Abdominal pain with elevated transaminases from inferior vena cava(IVC) obstruction is a relatively common reason for referral and further workup by a hepatologist. the differential for the cause of IVC obstruction is extensive, and the most common etiologies include clotting disorders or recent trauma. In some situations the common etiologies have been ruled out, and the underlying process for the patient's symptoms is still not explained. We present one unique case of abdominal pain and hepatomegaly secondary to IVC constriction from extrinsic compression of the diaphragm. Based on this patient's presentation, we urge that physicians be cognizant of the IVC diameter and consider extrinsic compression as a contributor to the patient's symptoms. If IVC compression from the diaphragm is confirmed, early referral to vascular surgery is strongly advised for further surgical intervention.

Lymphoepithelioma-like hepatocellular carcinoma(LELHCC)is a very rare neoplasm,with distinct epidemiologic,morphologic and clinical *** mechanistic insight into the pathogenesis of this carcinoma suggests a pivotal role for the host immune system in the proliferation and progression of this ***,while detailed genomic profiling of these hepatic tumors have revealed an intra-tumoral inflammatory mutational signature that may predispose to immune checkpoint inhibitor efficacy,no published report has described their use in this tumor ***,with near 100 cases of LEL-HCC reported in the literature to date and the majority of cases confined to localized and resectable disease,current evidencebased practices in the unresectable setting are lacking,with unknown benefit of chemotherapy or *** report on the case of a 68 year-old man with unresectable,advanced LEL-HCC who had evidence of disease stability after starting on the immune checkpoint inhibitor *** disease response persisted off therapy for over a year and was potentially augmented by radiotherapy at the site of local *** this extremely rare tumor subtype,this case highlights the potential efficacy and safety of immune checkpoint blockade in LEL-HCC and reinforces the need for more robust,large-scale analysis of patients with these rare tumors to better evaluate treatment strategies and outcomes.

transcatheter arterial chemoembolization(tACE) is an effective palliative intervention that is widely accepted for the management of hepatocellular carcinoma(HCC). Post-tACE pulmonary complications resulting in acute lung injury(ALI) or acute respiratory distress syndrome(ARDS) are rare events. Pulmonary complications after tACE are thought to be related to chemical injury subsequent to the migration of the infused ethiodized oil or chemotherapeutic agent to the lung vasculature, facilitated by arteriovenous(AV) shunts within the hyper-vascular HCC. We review herein the literature on pulmonary complications related to tACE for HCC. Post-tACE pulmonary complications have included pulmonary oil embolism, interstitial pneumonitis, chemical pneumonitis, ALI, ARDS, lipoid pneumonia, acute eosinophilic and neutrophilic pneumonia, bilious pleuritis, pulmonary abscess, pulmonary tumor embolism, and possibly pulmonary metastasis with HCC. the risk factors associated with post-tACE pulmonary complications identified in the literature include large hyper-vascular HCC with AV shunts, large-volume Lipiodol infusion, and embolization via the right inferior phrenic artery. However, the absence of known risk factors is not a guarantee against serious complications. An astute awareness of the potential post-tACE pulmonary complications should expedite appropriate therapeutic interventions and increase potential for early recovery.

Hepatocellular carcinoma （HCC） is a lethal disease in most patients, due to its aggressive course and a lack of effective systemic therapies for advanced disease. Surgical resection and liver transplantation remain the only curative options for a small subset of patients. Few patients with HCC are diagnosed early enough to be eli- gible for curative treatment. Angiogenesis inhibition is a natural therapeutic target for all solid tumors, but par- ticularly for the highly vascularized HCC tumors. With the approval of the targeted agent sorafenib, there are now additional options for patients with HCC. Although sorafenib does produce some improvement in survival in HCC patients, the responses are not durable. In addi- tion, there are significant dermatologic, gastrointestinal, and metabolic toxicities, and, as importantly, there is still limited knowledge of its usefulness in special sub- populations with HCC. Other angiogenesis inhibitors are in development to treat HCC both in the first-line set- ting and for use following sorafenib failure; the furthest in development is brivanib, a dual fibroblast growth factor pathway and vascular endothelial growth factor receptor inhibitor. Additional agents with antiangiogenic properties also in phase It and Ⅲ development for the treatment of patients with HCC include bevacizumab, ramucirumab, ABt-869, everolimus and ARQ 197.

Lactobacillus are considered promising therapeutic methods for nonalcoholic fatty liver disease(NAFLD).the effects of two strains of Ltmosilactobacillus mucosae on NAFLD were investigated in this ***-old male C57BL/6J mice were divided into 4 groups(n=8 per group,Control,Model,FZJtZ26M3,FGSYC17L3).*** FZJtZ26M3 reduced the mice 's body weight,liver weight,and adipose tissue weight after 12 weeks of *** to serum analysis,total cholesterol,triacylglycerol,and low-density lipoprotein cholesterol significantly decreased after *** FZJtZ26M3 *** pathology showed that *** FZJtZ26M3 was effective to ameliorate lipid deposition in NAFLD ***,the expression of the gene related to lipid metabolism in the liver and adipose tissue was analyzed,and the results indicated that *** FZJtZ26M3 could alleviate NAFLD by regulating lipid ***,the results of 16S rRNA gene sequencing revealed a drop in the relative abundance of Ruminococcaceae,which is linked to inflammation,but the relative abundance of a potential probiotic Akkermansia significantly increased after *** FZJtZ26M3 ***,*** FZJtZ26M3 could be a candidate to prevent NAFLD.

Hepatocellular carcinoma(HCC)ranks among the leading cancer-related causes of morbidity and mortality *** of HCC has prevailed as a key method to curative therapy for patients who present with unresectable HCC outside of the listing criteria for liver transplantation(Lt).Even though Lt paves the way to lifesaving curative therapy for HCC,perpetually severe organ shortage limits its broader *** over the optimal protocol and assessment of response to downstaging treatment has fueled immense research activity and is pushing the boundaries of Lt candidate selection *** implicit obligation of refining downstaging protocol is to ensure the maximization of the transplant survival benefit by taking into account the waitlist life *** the following review,we critically discuss strategies to best optimize downstaging HCC to Lt on the basis of existing literature.

For the first time,the mechanism of metal aluminum dissolution in NaF-ScF_(3) eutectic melts and the chemical interaction between the constituents of this mixture have been thoroughly studied by a combination of differential thermal analysis(DtA),high temperature and solid-state nuclear magnetic resonance(NMR),and X-ray diffraction(XRD) coupled with the molecular dynamic *** formation of an insoluble Al_(3)Sc alloy in molten(NaF-ScF_(3))_(eut)system was proven,and the chemical mechanism of this aluminothermic Al_(3)Sc alloy production was *** ex situ examinations bring to light the formation of NaScF_4 and solid solution of Na_(3)(Al,Sc)F_(6) in cooled *** molecular dynamics calculations of the bath allow us to construct the structural model and to predict viscosity,density and electrical conductivity of the reagent melt to help to optimize the conditions of the alloy synthesis.

Vγ9Vδ2 t cells are specialized effector cells that have gained prominence as immunotherapy agents due to their ability to target and kill cells with altered pyrophosphate *** our effort to understand how cancer cells evade the cell-killing activity of Vγ9Vδ2 t cells,we performed a comprehensive genome-scale CRISPR screening of cancer *** found that four molecules belonging to the butyrophilin(BtN)family,specifically BtN2A1,BtN3A1,BtN3A2,and BtN3A3,are critically important and play unique,nonoverlapping roles in facilitating the destruction of cancer cells by primary Vγ9Vδ2 t *** coordinated function of these BtN molecules was driven by synchronized gene expression,which was regulated by IFN-γsignaling and the RFX ***,an enzyme called QPCtL was shown to play a key role in modifying the N-terminal glutamine of these BtN proteins and was found to be a crucial factor in Vγ9Vδ2 t cell killing of cancer *** our research,we offer a detailed overview of the functional genomic mechanisms that underlie how cancer cells escape Vγ9Vδ2 t ***,our findings shed light on the importance of the harmonized expression and function of gene family members in modulating t-cell activity.

Development of tivantinib as treatment for Hepatocellular Carcinoma

Hepatocellular carcinoma (HCC) is a rapidly rising cause of liver-related death worldwide. Most patients are diagnosed at an advanced stage of disease, when systemic therapy is the only viable option for treatment. Significant strides have been made in the molecular understanding of HCC development and growth stimulation. the c-Met pathway has been found to be an important pathway in half of all patients with HCC. HCC tumors with high c-Met activation are associated with an aggressive phenotype and poor prognosis. tivantinib is a MEt receptor tyrosine kinase inhibitor with a broad spectrum of anti-tumor effects currently being studied for the treatment of HCC. Phase Ⅰ and Ⅱ data are available for tivantinib in the treatment of solid tumors, including HCC. there appears to be an adequate safety profile, with the main side-effect being neutropenia. In HCC patients with elevated c-Met activity, tivantinib results in an improved time to progression of 2.7 months, compared with 1.4 months in placebo-treated patients. Further studies are ongoing, but early data suggest that tivantinib is a therapy that deserves close attention in the coming years for patients with HCC.
