Liver Transplantation in a Patient with Situs inversus Associated with Biliary Atresia, Polysplenia and Absence of Inferior Vena Cava: A Case Report

Biliary atresia (BA) is the result of a process of unknown etiology which can occur as an isolated lesion or in association with several congenital abnormalities and remains the main indications of liver transplantation among infants. one of the malformations associated with biliary atresia is Situs inversus totalis, which is a rare congenital abnormality found in 10% - 20% of infants with biliary atresia and presents an incidence varying from 1:5000 to 1:20,000. This case report aims to present an extremely rare case of a patient with Situs inversus associated with biliary atresia, polysplenia and abscence of inferior vena cava that underwent liver transplantation. Case Report: A 10-month-old boy was referred to our hospital with the diagnosis of cirrhosis, due to biliary atresia. Abdominal ultrasound and Doppler scan showed the liver in the left upper quadrant as well as signs of chronic liver disease. In addition, we could notice that the spleen had several lobes, suggesting polysplenia, and was positioned in the right upper quadrant. The patient underwent living donor liver transplant. His mother was the donor and the graft, segment III, was positioned in the left upper quadrant associated with a Y en Roux reconstruction of the biliary tract. During the first post-operatory day, the patient presented signs of disseminated sepsis. Laboratory and imaging diagnostic tests were performed presenting a small intestinal malrotation associated with diffuse ischemia. The patient died at the third day of Po due to complications related to this condition. Conclusion: Any patient with Situs inversus with acute abdomen should be aggressively investigated for other structural and biochemical abnormalities. If liver transplant is indicated, the procedure can be successfully performed.

AIM: To study the benef icial effects of triterpene α,β-amyrin and the underlying mechanisms in an experimental pancreatitis model. METHoDS: Acute pancreatitis was induced in five groups of rats (n = 8) by L-arginine (2 × 2.5 g/kg, intraperitoneal, 1 h apart) and 1 h later, they received a single oral dose of α,β-amyrin (10, 30 and 100 mg/kg),methylprednisolone (30 mg/kg) and vehicle (3% Tween 80). A saline (0.9% NaCl) treated group served as a normal control. Efficacy was assessed at 24 h by determination of serum levels of amylase, lipase and proinflammatory cytokines [tumor necrosis factor (TNF)-α and interleukin (IL)-6], pancreatic myeloperoxidase (MPo) activity, lipid peroxidation [thiobarbituric acid reactive substances (TBARS)], nitrate/nitrite levels, and the wet weight/body weight ratio. Tissue histology and the immunoreactivity for TNF-α and inducible nitric oxide synthetase (iNoS) were performed. RESULTS: α,β-amyrin and methylprednisolone treatments significantly (P ody weight ratio, and decreased the serum levels of amylase and lipase, and TNF-α and IL-6, as compared to the vehicle control. Also, pancreatic levels of MPo activity, TBARS, and nitrate/nitrite were signifi cantly lower. Histological f indings and TNF-α and iNoS immunostaining further confirmed the amelioration of pancreatic injury by α,β-amyrin. CoNCLUSIoN: α,β-amyrin has the potential to combat acute pancreatitis by acting as an anti-in? ammatory and antioxidant agent.

Noninvasive diagnosis of periportal fibrosis in schistosomiasis mansoni:A comprehensive review

Schistosomiasis mansoni is a neglected disease and key public health problem,mainly due to its high prevalence,the scarcity of public policies,and the severity of some clinical *** fibrosis(PPF)is the commonest complication of chronic schistosomiasis mansoni and its diagnosis requires different *** though wedge biopsy of the liver is considered the gold standard,it is not justified in non-surgical patients,and percutaneous liver biopsy may be informative but does not have sufficient *** PPF tests mostly include biological(serum biomarkers or combined scores)or physical assessments(imaging assessment of fibrosis pattern or tissue stiffness).Moreover,imaging techniques,such as ultrasound,computed tomography,magnetic resonance imaging,and elastography are applied not only to support the diagnosis of schistosomiasis,but also to assess and detect signs of portal hypertension and organ damage due to chronic schistosomiasis.A combination between a comprehensive history and physical examination with biomarkers for liver fibrosis and imaging methods seems to offer the best approach for evaluating these *** addition,understanding their strengths and limitations will allow a more accurate interpretation in the clinical context and can lead to greater accuracy in estimating the degree of fibrosis in patients with Schistosomiasis mansoni(***)*** review will discuss the different noninvasive methods that are currently available for the evaluation of PPF in *** infection,and their application,advantages,and limitations in clinical practice.

Biomechanical Analysis of Implant Assisted-overdentures with Variations in the Attachments Systems

The aim of this study was to assess the stress patterns on the peri-implant zone and residual alveolar ridge in different overdenture attachment system designs by the photoelasticity method. Four attachments systems were tested: o-ring, ERA, Bar-clip, and Bar-clip/o-ring association. The prostheses were loaded with 100 N in five pre-determined points and the photoelastic model was evaluated by a circular polariscope. The anterior load, o-ring, and ERA showed better stress distribution in relation to the bars systems, which presented stress levels surrounding implants. The molar load, Bar-clip/o-ring association, presented the biggest stress concentration on the peri-implant region in relation to the others. When the second molar was loaded, there was a concentration of stress in the alveolar ridge in all situations analyzed. Within the limitation of this “in vitro” study, it could be concluded that there were biomechanical differences among the attachments systems analyzed, principally between isolated and the bars systems. The o-ring showed better stress distribution and the Bar-clip/o-ring showed higher stress concentration.

The Use of Erythropoietin in Liver Regeneration in Pigs after Partial Hepatectomy

A partial hepatectomy is a surgical procedure performed during the living-donor liver transplantation and sometimes the only option for patients with hepatocarcinoma. However the remnant liver after the hepatectomy is still a major concern. Therefore, the process of liver regeneration has been a constant theme of study in order to optimize this process. Erythropoietin, a hormone produced by the kidney and involved in protecting organs like heart, liver and kidney itself against injuries can be one of these factors that could accelerate the liver regeneration. This study aims to observe if erythropoietin can accelerate the process of liver regeneration after partial hepatectomy in pigs. Methods: 8 pigs were classified into 2 groups of 4 pigs each: the control group and the test group. The animals in the first group underwent an application of saline solution subcutaneous on the day before the hepatectomy. Instead of saline solution, the test groups received a subcutaneous injection of 200 UI/lg of recombinant erythropoietin also on the day before the surgical procedure. After 7 days since the hepatectomy, in each animal the liver was biopsied in two regions, one next to the hepatectomy section and other far from it. The liver regeneration was analyzed using Ki-67. Results: Pigs from control group presented the following results: Control pig I: 30% of regeneration in the hepatectomy section and 10% in the region far from it;control pig II 24% and 4%;control pig III 27% and 7%. The test group presented no significant liver regeneration since Ki-67 could not identify cell proliferation in neither the biopsied areas. Conclusion: Since the number of pigs was not statistically significant, we could not conclude any further hypothesis. We strong believe that enhancing the number of pigs and testing different doses, we will be able to reach further conclusions.

objective： The study aimed to evaluate the genotypic profiles of C. albicans （Candida albicans） sequentially isolated throughout the course of HIV infections, and to determine its MIC （minimal inhibitory concentrations） to AMB （amphotericin B）, FLC （fluconazole）, KTC （ketoconazole）, and ITC （itraconazole）. design： samples were collected from the oral cavity of HIV-positive individuals during 4 years, with a sterilized swab. MIC was performed by using the microdilution method AFST/EUCAST. The genetic similarities within and between sequential clones of C. albicans were assessed by DNA fingerprinting using the random amplification ofpolymorphic DNA technique. Results： A total of 142 oral samples were isolated from 59 HIV-infected individuals who attempted up to five visits each, with or without symptoms of oropharyngeal candidiasis. Profile analysis revealed that yeasts isolated over sequential visits from symptomatic or asymptomatic individuals showed 78% or 87% relatedness, respectively. The degree of similarity among C. albicans was higher for isolates from colonization than for those from infection. Genetically identical C. albicans samples also formed connected subelusters in sequential visits. In regard to susceptibility profile, all isolates were susceptible to AMB, FLC, KTC, and ITC and maintained this pattern all along, no differences in MICs of any given antifungal compound were observed for sequential C. albicans isolates. Conclusions： These data suggest that genotype and susceptibility to antifimgal drugs were maintained over time in sequentially isolates of C. albicans colonization and a diverse evolutionary genetic trend in C. albicans sequentially isolated from the oral eandidiasis of HIV infected individuals.

Aims We aimed at disentangling the effects of spatial distance,current and past environmental dissimilarity,and their combinations on tree community taxonomic and phylogenetic turnover by addressing the following questions:(i)Is tree community taxonomic and phylogenetic turnover related to the indirect effects of spatial distance via environmental dissimilarity?(ii)Does tree community taxonomic and phylogenetic turnover respond to paleoclimate(Last Glacial Maximum and Mid-Holocene)?Methods The study was carried out in 14 Atlantic rainforest sites in Brazil(20.4 ha sampled)containing 615 tree species from 83 plant *** obtained plot-level geographic coordinates and soil variables and site-level bioclimatic variables in the current,Mid-Holocene and Last Glacial *** used structural equation models with a distance-based approach to(i)test the direct effects of spatial distance and environmental dissimilarity and(ii)test the indirect effects of spatial distance via environmental dissimilarity on taxonomic(Bray–Curtis distance)and phylogenetic turnover(Comdist and Comdistnt distances).Important Findings our results suggest a weak indirect effect of spatial distance via environmental dissimilarity on taxonomic and phylogenetic *** community turnover was driven by the direct effects of neutral,niche-based and historical ***,we inferred that the paleoclimate(historical processes)promoted the selection of the clades that gave rise to the current flora,while spatial distances(neutral processes)limited the dispersal range of species from the regional pool and environmental conditions(niche-based processes)locally selected the taxa that are able to persist.

Background:Thrombocytopenia is a possible risk factor for bleeding after band ligation of esophageal ***,elevated von Willebrand factor(VWF)in cirrhosis improves platelet function and could decrease this *** objective was to assess platelet function in patients with cirrhosis undergoing esophageal variceal ligation(EVL).Methods:The assessment consisted of platelet count,antigen and activity of VWF and VWF-cleaving protease ADAMTS-13 activity,and a platelet adhesion and aggregation test simulating vascular flow in vivo(Impact-RR)prior to ***:Totally 111 patients were divided into three groups according to platelet count:(1)o 100×109/L(n=47,42.3%);and(3)>100×109/L(n=26,23.4%).No statistically significant difference was found in the aggregate size of platelets[group 1:41.0(31.8–67.3)μm 2;group 2:47.0(33.8–71.3)μm 2;and group 3:47.0(34.0–66.0)μm 2;P=0.60]and no significant correlation was found between aggregate size and platelet count(Spearman r=0.07;P=0.47).Surface coverage was 4.1%(2.8%–6.7%),8.5%(4.0%–10.0%),and 9.0%(7.1%–12.0%)(Poups 1,2 and 3,respectively and correlated with platelet count(Spearman r=0.39;Po significant difference between groups in VWF or ***-EVL bleeding occurred in six(5.4%)patients(n=2 in group 1,n=1 in group 2,and n=3 in group 3;P=0.32).Patients with bleeding had higher MELD scores[15.0(11.3–20.3)versus 12.0(10.0–15.0);P=0.025],but no difference was demonstrated for platelet function ***:Platelet function is preserved even in the presence of thrombocytopenia,including in the patients with post-EVL bleeding.

Analysis of Nursing Care Concept: Perspective of the Person Submitted to Care

This study aimed to analyze the concept of nursing care to patients with breast cancer on chemotherapy treatment, according to the perspective of the person submitted to care. The study followed the conceptual analysis methodology proposed by Walker and Avant according to Waldow, and it was developed through consultation to SCoPUS and CINAHL databases. The selection was carried out by inclusion and exclusion criteria, constituting a sample of 7 articles. There have been identified the main uses of the risk concept, defining attributes, antecedents and consequences of the concept. The evaluation of the concept and nursing care to breast cancer patient on chemotherapy treatment allowed a broader understanding of the phenomenon, with emphasis on the skills and competencies in nursing care dispensed to woman on chemotherapy treatment for breast cancer.
