BACKGROUND Acute fibroid complications are ***,failure to recognize and treat acute complications expeditiously when they occur can lead to catastrophic,even deadly,*** is a rare but potentially fatal condition resulting from infarction and infection of a fibroid through bacterial seeding and direct,hematogenous,or lymphatic *** though the diagnosis is established through clinical and laboratory findings,imaging is an important complementary method to support the suspected *** SUMMARY Herein,we report a case of a pyomyoma in a nulliparous woman previously diagnosed with uterine leiomyomatosis according to ultrasound *** patient had previously attended the emergency room due to hypogastric pain unresponsive to *** a week of persistent pain,she developed sepsis without any identifiable *** resonance imaging revealed findings compatible with uterine myomatosis with red degeneration,and a possible diagnosis of a pyomyoma was made according to the imaging findings along with the patient’s clinical *** decided to perform myomectomy(which is an infrequently performed surgical treatment due to the procedure’s intrinsic implic-ations)due to the patient’s desire to preserve *** results revealed a uterine leiomyoma with coagulative and liquefactive necrosis,while the tissue culture showed gram-negative cocci bacteria,which were successfully treated using antibiotic *** patient’s health status improved after several *** The main diagnostic tools to evaluate pyomyomas are the clinical and laboratory findings as well as tissue ***,magnetic resonance imaging can help to corroborate these findings as well as to better characterize myomas with its different complications.

Brazil is one of the countries that carry out cesarean delivery in the world. The pain after cesarean delivery (CD) is one of the main concerns of parturient. Intrathecal Morphine (ITM) is widely used for the treatment of pain after CD;however, the optimal dose with minimal side effects is to be established. Objectives: To compare the analgesia and side effects resulted by three different doses of ITM as part of a multimodal regimen for post-cesarean delivery analgesia. Methods: One hundred ninety-four patients were sampled in this randomized, double-blinded, three-arm study (group 0.075: ITM 0.75 mg;group 0.1: ITM 0.1 mg and group 0.15: ITM 0.15 mg). All patients received intravenous dexamethasone, dipyrone and ketoprofen. The primary endpoint was pain intensity measured at rest and on movement, while the secondary endpoint was overall satisfaction with pain relief, the incidence of nausea and vomiting, intensity of pruritus, and the use of additional tramadol, antipruritic and antiemetic use within 24 hours after postoperation. Results: The three groups showed similar pain intensity at rest and on movement. The pain intensity on movement was significantly higher than at rest in the groups (p 0.001). There were no significant differences among the groups regarding secondary endpoint. Conclusions: Our findings suggest that in cases of an elective CD, 0.075 mg of ITM produces postoperative analgesia of similar quality that provided by 0.1 or 0.15 mg ITM. The patients should have a multimodal analgesia approach that includes: ITM, anti-inflammatory agents and access to additional systemic analgesic if necessary.

The use of Animal House Facilities of IPEN-CNEN/SP （Nuclear and Energetic Research Institute-National Nuclear Energy Commission/SP, Brazil） has provided rats and mice with controlled sanitary quality for research and quality control in the production of radiopharmaceuticals lots, produced at IPEN, following rules of CONCEA-Brazilian National Council for Animal Experimentation Control and approved in CEUA （Ethics Committee on the Use of Animals）, before they are sent to hospitals and clinics spread out in Brazil, for use in nuclear medicine. The production and the supply of high quality laboratory animals have fundamental importance for the accomplishment of vanguard scientific research, with reproducibility and universality. The quality of those animals depends, largely, on the available facilities for their production and lodging, to assure the demanded sanitary control and animal＇s welfare, in agreement with the ethical principles that control the activity. Therefore, the facilities design is of vital importance so that the mentioned requirements can be reached. Nevertheless, pigs and miniature pigs have gained importance as large animal models in medicine. With their size, organ capacity, and physiology resembling in several aspects that of humans, pigs are well suited for preclinical experiments and long-term safety studies. Minipigs will be used for preclinical testing of radiopharmaceuticals and assays radioactive materials for cardiac tests. This paper describes the premises and preliminary activities that have been performed at IPEN for the design of new Animal House Facilities dedicated to keeping minipigs taking into account fundamental aspects such as： animal＇s welfare, sanitation, genetic, in agreement with the ethical principles that control the activity and environmental concerns.

The adsorption capacities for the removal of hexavalent chromium from aqueous solutions by six carbon nanomaterials have been evaluated. Single-walled and multi-walled carbon nanotubes as received and after oxidation treatment, graphene oxide and reduced graphene oxide are the materials with different dimension and functionalization compared in this research. Carbon nanotubes have been modified using hydrogen peroxide as oxidizing agent under microwave radiation. The oxidation treatment on carbon nanotubes has a positive effect increasing the adsorbent–adsorbate interaction. Rate-controlling mechanisms and equilibrium data are analyzed using non-linear models. Non-linear method is proposed as the most suitable method for determining the kinetic and equilibrium parameters. The values of adsorption energy(E) obtained from the Dubinin–Radushkevich isotherm,have been found around 0.371 and 0.870 k j·mol-1, indicating physical adsorption. Therefore, the pseudo-second order model represents better the kinetic experimental data. The results show that the Langmuir isotherm provides a slightly better fit to the experimental data compared with the Freundlich isotherm, indicating homogeneous distribution of active sites on carbon nanomaterials and monolayer adsorption. The separation factors RLare found in the range of 0–1, suggesting that the adsorption process is suitable for all adsorbents. The mechanisms for hexavalent chromium removal have been proposed as electrostatic interactions and hydrogen bonding.

AIM:To discuss the feasibility of single session endoscopic ultrasonography(EUS) to discuss and endoscopic retrograde cholangiopancreatography(ERCP) ***:Retrospective endoscopic and anesthetic outcome comparison of performing both EUS and ERCP in a single endoscopic session(Group Ⅰ) versus performing each procedure in two different sessions(Group Ⅱ) was *** following variables were evaluated:epidemiological variables,American Society of Anesthesiologists Physical Status Classification(ASA) level,procedural time,propofol dose,anesthetic complications,endoscopic complications and diagnostic yield,and therapeutic procedures on both groups.T-student,ChiSquare and Fisher test were used for ***:We included 39 patients in Group Ⅰ(mean age:69.85 ± 9.25;27 men) and 46 in Group Ⅱ(mean age:67.46 ± 12.57;25 men).Procedural time did not differ significantly between both groups(Group Ⅰvs Group Ⅱ:93 ± 32.78 vs 98.98 ± 38.17;P >0.05) but the dose of propofol differed(Group Ⅰ vs Group Ⅱ:322.28 ± 250.54 mg vs 516.96 ± 289.06 mg;P = 0.001).Three patients had normal findings on both *** anesthetic complications [O2 desaturation(2),broncoaspiration(1)] and 9 endoscopic complications [pancreatitis(6),bleeding(1),perforation(1),cholangitis(1)] occurred without significant differences between both groups(P > 0.05).We did not find any significant difference regarding age,sex,ASA scale level,diagnostic yield or therapeutic maneuvers between both ***:The performance of EUS and ERCP in a single session offers a similar diagnostic and therapeutic yield,does not entail a higher complication risk and requires a significantly smaller dose of propofol for sedation compared with performing each exploration in a different session.

Objective: To evaluate the synergic effects of a novel oral supplement formulation, containing prebiotics,yeast b-glucans, minerals and silymarin(Silybum marianum), on lipid and glycidic metabolism, inflammatory and mitochondrial proteins of the liver, in control and high-fat diet-induced obese ***: After an acclimation period, 32 male C57 BL/6 mice were divided into the following groups:nonfat diet(NFD) vehicle, NFD supplemented, high-fat diet(HFD) vehicle and HFD supplemented. The vehicle and experimental formulation were administered orally by gavage once a day during the last four weeks of the diet(28 consecutive days). We then evaluated energy homeostasis, inflammation, and mitochondrial protein expression in these groups of ***: After four weeks of supplementation, study groups experienced reduced glycemia, dyslipidemia,fat, and hepatic fibrosis levels. Additionally, proliferator-activated receptor-α, AMP-activated protein kinase-1α, peroxisome proliferator-activated receptor γ co-activator-1α, and mitochondrial transcription factor A expression levels were augmented;however, levels of inhibitor of nuclear factor-κB kinase subunit a and p65 nuclear factor-κB expression, and oxidative markers were reduced. Notably, the cortisol/C-reactive protein ratio, a well-characterized marker of the hypothalamic–pituitary–adrenal axis immune interface status, was found to be modulated by the ***: We discovered that the novel supplement was able to modify different antioxidant, metabolic and inflammatory pathways, improving the energy homeostasis and inflammatory status, and consequently alleviated hepatic steatosis.

BackgroundHuman rabies outbreak transmitted by bats continues to be a relevant public health problem not only in the Amazon region. The disease has affected one of the areas with the greatest poverty in southeastern Brazil, a region inhabited by the Maxakali indigenous *** presentationWe describe four cases of rabies among indigenous children that occurred in the indigenous village of Pradinho, municipality of Bertópolis, Minas Gerais, Brazil. Cases were notified between April and May 2022, all of whom died on average eight days after the first symptoms. All cases were observed in rural residents under 12 years of age. The probable form of exposure was through bat bites. The predominant symptoms were prostration, fever, dyspnea, sialorrhea, tachycardia, and altered level of consciousness. Half of the cases underwent late and/or incomplete post-exposure rabies prophylaxis, however, the other half underwent pre-exposure rabies prophylaxis, with only one case completing the scheme and another undergoing the adapted Milwaukee Protocol (Recife Protocol). All cases ended in *** was the first rabies outbreak among indigenous people in Brazil. Among the manifested clinical forms in the series, there was a disease atypical presentation in at least one case. We suggest active surveillance and an intercultural educational campaign to prevent new cases.

Daily Total Column Ozone (TCO) measurements compiled from Total Ozone Mapping Spectrometer (TOMS) and Ozone Monitoring Instruments (OMI) were used to analyze the global and hemispherical TCO interannual variations. Two periods of TCO measurements were analyzed separately covering full years. For the 1978-1994 period, the TCO showed a global decade decrease rate of 13.45 DU (about -4.3%). For the Northern Hemisphere(NH) the decade decrease rate was of 12.96 DU (-4.0%), while in the Southern Hemisphere (SH) was of 13.57 DU (-4.5%). These decreases in ozone trends, using the totality of TOMS and OMI satellite measurements, are greater than those reported in literature. The 1998-2014 period global TCO decade decrease rate was of 1.56 DU, corresponding 0.94 DU and 0.138 DU for the NH and SH, respectively. The global TCO variations must show a double annual periodicity, the first one with maxima in March due to the Northern Hemisphere (NH) and the second one during September due to the Southern Hemisphere (SH). However, the maxima due to SH TCO interannual variations have gradually vanished. A disturbance in the SH TCO interannual variations has appeared since 1980;graphically the periodicity brakes down and transforms to a double peak from 1985 and on. This effect can be attributed to the hemispheric impact of the ozone hole at the South Pole. Between October 1, 2004 and December 14, 2005 TOMS and OMI have recorded this disturbance unequivocally. We conclude that the disturbance in SH TCO has an irreversible character.
