) Set the Stage for Prognosis Communication, and 2) map the HF route. Attributes reflected the complex, dynamic, interactive and iterative nature of prognosis communication preferences. Conclusions: Prognosis communication occurs within a complex, adaptive healthcare system. While specific preferences exist, changing contextual elements within and outside of the system create conditions that require cardiologists to adjust their approach to individual patients. Patients with HF and cardiologists each have preferences that affect their willingness and ability to engage in dyadic HF-specific prognosis communication. Findings have relevance for the implementation of any efforts, including HF guidelines, aimed at improving prognosis communication. Our findings, informed by a complexity science approach, offer an innovative and robust alternative to traditional linear approaches to prognosis communication.

Dear Editor, In its latest edition, the World Health Organization （WHO） provides guidelines regarding the standardization of techniques for the measurement of sperm concentrationI and recommends use of a 100 μm-deep hemocytometer. However, these instructions are not always followed correctly.2 Furthermore, although hemocytometers provide more accurate and consistent results, they present several drawbacks： the chamber must be properly assembled prior to its use so as to ensure that the counts are correct and it needs to be cleaned thoroughly to remove all traces of sperm, which risks breaking the glass coverslip.
