Increased plasminogen activator inhibitor-1 (PAI-1) and decreased tissue-plasminogen activator (t-PA) activities lead to impaired fibrinolysis, which is critical for cardiovascular disease. We studied these hemostatic factors at fasting state and after an oral fat load in 12 type 2 diabetic and 17 nondiabetic obese adolescents, matched for age, sex, body mass index, and sexual maturation. Plasma PAI-1, t-PA, and glucose as well as serum C-peptide, insulin, total cholesterol, triglyceride, and HDL and LDL cholesterol levels were measured at 0, 2, 4, and 6 h after the fat load. Metabolic responses were expressed as the area under the curve (AUC). PAI-1 activities were signifi-cantly greater in patients than in control subjects fasting, 23.4 ± 2.6 versus 12.9 ± 2.0 U/mL (p sus 57.6 ± 6.5 U · h-1 · mL-1 (p sting t-PA activities were significantly lower in the patients than in the control subjects (0.8 ± 0.3 versus 6.5 ± 2.7 U/mL; p s the only lipid parameter that was significantly different in the patients than in the control subjects fasting, 1.5 ± 0.2 versus 0.9 ± 0.1 mM (p sus 7.9 ± 0.6 mmol· h-1· L-1 (p s decreased significantly during the loading tests (p s the t-PA activities did not change. Insulin resistance estimated by the homeostasis model assessment was greater in the patients than in the control subjects (14.4 ± 2.8 versus 4.6 ± 0.7; p shed t-PA activities, suggestive of suppressed fibrinolysis, are present in our adolescents with type 2 diabetes; adding another risk factor for cardiovascular disease and acute high fat load does not further negatively affect this suppressed fibrinolysis.

AIM To describe one case of bilateral Tapia's syndrome in a liver transplanted patient and to review the *** We report a case of bilateral Tapia's syndrome in a 50-year-old man with a history of human immunodeficiency virus and hepatitis C virus child. A liver cirrhosis and a bi-nodular hepatocellular carcinoma, who underwent liver transplantation after general anesthesia under orotracheal intubation. Uneventful extubation was performed in the intensive care unit during the following hours. On postoperative day(POD) 3, he required urgent re-laparotomy due to perihepatic hematoma complicated with respiratory gram negative bacilli infection. On POD 13, patient was extubated, but required immediate re-intubation due to severe respiratory failure. At the following day a third weaning failure occurred, requiring the performance of a percutaneous tracheostomy. Five days later, the patient was taken off mechanical ventilation and severe dysphagia, sialorrea and aphonia revealed. A computerized tomography and a magnetic resonance imaging of the head and neck excluded central nervous injury. A stroboscopy showed bilateral paralysis of vocal cords and tongue and a diagnosis of bilateral Tapia's syndrome was performed. With conservative management, including a prompt establishment of a speech and swallowing rehabilitation program, the patient achieved full recovery within four months after liver transplantation. We carried out MEDLINE search for the term Tapia's syndrome. The inclusion criteria had no restriction by language or year but must provide sufficientavailable data to exclude duplicity. 权力来源于影响力,而影响力本质上是一种说服,常常需要借助策略性的传播,传播的权力由此产生。传播力是一切权力运作的核心要件。社会秩序的维护和改变,无不有赖于有效的传播,因此传播本质上是社会各利益相关方在特定技术、政治、社会、文化环境下的利益和价值观争霸和博弈的过程。传播又是天然具有偏向的,具有政治、资本和技术权力优势的一方,总是在这样的博弈中居于有利位置,因此技术赋权并不总是均等的。网络时代的传播力,一方面体现在国家和大企业对普通用户个人隐私和使用行为数据日益变本加厉的收集和滥用上,这种监视和用户数据滥权行为,常常以维护国家安全或改善用户使用体验之名行之;另一方面,这种传播力体现在我们的日常生活中较之以往任何时候,都更加依赖于技术平台和各种内容所构建的日常生活场域,尽管这样的平台具有各种偏向,而网络内容也常常真假难辨,体现了典型的后真相时代的交往实践之特点。在这样的时代,谁拥有作为一种影响力的传播力,谁就拥有这个时代真正的权力,从定义现象的性质,到推动变革的方向。

Today, individuals can effectively use all kinds of communication tools with digital transformation. This transformation process is called ＂The Network society＂ by castells. According to castells, people are living with long cables or networks that connect each other. With the progressing and spreading technological advances, entertainment sector is booming since 1950s even more dramatically than the computer sector. Digital games sector, in this perspective, has been the intersection area of the entertainment sector and the computer sector. We can include to these three sectors a fourth one： Communication （or Media） sector. With the convergence of these four sectors, there is an enormous sea of possibilities, opportunities and problemles faced by humans. Our information society is turning into games. Today, the traditional understanding of play has been replaced by digital games. Digital games as an independent communication tool differ from traditional play in that they include a new communication medium into action. There is a different communication process in these digital games, which especially is an interest to younger generations. Digital games provide an interactive environment with individuals as they allow electiveness, mobility and non-linearity. In an era of economic angst and where real and virtual identities are being discussed, individuals of the information society are searching for something different other than mass communication. They seek after individual experiences. The main focus in this study will be to analyze the habits of play in digital games. The relationship between digital games and their users will also be examined. The quantitative study of digital games will be a useful source for future studies and will bridge the gap in this area.
