Heritage risk index:A multi-criteria decision-making tool to prioritize municipal historic preservation projects

The current pressure on the built historical heritage requires a global evaluation to develop strategies for its preservation.A combined vision of the state of conservation of architectural heritage is essential to highlight the vulnerabilities of the structure from which harmonized measures can be proposed for its *** study proposes a multi-criteria analysis methodology that considers a qualitative and quantitative approach to organize the global conservation state of heritage buildings hierarchically.A global index can be determined on the basis of the generation of an interoperable data matrix by defining a set of indicators to assess different components of a *** index describes the risk with respect to the stability,safety,and habitability of the *** methodology aims to achieve the state of the building in an integral way by obtaining a simple and objective index that enables efficient comparison among different *** application of this methodology to a historic area allows creating a hierarchical map of risk to stablish priorities for intervention and enable a sustainable and critical management and planning over the actions of maintenance,conservation,and *** this way,a holistic vision of the complex reality of built heritage and the urban landscape is obtained to identify critical areas of priority intervention.

Pulmonary arterial hypertension related to human immunodeficiency virus infection:A case series

AIM: To present 18 new cases of human immunodeficiency virus(HIV)-related pulmonary arterial hypertension(PAH) with presenting features,treatment options and follow-up ***: This is a single-centre,retrospective,observational study that used prospectively collected data,conducted during a 14-year period on HIV-related PAH patients who were referred to a pulmonary hy-pertension unit. All patients infected with HIV were consecutively admitted for an initial evaluation of PAH during the study period and included in our study. Right heart catheterisation was used for the diagnosis of PAH. Specific PAH treatment was started according to the physician's judgment and the recommendations for idiopathic PAH. The data collected included demographic characteristics,parameters related to both HIV infection and PAH and disease ***: Eighteen patients were included. Intravenous drug use was the major risk factor for HIV infection. Risk factors for PAH,other than HIV infection,were present in 55.5% patients. The elapsed time between HIV infection and PAH diagnoses was 12.2 ± 6.9 years. At PAH diagnosis,94.1% patients had a CD4 cell count > 200 cells/μL. Highly active antiretroviral therapy(present in 47.1% patients) was associated with an accelerated onset of PAH. Survival rates were 93.8%,92.9% and 85.7% at one,two and three years,respectively. Concerning specific therapy,33.3% of the patients were started on a prostacyclin analogue,and the rest were on oral drugs,mainly phosphodiesterase-5 inhibitors. During the follow-up period,specific therapy was de-escalated to oral drugs in all of the living ***: The survival rates of HIV-related PAH patients were higher,most likely due to new aggressive specific therapy. The majority of patients were on oral specific therapy and clinically stable. Moreover,sildenafil appears to be a safe therapy for less severe HIVrelated PAH.

Multinational Measles Outbreak in Post-Elimination Era,Involves Three Countries of North America and a European Country in a Short Transmission Chain

Background: Four measles cases in Canada and one in the United States are linked to international importation of measles in Playa del carmen, Quintana Roo, Mexico. Objective: To describe characteristics of transmission and not spillover to local population in Mexico. Material and Methods: The outbreak investigation was based on active search of cases and in the rapid monitoring of vaccination coverage in children aged 1 - 7 years old. Laboratory confirmation by Enzyme-Linked Immunosorbent Assay (ELISA) and molecular detection by Reverse Transcriptase-Polymerase Chain Reaction (RT-PCR) assay in throat swab and urine were done. Results: One transmission chain with three generations of cases was identified. The primary case was a 9-year-old boy who was infected in Wales, UK. His sisters aged 7 and 15 years old respectively, were the first generation of cases in Mexico. The second generation was related to the imported cases, and affected two Canadian tourists and an American woman aged 39 years old. A third generation occurred in Canada and affected an infant of 15 months of age and his sister aged 4 years old. The genotype D8 which was circulating in UK was identified in these patients. One probable case was detected in Quintana Roo, but was discarded by laboratory testing. The coverage with at least one dose of Measles-Mumps-Rubella (MMR) vaccine ranged from 95% to 99.5%. Conclusion: International travelers with no history of vaccination are at risk of acquiring measles even in countries that have interrupted endemic transmission. The high immunization coverage of measles containing vaccine could explain the absence of cases in Mexican population. Highlights: Multinational measles outbreak in a country without endemic transmission. The findings exhibit the importance of immunization in international travelers in the post-elimination era.
