Evaluation of Opioid Reversal with Naloxone before and after Implementation of a Computerized Physician Order Entry System at a Tertiary Medical Center

Objectives： Opioid medications consistently rank among the drugs most frequently associated with adverse events. We reviewed cases of naloxone administration to assess our opioid prescribing and administration practices before and after implementation of a computerized provider order entry （CPOE） system. Our primary measure was the rate of opioid reversal with naloxone in the two time periods. Methods： We systematically reviewed all cases of naloxone administration at our institution from March 2016 through May 2016 （before period） compared with cases from October 2017 through December 2017 （after period）. Results： In the before period, 0.58% of patients who received an opioid required reversal with naloxone compared with 0.16% in the after period （p 〈 0.001）. Subject demographics such as age, sex, BMI, and serum creatinine were not significantly different between the two groups. Similarly large proportions of patients in the before and after groups had at least 2 risk factors for over-sedation （79.5% and 75%, respectively, p = 0.17）. More than 84% and 61% of patients in the before and after groups, respectively, did not have documentation of an opioid on the admission medication history, suggesting they were opioid-naYve （p= 0.10）. Per our institution＇s policy on range orders, nurses should begin with the lowest prescribed opioid dose; this policy was followed in 58.3% in the before period and 69.2% in the after period （p = 0.36）. Conclusions： Doses of naloxone that were required for opioid reversal were significantly reduced after implementation of a CPOE system. Our opioid-reversal rate was comparable to similar studies; however, secondary endpoints indicate that opioid prescribing and administration habits may not account for patient-specific risk factors for over-sedation.

The quality of a breeding site may have major fitness consequences.A fundamental step to understanding the process of nest-site selection is the identification of the information individuals use to choose high-quality nest *** secondary cavity-nesting bird species that do not add nest lining material,organic remains (faeces,pellets)accumulated inside nest cavities during previous breeding events may be a cue for high-quality nest-sites,as they contain information about past successful breeding and may improve thermal insulation of eggs during ***,cavities in which breeding was successful might also contain more nest-dwelling ectoparasites than unoccupied cavities, offering an incentive for prospective parents to avoid *** exposed breeding cavity-nesting lesser kestrels (Falco naumanni)to nestbox dyads consisting of a dirty (with a thick layer of organic substrate) and a clean nestbox (without organic material).Dirty nestboxes were strongly preferred,being occupied earlier and more frequently than clean *** success in dirty nestboxes was significantly higher than in clean ones,suggesting a positive effect of organic nest material on incubation efficiency, while nestbox dirtiness did not significantly affect clutch and brood *** from dirty nestboxes had significantly higher ectoparasite load than those from clean nestboxes soon after egg hatching,but this difference was not evident a few days *** substrate did not significantly affect nestling *** concluded that nest substrate is a key driver of nest-site choice in lesser kestrels,although the adaptive value of such a strong preference appears elusive and may be context-dependent.

Epoxy Thermosets with Self-Healing Ability

A hybrid epoxy resin with intrinsic self healing properties has been prepared from a bifunctional Diels-Alder (DA) adduct. The obtained network, based on conventional Diglycidyl ether of BisphenolA (DGEBA) and DA epoxy, leads to simultaneous tuning of physical-chemical properties and healing capability. The self-repairing behavior has been verified by means of scratch recovery observation and micromechanical analysis. A satisfactory morphological and mechanical recovery has been achieved by thermal stimulus, leading to very promising application in the field of adhesives and structural applications.

Pregnancy Craving and Pica: 60 Years Later

Purpose: To study pregnancy craving and pica in a cohort of women and compare our findings to a historic study. Methods: Prospective survey of women in the third trimester of pregnancy. Data were collected using the same questionnaire from a study published in Obstetrics and Gynecology in 1957. All surveys were conducted by clinicians at 35 0/7-41 6/7 weeks gestational age. High risk pregnancies were excluded. Results: We included 547 women in the analysis. Of those, 60.8% reported food craving vs 65.7% in the 1957 cohort, p-value 0.09. The most common food craving was fruit, seen in 31.1% vs 13.6%, p-value p-value 0.00001. The cohorts differed in fruit craving, (31.1% vs 13.6%), meat, poultry, fish, dry beans, eggs, and nuts (21.0% vs 5.0%), milk, yogurt, and cheese, (17.0% vs 2.2%) and vegetable, (14.1% vs 6.9%), all with p-value

Travel in Pregnancy: The Impact of Zika

OBJECTIVE: The purpose of our study was to examine current travel patterns and behaviors of pregnant women. METHODS: We developed a questionnaire adapted from a publication by Kingman and Economides to examine the travel behavior of women during pregnancy in a suburban community. RESULTS: A convenience sample of 102 patients completed the travel questionnaire. Forty-six (45.1%) traveled during the current pregnancy. Thirty-one (30.4%) traveled more than once. A total of 257 trips were taken by the participants: 113 (43.9%) trips were taken in the first trimester, 87 (33.8%) in the second and 59 (22.9%) in the third trimester. Trip length ranged from 2-90 days with a mean of 11 days. Reasons for travel included: 193 (75.1%) for leisure, 37 (14.4%) work related, 10 (3.9%) trips for emergencies, and 3 (0.4%) trips for relocation.?Eighteen women (17.6%) traveled internationally. One (1.0%) woman was hospitalized while traveling. Manner of travel was as follows: car 167 (65.0%), plane 67 (26.1%), train 13 (5.1) %, bus 10 (3.9%) and none by boat. Nineteen (41.3%) women sought travel advice. Thirteen (68.4%) asked for advice from a doctor, 2 (10.5%) from a nurse, 2 (10.5%) from family and/or friends, 1 (5.3%) from the Internet and 1 (5.3%) from a travel book. Seventeen (37%), traveled without suitable insurance. DISCUSSION: Travel rates during pregnancy have remained surprisingly stable over the past 60 years. Almost 50% of our cohort traveled during pregnancy, and the majority did not seek advice prior to travel. In light of new infectious disease threats, obstetric practice and advice needs reassessment. The majority of travel during pregnancy remains optional. Improved patient education and consultations prior to travel could decrease health risks.

目的:检测大鼠视网膜色素上皮细胞系(RPE-J)是否表达载脂蛋白的基因及其是否具有合成和分泌中性脂质的功能。方法:将RPE-J细胞培养于组织培养板及Transwell上,用RT-PCR检测载脂蛋白mRNA的表达。将荧光标记颗粒加入细胞中培养以检测细胞吞噬功能。细胞中加入光感受器细胞外节,培养不同时间点后提取脂质,用薄层层析法和酶学荧光法分析细胞中脂质含量及变化。用放射性标记的OleicAcid([3H]-oleate)检测细胞新合成和分泌中性脂质的功能。结果:RPE-J细胞有载脂蛋白A-I,A-II,B,E,C-I,C-II,C-III,以及微粒体甘油三酯酸转移蛋白基因的表达。RPE-J细胞具有吞噬功能。吞噬光感受器细胞外节后,RPE-J细胞中甘油三酯含量增加5.8倍,磷脂含量增加2.6倍,非酯化胆固醇增加1.5倍,酯化胆固醇增加0.3倍。RPE-J细胞和培养基内可以检测到新合成的放射标记的中性脂质。细胞内脂质含量随时间延长而增加;培养基中只有磷脂随时间延长而增多。结论:RPE-J细胞有多种载脂蛋白及微粒体甘油三酯酸转移蛋白的基因表达。吞噬光感受器细胞外节后,RPE-J细胞中脂质增加。培养基中加入长链脂肪酸后,RPE-J细胞可以新合成和分泌中性脂质。

一、卫星通信的革命2020年的头两个月对太空行业来说很特别,尤其是在卫星领域出现了意义非凡的进展。最令人印象深刻的可能是“星链”(Starlink)卫星的发射。2019年9月,在巴黎举行的世界卫星商业周上,美国太空探索技术公司(SpaceX)的首席运营官表示,2020年,SpaceX将每两周进行一次“星链”卫星发射,将60颗卫星送入太空,组成该公司的全球卫星互联网星座。当时,笔者对如此高密度的制造和发射仍持怀疑态度,因为这对于相对较小的卫星也很困难,而每颗“星链”卫星的质量略高于200kg。

Covered self expandable metallic stent with flared plastic one inside for pancreatic pseudocyst avoiding stent dislodgement

Endoscopic ultrasound-guided drainage has recently been recommended for increasing the drainage rate of endoscopically managed pancreatic fluid collections and decreasing the morbidity associated with conventional endoscopic trans-mural drainage. The type of stent used for endoscopic drainage is currently a major area of interest. A covered self expandable metallic stent (CSEMS) is an alternative to conventional drainage with plastic stents because it offers the option of providing a larger-diameter access fi stula for drainage, and may increase the fi nal success rate. One problem with CSEMS is dislodgement, so a metallic stent with flared or looped ends at both extremities may be the best option. An 85-year-old woman with severe comorbidity was treated with percutaneous approach for a large (20 cm) pancreatic pseudocyst with corpuscolated material inside. This approach failed. The patient was transferred to our institute for EUS-guided transmural drainage. EUS confi rmed a large, anechoic cyst with hyperechoic material inside. Because the cyst was large and contained mixed and corpusculated fluid, we used a metallic stent for drainage. To avoid migration of the stent and potential mucosal growth above the stent, a plastic prosthesis (7 cm, 10 Fr) with flaps at the tips was inserted inside the CSEMS. Two months later an esophagogastroduodenoscopy was done, and showed patency of the SEMS and plastic stents, which were then removed with a polypectomy snare. The patient experienced no further problems.
